V retrospektivní pozorovací studii hodnotíme míru úspěšnosti operační techniky rekonstrukce strikturybulbární části uretry (za období 1988–2013) v italském centru s velkým objemem případů. Do studie byli zařazeni pacienti vyžadující uretroplastiku bulbární části močové trubice. Ze studie byli vyloučeni pacienti se strikturou penilní uretry a pacienti s neúplnými zdravotními záznamy. Primární cíl studie spočíval v hodnocení celkové míry úspěšnosti operační rekonstrukce striktury bulbární části močové trubice.Sekundární výsledné parametry zahrnovaly hodnocení úspěšnosti jednotlivých operačních technik. Pro účely studie jsme užili deskriptivní statistické metody. Do studie bylo zařazeno celkem 614 mužů ve věku 39 let (medián). Medián sledování činil 82 měsíců (rozmezí 12–302 měsíců). Z celkem 614 pacientů podstoupilo 338 (55 %) mužů uretroplastiku technikou ventrálního onlay graftu, 208 (33,9 %) mužů anastomózu konec ke konci, 44 (7,2 %) mužů uretroplastiku technikou dorzálního onlay graftu a 24 (3,9 %) mužů anastomózu s augmentací. Jako úspěšných bylo klasifikováno 520 (84,7 %) výkonů a v 94 (15,3 %) případech došlo k selhání. Technika anastomózy konec ke konci byla úspěšná u 87 % pacientů, uretroplastika technikou ventrálního onlay graftu u 85,5 % pacientů, technika dorzálního onlay graftu u 72,7 % pacientů a anastomóza s augmentací u 75 % pacientů. Rekonstrukce striktury bulbární uretry s využitím jedné ze čtyř jednodobých operačních technik dosáhla uspokojivých výsledků u 84,7 % pacientů., We performed a retrospective, observational study to evaluate the success rate of surgical techniques used to repair bulbar urethral strictures in an Italian high‑volume centre from 1988 to 2013. Patients requiring bulbar urethroplasty were included. Patients with penile stricture or incomplete clinical records were excluded from the study. The primary outcome of the study was to evaluate the overall success rate of surgical repair of bulbar urethral strictures. The secondary outcome of the study was to investigate the success rate of the single surgical techniques. Descriptive statistical methods were applied. Six hundred fourteen male patients with a median age of 39 years were included in the study. The median followup was 82 months (range 12–302 months). Out of 614 patients, 338 (55%) underwent ventral onlay graft urethroplasty, 208 (33.9%) end‑to‑end anastomosis, 44 (7.2%) dorsal onlay graft urethroplasty and 24 (3.9%) augmented anastomotic repair. Out of 614, 520 (84.7%) were classified as success and 94 (15.3%) as failures. End‑to‑end anastomosis provided 87% success rate, ventral onlay graft technique, 85.5%, dorsal onlay graft technique 72.7% and augmented anastomotic repair 75%. We studied the relationship of connective tissue state with magnesium and hydroxyproline levels in blood and urine of young women with primary dysmenorrhea. Study showed that in girls with dysmenorrhea and phenomena of connective tissue dysplasia hydroxyproline level in urine was increased which was associated with increased degradation of collagen and decreased level of magnesium in blood serum. Účinná léčba nádorových onemocnění často způsobuje trvalou ztrátu reprodukčních schopností pacienta. Jsou představeny moderní ukazatele ovariální rezervy, které mohou v běžné klinické praxi významně pomoci při zhodnocení míry rizika poškození gonadálních buněk v důsledku onkologické léčby. Práce dále popisuje zavedené metody reprodukční ochrany – kryokonzervace embryí a oocytů, kryokonzervace ovariální tkáně včetně možnosti její autotrasplantace a metody ochrany ovariální funkce aplikací analog gonadoliberinu. V diskuzi je pozornost věnována zejména výhodám a limitům popsaných metod ovariální protekce. V závěru jsou formulována stručná doporučení pro konzultace s pacientkami v adolescentním a reprodukčním věku před onkologickou léčbou, která významně ohrožuje jeho fertilitu., Effective treatment of cancer often causes patients irreversible damage of reproductive abilities. Modern markers ovarian reserves are introduced. These clinical examinations can effectively help to evaluate