Cíl: Cílem této normativní studie Mini‑Mental State Examination (MMSE), neuropsychologické metody pro orientační zjištění kognitivního stavu a skríningu kognitivních poruch, je potvrdit zahraniční zjištění o závislosti výkonu v MMSE na věku a vzdělání a poskytnout normativní data české starší populace. Soubor a metodika: Soubor 540 osob ve věku 60 a více let splňujících daná kritéria zařazení, z 12 krajů České republiky, bez suspektní poruchy kognice (na základě anamnézy a výsledků neuropsychologických testů). Soubor byl rozdělen na čtyři skupiny dle věku (mladší, 60–74 let, a starší, 75 a více let) a dle vzdělání (nižší – bez maturity, vyšší – s maturitou a vyšším). Poměr pohlaví byl muži : ženy, 1 : 1. Výsledky: Byl zjištěn významný efekt věku (Pearson r = –0,308) i vzdělání (Cohenovo d = 0,43; oba p < 0,001) na získaný skór v MMSE. Nejhorších výsledků dosahovali starší, méně vzdělaní jedinci (MMSE 26,88; ? 1 SD 24,27–28,48) a nejlepších výsledků mladší vzdělanější (MMSE 28,60; ? 1 SD 26,83–29,62). Závěry: Potvrdili jsme závislost výsledků v MMSE u starších zdravých osob na jejich věku a vzdělání. Z toho vyplývá nutnost ve studiích klinické užitečnosti prověřit užívaný jednotný hraniční skór při odhadu kognitivních poruch, a to především u syndromu demence u různých neurodegenerativních onemocnění. Studie poprvé poskytuje normativní data MMSE na velkém souboru osob z české populace upravená dle věku a vzdělání., Aim: The aim of this normative study of Mini-Mental State Examination (MMSE) – a neuropsychological method to screen cognitive status and disorders - is to confirm international findings on the effects of age and education on MMSE scores and to obtain normative data for the Czech older population. Sample and methods: The sample consisted of 540 adults of 60 plus years of age meeting the inclusion criteria and without suspected cognitive disorder (based on anamnesis and neuropsychological assessment) from 12 regions of the Czech Republic. The sample was divided into four groups according to their age (younger 60–74, older 75+), and education (lower, higher). Men to women ratio in the sample was 1 : 1. Results: We found a significant effect of age (Pearson r = –0.308) and education (Cohen‘s d = 0.43; both p < 0.001) on MMSE score. The lowest scores were obtained by older, less educated individuals (MMSE 26.88; ? 1 SD 24.27–28.48), the highest scores were obtained by younger, more educated individuals (MMSE 28.60; ? 1 SD 26.83–29.62). Conclusion: We confirmed correlations between MMSE scores and age and education in older, cognitively healthy persons. These results necessitate further investigation of the MMSE clinical utility to determine the cut-off scores for dementia due to different neurodegenerative diseases. Pro pacienty s demencí je charakteristická porucha soběstačnosti v důsledku postižení více kognitivních domén zahrnujících i prostorovou orientaci, která se dělí na tři základní komponenty závisející na odlišných mozkových strukturách: orientaci podle blízké značky, orientaci podle výchozí pozice těla a orientaci podle vzdálené značky. Cílem této studie bylo porovnat rozdíly v postižení těchto tří základních komponent prostorové orientace u nejčastějších demencí neurodegenerativní etiologie. Studie se zúčastnilo celkem 78 pacientů s degenerativními demencemi různé etiologie: s Alzheimerovou chorobou (n = 61), frontotemporální lobární degenerací (n = 9) a demencí s Lewyho tělísky (n = 8). Pacienti byli vyšetřeni v experimentálním zařízení Blue Velvet Arena, které umožňuje měřit výkon v každé ze tří základních komponent prostorové orientace, a následně byly jejich výsledky v jednotlivých testech vzájemně porovnávány. V testu orientace podle blízké orientační značky měla skupina pacientů s frontotemporální lobární degenerací lepší výkon než skupina s Alzheimerovou chorobou (p = 0,03) a demencí s Lewyho tělísky (p = 0,006). V testu orientace podle výchozí pozice měla skupina s demencí s Lewyho tělísky horší výkon než skupina s Alzheimerovou chorobou (p = 0,012) a pacienti s frontotemporální lobární degenerací (p = 0,012). V testu orientace podle vzdálené orientační značky jsme mezi skupinami nenalezli signifikatní rozdíly (p = 0,069). Naše výsledky