Nádory tlustého střeva a konečníku patří v České republice k nejčastějším zhoubným nádorům jak v populaci mužů, tak v populaci žen. Více než 50 % nově diagnostikovaných kolorektálních nádorů je zjištěno v pokročilém stadiu vývoje a zhruba 20 % má v době stanovení diagnózy založeny vzdálené metastázy. Játra jsou nejčastější orgánovou lokalizací metastáz kolorektálních karcinomů. Kurativní resekce metastáz je primárně možná pouze u malé části pacientů. Proto systémová léčba, chemoterapie a v posledních letech biologická léčba hrají významnou úlohu v léčbě metastazujícího kolorektálního karcinomu. Cetuximab (Erbitux) je monoklonální protilátka specificky namířená proti EGFR receptoru nádorových buněk, včetně buněk kolorektálního karcinomu. Inhibicí aktivity EGFR receptoru a jeho signální kaskády může Erbitux zastavit další růst nádorových ložisek a navodit jejich regresi. Přestože je cetuximab účinný i jako monoterapie, jeho účinek se zvyšuje v kombinaci s chemoterapií, opírající se jak o irinotekan, tak o oxaliplatinu. Přidání cetuximabu k chemoterapii navíc pomáhá zvrátit refrakternost nádorových buněk vůči chemoterapii samotné. V této kazuistice popisujeme případ staršího pacienta mužského pohlaví, u kterého byl cetuximab nasazen v době, kdy předcházející chemoterapie (režimy FOLFOX4 i FOLFIRI) nedokázala zabránit progresi onemocnění. Léčba cetuximabem v kombinaci s režimem FOLFIRI dosáhla významné regrese metastatického postižení a při pokračující udržovací léčbě cetuximabem samotným zůstává pacient v klinické remisi, která již trvá déle než 15 měsíců. Pacient dlouhodobou léčbu cetuximabem vcelku dobře toleruje, hlavním nežádoucím účinkem léčby je kožní akneiformní reakce, která však nebrání v pokračování terapie., In the Czech Republic, colorectal cancer is one of the most common malignancies both among men and women. More than 50 % of newly diagnosed colorectal cancers are found in an advanced stage and about 20 % have distant metastases at diagnosis. The liver is the most common site of metastasis from colorectal cancer. Curative resection of metastases is primarily feasible only in a small proportion of patients. Thus, systemic therapy, chemotherapy and, more recently, biological therapy play a significant role in treating metastatic colorectal cancer. Cetuximab (Erbitux) is a monoclonal antibody targeted specifically at the EGFR receptor of tumour cells, including those of colorectal cancer. By inhibiting the activity of the EGFR receptor and its signal cascade, Erbitux can stop further growth of tumour foci and induce their regression. Although cetuximab is also effective as monotherapy, its effect increases in combination with chemotherapy involving both irinotecan and oxaliplatin. Moreover, adding cetuximab to chemotherapy helps to reverse the refractoriness of tumour cells to chemotherapy alone. The present case report describes an elderly male patient who was started on cetuximab after previous chemotherapy regimens (FOLFOX4 and FOLFIRI) had failed to prevent the progression of disease. Treatment with cetuximab in combination with the FOLFIRI regimen resulted in a significant regression of metastatic disease and, while being continued on maintenance therapy with cetuximab alone, the patient has remained in clinical remission for more than 15 months now. Autor v článku popisuje šest případů emfyzematózní pyelonefritidy. Průměrný věk nemocných byl 63,5 roku, onemocnění se vyskytovalo častěji u mužů a ani v jednom případě nebyl příčinou konkrement. Dvě třetiny nemocných byli diabetici a nejčastěji byla prokázána jako etiologické agens Escherichia coli a v jednom případě Klebsiella pneumonie. Autor uvádí způsoby léčby jednotlivých případů onemocnění. Pouze v jednom případě vedla emfyzematózní pyelonefritida ke smrti pacienta., The author of this communication describes six cases of emphysematous pyelonephritis. The mean age of patients was 63.5, with the disease seen most frequently in men. None of the cases were associated with stone disease. Two thirds of patients had diabetes mellitus. Escherichia coli was confi rmed in the urine most frequently, and in one case, Klebsiella pneumoniae was identifi ed. The author presents the methods of treatment used in each case. Cíl: Prezentovat neobvyklý případ 49leté ženy s pozitivní anamnézou metastazujícího maligního melanomu, u které se při 18F-FDG PET/CT vyšetření po léčbě zobrazil enchondrom v hlavičce fibuly napodobující solitární metastázu maligního melanomu. Metodika: Pacientka v rámci kontroly efektu léčby absolvovala celotělové PET/ CT zobrazení, a to 60 minut po intravenózní aplikaci radiofarmaka 18F-FDG o aktivitě 386 MBq. Akviziční čas PET skenu byl 2,5 minuty na 15 cm projekci, celkem 14 projekcí. Kontrastní (i.v. + per os kontrast) CT sken byl proveden ve venózní fázi. Výsledky: 18F-FDG PET/CT prokázalo