Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. Úvod: Terapie ran kontrolovaným podtlakem (NPWT) je efektivní metodou lokální terapie širokého spektra nehojících se ran. V současnosti je využívána především u hospitalizovaných pacientů, pro systematické použití v ambulantním režimu nejsou v České republice dostupná klinická data. Metody: V prospektivní nerandomizované studii autor porovnává klinickou a nákladovou efektivitu léčby ulcerací na dolní končetině u diabetiků kontrolovaným podtlakem (NPWT) ambulantně versus za hospitalizace. Cílem sledování je stanovit účinnost NPWT v ambulantním režimu na základě hodnocení vývoje spodiny rány, kvantifikovat předpokládanou redukci celkových nákladů na terapii a porovnat efektivitu obou režimů navzájem. Do sledování bylo zařazeno 36 hospitalizovaných (NPWT systémy RENASYS GO nebo PICO) a 28 ambulantních pacientů (PICO). NPWT byla ukončena v momentě, kdy byla spodina rány zcela pokryta granulační tkání nebo při vzniku vážných komplikací léčby. Hodnotili jsme velikost ulcerace, charakter její spodiny, počet NPWT výměn, její celkovou délku, intenzitu bolesti, přítomnost komplikací a náklady na terapii. Výsledky: Sledování prokázalo porovnatelnou klinickou účinnost NPWT v ambulantním režimu (n=28) versus za hospitalizace (n=36, statisticky nesignifikantní rozdíly v hojení ulcerací verifikované na základě vývoje spodiny rány a redukce plochy ulcerací v čase), stejné riziko komplikací spojených s NPWT (statisticky nesignifikantní rozdíly ve funkčnosti obou technologií a ve výskytu komplikací vzniklých v průběhu sledování) i obdobné dlouhodobé výsledky (statisticky nesignifikantní rozdíly v počtu amputací). Celkové náklady na ambulantní terapii ran byly statisticky signifikantně nižší (16 700 Kč versus 36 000 Kč, p 0,001), stejně tak průměrné náklady na 1 den podtlakové terapie (817 Kč versus 3400 Kč, p 0,001). Závěr: Analýza klinických výsledků jednoznačně prokázala, že ambulantní podtlaková terapie ulcerací syndromu diabetické nohy (metoda PICO) má stejnou efektivitu jako podtlaková terapie za hospitalizace. V ambulantním provozu je NPWT levnější, převazy jsou vnímány méně bolestivě a NPWT je pacienty lépe snášena., Introduction: The negative pressure wound therapy (NPWT) is an effective local treatment method of many non-healing wounds. NPWT is routinely used for inpatient treatment in the Czech Republic; however, no clinical data is available for systematic outpatient treatment. Methods: The prospective non-randomized study was used to compare the clinical impact and cost-effectiveness of diabetic leg-ulcer and foot-ulcer negative pressure treatment in outpatient and inpatient settings. The aim of the study was to assess the effectiveness of outpatient NPWT on the basis of wound bed evaluation, to detect and quantify pre-expected reductions in the total costs of the therapy, and to compare the efficacy of both treatment options. Enrolled subjects included inpatients (n=36, NPWT systems RENASYS GO or PICO) and outpatients (n=28, PICO system). NPWT was concluded when the wound bed had been totally covered by granulation tissue, or when serious health problems occurred. We monitored the ulcer area, wound bed trait, number of NPWT dressing changes, length of NPWT, intensity of pain (VAS scale), presence of complications, and treatment costs. Results: We found comparable clinical efficacy of outpatient (n=28)/inpatient (n=36; statistically non-significant differences in healing of ulcers verified based on wound bed development and reduction of ulcer areas in time) NPWT, the same risk of complications associated with NPWT (statistically non-significant differences in the function of both technologies and in complications developed during follow-up) as well as similar long-term results (statistically non-significant differences in the count of amputations). The total costs for outpatient wound treatment were statistically significantly lower (EUR 600 versus EUR 1300, p=0.001), and so were the average one-day-NPWT costs (EUR 30 versus EUR 120, p=0.001). Conclusion: We demonstrated that outpatient diabetic-foot-ulcer NPWT provided the same clinical efficiency as inpatient NPWT, while outpatient NPWT was less expensive, less painful and better tolerated than inpatient NPWT., and J. Účel studie: Hlavním cílem studie bylo odhadnout dosažení patologické kompletní odpovědi nemocných trpících lokálně pokročilým adenokarcinomem rekta po předoperační (neoadjuvantní) chemoradioterapii (CHRT) s využitím individuálních farmakokinetických parametrů 5-fluorouracilu (5-FU). Sekundárním cílem bylo hodnocení bezpečnosti a snášenlivosti léčby. Tato otevřená prospektivní studie podporovaná grantem IGA NS 9693–04/2008 zahrnula 34 dospělých s lokálně pokročilým karcinomem konečníku ozařovaných do celkové dávky 50,4 Gy ve 28 