Backround: Worldwide, approximately 1 billion people, from which 200250 million are women, smoke. Based on the Estonian Medical Birth Register data, in 2010, 7,5% of pregnant women were smokers, and in 2012, 7% of pregnant women were smoking during their pregnancy. Thus, smoking rates among pregnant women fell only 0,5%. The objective of the research was to find out the views of pregnant women who were smoking during pregnancy of the effect of smoking on the health of them and their babies, and experiences in quitting smoking, changing indicators, e.g. smoking and socioeconomical status (initial exposure to smoking, employment, longterm partnerships etc.) in 20092013. Methods: This research is a phenomenological study, which was being carried out from October 2009 until January 2013. Data collection methods were semistructured interviews with 45 pregnant women smoking during their pregnancy and being registered for antenatal care in three health care institutions of Republic of Estonia. An inductive approach for qualitative analysis was used. Results: Most women smoking during pregnancy started smoking in their teens, their parents smoked, they were under 30 years old housewives or unemployed. On an average of 1120 cigarettes were consumed each day, the pregnancy of the research group did not influence frequency and tobacco intake. The participants in the research group reported that tobacco consumption helped them relax, gave them a chance to spend time in a good company, and it was not regarded as a bad habit but a social addiction, being caused by availability of tobacco products. Although most of them had an opinion about harmful effects of smoking, e.g. a cough, asthma and rapid fatigue, on the health of them and less on their expected babies, they continued smoking. Conclusions: The smoking and socioeconomical status of women smoking during pregnancy in 20092013 has not changed, health awareness of them is low and has not improved. Support groups with close relatives are needed to motivate cessation. In the first place, prevention needs a good example, purposeful cooperation, which starts in daily life context at homes, in educational institutions and in the community. Limiting the availability of tobacco products, postponing and preventing an initial exposure to smoking are thought to be of great importance. Podmínkou zdraví, nejenom reprodukčního, je zdravý způsob života vycházející z kvalitní informovanosti. Výsledky současných výzkumů ukazují, že nezdravý způsob života matky/rodičů již před početím dítěte a následně prenatálně hraje roli v etiologii různých poruch zdraví a abnormalit vývoje jedince, které se mohou manifestovat okamžitě, ale také teprve s dlouhou latencí. Znalosti a návyky žen mohou svým poradenstvím významně ovlivnit porodní asistentky prenatálně v kurzech, v optimálním případě již prekoncepčním poradenstvím. Záměrem práce bylo zjistit, jaká je současná situace v oblasti přípravy žen na těhotenství a mateřství, které faktory ji determinují a jaký je její případný benefit. Užito bylo dotazníkového šetření a hodnocení vybraných antropometrických parametrů. Práce zjistila, že příprava žen na těhotenství a mateřství vykazuje značné rezervy projevené nedostatečnou informovaností i výskytem nevhodného či rizikového chování. Výsledky ve většině sledovaných parametrů korelovaly s dosaženým stupněm vzdělání a byly lepší u žen, které se na své těhotenství a mateřství připravovaly. Prenatální kurzy navštěvuje ale celkově malý počet žen, nejméně ženy s nižšími stupni vzdělání, pro které ale paradoxně kurzy přinášejí vyšší benefit., Healthy lifestyle resulting from well-informed parental knowledge is a condition of both reproductive and general health. Results of current studies show that both pre-conceptual and prenatal unhealthy lifestyles of mothers/parents play an important role in the etiology of different health malfunctions and abnormalities in ontogeny. These malfunctions and abnormalities can be manifested either immediately or with a long latency. The knowledge and habits of women can be significantly influenced by midwives in prenatal courses or, optimally, in a pre-conception advisory capacity. The goal of our work was to ascertain the current situation of pregnanacy and maternity preparedness and associated