Pro indikaci spinocelulárních karcinomů hlavy a krku je v rámci cílené léčby zaregistrován pouze jediný přípravek a sice cetuximab, jehož nižší toxicita je nespornou výhodou oproti chemoterapii. Díky vysokému počtu dosažených kompletních remisí u lokoregionálně pokročilých onemocnění v našem souboru máme zkušenosti s toutu léčbou velmi dobré. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). In palatine cancer, the incidence rate is 0.34/100 000 inhabitants with a mortality rate of 0.15/100 000 inhabitants. The most prevalent clinical stage is also clinical stage IV (26.7%). Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Nádory tlustého střeva a konečníku patří v České republice k nejčastějším zhoubným nádorům jak v populaci mužů, tak v populaci žen. Více než 50 % nově diagnostikovaných kolorektálních nádorů je zjištěno v pokročilém stadiu vývoje a zhruba 20 % má v době stanovení diagnózy založeny vzdálené metastázy. Játra jsou nejčastější orgánovou lokalizací metastáz kolorektálních karcinomů. Kurativní resekce metastáz je primárně možná pouze u malé části pacientů. Proto systémová léčba, chemoterapie a v posledních letech biologická léčba hrají významnou úlohu v léčbě metastazujícího kolorektálního karcinomu. Cetuximab (Erbitux) je monoklonální protilátka specificky namířená proti EGFR receptoru nádorových buněk, včetně buněk kolorektálního karcinomu. Inhibicí aktivity EGFR receptoru a jeho signální kaskády může Erbitux zastavit další růst nádorových ložisek a navodit jejich regresi. Přestože je cetuximab účinný i jako monoterapie, jeho účinek se zvyšuje v kombinaci s chemoterapií, opírající se jak o irinotekan, tak o oxaliplatinu. Přidání cetuximabu k chemoterapii navíc pomáhá zvrátit refrakternost nádorových buněk vůči chemoterapii samotné. V této kazuistice popisujeme případ staršího pacienta mužského pohlaví, u kterého byl cetuximab nasazen v době, kdy předcházející chemoterapie (režimy FOLFOX4 i FOLFIRI) nedokázala zabránit progresi onemocnění. Léčba cetuximabem v kombinaci s režimem FOLFIRI dosáhla významné regrese metastatického postižení a při pokračující udržovací léčbě cetuximabem samotným zůstává pacient v klinické remisi, která již trvá déle než 15 měsíců. Pacient dlouhodobou léčbu cetuximabem vcelku dobře toleruje, hlavním nežádoucím účinkem léčby je kožní akneiformní reakce, která však nebrání v pokračování terapie., In the Czech Republic, colorectal cancer is one of the most common malignancies both among men and women. More than 50 % of newly diagnosed colorectal cancers are found in an advanced stage and about 20 % have distant metastases at diagnosis. The liver is the most common site of metastasis from colorectal cancer. Curative resection of metastases is primarily feasible only in a small proportion of patients. Thus, systemic therapy, chemotherapy and, more recently, biological therapy play a significant role in treating metastatic colorectal cancer. Cetuximab (Erbitux) is a monoclonal antibody targeted specifically at the EGFR receptor of tumour cells, including those of colorectal cancer. By inhibiting the activity of the EGFR receptor and its signal cascade, Erbitux can stop further growth of tumour foci and induce their regression. Although cetuximab is also effective as monotherapy, its effect increases in combination with chemotherapy involving both irinotecan and oxaliplatin. Moreover, adding cetuximab to chemotherapy helps to reverse the refractoriness of tumour cells to chemotherapy alone. The present case report describes an elderly male patient who was started on cetuximab after previous chemotherapy regimens (FOLFOX4 and FOLFIRI) had failed to prevent the progression of disease. Treatment with cetuximab in combination with the FOLFIRI regimen resulted in a significant regression of metastatic disease and, while being continued on maintenance therapy with cetuximab alone, the patient has remained in clinical remission for more than 15 months now. In order to study cardiac pathology in different subtypes of ischemic stroke we examined 165 stroke patients. The 1st group included 90 (54.5%) patients with hemodynamic stroke, the 2nd one - 75 (45.5%) patients with cardioembolic stroke. Control group consisted of 45 individuals without cerebrovascular diseases. Cardiac pathologies with the prevalence of various types of ischemic heart disease were observed in all groups. The parameters of left ventricular stroke volume and ejection fraction were lower in patients with hemodynamic stroke than in other groups, but were within normal limits. Potential sources of cardiogenic embolism were found in all groups: in the 1st group at 74.4%, in the 2nd one at 100%. