Karcinom jícnu je onemocnění s 90% mortalitou, ale část pacientů s lokálně pokročilým onemocněním může dlouhodobě přežívat díky multidisciplinární léčbě. Dokladem je tato kazuistika 40letého muže s dlaždicovým karcinomem středního jícnu prorůstajícím do dýchacích cest ve stadiu T4N1M0. Prvním léčebným zákrokem bylo zavedení Dumon Y-stentu do dýchacích cest jako urgentní zábrana sufokace. V dalším postupu byla použita konkomitantní chemoradioterapie, s dávkou záření 66Gy/33 frakcí/47 dní normofrakcionací. Chemoterapie v kombinaci cisplatina a fluorouracil byla podávána v celkové délce 6 cyklů, s ozářením během 2. a 3. cyklu. Na CT po léčbě byla zjištěna částečná regrese nádoru, endoskopické biopsie z jícnu i průdušek byly negativní. V prvním roce po chemoradioterapii bylo nutno řešit těsnou stenózu jícnu s příznaky až úplné afagie, což se podařilo zvládnout dilatacemi. V dalším období stav nemocného komplikovaly endobronchiální granulace řešené lokálním ošetřením laserem a jejich mechanickým snesením. Opakované biopsie ukazovaly na lokální progresi tumoru endobronchiálně, takže byla indikována paliativní chemoterapie docetaxelem, která byla podávána po dobu 1 roku. Během tohoto období opakované endobronchiální biopsie neprokázaly karcinom, CT hrudníku se normalizovalo, stav pacienta se zlepšil. Posléze byl odstraněn Y-stent 25 měsíců po zavedení a ukončena chemoterapie. Při kontrole 3 roky po diagnóze byl pacient bez známek nádoru., Esophageal cancer is characterized by 90 % mortality. Nevertheless, the part of the patients suffering from locally advanced disease can survive for a long time thanks to the multidisciplinar treatment. This fact can be documenetd by this case report of 40 year old man with squamous cell carcinoma of medial oesophagus intergrowing to respiratory tract, stage T4N1M0. At the start of the treatment Dumon Y stent was introduced into airways to avoid suffocation. Then, the concurrent chemoradiotherapy was used, with the radiation dose of 66Gy/33 fractions/47 days. Chemotherapy using combinationof cisplatin and fluorouracil was administered in the total length of 6 cycles, with the radiation applied during the second and third cycle. After the treatment, CT revealed the partial regression of the tumor, while endoscopic biopsies from oesophagus and bronchial tubes being negative for malignancy. During the first year after the chemoradiotherapy the tight stenosis of the oesophagus with the total aphagia was to be solved using the repeated dilatations. During the following period, the illness course was complicated by endobronchial granulations, treated by the local laser treatment and mechanical ablation, as well. Repeated biopsies shown the local endobronchial progression of the tumor, so that the paliative chemotherapy with the docetaxel was applicated, lasting one-year. During this period, repeated endobronchial biopsies did not proved a malignancy, CT finding was normalized and the patient´s status was improved. Finally, the Y stent was removed 25 months after insertion, and the chemotherapy was ended. Dideoxynucleotide DNA sequencing is one of the principal procedures in molecular biology. Loss of an initial part of nucleotides behind the 3' end of the sequencing primer limits the readability of sequenced amplicons. We present a method which extends the readability by using sequencing primers modified by polyadenylated tails attached to their 5' ends. Performing a polymerase chain reaction, we amplified eight amplicons of six human genes (AMELX, APOE, HFE, MBL2, SERPINA1 and TGFB1) ranging from 106 bp to 680 bp. Polyadenylation of the sequencing primers minimized the loss of bases in all amplicons. Complete sequences of shorter products (AMELX 106 bp, SERPINA1 121 bp, HFE 208 bp, APOE 244 bp, MBL2 317 bp) were obtained. In addition, in the case of TGFB1 products (366 bp, 432 bp, and 680 bp, respectively), the lengths of sequencing readings were significantly longer if adenylated primers were used. Cíl: Cílem studie bylo vyhledat poznatky týkající se NANDA International ošetřovatelské diagnózy Porucha polykání (00103) se zaměřením na výskyt této ošetřovatelské diagnózy u různých skupin pacientů a její stanovení prostřednictvím znaků uvedených v klasifikačním systému dle NANDA International. Metodika: Senzitivní vyhledávací strategií byly vyhledány publikované informace k ošetřovatelské diagnóze dle NANDA International Porucha polykání (00103), k jejímu diagnostikování a k výskytu této diagnózy u pacientů. K