Cíl: Práce se zabývá porovnáním posturální stability u dvou vybraných souborů probandů, a to u dětí s obezitou a u atletů. Metody: U obou skupin bylo provedeno posturografi cké vyšetření pomocí dvou testů – Limity stability v náklonu tzv. Limits of Stability (LOS) a Senzorický test – Sensory Organisation Test (SOT). Na základě takto získaných výsledků byly porovnány a statisticky vyhodnoceny rozdíly mezi jednotlivými soubory. Výsledky: Práce prokázala významný vliv obezity na individuální posturální stabilitu jedince. Prokázali jsme statisticky signifi kantní rozdíly jak v reakčním čase (RT) a maximální exkurzi při testu LOS, tak i ve volbě pohybových strategií pro udržení posturální stability při testu SOT. Závěry: U dětí s obezitou je, v rámci fyzioterapie, vhodné zaměřit pohybovou aktivitu na zlepšení celkové postury a posturální stability a to např. využitím funkčního tréninku., Aim: The paper deals with the comparison of postural stability in two selected groups of probands: children with obesity and athletes. Methods: Posturographic examination was carried out in both groups through two tests – Limits of Stability (LOS) and Sensory Organization Test (SOT). Variations between individual groups were compared and statistically evaluated based on the acquired results. Results: Obesity as well as overall posture has a signifi cant impact on postural stability. We demonstrated statistically signifi cant variations in reaction time (RT) and maximum excursion in LOS test, and in SOT test in the choice of movement strategies for maintaining postural stability. Cíl: Cílem studie bylo zhodnotit míru subjektivně vnímaných potíží osob s poruchou čichu. Výsledky porovnat s 2 standardizovanými olfaktometrickými testy. Materiál: Celkem bylo vyšetřeno 134 osob (59 mužů a 75 žen) průměrného věku 53 let. Se sinonazální poruchou čichu bylo vyšetřeno 58 osob (průměrný věk 50 let). 21 osob s poúrazovou poruchou čichu (průměrný věk 39 let), 19 osob s povirovou poruchou čichu (průměrný věk 58 let) a 36 osob s jinou etiologií poruchy čichu (průměrný věk 54 let). Metodika: Subjektivní hodnocení závažnosti čichové poruchy a její vnímání v každodenním životě bylo zjišťováno formou dotazníku. K vyšetření čichu byl použit Test parfémovaných fi xů (Odorized Markers Test – OMT) a Sniffi n’Sticks test (16 složkový identifi kační test). Výsledky: Subjektivně udalo potíže s čichem 134 osob, které byly zařazeny do studie. Potíže s čichem trápí pacienty ve 42 % výrazně, ve 43 % mírně a 15 % osob poruchu čichu nevnímá vůbec. V každodenním životě vnímá poruchu čichu 62 % respondentů. Závěr: Ženy trápí potíže s čichem výrazněji než muže. Souvislost mezi mírou vnímání tíže poruchy čichu a vlivem na kvalitu života nebyla zjištěna. Výstup studie bude sloužit k vytvoření dotazníku pro hodnocení kvality života osob s poruchou čichu., Aim: Th e aim of study was to determine the degree of subjectively perceived problems in people with smell disorders. Th e results were compared with two standardized smell identifi cation tests. Methods: Th e total of 134 persons (59 men and 75 women) aged 53 years on average were examined. 58 persons with sinonasal disease (aged 50 years on average), 21 persons aft er head trauma (aged 39 years on average), 19 persons with smell loss due to upper respiratory tract infection (aged 58 years on average), and 36 persons with other etiology of smell disorders (aged 54 years on average). Subjective assessment of severity of olfactory disorders and its perception in daily life was examined by questionnaire. Th e sense of smell was tested by Odorized Markers Test (OMT) and Sniffi n’Sticks test (16 – items identifi cation test). Results: All 134 persons who were included to the study reported diffi culty with smell. Th e problems with smell were perceived in 42 % of patients strongly, 43 % mildly and 15 % of patients did not smell anything at all. 62 % of respondents realized their smell disorder in everyday life. Conclusion: Women suff ered from problems with smell more strongly compared to men. Th e relationship between the level of perception of severity of smell disorders and the eff ect on life was not confi rmed. Cíl: Hodnocení komunikačních a vybraných psychických oblastí (napětí, obavy, podráždění, deprimace) dispenzarizovaných nemocných se zhoubnými nádory hrtanu a slinných žláz, pomocí standardizovaných dotazníků kvality života. Východiska: Zhoubná nádorová onemocnění hlavy a krku se v četnosti výskytu řadí na šesté místo mezi zhoubnými nádory na celém světě. Otázka léčby a následné péče je velmi aktuální zvláště z hlediska rizika mutilujících léčebných výkonů a jejich vlivu na kvalitu života. Metody: Do výzkumu byli zařazeni aktivně dispenzarizovaní nemocní ORL kliniky se zhoubnými nádory hrtanu a