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. Cíl: Cílem studie bylo zjistit, jak rodinní pečovatelé pečující o blízkou osobu v České republice hodnotí definující charakteristiky ošetřovatelské diagnózy Přetížení pečovatele. Metodika: Na klinickou validizaci ošetřovatelské diagnózy Přetížení pečovatele byl použit „Na pacienta zaměřený Fehringův model klinické diagnostické validity“. Výzkum u rodinných pečovatelů probíhal v měsících srpen a září 2010 v Agentuře domácí péče Victoria na území města Šumperk. Do výzkumného souboru bylo vybráno 50 rodinných pečovatelů, kteří pečovali o své blízké v domácím prostředí déle než 2 roky. Souhlas se zařazením do výzkumu vyjádřilo 23 rodinných pečovatelů, co je 46 %. Výsledky: Rodinní příslušníci za hlavní definující charakteristiky Přetížení pečovatele označili celkem 8 charakteristik: obavy z možné institucionalizace péče příjemce; obavy, jak vyřešit péči o člena rodiny, když nebudu schopen tuto péči vykonávat; obavy o zdraví příjemce péče v budoucnosti; obavy související se schopností pečovatele poskytovat péči v budoucnosti; konflikt v rodině; nejistota ze změny vztahu s příjemcem péče; smutek ze změny vztahu s příjemcem péče; starost o členy rodiny. Za vedlejší bylo označeno 10 definujících charakteristik: obtíže v dokončování požadovaných úkolů, obtíže se sledováním příjemce péče v průběhu nemoci, obtíže ve vykonávání požadovaných úkolů, vysoký krevní tlak, kardiovaskulární onemocnění, somatizace, frustrace, depresivní nálada, porušený spánek, stres. Závěr: Pro skupinu vybraných pečovatelů nebyly všechny definující charakteristiky uvedeny v klasifikačním systému NANDA International významné., Aim: The aim of the study was to find out how the family caregivers caring for their close relatives in the Czech Republic evaluate defining characteristics of the nursing diagnosis Caregiver Role Strain. Methods: Fehring´s patient – focused clinical diagnostic validity model was used to clinical validation of the nursing diagnosis Caregiver Role Strain. The research was ongoing during the months August and September 2010 at Home Care Agency Victoria in region of city Šumperk. Fifty family caregivers, who take care for their family members more than 2 years, were chosen to the research sample. Twenty three caregivers (46 %) agreed with their including to the research. Results: Caregivers classified in total the following eight defining characteristics as major ones: apprehension about possible institutionalization of care receiver, apprehension about care receiver´s care if caregiver unable to provide care, apprehension about the future regarding care receiver´s health, apprehension about the future regarding caregiver´s ability to provide care, family conflict, uncertainty regarding changes relationship with care receiver, grief regarding changed relationship with care receiver, concerns about family members. Caregivers classified ten defining characteristics as minor ones: difficulty completing required tasks, difficulty watching care receiver go through the illness, difficulty performing required tasks, hypertension, cardiovascular disease, somatization, frustration, feeling depressed, disturbed sleep, stress. U pacienta s diabetes mellitus 1. typu je výrazný, opakující se nesoulad mezi vyšší glykémii v plazmě nalačno a glykovaným hemoglobinem. Příčinou snížených hodnot glykovaného hemoglobinu je zkrácená životnost erytrocytů způsobená hemolýzou po podání léku dapsonu. Vhodnější metoda ke sledování dlouhodobé kompenzace diabetes mellitus je v tomto případě stanovení fruktosaminu, jelikož koncentraci fruktosaminu neovlivňuje hemolýza., There is significant recurrent discrepance between elevated fasting plasma glukose and glycohemoglobin in a patient with diabetes mellitus type 1. The fall in glycohemoglobin results is due to short life span of erythrocytes as a product of dapson-induced hemolysis.