vyhledání relevantních dat byly využity elektronické databáze: Bibliographia Medica Čechoslovaca, MEDLINE, Springer, ProQuest a vyhledávač Google. Výsledky: V odborných publikacích se autoři velmi často věnují problematice porušeného polykání a jejímu diagnostikování, nikoli však pomocí ošetřovatelské diagnostiky dle NANDA International. Ošetřovatelská diagnóza Porucha polykání (00103), diagnostikována prostřednictvím ošetřovatelské diagnostiky dle NANDA International, se vyskytovala ve velkém zastoupení u pacientů na jednotkách intenzivní péče, u pacientů s onkologickým onemocněním a u pacientů s onemocněním jícnu. Závěr: V ošetřovatelství je porucha polykání častým problémem, který lze diagnostikovat prostřednictvím klasifikačního systému NANDA International. Další výzkumy by mohly přispět ke zjištění, zda diagnostika poruchy polykání pomocí NANDA International je vhodná pro použití v klinické praxi., Aim: The aim of the study was to search for findings related to NANDA International nursing diagnosis Impaired Swallowing (00103), focusing on the incidence of this nursing diagnosis in different groups of patients and its establishment using the NANDA International classification system. Methods: Published information concerning NANDA International nursing diagnosis Impaired Swallowing (00103), its establishment and incidence in patients were found by a sensitive search strategy. The following electronic databases were searched for relevant information: Bibliographia Medica Čechoslovaca, MEDLINE, Springer and ProQuest, together with Google. Results: Although paper authors frequently mention impaired swallowing, it is rarely diagnosed using the NANDA International classification system. Nursing diagnosis Impaired Swallowing (00103) as diagnosed according to NANDA International was largely present in intensive care unit patients, oncology patients and those with oesophageal disease. Conclusion: Impaired swallowing is a frequent condition in nursing which can be diagnosed using the NANDA International classification system. Narůstající počet závislých osob seniorského věku si vyžaduje adekvátní reakci ze strany sociálně zdravotní péče o tyto osoby. Jde zejména o hledání optimálního nastavení spolupráce mezi formální a neformální péčí, a dále o rozvíjení vhodných forem péče - terénních, ambulantních i pobytových služeb a služeb zdravotních a sociálních tak, aby na sebe navazovaly a dokázaly pružně uspokojovat aktuální potřeby seniorů a jejich rodinných pečujících. Článek se zaměřil na situaci v oblasti sociálně-zdravotních služeb určených pro seniorskou populaci v hlavním městě Praha (HMP) ve srovnání s dalšími re - giony ČR a identifikoval nejvýznamnější neuralgické body současného systému dlouhodobé sociálně-zdravotní péče v HMP. Využívá kvantitativní i kvalitativní metody, na jejichž základě byla podrobně analyzována existující nabídka sociálně-zdravotních služeb v HMP a byla reflektována očima zúčastněných aktérů, tj. uživatelů služeb a jejich rodinných příslušníků, poskytovatelů a zadavatelů sociálních a sociálně zdravotních služeb. Výsledky kvantitativního i kvalitativního výzkumu ukazují na závažné deficity jak v nabídce těchto služeb, tak v jejich vzájemné propojenosti., The increasing number of dependent seniors requires an adequate response from long-term care system for these citizens. This mean a search for setting of cooperation between formal and informal care, and need for appropriate forms of long-term care (field care, outpatient care and residential services) There is an increasing need for health and social services to relate mutually and cooperate flexibly, so that they would appropriately meet the current needs of seniors and their family caregivers. The article is focused on long-term care for the senior population in the capital Prague in comparison with other Czech regions and identifies the most neuralgic points of long- term care in Prague. It uses both qualitative and quantitative methods to analyse in detail supply of long-term care services in Prague from the point of view of main stakeholders. The colorectal cancer ranks high among the malignant tumours in incidence and mortality and irinotecan is standardly used in palliative treatment of metastatic disease in every therapeutic line. Unfortunately, the treatment with irinotecan is often associated with severe toxicities, especially neutropenia and diarrhea. The majority of the toxic manifestation is caused by the insufficient deactivation (glucuronidation) of