determining factors. A questionnaire survey and evaluation of chosen anthropometrical parameters were used. The study revealed that existing pregnancy and maternity preparation is insufficient, with associated inadequacies or risks in behaviour. In most cases the results correlated with the educational capacity of mothers and were better in cases where the subjects had prepared for their pregnancy and maternity. Současný stav: Výzkumné studie prokázaly, že nejenom rozdíly v pohlaví mají vliv na zdravotní stav, ale též etnická příslušnost a společenská třída. Imigranti jsou zranitelnou skupinou, která je ve vyšším riziku řady onemocnění ve srovnání s majoritou. Sociální determinanty zdraví jsou významné při hledání příčin sociálních nerovností ve zdraví (Bates et al., 2009; Lindström et al., 2001). Cíl: Hlavním záměrem výzkumu bylo zmapování využití zdravotní péče imigranty. Metodika: Kvantitativní metoda byla realizována pomocí dotazování. Výzkumné šetření probíhalo v období leden až červenec 2012. Bylo kontaktováno 236 imigrantů ve dvou regionech (Jihočeský kraj a hlavní město Praha). Byli osloveni legálně usazení imigranti, ve věkové kategorii 18-65 let, vietnamské, mongolské a ukrajinské národnosti. Výsledky: Čerpání zdravotní péče imigranty bylo ovlivněno typem zdravotního pojištění, délkou pobytu, pohlavím a znalostí českého jazyka. Imigranti, kteří mohli vstoupit do systému veřejného zdravotního pojištění, čerpali zdravotní péči častěji než imigranti s komerčním zdravotním pojištěním. Závěry: Mezi opatření, vedoucí k odstranění sociálních nerovností a zlepšení situace imigrantů v ČR patří zvyšování jazykových znalostí imigrantů a umožnění všem legálně usazeným imigrantům přístup k veřejnému zdravotnímu pojištění., Background: Research shows that health state is influenced not only by gender but also by class and ethnic membership. Immigrants form a vulnerable group in comparison to the majority, more susceptible to a wide range of diseases. Social determinants of health play an important role in search for reasons for social inequality in health (Bates et al., 2009; Limdström et al., 2001). Goal: The main aim of the research was to map use of healthcare by immigrants. Methodology: The quantitative method used were interviews. The survey was carried out in the period from January to July 2012. 246 immigrants were interviewed from two regions (South Bohemian Region and the capital Prague). They were legally settled immigrants in the age group 18-65, of Vietnamese, Mongolian and Ukrainian nationality. Results: The factors influencing use of healthcare by immigrants are the type of health insurance, length of residence, sex and knowledge of Czech language. Obezita dětí je v oblasti veřejného zdraví vážným celosvětovým problémem. Výživa, fyzická (in)aktivita a sedavý životní styl, ke kterému patří sledování televize, práce či hraní her na počítači, jsou nezávislými rizikovými faktory nárůstu BMI (nárůstu hmotnosti). Cílem předkládané práce bylo posoudit asociace mezi stravovacími návyky, fyzickou aktivitou, životním stylem, nadváhou a obezitou u dětí ve věku 8–14 let v Královéhradeckém, Libereckém a Pardubickém kraji. Cílovou skupinou (n = 2042) byli žáci ze základních škol z 2., 4., 6. a 8. ročníku. Všechny sledované parametry (vynechávání jídla, celkový energetický příjem, preference potravin a nápojů...) byly hodnoceny formou četnosti (%) v celé skupině, dle pohlaví a dle tělesné hmotnosti. Ve všech sledovaných parametrech měly děti s nadváhou/obezitou vždy horší výsledky než děti s normální hmotností, i když ne vždy signifikantní. Největší rozdíly byly zjištěny při dodržování pitného režimu, kde děti s nadváhou/obezitou pijí především slazené nápoje., Childhood obesity is a serious public health concern worldwide. Dietary behaviour, physical activity and a sedentary lifestyle which includes television viewing, working or playing games on the computer are independent risk factors for increased BMI (increasing weight). The aim of this study was to explore dietary habits, physical activity and lifestyle associated with overweight and obesity among children 8–14 years of age, in Hradec Králové, Liberec and Pardubice district. The target population (N = 2042) comprised elementary school children in the 2nd, 4th, 6th and 8th