Cíl: Průzkumné šetření zjišťovalo schopnosti spolupráce nemocných při léčbě Warfarinem a dodrţování léčebného reţimu v průběhu dlouhodobé antikoagulační terapie Warfarinem. Konkrétně byla mapována a vyhodnocena úroveň edukace sestrou, informovanost a adherence k léčbě u pacientů léčených Warfarinem. Metodika: Průzkumný soubor tvořilo 108 pacientů evidovaných v ambulantním léčení interní a kardiologické ambulance při interním oddělení Nemocnice ve Frýdku-Místku. Průzkumné šetření probíhalo v prosinci roku 2010 a lednu roku 2011. Pro sběr dat byla vyuţita metoda dotazníkového šetření. Výsledky: Výsledky průzkumného šetření ukazují, ţe naprostá většina pacientů léčených Warfarinem se aktivně podílí na léčebném procesu. Nemocným se od zdravotníků dostává poučení o léčbě, sestry se na edukaci podílejí v 75 %. Většina dotázaných dochází na pravidelné kontroly a zná zpaměti uţívanou dávku léku. Čtvrtina nemocných však není schopna uvést krvácení jako hlavní riziko léčby. Převáţná většina respondentů si je vědoma skutečnosti, ţe účinek léčby můţe být ovlivněn sloţením přijímané potravy, méně neţ polovina respondentů zná interakci Warfarinu s jinými léky či doplňky stravy. Nemocní mají na zřeteli, ţe je nutno informovat kaţdého lékaře o této léčbě, jen polovina respondentů však informuje i lékárníka. Závěr: Adherence k léčbě ze strany pacienta je základem úspěšné a bezpečné terapie Warfarinem. Cílem všech zdravotníků by měla být snaha o kvalitní a efektivní komunikaci s nemocnými, snaha o dosaţení co nejvyšší úrovně spolupráce, ochoty a schopnosti pacientů řídit se pokyny zdravotníků. Tím se výrazně zvýší úspěšnost léčby a eliminuje se riziko moţných komplikací léčby., Aim: Exploratory investigation ascertained capabilities of cooperation of patients when treated with warfarin and adherence to treatment regimen during long term anticoagulant therapy by warfarin. Specifically was mapped and assessed the level of education by nurse, foreknowledge and adherence to treatment in patients treated with warfarin. Methods: The exploratory set consisted of 108 patients, who were registered in ambulant treatment of internal and cardiac ambulance at internal department of Hospital in Frydek-Mistek. Exploratory investigation took place in December 2010 and January 2011. For data collection was used method of questionnaire survey. Results: The results of questionnaire survey show, that absolute majority of patients treated with warfarin is actively participating in the treatment process. The patients receive from the health care providers education about the treatment, nurses are participating in education in 75%. Majority of respondents have regular check ups and knows by heart the dose of the drug. A quarter of patients is not able to state bleeding as the main risk of treatment. The vast majority of respondents are aware of the fact, that the treatment effect may be influenced by the composition of the diet, less than half of the respondents know the warfarin interactions with other medication or supplements. Patients have in mind that every doctor should be informed about this treatment, only half of the respondents, however, informs as well the pharmacist. Conclusion: Adherence to treatment by the patient is the foundation of a successful and safe warfarin therapy. The goal of all health professionals should be to aim for quality and effective communication with patients, effort to achieve the highest level of cooperation, willingness and ability of patients to follow medical instructions. Kvalita ţivota je ovlivněna zdravotním stavem, který zahrnuje fyzické, psychické a sociální charakteristiky. Největším zdravotním, ale i společenským problémem současné epochy nejsou nemoci akutní, ale nemoci dlouhodobé, chronické a trvalé. Výzkum se zaměřuje na oblast sexuality u pacientů s hypertenzí, s důrazem na holistické pojetí lidské osobnosti, hlubší vnímání a proţívání sexuálního ţivota, sexuálních potřeb pacientů. Cíl: Cílem kvalitativního šetření bylo zjistit, zda léčba hypertenze ovlivňuje sexuální potřeby pacientů. Metodika: Výzkumný