A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. The patient underwent multiple consecutive operations and interventions having, among others, finally replaced the entire thoracic aorta with the use of the hybrid elephant trunk technique. and T. Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary. Key words: hereditary hemorrhagic telangiectasia – Rendu-Osler-Weber disease – brain abscess – arteriovenous malformation – epistaxis – endoglin The authors declare they have no potential conflicts of interest concerning drugs, products, or services used in the study. The Editorial Board declares that the manuscript met the ICMJE “uniform requirements” for biomedical papers., and G. Hanoun, V. Vybíhal, M. Sova, J. Boudný, T. Svoboda, M. Smrčka
Prezentujeme praktický postup riešenia tejto komplikácie, ktorý sme zostavili na základe skúseností s tromi prípadmi kompletnej inverzie maternice a literárnych údajov., Acute uterine inversion is severe obstetric and life threatening complication. As immediate consequences of uterine inversion are different degrees of uterine prolaps. The pathogenesis of uterine inversion is usually associated with an incorrect management of the third stage of labour. Immediate diagnosis and prompt action minimize maternal morbidity and mortality. We report three cases of the uterine inversion with a guideline supported with a literature review., Erik Dosedla, Dušan Frič, Ján Richnavský, Štefan Lukačín, and Lit.: 19
Biopsie kontralaterálního varlete u pacientů s germinálním tumorem k vyloučení IGCNU by měla být provedena v případě kryptorchizmu, poruchy spermiogeneze a atrofií varlete s objemem pod 12 ml, vhodná je rovněž u pacientů s mikrolitiázou varlete a předchozím výskytem IGCNU., Objective: We are presenting a case report of young man who developed an asynchronous bilateral seminoma based on an intratubular germ cell neoplasia of unclassified type (IGCNU). Case report: A patient, with history of a stage I seminoma of the left testis, based on an IGCNU of the left testis, after a radical orchidectomy and adjuvant radiotherapy, was referred to the urology clinic with a suspected tumor of the right testis. A testicular biopsy was made and the histological examination showed an IGCNU. A radical orchidectomy was performed and a seminoma was found. Conclusion: IGCNU is a precursor lesion of germ cell testicular tumors. Treatment of the IGCNU includes radiotherapy or chemotherapy with cisplatin. Surveillance remains an option for those with a solitary testicle who wish to preserve fertility and endocrine function. A contralateral testicular biopsy, in patients with a history of germ cell tumors, should be made in cases of cryptorchidism, poor spermatogenesis, and testicular atrophy with a testicular volume of less than 12 ml. Biopsy is also recommended in patients with testicular microlithiasis and history of ITGCNU., Jakub Musil, Daniel Bulíř, Jan Jandejsek, and Literatura
Unilateral otosclerosis combined with avascular necrosis of stapes crura is a rare entity. It should be considered in a case of high grade otosclerosis. Symptoms are the same as in patients who suffer from common otosclerosis. Patients complain on progressive hearing loss and tinnitus. The diagnosis is made clinically by conventional audiologic evaluation and radiologically by x-ray mastoid Schuller's view and CT scan. HRCT scan makes visible all parts of ossicular chain and gives surgeon some information about ossicular chain damage. Surgery with stapedotomy and stapes prosthesis implantation in a case of otosclerosis with avascular necrosis of stapes crura can be success therapy to improve patient's hearing and M. Erdoglija, J. Metastatický karcinom prsu je nevyléčitelné onemocnění, ale použití nových léčebných možností může zmírnit obtíže pacientky, prodloužit dobu do progrese a prodloužit celkové přežití. Uvádíme kazuistiku ukazující naši zkušenost s efektem léčby kombinace bevacizumabu s chemoterapií u pacientky s triplet negativním karcinomem prsu., Metastatic breast cancer is incurable disease, but application of new therapeutic options reduce complaint of patient, extend time to progression and extend overall survival. Treatment of chemotherapy is often concomitant with undesirabels effects and during of the treatment come to pass chemoresistance. New chance of treatment is combination of chemotherapy with targeted treatment. The vascular endothelial growth factor (VEGF) is the main regulator of angiogenesis. The humanized monoclonal antibody antiVEGF – bevacizumab – evidenced efficiency in series cancer diseases. The case report presents a our experience of the combined treatment of bevacizumab with chemotherapy at patient with chemoresistant triple negative breast cancer., Marta Holíková, and Literatura