irinotecan active metabolite SN-38 by UGT1A enzyme. The elevated SN-38 plasma concentration is responsible for the hematological and gastrointestinal toxicity that can become life-threatening. The patients carrying the mutation of the gene encoding UGT1A enzyme lack the ability of bilirubin glucuronidation, and suffer from the inherited un-conjugated hyperbilirubinemia (Gilbert syndrome, Crigler-Najjar type 1 and 2 syndrome). The mutations in other enzyme systems also play role in the etiopathogenesis of the irinotecan toxicity: CYP3A (cytochrome P-450), ABC family of transmembrane transporters (adenosine-triphosphate binding cassette). Tento článek pojednává o problému cetérů nedaří zastavit. Přitom důsledky obezity jsou značné: zhoršování zdravotního stavu a ekonomické ztráty. V poslední 40 letech došlo ke znatelnému pokroku ve vývoji teoretických poznatků o obezitě. Významným posunem je to, že obezita začala být nahlížena jako komplexní a dynamický problém, který je výsledkem působení mnoha faktorů, z nichž mnohé byly a stále jsou opomíjeny (zejména socioenvironmentální faktory) a přetrvává zažitý biomedicínský přístup, který neřeší problém jako celek a nepřináší tak žádné zlepšení. Je tedy nutné, aby tvůrci politik začaly tyto nové poznatky reflektovat, aby došlo k revizi dosavadních postupů řešení problému obezity a aby byl problém řešen systémově, tj. cestou víceúrovňové a víceoborové spolupráce se zapojením širokého okruhu aktérů i ze strany soukromého a občanského sektoru., This article deals with a problem of worldwide obesity epidemic, whose growing trend over considerable endeavour of many participants goes badly to a stop. Withal consequences of obesity are considerable: worsening of health and economic waste. In last 40 years it has come to a considerable progress in the development of theoretical knowledge in obesity. There is a significant shift: the obesity has begun to be regarded as a complex dynamic problem, which results from the coincidence of many factors, from which many of them have been neglected (especially socio-environmental factors), and there still persists the biomedical approach, which does not solve the problem as a whole and therefore brings no improvement. Cíl studie: Cílem studie bylo prostřednictvím prvkové analýzy konkrementů močového traktu stanovit obsahy minoritních prvků v nich, prokázat souvislosti mezi obsahem minerálů a minoritních prvků, a tím posoudit, za jakých okolností lze použít močové konkrementy k biomonitoringu toxických/esenciálních prvků. Typ studie: observační Materiál a metody: 489 vzorků močových konkrementů z ČR bylo podrobeno mineralogické analýze infračervenou spektrometrií a dále prvkové analýze s využitím hmotnostní spektrometrie s indukčně vázaným plazmatem (ICP-MS). Obsah prvků byl korelován s obsahem minerálů a dále byla provedena statistická analýza obsahů prvků u jednotlivých skupin minerálů. Výsledky: Metoda ICP-MS je vhodná jak pro stanovení obsahu minoritních prvků, tak pro stanovení obsahu majoritních prvků, na jejichž základě je možno dopočítat zastoupení minerálů. Byly zjištěny asociace prvků s fosforečnanem vápenatým (Na, Zn, Sr, Ba), struvitem (K, Rb) i rozdíly v obsahu prvků v konkrementech z whewellitu a kyseliny močové. Závěr: Močové konkrementy lze použít k monitoringu toxických/esenciálních prvků v organismu, nicméně pro spolehlivé hodnocení vlivu různých klinických faktorů (věk, pohlaví, stravovací návyky apod.) na obsah minoritních prvků je prvořadé stanovení mineralogického složení konkrementu., Objective: Quantification of minor elements in urinary calculi by means of elemental analysis, demonstration of the relations between mineral and minor element contents and finally evaluation of applicability of urinary stones for biomonitoring of toxic/essential elements. Design: Observational Material and Methods: 489 samples of urinary calculi from the Czech Republic was analyzed for mineralogical composition using infrared spectrometry and further for content of elements using inductively coupled plasma – mass spectrometry (ICP-MS). Element content was statistically correlated to mineral content and differentiated according to mineral composition. Results: ICP-MS method is suitable for quantification of minor elements as well as for major elements, whose determination enables calculation of mineral content. Two groups of elements including (Na, Zn, Sr, Ba) and (K, Rb) associate with calcium phosphate and struvite minerals, respectively. Also different content of elements in whewellite and uric acid concrements was confirmed. Conclusion: Urinary stones can be used for monitoring of toxic/essential elements in the human body however mineralogical analysis is crucial for credible assessment of the effect of clinical factors (e.g. age, gender, eating habits, etc.) on minor element content., and Kuta J., Machát J., Benová D., Kořistková T.