slinných žláz. Vlastní šetření absolvovalo 113 respondentů, výzkum probíhal od ledna do prosince 2011. Pro sběr dat byly využity dva standardizované dotazníky kvality života (EQRTC QLQ-30, EQRTC QLQ-H&N35). Dále informace z nestandardizovaného dotazníku a zdravotnické dokumentace. Použité statistické metody – dvouvýběrový Wilcoxonův test a analýza rozptylu. Výsledky: U pacientů s nádorovým onemocněním hrtanu s tracheostomickou kanylou a bez ní byl statisticky významný rozdíl v komunikaci. Stádium nemoci dle klasifi kace TNM nemělo vliv na vybrané psychické oblasti u respondentů s nádorovým onemocněním hrtanu a slinných žláz. Závěry: Závažnost obtíží u dispenzarizovaných respondentů závisí na lokalizaci a velikosti nádoru, terapeutické strategii, věku pacienta, polymorbiditě a dalších faktorech. Trvalá tracheostomická kanyla má vliv na komunikaci respondentů s nádory hrtanu. V našem souboru respondentů jsme neprokázali vliv stádia nemoci dle klasifi kace zhoubných novotvarů TNM, na vybrané oblasti psychiky (napětí, obavy, podráždění, deprimace)., Aim: Assessment of communication and selected psychological areas (e. g. tension, concerns, irritation, depression) related to the dispensarized patients with malignant laryngeal and salivary glands tumors using standardized questionnaires assessing quality of life. Background: The head and neck malignant tumors rank sixth in frequency among malignant tumors worldwide. The question of treatment and follow-up care is very current especially in terms of the risk of mutilating surgical procedures and their impact on quality of life. Methods: In the research actively dispensarized patients of ENT clinic with malignant tumors of the larynx and salivary glands were included. 113 respondents took part in the research, which was conducted from January to December 2011. Standardized (EQRTC QLQ-30, EQRTC QLQ-H&N35) and non-standardized questionnaires assessing quality of life and medical records were used for data collection. Statistical methods such as double selection Wilcoxon test and analysis of dispersion were used. Results: There was a statistically signifi cant diff erence in communication between patients with tracheostomy cannula and without it. Stage of the illness according to TNM classifi cation did not infl uence selected psychological areas of the respondents with cancer of the larynx and salivary glands. Conclusions: The seriousness of the diffi culties of the dispensarized patients depends on localization and size of the tumor, therapeutic strategy, age of the patient, polymorbidity and other factors. Permanent tracheostomy cannula has an impact on communication of the respondents with laryngeal tumors. Příspěvek přináší přehled rozhodnutí Evropského soudu pro lidská práva, která se problematikou asistované reprodukce zabývaly. Na pozadí těchto rozhodnutí se příspěvek snaží analyzovat hranice rozšiřujícího pojetí práva na respektování rodinného a soukromého života podle čl. 8 Úmluvy a poukázat na skutečnost, že v oblasti řešení složitých biomedicínckých a bioetických otázek nemá Soud dostatečnou autoritu na to, aby se stal finálním arbitrem při posuzování hodnotových měřítek společnosti v jednotlivých evropských státech., In the last few decades decided European Court of Human Rights very problematic cases involving “right to access to assisted reproduction”. These cases are concerning very controversial ethical aspects of biomedical developments in the field of medical techniques of artificial procreation and reshape the content (meaning) of Article 8 of European Convention of Human Rights. This paper provides short overview of cases of European Court of Human Rights involving “right to access to assisted reproduction” and focuses especially on the latest developments in this field (case S.H. and Others v. Austria - Application no 57813/00) . Although the technology of medically assisted procreation had been available in Europe for several decades, many of the issues to which it gave rise, remained the subject of debate. There is no consensus towards the use of IVF treatment in general and even less clear are trends in some particular questions (f.e. allowing gamete donation). In the department of coloproctology of NORC MH RUz 17 patients with disseminated forms of colorectal cancer was made the study of oncogenes and complex treatment by 2 protocols using FOLFOX4 regime and FOLFIRI regime. In second protocol there used 2 sessions of endolymphatical polychemotherapy FOLFOX4 regime against EHFhyperthermia. All patients were performed additional investigations directed to study the presence of multiple drug resistance in them where definition of р53, bcl2 oncogene expression. In our observations we followed resistance to FOLFOX4 scheme in 4 patients, and