frakcích po 1,8 Gy 10–15 MV paprsky v režimu 5 + 2 (5 dní radioterapie, 2 dny volno) s kontinuálně podávaným 5-FU v i. v. infuzi 200–1000 mg/m2 po dobu 4–5 týdnů kontinuálně 7denní infuzí. Chirurgická resekce následovala 4–6 týdnů po ukončení CHRT a po klinickém restagingu. Hodnocena byla klinická a patologická odpověď na chemoradioterapii pomocí zobrazovacího (MR) a histopatologického vyšetření, vyjádřená v % jako reziduální choroba. Výsledky a závěr: Výsledek dokládá vztah mezi velikostí kumulativní dávky 5-FU a kumulativní AUC 5-FU (r = 0,61, p < 0,001). Podobný vztah byl prokázán mezi kumulativní AUC 5-FU a metabolickým poměrem (poměr dosažených plazmatických koncentrací neaktivního metabolitu dihydrofluorouracilu (5-FUH2) ku koncentracím 5-FU, r = -0,80, p < 0,0001). Kumulativní AUC korelovala se stupněm odpovědi na léčbu (r = – 0,53, p < 0,005) a určovala také stupeň toxicity léčby. Pokud bychom chtěli dosáhnout kompletní patologickou odpověď, pak by denní dávka 5-FU měla být u středně rychlého metabolizéra >350 mg/m2 a kumulativní AUC1–39 dní > 50 mg/L*h. U žádného z nemocných nebyla nalezena mutace v genu pro dihydropyrimidindehydrogenázu (DPD) a multidrug resistance-1 protein (MDR-1), přesto byla nalezena vysoká interindividuální variabilita v dosažených plazmatických koncentracích 5-FU, a to i s ohledem na cirkadiální rytmus kinetiky léčiva., Background and Purpose: The main goal of the present study was to estimate the early patients´response following neoadjuvant chemoradiotherapy (CHRT) based on 5-fluorouracil (5-FU) with curative aim in relation to plasma concentrations and pharmacokinetic parameters of 5-FU. Secondary objectives included evaluation of the safety and tolerability of the regimen. Patients and Methods: This open prospective study enrolles 34 adult patients with locally advanced rectal cancer, who received 5-FU 200 -1000 mg/m2 administered as a continuous i. v. infusion over 4–5 week and radiotherapy delivered with 10–15 MV photon beams at 1.8 Gy/fraction up o 50.4 Gy in 28 daily fractions for 5 days a week. Surgical resection with curative aim followed 4–6 weeks after the completion of CHRT and clinical restaging. Pathologic response evaluation and the rate of tumor regression was evaluated using tumor downstaging by MR, histopathological staging, and expressed as residual disease (%). Results and Conclusion: The outcome evidenced the correlation between the cumulative 5-FU dose and cumulative AUC of 5-FU (r = 0.61; p < 0.001). The similar relationship was demonstrated between the cumulative AUC and metabolic ratio (the plasma concentration od inactive metabolite dihydrofluotouracile 5-FUH2 to 5-FU; r = -0.80; p < 0.0001). The cumulative AUC was correlated with tumor regression rate (r = -0.53; p < 0.005) and determined toxicity grade. To reach pCR, the daily dose of 5-FU in patient with average metabolic ratio of 5-FUH2/5-FU should be >350 mg/m2 and the cumulative AUC1–39days > 50 mg/L*h. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Hypotalamický hamartom může způsobovat rozvoj katastrofické a extrémně farmakorezistentní epilepsie s epileptickou encefalopatií. První epileptický záchvat je typicky gelastický, posléze následují další typy záchvatů. Diagnostika malého hypotalamického hamartomu pomocí magnetické rezonance mozku může být obtížná. Neurochirurgická léčba včetně radiochirurgie je vzhledem k uložení komplikovaná s rizikem vedlejších účinků. Odstranění hypotalamického hamartomu však velmi účinně ovlivňuje epileptické záchvaty a brání rozvoji encefalopatie. Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis. Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %. Úvod: Kombinace monoklonální antiCD20 protilátky rituximabu s chemoterapií je vysoce účinná v léčbě chronické lymfocytární leukemie (CLL). Hlavním nežádoucím účinkem rituximabu jsou infuzní reakce, např. syndrom uvolnění cytokinů (cytokine release syndrome – CRS), který zpravidla doprovází první podání rituximabu. Cílem naší retrospektivní analýzy bylo zhodnotit četnost výskytu infuzních reakcí v rutinní klinické praxi, jejich vztah k parametrům nádorové nálože a zjistit možnou spojitost infuzních reakcí s účinností léčby. Dále byla posouzena bezpečnost rychlé infuze s rituximabem u nemocných s CLL. Charakteristika souboru: Hodnoceno bylo 108 nemocných s CLL léčených chemoimunoterapií s rituximabem od května roku 2005 do března roku 2011 v našem centru. V rámci 1. linie (n = 66, 71 % mužů, věkový medián 63 let) byly použity tyto léčebné režimy: fludarabin, cyklofosfamid, rituximab – FCR (n = 43) a nízkodávkovaný FCR (n = 13); 10 nemocných bylo léčeno jinými protokoly. 