grade. All monitored parameters (skipping meals, total energy intake, food and beverage preferences...) were evaluated by frequency (%) in the group, by gender and according to body weight. In all monitored parameters overweight/obese children have worse results, although not always significantly, than children with normal weight. Cílem pilotní studie bylo vyhodnotit kvalitu života (Quality of Life – QoL) u pacientek s karcinomem prsu ve vztahu k léčebným modalitám a jejich kombinacím. Ve studii QoL participovalo 71 pacientek s karcinomem prsu, QoL byla hodnocena pomocí Amerického dotazníku zdraví (SF36), Dotazníku životní spokojenosti a české výzkumné verze Functional Assessment of Breast Cancer Therapy (FACT-B). Průměrná doba od diagnózy do doby hodnocení QoL ve výzkumném souboru byla 4,5 roku. Statistická významnost rozdílů v kvalitě života mezi pacientkami s různou léčbou byla testována Mannovým-Whitneyovým testem pro jednotlivé typy terapie a Kruskalovým-Wallisovým testem pro kombinace terapie. Vztah mezi kvalitou života a léčebnou modalitou byl zjištěn pro dotazník SF36 – pro dimenzi emocionální funkční role a operační léčbu (vyšší kvalita života u operovaných, p = 0,049), pro dotazník FACT-B – fyzický stav a chemoterapii (vyšší kvalita života u pacientek bez chemoterapie, p = 0,012), pro FACT-B – sociální/rodinný stav a operační léčbu (vyšší kvalita života u operovaných, p = 0,036), pro FACT-G skóre a operační léčbu (vyšší kvalita života u operovaných, p = 0,049) a dále pro Dotazník životní spokojenosti – pro dimenzi zdraví a kombinaci léčby (nejvyšší kvalita života u kombinace operace, chemoterapie, radioterapie, biologické a hormonální terapie, nejnižší u kombinace předešlých bez biologické terapie, p = 0,027). Pro úplnost uvádíme typ a četnost vedlejších efektů léčebných modalit. Zjistili jsme vyšší QoL u operovaných pacientek a nižší QoL u pacientek vystavených chemoterapii, avšak naše závěry vyžadují další výzkumná ověření. A total of 71 patients with breast carcinoma participated in this study. QoL was assessed using the 36-item Short-Form Health Survey (SF-36), Life Satisfaction Questionnaire and Czech research version of Functional Assessment of Breast Cancer Therapy (FACT-B). The average time from diagnosis at the time of QoL assessment was 4.5 years. Mann-Whitney U test was used to test the differences in QoL between groups of patients with different type of therapy and Kruskal-Wallis test for combinations of therapies. A relation between quality of life and a treatment modality was found for questionnaire SF 36 – for the dimension of emotional functional role and operation treatment (higher quality of life in operated patients, p = 0.049), for questionnaire FACT-B – physical condition and chemotherapy (higher quality of life in patients without chemotherapy, p = 0.012), for FACT-B – social/family condition and operation treatment (higher quality of life in operated patients, p = 0.036), for FACT-G score and operation treatment (higher quality of life in operated patents, p = 0.049) and further for Life Satisfaction Questionnaire – for the dimension of health and treatment combination (the highest quality of life in case of the combination of operation, chemotherapy, radiotherapy, biological and hormonal therapy, the lowest quality of life in case of the combination of the previous without biological therapy, p = 0.027). Additionally, we described type and occurrence of side effects of treatment modalities. Generally we found higher QoL in operated patients and lower in patients who underwent chemotherapy, however our findings require further research verifications. Cílem studie bylo prozkoumat vztah mezi sociální oporou, socioekonomickými determinantami a průběhem onkologické léčby. Byl sledován výskyt komplikací souvisejících s toxicitou protinádorové terapie, výskyt viróz během 6 měsíců po operaci a pooperační komplikace. Ve studii participovalo 139 pacientek s karcinomem prsu (medián věku 65 let), sociální opora (SO) a socioekonomický status (SES) byly hodnoceny na základě strukturovaného interview. SES byl odhadován na základě kategorií ekonomického příjmu a dosaženého vzdělání. Ze zdravotnické dokumentace byly do analýz zařazeny údaje o výskytu obecně léčebných komplikací, komplikací po operaci a o výskytu