to FOLFIRI scheme in 2 cases. Introduction Cutaneous malignant melanoma prevalence, incidence and mortality rates are increasing in white populations worldwide more rapidly than any other cancer site (American Cancer Society, 2006). Despite the potential importance of regular skin selfexamination and promotion of selfprotection practices, little is known about the prevalence of these practices in medical students in Albanian population. Methods This is a descriptive, quantitative crosssectional study. In this study were included a sample of 150 individuals chosen among the students of Faculty of Medicine based on their family history for skin cancer. This study was started on October the 3rd and finished on November the 12th. Subjects had to fill in a structured, selfadministered questionnaire. All participants lived within Republic of Albania but at the time of the study were students in University of Tirana, Faculty of Medicine. Results In this study that we conducted we included 150 individuals supposed to have a risk for skin cancer based on their family history. 200 individuals were approached for participation. Of these, 150 individuals returned questionnaire data, yielding a response rate of 75% among eligible, successfully contacted participants. The mean age of the sample was 20.05 years (Std. Dev. = 0.925), with males (22%) and females (78%) represented unequally, because the gender configuration of the faculty itself has gender disparities with more than 80% females and only 20% male students. The majority of the sample was born in city or town (85.3%), 14.7% was born in a village. In terms of income level, 79.3% of participants had a medium income family background, 15.3% high income and 5.3% had a low income level family background. Conclusion It is important for those individuals with family history for skin cancer (which inherently indicates risk for skin cancer) to develop selfexamination and SSE behaviours and practices in order to have a protection and at least an early detection (if onset) of the different forms of Melanoma. It is easier to prevent a disease than to cure it. This postulate is a foundation stone of the contemporary medicine, furthermore its mission. The Chronic Kidney Diseases (CKD), amongst them the Chronic Pyelonephrites (CP) and the mass kidney reduction take an important place in human pathologies in general, and in particular in renal ones. The Chronic Pyelonephrites are chronic renal pathologies, which on one side are of various causes and on the other side are multi systemic. At the same time they tend, earlier or later, depending on their course, to bring the patient towards the Chronic Kidney Insufficiency in stage of uremia, consequently in need of substitution therapies e.g. dialysis, peritoneum dialysis or transplant. It is worthy to emphasize that from the prevention and correct cure of CP make profit the patients, the family, the state and in the last analyses the entire society, because in that way the budget expense destined for the fore going substitution cures, dialysis, peritoneum dialysis or transplant, is considerably reduced. The same should be mentioned in relation to the CP and the mass kidney reduction, speaking about our country, which are still at the first place as the very cause of Chronic Kidney Insufficiencies (CRI), later on advancing toward uremia and terminal uremia along with its grave consequences. Cieľ: Cieľom prieskumu bolo zistiť prevalenciu a rizikové faktory hypertenzie bieleho plášťa (WCH) u detí. Metodika: Formou retrospektívnej štúdie sme analyzovali zdravotné záznamy 140 detí (36 dievčat, 104 chlapcov) s vysokým krvným tlakom nameraným v kardiologickej ambulancii, u ktorých bolo vykonané 24-hodinové ambulantné monitorovanie krvného tlaku (ABPM) a určený typ hypertenzie. Hypertenzia bola diagnostikovaná, keď bol systolický a/alebo diastolický krvný tlak meraný auskultačnou technikou ≥ 95. percentil pre pohlavie, vek a výšku. WCH bola definovaná ako priemerná hodnota krvného tlaku v ambulancii nachádzajúca sa v percentilovom pásme hypertenzie a hodnota tlaku krvi počas 24-hodinového monitorovania ambulantného tlaku nachádzajúca sa v pásme normálnych hodnôt krvného tlaku. Výsledky: V skupine 140 detí bola potvrdená prítomnosť WCH u 51 detí (36,4 %) – 13 dievčat a 38 chlapcov. Z celkového súboru sa WCH vyskytovala u 36,1 % zaradených dievčat a 36,5 % zaradených chlapcov. Systolický TK sa v kardiologickej ambulancii nachádzal nad 95. percentilom, diastolický TK pod 90. percentilom. Väčšina pacientov s WCH bola vo veku 15 – 19 rokov (80,4 %) a v pásme od 3. do 90. percentilu BMI (72,5 %). U pacientov prevládala negatívna rodinná anamnéza hypertenzie (54,9 %) a zriedkavé návštevy kardiologickej ambulancie (2-4x). Krvný