The main aim of the initial examination is to decide whether we are facing a patient with life-threatening disease, e.g. stroke, or a benign disease such as vestibular neuritis. This decision is usually made under a time pressure and knowing that neuroradiology examination may not provide sufficient information. Clinical examination of the vestibular and oculomotor function is sufficient to make the initial decision. Vestibular examination requires basic knowledge of the vestibular system function and reflex circuits involved in the organization and control of the multisensoric balance system. Key words: diziness – vertigo – instability – unsteadiness – head impulse test – vestibular system – stroke The author declare he has no potential conflicts of interest concerning drugs, products, or services used in the study. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. Mezi hlavní léčebné strategie patří agresivní léčba relapsů (vysokodávkované steroidy či plazmaferéza) následované zahájením chronické terapie (mezi léky první volby dnes řadíme azathioprin a rituximab)., Neuromyelitis optica (NMO), also known as Devic‘s disease, is a devastating autoimmune disorder of the central nervous system and is clinically characterized by inflammation of the optic nerve, spinal cord, and, less frequently, brainstem. The discovery of antibodies against aquaporin‑4 (NMO‑IgG/AQP4-IgG) has definitively distinguished this disease from multiple sclerosis. AQP4-IgG testing is crucial in differential diagnosis that also includes magnetic resonance imaging of the brain and spinal cord. Typically, spinal cord lesions exceeding the length of three vertebral segments are found. Changes in magnetic resonance imaging of the brain do not exclude the diagnosis of NMO; by contrast, they are relatively common and are fairly typical. Optical coherence tomography provides important information, and quantifies the involvement of the retina and optic nerve. Neurologic deficit in this disease is a result of disease relapse, and it is therefore crucial to establish the diagnosis early and initiate an appropriate treatment that can change the unfavorable prognosis. The main therapeutic strategies include aggressive treatment of relapses (with high‑dose corticosteroids or plasmapheresis), followed by chronic therapy (first choice drugs include azathioprine and rituximab).