soubor tvořilo 20 respondentů, výběr zkoumaného vzorku byl záměrný, byli to pacienti, kteří se léčili pro hypertenzi minimálně 1 rok ve Fakultní nemocnici Ostrava. Výzkum byl proveden pomocí kvalitativní metody, sběr dat byl proveden rozhovorem, který byl nahráván na diktafon v příjemném, nerušeném prostředí. Ve výzkumu byla pouţita metoda barvení textu s definovanými 4 kategoriemi a podkategoriemi. Výsledky: Z analýzy rozhovorů vyplynulo, ţe u některých pacientů antihypertenzní terapie negativně ovlivnila jejich sexuální potřeby. Byl zjištěn výskyt sexuálních dysfunkcí, u ţen se jednalo o sníţený sexuální apetit, sníţenou lubrikační schopnost, delší interval k vyvrcholení a u muţe byl objeven problém s erekcí. Závěr: V rámci subjektivní spokojenosti s vlastní sexualitou u pacientů, kteří se léčí s arteriální hypertenzí je důleţitá znalost vedlejších účinků a následná edukace s vyuţitím výsledků vědeckých studií domácích i zahraničních odborníků., The quality of life is influenced by health condition, which include physical, psychological and social features. The biggest, not only medical but also social problem nowadays are not acute diseases but long-term diseases, chronical and permanent. The research is concentrated on the sphere of sexuality among patients with hypertension, with stress on holistic concept of human being, deeper perception and experiencing of sexual life and sexual needs of patients. Aim: The aim of qualitative research was to find out how the treatment of hypertension influences sexual needs of patients. Methods: Researched sample was formed by 20 respondents. The choice of researched sample was intentional. They were patients who were treated for hypertension minimally one year in University Hospital Ostrava. The research was done by qualitative method, collection of datas was done by interview, which was recorded on voice recorder in a pleasant, by nothing disturbed place. In the research was used method of coloring the text with 4 defined categories and subcathegories. Results: From the analysis of interviews was clear that at some patients antihypertension therapy influenced their sexual needs in a negative way. There was found out the occurrence of sexual dysfunctions. Among women it was lower sexual appetite, lower lubricative ability, longer time for climax and among men was found the problem with erection. Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. Spánek se významně podílí na celkovém zdraví a kvalitě života člověka, je důležitý nejen pro duševní a fyzickou regeneraci, ale také pro normální funkci metabolických pochodů, endokrinního a imunitního systému. V moderní společnosti dochází k postupnému zkracování délky spánku v důsledku celé řady faktorů. Epidemiologické studie v posledním desetiletí potvrdily, že spánek kratší než 7 hodin představuje rizikový faktor pro rozvoj obezity, diabetu mellitu 2. typu, hypertenze, kardiovaskulárních onemocnění, depresí a zvyšuje riziko mortality. Cílem této práce bylo zhodnotit délku spánku a posoudit vztah mezi délkou spánku a BMI u dospělé populace (n=478, 18–65 let). Metoda sběru dat zahrnovala 7denní záznam o délce spánku za den a antropometrická měření. Z výsledků vyplynulo, že průměrná délka spánku v souboru činila 7,45 (±0,91) hodin za den. 28 % dospělých osob přesto spalo méně než doporučených 7 hodin denně. Statisticky nejméně spali jedinci ve věku 40–49 let (průměr 7,2±0,80 hodin). Riziko obezity definované jako BMI ≥30 kg/m2 v souboru stoupalo se zkracující se délkou spánku: pro kategorie spánku [<6,3 h], [≥6,3 až <8,3 h] a [≥8,3 h] bylo riziko obezity na základě relativních četností vyjádřeno poměrem 2 : 1,5 : 1,2. Jedinci spící průměrně <6,3 hodiny vykazovali 1,3krát vyšší riziko obezity než ostatní jedinci spící déle i při zohlednění faktoru věku, tento závěr však není statisticky významný (OR=1,256; 95% IS: 0,631; 2,422). Z hlediska prevence obezity se dostatečný a kvalitní spánek jeví jako významný ochranný faktor, který by zejména v současné době rychlého životního tempa a při stoupajícím počtu obézních jedinců neměl být, vedle