tlak v ambulancii merala deťom výlučne sestra. Záver: Zistenie prevalencie WCH a jej rizikových faktorov je u detí významné, pretože prevažuje názor, že v detskom veku je WCH predispozíciou k trvalej hypertenzii. Na základe zistení rizikových faktorov, môžeme vykonávať prevenciu WCH., Aim: The aim of this study was to find out the prevalence and risk factors of white coat hypertension (WCH) in children. Methods: We realized the retrospective study and analyzed records of 140 children (36 girls, 104 boys) with high blood pressure measured in cardiology outpatients' clinic, to whom a 24-hour ambulatory blood pressure monitoring (ABPM) was made and the type of hypertension was specified. Hypertension was diagnosed when systolic and/or diastolic blood pressure measurements with auscultatory technique were ≥ 95th percentile for sex, age and height. White coat hypertension (WCH) has been defined as mean blood pressure (BP) readings in outpatients' clinic being located in the hypertensive range while 24-hour ambulatory blood pressure monitoring is in the normal range. Results: In the group of 140 children the presence of WCH was confirmed in 51 children (36,4 %) - 13 girls and 38 boys. WCH occurred in 36,1 % of girls and 36,5 % of boys in the total sample of respondents. Systolic BP in cardiology outpatients' clinic reached the value above the 95th percentile, diastolic BP below the 90th percentile. The most patients with WCH were 15 – 19 years old (80,4 %) and in 3rd - 90th percentile of BMI (72,5 %). Patients prevailed negative family history of hypertension (54,9 %) and fewer visit (2-4 times) of cardiology outpatients' clinic. Blood pressure of children was measured exclusively by a nurse in this outpatients' clinic. Conclusion: Determination of the prevalence of WCH and its risk factors in children is important because of prevailing opinion that WCH in children is predisposition in permanent hypertension. Purpose: To investigate the protective action of Ceruloplasmin (Cp) on the lysis of erythrocytes in men with prostate tumors. Material and Methods: The blood erythrocytes of the patients with benign hyperplasia of prostate (BHP) and prostate adenocarcinoma (CaP) were studied. Patients at the age 60-75 with early stages of a cancer have been investigated. The control group was consisted of apparently healthy males with the compatible age. n=15 for each group. The clinical stage of the disease was diagnosed by means of rectal, histological and echographic examination of the prostate gland. Photometric methods were applied to register lysis dynamics in order to test out the protective action of Cp. Results: The nonspecific protective function of Cp preparation on BHP and CaP erythrocytes as well as on the control group erythrocytes was revealed. CaP erythrocytes have shown more sensitivity to the lysis provocative factor than BHP and the control group erythrocytes, that was presumably attributed to the structural and functional changes of the erythrocytes developed in the presence of malignant tumor., N. Pseudomyxoma peritonei je vzácný klinický syndrom; nádorový proces, pro který je charakteristické hromadění mucinózního ascitu a peritoneálních implantátů v břišní dutině. Primárním zdrojem je téměř vždy low grade mucinózní tumor apendixu. Jedná se o pomalu progredující, nízce agresivní, ale smrtící onemocnění s častými recidivami. Optimálním léčebným postupem je kompletní chirurgická cytoredukce (CRS) v kombinaci s peroperačně aplikovanou intraperitoneální hypertermickou chemoterapií (HIPEC) – tzv. Sugarbakerova metoda., Pseudomyxoma peritonei (PMP) is an uncommon clinical syndrome characterized by the slow and progressive accumulation of peritoneal implants and mucinous ascites, intraabdominal gelatinous collection (jelly belly). PMP is reported to originate from the low grade appendiceal mucinous neoplasm. Pseudomyxoma peritonei often recurs after treatment and may eventually cause death by abdominal visceral dysfunction via compression with mucinous ascites. Cytoreductive surgery (CRS) and perioperative intraperitoneal chemotherapy comprising hyperthermic intraperitoneal chemotherapy (HIPEC) with or without postoperative intraperitoneal chemotherapy (EPIC) has been regarded as the standard of care in specialized centers. It is often referred to as the „Sugerbaker procedure“. Conventionally, PMP is considered resistant to systemic chemotherapy. Even though complete cytoreduction is associated with prolonged overall survival, recurrence of disease is common and multiple operations are frequently required. Patients may enjoy sustained period of remission, free of symptoms, but long – term disease free survival is distinctly uncommon. The pathologic subtype remains the dominant factor in survival.