In this paper we describe the influence of NU7026, a specific inhibitor of DNA -dependent protein kinase, phosphoinositide 3-kinase, and AT M-kinase on molecular and cellular mechanisms triggered by ionising irradiation in human T-lymphocyte leukaemic MOLT -4 cells. We studied the effect of this inhibitor (10 μM) combined with gammaradiation (1 Gy) leading to DNA damage response and induction of apoptosis. We used methods for apoptosis assessment (cell viability count and flow-cytometric analysis) and cell cycle analysis (DNA content measurement) and we detected expression and post-translational modifications (Western blotting) of proteins involved in DNA repair signalling pathways. Pre-treatment with NU7026 resulted into decreased activation of checkpoint kinase-2 (Thr68), p53 (Ser15 and Ser392), and histone H2A.X (Ser139) 2 hours after irradiation. Subsequently, combination of radiation and inhibitor led to decreased amount of cells in G2-phase arrest and into increased apoptosis after 72 hours. Our results indicate that in leukaemic cells the pre-incubation with inhibitor NU7026 followed by low doses of ionising radiation results in radio-sensitising of MOLT -4 cells via diminished DNA repair and delayed but pronounced apoptosis. We report the case of a 22-year old male, a self-confessed recreational drug user who developed cardiogenic shock because of severe destruction of the aortic valve by rapidly progressive aortic valve endocarditis. The disease progression was acute; in a matter of days, the clinical manifestations were life-threatening necessitating urgent aortic valve replacement surgery. Cultivation revealed Streptococcus viridans as the microbial agent. Subsequent recovery with antibiotic treatment was without complication. This case report shows that immediately performed transoesophageal echocardiography and early consultation with a cardiac surgeon has fundamental importance in diagnosis and management of acute infective endocarditis in haemodynamically instable patients., Malkia S. Práce se zabývá řízením péče o nemocné celiakií. Jedná se o modelový případ řízení péče o chronicky nemocné. Analytickým rámcem práce je víceúrovňový inovativní model péče o chronicky nemocné. Klíčová je interakce mezi zdravotnickým týmem, který reprezentuje systém zdravotní péče, mezi komunitními partnery a mezi pacienty a jejich rodinami. V případu celiakie je pak nutno zahrnout také roli zástupců trhu s bezlepkovými potravinami. Interakce těchto aktérů se odehrává v širším politickém kontextu, jehož stav tuto interakci zásadním způsobem ovlivňuje a odráží se tak v konečných výsledcích stavu pacientů a kvalitě jejich života. Pomocí nástroje hodnocení kvality života pod - míněné zdravím SF-36 bylo zjištěno, že kvalita života celiaků je nižší ve srovnání s evropským standardem zdravé populace a že příčiny tohoto problému je nutno hledat a systematicky odstraňovat na všech úrovních systému péče o chronicky nemocné. Zjištění tak potvrzují teoretický předpoklad, že péče o chronicky nemocné je stále značně podceňována a neodpovídá specifickým potřebám chronicky nemocných pacientů, což se odráží právě na celkové kvalitě života., This work deals with coeliac disease management. The analytical framework of this work is the multilevel Innovative Care for Chronic Conditions Framework. Crucial is the interaction between health care team that represents health care system, community partners and patients with their families. In the case of coeliac disease it is necessary to include also the role of representatives of market with gluten free products. The interaction of these participants takes place in wider political context, whose state influences this interaction fundamentally and reflects so in final results state of patients and quality of their live. Using the evaluation of the quality of life tool SF-36, it has been ascertained, that the quality of life of coealiac patients is lower in comparison with European standard of healthy population and that the causes of the problem have to be searched and systematically removed at all levels of the system of chronic health care.
