Cíl: Minireview podává přehled aktuálního stavu měření 25-hydroxyvitaminu D v krevním séru. Metoda: Hodnocení analytické kvality měření 25-hydroxyvitaminu v programech externího hodnocení kvality. Výsledky separačních a imunochemických metod, stav metrologické reference a standardizace, předměty měření (analyty), preanalytická stabilita. Závěr: Je nutná standardizace měření jako podmínka vyšší úrovně srovnatelnosti mezi metodami a mezi laboratořemi využitím a rozvojem referenční metodiky a aplikací již existujících referenčních materiálů., Objective: A mini-review on the current state in serum 25-hydroxyvitamin D measurement. Methods: The review of used routine methods for the measurement the 25-hydroxyvitamin D in serum. The assessment of analytical quality by means of inter-laboratory comparison programs is presented. The current state of metrological reference and standardization is outlined. The methods, based on separation, with immunochemistry methods are compared. Problem of suitable analytes as subjects of measurement. Stability of analyte is described. Neopterin, derivát pteridinu, je produkován v signifikantním množství lidskými monocyty-makrofágy po stimulaci interferonem gama. Představuje užitečný a důležitý parametr monitorování akrivace imunitního systému, který doprováži mnoho onemocněni, v poslední době se objevila řada prací spojujících neopterin a 7,8-dihydroneopterin s intracelulárním oxidativním stresem a je popisována i možná spojitost s apoptózou, vyvolanou tímto oxidativním stresem. v souvislosti s tímto pozorováním se výzkum v oblasti pteridinu obrací v současné době k takovým onemocněním, jako je neurodegenerativní onemocnění mozku, Alzheimerova choroba, Parkinsonova choroba a mnoho dalších nemocí, které jsou provázeny ztrátami a změnami nervových buněk. V této práci jsou uvedeny výsledky dvou studií, ve kterých bylo stanovení neopterinu v moči provedeno u dlouhověkých pacientů a které ukazují na možnost využití stanovení neopterinu v moči pro monitorování zdravotního stavu u gerontologických pacientů., Neopterin, a pteridine derivative, is produced in significant quantity by human monocytes/macrophages stimulated by interferon gamma. Neopterin represents an useful and important parameter for monitoring the immune system activation that accompanies different disorders. In recent years, several studies have demonstrated a link between neopterin and 7,8-dihydroneopterin and intracellular oxidative stress, and indicated possible association with apoptosis induced by oxidative stress. Based on these observations, the research on pteridines is currently being focused on degeneratíve disorders such as Alzheimer's disease, Parkinson's disease and other disorders accompanied by loss and disturbances of neural cells. We report here the results of 2 studies in which we investigated urinary neopterin in elderly patients. Náplní sdělení je aktuální informace o přínosu nové, vysoce senzitivní metody analýzy imunoglobulinů séra (Hevylite™), umožňující separátní kvantitativní vyšetřování párů těžkých/lehkých řetězců imunoglobulinu (HLC), tj. monoklonálního (involved) a polyklonálního (noninvolved) izotypu včetně jejich poměru (HLC-r) u monoklonálních gamapatií. Hlubší pozornost je věnována východiskům a charakteristice této analytické metody, srovnání jejího klinického přínosu se standardními technikami používanými v diagnostice, sledování průběhu, časné detekce relapsu a progrese nemoci, stratifikace, hodnocení léčebné odezvy a prognózy monoklonální gamapatie nejistého významu, mnohočetného myelomu, Waldenströmovy makroglobulinemie, systémové AL-amyloidózy a některých nehodgkinských lymfomů. Z dosavadních výsledků vyplývá, že ve srovnání s dosavadními standardně používanými metodami obohacuje metoda Hevylite™ klinickou praxi o vyšetření hladiny noninvolved Ig s možností vyhodnocení hloubky imunoparézy, stanovení indexu HLC-r uplatňujícího se při stratifikaci MM do rizikových skupin a přispívajícího k odhadu prognózy a zlepšení hodnocení hloubky léčebné odezvy, zatímco v případě MGUS poskytuje informaci o riziku maligní evoluce. Ke zpřesnění indikací metody Hevylite™ u MG v běžné klinické praxi zřejmě přispěje zveřejnění výsledků aktuálně probíhajících validačních studií., The aim of the paper is to inform about the contribution of novel, highly sensitive analytic technique for the assessment of serum immunoglobulins (Hevylite™), enabling separate quantitative assessment of heavy/light chain pairs of immunoglobulin (HLC), i. e. the monoclonal („involved“) and polyclonal („noninvolved“) isotype including their ratio (HLC-r) in monoclonal gammopathies. We particularly target the characteristics of this technique, the comparison of its clinical contribution with standard methods used in the diagnostics, course and the detection of relapse and progression of the disease, as well as the stratification, assessment of therapeutic outcome and prognosis in monoclonal gammopathy of undetermined significance, multiple myeloma, Waldenström´s macroglobulinemia, systemic AL-amyloidosis and some non-Hodgkin lymphomas. Present results show that in comparison with existing routinely used techniques the Hevylite™ method enriches clinical practice with the assessment of serum levels of „uninvolved“ Ig. It enables the evaluation of the depth of „immunoparesis“, and the determination of HLC-r index that is needful for the stratification of MM into „risk cohorts“. It also contributes to prognostic assessment and improvement of the evaluation of the depth of therapeutic response. In MGUS individuals the HLC-r index provides information about the risk of malignant transformation.