osteolytické ložisko s jemným sklerotickým lemem v hlavičce levé fibuly s zvýšenou akumulaci 18F-FDG. Vzhledem k pozitivní anamnéze metastazujícího maligního melanomu bylo vysloveno podezření na solitární metastázu a doporučena biopsie. Následně pacientka podstoupila resekci hlavičky fibuly a histologické vyšetření zjistilo přítomnost enchondromu. Závěr: Enchondrom může vykazovat zvýšenou ložiskovou akumulací 18F-FDG v kosti, a tudíž má schopnost napodobovat solitární metastázu maligního melanomu při PET/CT zobrazení a tím způsobit potenciální obtíže při hodnocení nálezu., Aim: To present a rare case report of the fibular head enchondroma mimics a solitary malignant melanoma metastasis on 18F-FDG PET/CT imaging. Method: The 49-year-old woman with positive history of the metastatic malignant melanoma after treatment underwent 18F-FDG PET/CT. The whole body PET/CT scan was obtained 60 minutes after intravenous injection of 18F-FDG (386 MBq). The PET scan acquisition time was 2.5 minutes per bed position for whole body imaging and fourteen bed positions were necessary. Contrast-enhanced CT (venous phase) provided both full CT evaluation (including intravenous and oral contrast) and PET attenuation correction. Results: The examination revealed 18F-FDG avid bone lytic lesion with the mild sclerotic border in the left fibular head. The PET/CT finding was suspected for solitary melanoma metastasis. The patient underwent surgery and enchondroma was proven by histopathology. Conclusion: Benign enchondroma shows increased 18F-FDG uptake in bone lesion and can mimic solitary malignant melanoma metastasis on PET/CT imaging and can cause potential pitfall in the PET/ CT evaluation., Jiří Doležal, Martin Slanina, and Literatura
Autoři prezentují formou kazuistického sdělení případ 57leté ženy s velmi vzácným extragenitálně lokalizovaným maligním Mülleriánským karcinosarkomem retroperitonea infiltrujícím dolní dutou žílu. Je popsána resekce tumoru s částí stěny duté žíly i resekce metastázy v oblasti pánve. Dále autoři diskutují možnosti diagnostiky rozsevu tohoto typu nádoru a doporučenou adjuvantní onkologickou terapii., The authors present the case of a 57-year-old woman with a very rare extragenital malignant retroperitoneal Müllerian carcinosarcoma invading the inferior vena cava. Tumor resection with partial resection of the vena cava wall and resection of metastases in the pelvic area is described. BACKGROUND: Occurrence of gastric metastasis as the first symptom of breast carcinoma with a long period of latency before presentation of the primary breast carcinoma is rare. CASE REPORT: A patient with gastric metastasis as the first symptom of lobular breast carcinoma, treated by neoadjuvant preoperative chemoradiotherapy and total gastrectomy, with complete local control. Fourteen months after presentation of the gastric metastasis a primary lobular breast carcinoma was discovered, treated by radiotherapy, chemotherapy and hormonal treatment with complete local response. Twenty-three months after diagnosis of breast cancer multiple colorectal metastases from the breast cancer occurred, which were treated by chemotherapy and hormonal treatment. Eighty-six months after diagnosis of gastric metastasis the patient died due to progression of cancer. CONCLUSIONS: Metastases to gastrointestinal or gynaecological tracts are more likely in invasive lobular carcinoma than invasive ductal cancer. The pathologist should determine whether or not they check estrogen and progesterone receptor status not simply by signet ring cell morphology but also by consideration of clinic-pathological correlation of the patient, such as the presence of a past history of breast cancer, or the colorectal localization of poorly differentiated carcinoma, which may occur less frequently than in the stomach. and D. Pro hemodialyzované pacienty, u kterých byl diagnostikován diferencovaný karcinom štítné žlázy, může mít zásadní význam léčba 131I. Diagnóza nádorového onemocnění tyto pacienty vyřazuje z registru čekatelů pro transplantaci ledviny do doby, než bude prokázána 3-5letá remise. Jelikož je 131I vylučován z těla převážně ledvinami, je efektivní poločas 131I u pacientů s renálním selháním prodloužen. To je důvod, proč by měla být redukována podávaná léčebná aktivita, aby došlo ke snížení radiační zátěže především pro krvetvornou kostní dřeň. Prezentujeme kazuistiku 48letého muže v terminálním renálním selhání, léčeného 131I pro papilární karcinom štítné žlázy. Terapeutické aktivity jsme snížili o 30 %. Hemodialýza byla prováděna po podání léčebné aktivity 131I na lůžkové části kliniky nukleární medicíny. 131I je dialyzovatelný a po každé dialýze klesla zbytková aktivita o 33-67 %. Radiační zátěž personálu hemodialyzačního centra byla minimální. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis.