ukazují nejmírnější postižení prostorové orientace u pacientů s frontotemporální lobární degenerací a nejvýraznější u pacientů s demencí s Lewyho tělísky. V postižení prostorové orientace u pacientů s Alzheimerovou chorobou, frontotemporální lobární degenerací a demencí s Lewyho tělísky existují specifické rozdíly, které lze změřit v experimentálním zařízení Blue Velvet Arena., Impairment of multiple cognitive domains, including spatial navigation, leads to deterioration of self-sufficiency, a characteristic feature of patients with dementia. Spatial navigation is divided into three basic components that depend on different brain structures – navigation using a close orientation cue (cued), starting position of the body (egocentric) and distant orientation cue (allocentric). The aim of the study was to compare differences in impairment of these spatial navigation components in patients with the most common types of neurodegenerative dementias. In total, 78 patients with neurodegenerative dementias of various etiologies were divided into three groups: Alzheimer’s disease (AD, n = 61), frontotemporal lobar degeneration (FTLD, n = 9) and dementia with Lewy bodies (DLB, n = 8). All subjects were examined in an experimental device called the Blue Velvet Arena that allows researchers to measure performance in each of the three navigation components. The results of the different tests were subsequently compared. In the cued navigation test, the FTLD group performed better than the AD (p = 0.03) and DLB (p = 0.006) groups. Furthermore, in the egocentric navigation test, the DLB group was outperformed by AD (p = 0.012) and FTLD (p = 0.012) groups. Finally, in the allocentric navigation test there were no differences among the groups (p = 0.069). Our results show that spatial navigation impairment may be least pronounced in FTLD patients and most pronounced in DLB patients. Figurální (neboli neverbální) fluence je schopnost exekutivních funkcí, která nám poskytuje informace o divergentním myšlení, rozdělené pozornosti, plánování a mentální flexibilitě. Zhoršený výkon v testech zaměřených na figurální fluenci nacházíme u pacientů s neurologickým i psychiatrickým postižením. Pětitečkový test (Five Point Test, 5TT) je jeden z neuropsychologických testů, jenž slouží ke zhodnocení figurální fluence. Úkolem probanda je vytvořit co nejvíce obrazců v časovém limitu. Cílem této studie bylo vytvořit normy k 5TT pro českou dospělou populaci. Předkládáme normativní data pro dospělé ve věku od 20 do 85 let (n = 503). Hodnotili jsme počet správných odpovědí a počet perseverací. Počet správných odpovědí je ovlivněn věkem a vzděláním (r = –0,3; resp. 0,4; p < 0,0001), proto jsou normy rozděleny na pásma po 10 letech a dále podle ukončeného vzdělání. Počet perseverací s těmito proměnnými souvisí jen slabě (rs = 0,1; resp. –0,1; p < 0,05). Pohlaví nemá vliv na počet správných odpovědí ani perseverací (t = 0,09; p > 0,9 pro oba skóry)., Figural (or nonverbal) fluency is the ability of executive functions to provide information about divergent reasoning, divided attention, planning and mental flexibility. Impairments of figural fluency have been found in individuals with various neurological or psychiatric diseases. Five Point Test (5TT) is a neuropsychological test that assesses figural fluency. A participant is asked to generate as many unique designs as possible in a certain time limit. The aim of this study was to create Czech population norms for the Five Point Test. Normative data for adult population aged between 20 and 85 years (n = 503) are presented. We assessed the number of correct answers and the number of perseverations. The number of correct answers is influenced by age and education (r = –0.3 and 0.4, respectively, p < 0.0001); for this reason the norms are stratified into ten age ranges and also according to completed education. The number of perseverations correlates with these variables only weakly (rs = 0.1 and –0.1, respectively, p < 0.05). Gender has no impact neither on the number of correct answers nor on perseverative responses (t-test, p > 0.9 for both scores). Cíl: Cílem našeho článku je předložit normativní data pro fonemickou verbální fluenci (VF), pro hlásky