42 nemocných (76 % mužů, věkový medián 65 let) podstoupilo 2. linii: rituximab-dexametazon (n = 18), FCR/nízkodávkované FCR (n = 14) a 10 nemocných jiný režim. Součástí premedikace před infuzí rituximabu byl metylprednisolon 80 mg i.v., paracetamol 1 000 mg p. o. a bisulepin 1 mg i. v., všichni nemocní měli nitrožilní hydrataci minimálně 2 000 ml/den a alopurinol 300–600 mg/den. V 1. cyklu byl rituximab podáván frakcionovaně v celkové dávce 375 mg/m2 a od 2. cyklu formou rychlé infuze v dávce 500 mg/m2. Výsledky: K rozvoji infuzní reakce po rituximabu došlo u 32 % nemocných v 1. linii a 19 % v 2. linii. Jednalo se převážně o mírné reakce a k závažné toxicitě (stupeň III/IV dle NCI CTCAE – National Cancer Institute – The Common Terminology Criteria for Adverse Events) došlo výjimečně (3 % nemocných v 1. linii a 2 % v 2. linii). Reakce se nejčastěji projevila třesavkou, zimnicí, dušností, horečkou či hypotenzí. U všech nemocných bylo možné dokončit infuzi rituximabu v plánované dávce ve stejný den. Hodnocení léčebné odezvy (overall response rate – ORR) neprokázalo statisticky významné rozdíly mezi nemocnými s reakcí či bez infuzní reakce. Nemocní s infuzní toxicitou měli vyšší absolutní počet lymfocytů (1. linie, 87 vs 56 × 109/l, p = 0,21; 2. linie, 101 vs 14 × 109/l, p = 0,043). Při mediánu sledování 36 měsíců nebyl prokázán statisticky významný rozdíl v PFS (doba přežití bez progrese – progression-free survival) a OS (celkové přežití – overall survival) mezi oběma skupinami. Závěry: Výskyt infuzních reakcí u nemocných s CLL léčených rituximabem je poměrně běžný (32 %), avšak při pečlivé premedikaci jsou reakce velmi dobře zvladatelné a těžká forma je vzácná. Potvrdili jsme velmi dobrou snášenlivost rychlé infuze rituximabu. Naše data neprokázala statisticky významnou souvislost mezi výskytem infuzních reakcí a efektem léčby., Background and Aims: Rituximab in combination with chemotherapy is an effective treatment of patients (pts) with chronic lymphocytic leukemia (CLL). The most frequent adverse event of rituximab is infusion-related toxicity, e.g. cytokine-release syndrome that occurs usually during the first infusion. However, there is scarce data on feasibility and tolerability of rituximab infusions in CLL outside clinical trials. Therefore, we performed a single-center retrospective analysis of the frequency of rituximab infusion-related adverse events during the first- and the second line CLL treatment administered in the routine practice. We also analyzed its relation to parameters of tumor load and possible association with treatment efficacy. The safety of rapid infusion of rituximab in CLL pts was also evaluated. Patients and Methods: We analyzed 108 pts with CLL treated with rituximab-containing regimens between March 2005 and May 2011 at our institution. The most common first-line regimens (n = 66, 47 males, median age 63 years) were FCR (43 pts) and low-dose FCR (13 pts); 10 pts were treated by other protocols. Forty-two pts (32 males, median age, 65 years) underwent second line treatment: 18 pts rituximab-dexamethasone, 7 pts FCR, 7 pts low-dose FCR and 10 pts other regimens. Intravenous hydration (2 000 ml daily on days 0 and 1 of cycle), allopurinol 300–600 mg p. o. daily and premedication with methylprednisolone 80 mg i. v., acetaminophen 1 000 mg p. o. and bisulepine 1 mg i. v. were administered before rituximab infusion. Rituximab was given by fractionated infusion (100 mg for 2 hours, then if tolerated well, the rest of the dose with infusion rate escalation from 100 mg/hour up to 400 mg/hour) at the dose of 375 mg/m2 in the first cycle. Subsequent doses (500 mg/m2) were administered by rapid-infusion protocol. Results: Rituximab infusion-related toxicity occurred in 32 % pts (n = 21) during the first line treatment and 19% pts (n = 8) during the second line treatment. Adverse events were predominantly mild and NCI CTCAE grade III/IV occurred rarely (3% in the first line, 2% in the second line). Infusion toxicity manifested predominantly as rigors, chills, fever and hypotension. All patients with adverse events could finish rituximab infusion as initially planned on the same day. Treatment response analysis did not demonstrate statistically significant differences between patients with and without rituximab infusion toxicity. Patients who developed rituximab infusion toxicity had higher absolute lymphocyte count (first line, 87 vs 56 × 109/l, p = 0.21; second line, 101 vs 14 × 109/l, p = 0.043). At the median follow-up of 36 months, there were no statistically significant differences in PFS or OS in both cohorts. Conclusions: Rituximab infusion-related toxicity in pts with CLL is relatively frequent (32%). However, occurrence of infusion-related symptoms can be reduced by proper premedication and severe adverse events are uncommon. In our experience, all patients were able to receive the planned dose of rituximab. Subsequent doses of rituximab could be safely administered by rapid-infusion protocol.