zdravé stravy a dostatečné pohybové aktivity, opomíjen., Sleep plays a significant role in overall health and in the quality of human life. It is not only important for mental and physical regeneration, but also for the normal functioning of metabolic processes, for the endocrine and immune systems. In modern society there is a gradual shortening of the length of sleep due to a number of factors. In the last decade a number of epidemiological studies suggest that sleeping less than 7 hours a day increases the risk of developing obesity, type 2 diabetes mellitus, hypertension, cardiovascular diseases, and depression and is associated with premature mortality. The aim of this study was to evaluate sleep duration and to assess the relationship between sleep duration and BMI in the adult population (n=478, 18 to 65 years). The method of data collection consisted of a 7-day record of the length of the sleep per day and anthropometric measurements. The results showed that the average sleep duration was 7.45 (±0.91) hours per day. However, 28% of adults slept less than a recommended 7 hours a day. The individuals aged 40–49 years slept statistically the shortest time (mean 7.2±0.80 hours). The risk of obesity increased indirectly with duration of sleep in this population, for sleep categories [<6.3 h], [≥6.3 to <8.3 h] and [≥8.3 h] the risk of obesity defined as BMI ≥30 kg/m2 was 2 : 1.5 : 1.2 based on the relative frequencies of obese and non-obese participants. Cíl: Prezentovat zajímavý případ 44leté ženy s metastázou karcinomu prsu v axilární lymfatické uzlině vpravo s negativní mamografií, biopsií a ultrasonografií prsů, kdy 18F-FDG PET/CT odhalil multifokální karcinom prsu vpravo. Materiál a metoda: Pacientka vyhledala lékaře pro hmatnou rezistenci v pravé axile. Následovala exstirpace paketu lymfatických uzlin z pravé axily, histologické vyšetření prokázalo metastázu karcinomu prsu. Mamografie a ultrasonografie obou prsů byla s negativním výsledkem. Vzhledem k pozitivitě metastáz v lymfatických uzlinách v pravé axile následovala ještě biopsie z pravého prsu, také s negativním nálezem. V rámci pátrání po primárním ložisku karcinomu prsu pacientka absolvovala celotělové PET/CT zobrazení v rozsahu od baze lební do proximální třetiny stehen, a to 60 minut po intravenózní aplikaci radiofarmaka 18F-FDG o aktivitě 377 MBq (5 MBq/kg). Akviziční čas PET skenu byl 2,5 minuty na 15 cm projekci, celkem 7 projekcí. Kontrastní (i.v. + per os kontrast) CT sken byl proveden ve venózní fázi. Výsledky: PET/CT odhalil multiložiskový viabilní tumor v laterální části pravého prsu a viabilní metastázy v lymfatických uzlinách v pravé axile a infraklavikulárně vpravo. Závěr: 18F-FDG PET/CT správně detekoval multifokální karcinom prsu u pacientky s pozitivní metastázou tohoto karcinomu v lymfatické uzlině a neznámým primárním ložiskem, čímž zkrátil dobu diagnostického procesu a umožnil zvolit správnou léčbu., Aim: To present a rare case report of the multifocal breast cancer finding on 18F-FDG PET/CT in a 44-year-old woman with the positive right axillary breast metastases and negative mammography, ultrasound and right breast biopsy. Material and Method: The patient underwent 18F-FDG PET/CT for the positive right axillary lymph node breast cancer metastases and negative mammography, ultrasound a right breast biopsy. The whole body PET/CT was obtained 60 minutes after intravenous injection of 18F-FDG (377 MBq – 5 MBq/kg). The PET scan acquisition time was 2.5 minutes per bed position for whole body imaging from base of the skull to mid femur and seven bed positions were necessary. Contrast-enhanced CT (venous phase) provided both full CT evaluation (including intravenous and oral contrast) and PET attenuation correction. Results: The examination revealed PET/CT positive multifocal tumour in the right breast and regional metastases in the right axilla and infraclavicular region without distant metastases. The patient underwent the subsequent neoadjuvant chemotherapy, surgery and adjuvant radiotherapy and the complete remission was induced.