K, P, S, i sémantickou VF, pro kategorie zvířata a zelenina. Úvod: Testy verbální fluence jsou jedna z nejrozšířenějších metod hodnocení kognitivní výkonnosti. V českém prostředí dosud neexistuje kvalitní normativní studie testů VF se zaměřením na populaci starších osob, u kterých se tato metoda používá nejčastěji. Metodika: Testy VF jsme administrovali v rámci neuropsychologické baterie 540 zdravým osobám (292 žen, 248 mužů) nad 59 let. V části fonemické VF jsme použili hlásky K, P, S a celkový skór všech hlásek (K + P + S) jako analogii anglického originálu F, A, S v Controlled Oral Word Association Test (COWAT/<span class=""></span>FAS). V rámci sémantické VF jsme hodnotili výkonnost ve dvou kategoriích, zvířata a zelenina. Výsledky: Věk má statisticky významný (p < 0,001) slabý až středně silný vliv na výkon ve VF, reprezentovaný součtem skórů K + P + S (r = –<span class=""></span>0,236), zvířata (r = –<span class=""></span>0,359) a zelenina (r = –<span class=""></span>0,264). Rovněž počet let vzdělání ovlivňuje slabě až středně silně výkon ve VF. Statisticky významně (p < 0,001) pouze ve skórech K + P + S (r = 0,297) a zvířata (r = 0,357). Mezi věkem a skórem zelenina vztah není signifikantní (r = 0,028; p = 0,523). Ve skóru zelenina existuje signifikantní (p < 0,001) rozdíl mezi muži a ženami. Dále předkládáme normativní data pro věkové skupiny 60–<span class=""></span>75 let, 70–<span class=""></span>85 let a 80–<span class=""></span>96 let. Závěr: Výsledky naší studie prokazují statisticky významný vliv věku na všechny uvedené zkoušky VF. Fonemická VF a kategorie zvířata jsou závislá i na výši vzdělání, v kategorii zelenina není výkonnost závislá na vzdělání, ale na pohlaví., Aim: The aim of the study was to report normative data on letter (LF; letters K, P, S) and semantic fluency (SF; animals and vegetables). Introduction: Verbal fluency (VF) is one of the most frequently used neuropsychological methods for the assessment of cognitive performance in clinical and experimental neuropsychology. However, representative normative data for the Czech population of older and very old adults are so far lacking. Methods: We administered VF as part of neuropsychological battery to 540 (292 women, 248 men) healthy older adults (60–<span class=""></span>96 years of age). In LF, the letters K, P, S and their total score were used as analogous to the original Controlled Oral Word Association Test (COWAT/<span class=""></span>FAS). In SF, we evaluated performance in two categories –<span class=""></span> animals and vegetables. Results: Age was significantly (p < 0.001) related to a sum of K + P + S (r = –<span class=""></span>0.236) as well as animals (r = –<span class=""></span>0.359) and vegetables (r = –<span class=""></span>0.264). However, the association was moderate. Education was also moderately related (p < 0.001) to the sum of K + P + S (r = 0.297) and animals (r = 0.357). However, we did not find a significant relationship between age and vegetables (r = 0.028; p = 0.523). Vegetables were also the only measure that showed highly significant sex differences (p < 0.001). We present normative Czech data for 60–<span class=""></span>75, 70–<span class=""></span>85 and 80–<span class=""></span>96 age groups. Conclusion: The results of our study confirm a significant moderate influence of age and education (with the exception of vegetables for the latter) on all VF measures. There were highly significant sex differences in the vegetable category. Cílem studie bylo prozkoumat vztah mezi sociální oporou, socioekonomickými determinantami a průběhem onkologické léčby. Byl sledován výskyt komplikací souvisejících s toxicitou protinádorové terapie, výskyt viróz během 6 měsíců po operaci a pooperační komplikace. Ve studii participovalo 139 pacientek s karcinomem prsu (medián věku 65 let), sociální opora (SO) a socioekonomický status (SES) byly hodnoceny na základě strukturovaného interview. SES byl odhadován na základě kategorií ekonomického příjmu a dosaženého vzdělání. Ze zdravotnické dokumentace byly do analýz zařazeny údaje o výskytu obecně léčebných komplikací, komplikací po operaci a o výskytu infekčních onemocnění typu viróz do 6 měsíců po operaci. Statistická