infekčních onemocnění typu viróz do 6 měsíců po operaci. Statistická významnost rozdílů v kategoriálních proměnných mezi testovanými skupinami byla hodnocena Fisherovým přesným testem. Statisticky významné rozdíly mezi skupinami s a bez SO byly zjištěny u celého souboru pacientek v méně častém výskytu viróz u pacientek se SO (bez viróz 70,3 % vs 48,3 % bez SO). U pacientek se SO ve věku do 60 let byl zjištěn nižší výskyt pooperačních komplikací (56,7 % vs 0 %) i obecně nižší počet komplikací v souvislosti s onkologickou léčbou (více než 4 komplikace se projevily u 47,1 % se SO vs 100 % bez SO). U pacientek se SO ve stadiu nemoci II–III byl zjištěn nižší výskyt viróz (bez viróz 63,2 % vs 28,6 %) a nižší četnost komplikací (více než 4 komplikace 35,5 % vs 73,3 %). Statisticky významný vztah (P = 0,015) byl zjištěn mezi vzděláním a počtem komplikací u pacientek se stadiem II–III, u pacientek se základním a učňovským vzděláním byl vyšší podíl více než 4 komplikací (55,9 %) oproti skupině se středním a vyšším vzděláním (32,6 %). Zvýšený trend byl nalezen pro vztah mezi SES a výskytem komplikací souvisejících s onkologickou léčbou (P = 0,06) ve skupině pacientek s karcinomem prsu ve stadiu II–III.Zjistili jsme indicie pro vztah sociální opory a průběh onkologické léčby pro starší pacientky napříč klinickými stadii a pro vztah SES a výskyt léčebných komplikací pro starší ženy ve stadiu II–III. Sociální opora je velmi důležitý faktor pro zdraví pacientek, významnou pozitivní roli hraje také vyšší socioekonomický status a vzdělání. Klíčová slova: karcinom prsu – onkologická léčba – sociální opora – socioekonomický status, The objective of the study was to investigate the relationship between social support, socioeconomic determinants and oncological treatment progress in patients with complications due to the toxicity of antitumor therapy, viral diseases during 6 months following surgery, as well as postoperative complications. The study sample consisted of 139 female patients with breast cancer (age median: 65 years), and social support (SS) and socioeconomic status (SES) were evaluated based on a structured interview. SES was estimated based on income and education classification. The analyses performed included information from patients’ documentation, including data on the occurrence of general therapy complications, postoperative complications, and the occurrence of infectious viral diseases within the first 6 months following surgery. The level of statistical significance of the differences within category-specific variables between the analysed groups was evaluated with Fisher’s exact test. The differences in statistical significance between groups with and without SS were found within the entire group of patients, while viral diseases occurred less in patients with SS (no viral diseases 70.3% vs 48.3% without SS). Patients with SS and below 60 years of age experienced a lower occurrence of postoperative complications (56.7% vs 0%), as well as generally lower numbers of complications resulting from oncological treatment (more than 4 complications occurred in 47.1% of patients with SS vs 100% of patients without SS). Patients with SS in the Stage II–III experienced a lower occurrence of viral diseases (63.2% no virosis vs 28.6%) and a lower occurrence of complications (35.5% more than four complications vs 73.3%). A statistically significant relationship (P = 0.015) was determined between the education of patients and the number of complications in patients in Stage II–III: more than four complications occurred in patients with basic and vocational education (55.9%), compared to patients with secondary and higher education (32.6%). An increasing trend was determined for the relationship between SES and the occurrence of complications due to oncological treatment (P = 0.06) within the group of patients with Stage II–III breast cancer. We have found evidence supporting the idea of a relationship between social support and oncological treatment for older patients across clinical stages and for the relationship between SES and the occurrence of complications for older women in Stage II–III. Social support is a factor which is of great importance for patient health, while higher socioeconomic status and education play a significant role, too.