the risk of irreversible damage of gonadal cells done by oncology disease or its curative treatment. The paper describes current oncofertility techiques implemented to clinical practice – embryo and oocyte banking, ovarian tissue cryopreservation including further possibility of its orthotopic autotransplantation and administration of gonadoliberin analogues for ovarian protection. Attention is applied to pro and contras of previously described methods of ovarian protection in the discussion. The purpose of study was to analyze clinical and genetic polymorphism of Duchenne/Becker progressive muscular dystrophies among patients with neuromuscular diseases in Uzbekistan. 106 male patients with progressive pseudohypertrophic forms of muscular dystrophy were retrospectively and prospectively analyzed in the period from 2004 till 2014: 93 patients with Duchenne PMD aged from 3 years to 18 years and 13 patients with Becker PMD aged from 10 years to 25 years, who had been examined in the medico-genetic consulting department of the Republican Center “Mother and Child Screening” of Tashkent city. Olovo je kumulativní jed, který je zvláště nebezpečný pro malé děti. I při nízké úrovni expozice, která byla dříve považována za bezpečnou, poškozuje vývoj mozku a nervového systému, což vede k poruchám chování a snížení inteligence dětí. Hlavním zdrojem expozice olovu je potrava. Pitná voda může představovat významný příspěvek expozice olovu ve starých budovách s původními olověnými rozvody vody. Cílem prezentované práce bylo získat základní informaci o expozici olovu z pitné vody ve vytipovaných školských zařízeních ve starých budovách v Pardubickém kraji a o možném zdravotním riziku pro děti. V letech 2013 a 2014 byly v součinnosti se Zdravotním ústavem vyšetřeny vzorky pitné vody ve 20 školských zařízeních. Vzorky byly odebrány jak ráno po noční stagnaci vody v potrubí, tak náhodně během dne. Zvýšený obsah olova ve srovnání s dodávanou vodou z vodovodu byl zjištěn ve většině zařízení. V téměř polovině zařízení byly nalezeny zvýšené koncentrace olova i ve vzorcích náhodně odebraných během dne při provozu školy a ve 3 zařízeních bylo zjištěno překročení současného hygienického limitu 10 µg/l. Při charakterizaci rizika vývojové neurotoxicity u dětí byla použita Metodika rozdílu expozic (MOE), doporučená EFSA. Výsledkem je konstatování, že i podlimitní obsah olova v pitné vodě ve školském zařízení může představovat sice malé, ale ne úplně zanedbatelné riziko nepříznivého ovlivnění nervového vývoje u dětí. Do péče o zdravé prostředí předškolních a školních zařízení by proto měla patřit i eliminace tohoto potenciálního rizika., Lead is a cumulative toxicant that is particularly harmful to young children. Even at lower levels of exposure that were previously considered safe, lead damages brain and nervous system development resulting in behavioural changes and decreased intelligence of children. The main source of lead exposure is food. An important contributor to lead exposure may be the tap water in old buildings, having the original lead plumbing. The aim of the present study was to obtain basic information about lead exposure from drinking water in selected educational facilities placed in old buildings in the Pardubice Region as well as the possible health risks for the children. Between 2013 and 2014, in collaboration with the Public Health Institute, samples of drinking water in 20 educational facilities were examined. The samples were taken both in the morning after overnight water stagnation in the pipes and randomly during the day. In most facilities the lead content was increased as compared to water regularly supplied in the mains. In almost half of the monitored facilities increased lead content was detected even in the samples taken during the day. In three facilities the lead content exceeded the current lead content limit 10 µg/l. To characterize the risk of developmental neurotoxicity in children, the methodology "margin of exposure" (MOE), recommended by EFSA, was used. The result is, that even a lower lead content in drinking water of school