Roztroušená skleróza je chronické onemocnění centrálního nervového systému neznámé etiologie s projevy autoimunitního zánětu a neurodegenerace. Onemocnění je heterogenní s nepředvídatelnou prognózou. Průběh choroby lze monitorovat klinickými parametry a sledováním vývoje patologických změn na magnetické rezonanci. I když máme znalosti o efektu nově zaváděných léků na základě klinických studií, není možné předvídat jejich účinnost u konkrétního pacienta. Proto se v posledních letech prosazuje snaha najít takové laboratorní markery, které by co možná nejspolehlivěji odpověděly na otázky spojené se subklinickou aktivitou onemocnění, jeho progresí a usnadnily terapeutické rozhodnutí na základě personifikované medicíny., Multiple sclerosis is a chronic disease of the central nervous system of unknown etiology with manifestations of autoimmune inflammation and neurodegeneration. The disease is heterogeneous with an unpredictable outcome. The course of the disease can be monitored with clinical parameters as well as pathological changes on magnetic resonance imaging. Even though the effects of newly introduced drugs are known from clinical trials, it is not possible to predict their efficacy in a specific patient. Therefore, efforts have intensified over the recent years to identify laboratory markers that would as reliably as possible answer questions on subclinical disease activity, its progression and would facilitate therapeutic decisions based personalized medicine. Nová cílená léčiva nemají výraznější toxicitu než standardní cytostatika a jejich efektivita byla prokázána u řady jiných typů nádorů., This article is aimed to summarize new targeted therapy used for treatment of advanced cancer of stomach and gastroesophageal junction. Their efficacy is being tested in numerous clinical trials worldwide. Recent findings have suggested that this type of malignancy is considered as a group of heterogenous tumors that differ in the expression of cell-signalling molecules. Karcinóm prsníka je príkladom rýchleho rozvoja vedeckých poznatkov, začínajúc od molekulárnej diagnostiky a končiac cielenou liečbou na molekulárnej úrovni. Neoadjuvantná chemoterapia je indikovaná u veľmi heterogénnej skupiny nádorov. O type liečby rozhoduje nielen biologická variabilita (stav receptorov, stav HER-2), ale tiež individuálna variabilita (PS, vek, komorbidita, rozsah ochorenia). Primárna systémová liečba (neoadjuvantná, indukčná, iniciálna) je dnes štandardná pre lokálne pokročilé nádory a niektoré vybrané operabilné typy karcinómov prsníka. Pre neuspokojivú liečebnú odpoveď sa hľadajú nové režimy s iným konceptom dávkovej intenzity, denzity, sekvencie. V neoadjuvancii je možné využiť všetky necielené aj cielené liečebné metódy na podklade výsledkov mnohých klinických štúdií., Breast cancer is a model of quickly development scientific knowledges begining from molecular diagnostic and termination with targeted therapies on the molecular level. Neoadjuvant chemotherapy is indicated for very heterogeneous group of tumours. Treatment type is decided not only by biologic variability (receptor status, HER-2 status) but with individual variability (PS, comorbidity, extent of illness). Primary systemic treatment (neoadjuvant, induction, inicials) is standard treatment for locally advanced tumor and for some eligible type of surgery breast cancer, too. Výběr terapeutického postupu u konkrétního pacienta s metastatickým renálním karcinomem je obtížný a opírá se hlavně o charakteristiku nádoru, výkonnostní stav, komorbidity pacienta a předpokládanou toxicitu terapie. V poslední době bylo publikováno několik analýz, které identifikují prognostické faktory pro přežití nemocných léčených cílenými léky, jako jsou sunitinib, sorafenib, pazopanib, bevacizumab, temsirolimus a everolimus.