významnost rozdílů v kategoriálních proměnných mezi testovanými skupinami byla hodnocena Fisherovým přesným testem. Statisticky významné rozdíly mezi skupinami s a bez SO byly zjištěny u celého souboru pacientek v méně častém výskytu viróz u pacientek se SO (bez viróz 70,3 % vs 48,3 % bez SO). U pacientek se SO ve věku do 60 let byl zjištěn nižší výskyt pooperačních komplikací (56,7 % vs 0 %) i obecně nižší počet komplikací v souvislosti s onkologickou léčbou (více než 4 komplikace se projevily u 47,1 % se SO vs 100 % bez SO). U pacientek se SO ve stadiu nemoci II–III byl zjištěn nižší výskyt viróz (bez viróz 63,2 % vs 28,6 %) a nižší četnost komplikací (více než 4 komplikace 35,5 % vs 73,3 %). Statisticky významný vztah (P = 0,015) byl zjištěn mezi vzděláním a počtem komplikací u pacientek se stadiem II–III, u pacientek se základním a učňovským vzděláním byl vyšší podíl více než 4 komplikací (55,9 %) oproti skupině se středním a vyšším vzděláním (32,6 %). Zvýšený trend byl nalezen pro vztah mezi SES a výskytem komplikací souvisejících s onkologickou léčbou (P = 0,06) ve skupině pacientek s karcinomem prsu ve stadiu II–III.Zjistili jsme indicie pro vztah sociální opory a průběh onkologické léčby pro starší pacientky napříč klinickými stadii a pro vztah SES a výskyt léčebných komplikací pro starší ženy ve stadiu II–III. Sociální opora je velmi důležitý faktor pro zdraví pacientek, významnou pozitivní roli hraje také vyšší socioekonomický status a vzdělání. Klíčová slova: karcinom prsu – onkologická léčba – sociální opora – socioekonomický status, The objective of the study was to investigate the relationship between social support, socioeconomic determinants and oncological treatment progress in patients with complications due to the toxicity of antitumor therapy, viral diseases during 6 months following surgery, as well as postoperative complications. The study sample consisted of 139 female patients with breast cancer (age median: 65 years), and social support (SS) and socioeconomic status (SES) were evaluated based on a structured interview. SES was estimated based on income and education classification. The analyses performed included information from patients’ documentation, including data on the occurrence of general therapy complications, postoperative complications, and the occurrence of infectious viral diseases within the first 6 months following surgery. The level of statistical significance of the differences within category-specific variables between the analysed groups was evaluated with Fisher’s exact test. The differences in statistical significance between groups with and without SS were found within the entire group of patients, while viral diseases occurred less in patients with SS (no viral diseases 70.3% vs 48.3% without SS). Patients with SS and below 60 years of age experienced a lower occurrence of postoperative complications (56.7% vs 0%), as well as generally lower numbers of complications resulting from oncological treatment (more than 4 complications occurred in 47.1% of patients with SS vs 100% of patients without SS). Patients with SS in the Stage II–III experienced a lower occurrence of viral diseases (63.2% no virosis vs 28.6%) and a lower occurrence of complications (35.5% more than four complications vs 73.3%). A statistically significant relationship (P = 0.015) was determined between the education of patients and the number of complications in patients in Stage II–III: more than four complications occurred in patients with basic and vocational education (55.9%), compared to patients with secondary and higher education (32.6%). An increasing trend was determined for the relationship between SES and the occurrence of complications due to oncological treatment (P = 0.06) within the group of patients with Stage II–III breast cancer. We have found evidence supporting the idea of a relationship between social support and oncological treatment for older patients across clinical stages and for the relationship between SES and the occurrence of complications for older women in Stage II–III. Social support is a factor which is of great importance for patient health, while higher socioeconomic status and education play a significant role, too.