facilities than the regulatory limit value may pose a small but not entirely negligible risk of neurodevelopmental effects. Úvod: Systémový lupus erythematodes (SLE) je chronické autoimunitní onemocnění charakterizované multisystémovým postižením. Cílem naší studie bylo ozřejmit výskyt snížených hodnot koncentrací imunoglobulinů (s-Ig) v séru v populaci nemocných se SLE. V retrospektivní studii bylo zhodnoceno 799 výsledků vyšetření s-Ig získaných od 157 nemocných splňujících revidovaná klasifikační kritéria. Výsledky: Sérová koncentrace imunoglobulinů nižší než dolní hranice normy byla zjištěna celkem u 29 ze 157 nemocných (18,5 %). Nejčetnější byl výskyt izolovaného snížení IgG 12 ze 157 (7,6 %), 2 nemocné naplňovaly kritéria selektivního deficitu IgA a v jednom případě se jednalo o vzácnou koincidenci s běžnou variabilní imunodeficiencí (common variable immunodeficiency – CVID). Článek podrobněji prezentuje 2 případy nemocných se SLE komplikované těžkou hypogamaglobulinemií a infekcemi vyžadujícími dlouhodobou substituční léčbu imunoglobuliny. V prvním případě se jedná o vysoce pravděpodobný případ běžné variabilní imunodeficience a v případě druhém pak o léky indukovanou hypogamaglobulinemii. Tato žena s anamnézou maligního lymfomu a současným závažným multiorgánovým postižením při SLE byla léčena rituximabem po selhání konvenční léčby. Závěr: S protilátkovými deficiencemi se můžeme setkat i u nemocných se SLE. Opakované měření sérové koncentrace imunoglobulinů by mělo být součástí rutinního sledování těchto nemocných. Na možnost CVID bychom měli myslet v případě opakujících se sinopulmonárních infekcí, zejména v kombinaci s absencí aktivity SLE. Léčba rituximabem může vést i u nemocných se SLE k dlouhodobé supresi B-lymfocytů s rozvojem sekundárního protilátkového deficitu, který v některých případech vyžaduje substituční léčbu imunoglobuliny., Introduction: Systemic lupus erythematosus (SLE) is a chronic autoimmune multisystem disease. The aim of our study was to clarify the frequency of decreased serum immunoglobulin levels in SLE patients. There were evaluated 799 results of serum immunoglobulin levels gained from 157 patients fulfilling revised ACR criteria in the retrospective study. Results: The immunoglobulin levels under the normal range were found in 29/157 (18.5 %) patients. The most frequent was isolated reduction of IgG 12/157 (7.6 %), two persons fulfilled criteria for selective IgA deficiency, and one case possible diagnosis of common variable immunodeficiency (CVID). Additionally we report two cases of SLE patients complicated by severe hypogammaglobulinaemia and infectious complications with necessity of long-term immunoglobulin substitution therapy. The diagnosis of CVID is highly probable in the first case. The second case presents sever drug-induced hypogammaglobulinaemia. This female with lymphoma history and multiorgan impairment due to acute SLE was treated with rituximab after convention therapy failure. Conclusion: Humoral immunodeficiency may occur in SLE patients. The monitoring of serum immunoglobulin levels could be a routine in these patients. The CVID diagnosis is possible in patients suffering from recurrent sinopulmonary infections, especially in combination with absence of lupus activity. Aim: The principal aim was to establish reference paediatric data for the serum homocysteine levels in Czech children and adolescents. Methods and Results: 144 children either healthy or not sufferig from acute or chronic inflammation, autoimmune disorders including rheumatic diseases, inflammatory musculoskeletal disorders, inflammatory bowel disease, diabetes mellitus, hypercholesterolemia, epilepsy, chronic renal failure, aged 0–19.9 years (0–6.9 years, n = 40; 7–10.9 y, n = 28; 11–15.9 y, n = 45; 16–19.9 y, n = 31) had their blood samples collected and the serum homocysteine level (S-homocysteine) was evaluated by chemiluminescence. A significant age dependence of the S-homocysteine levels was observed (R = 0.35, p < 0.01); with highest values of upper reference range in the 11–15.9 and 16–19.9 years’ group, respectively.
