Diagnostika a léčba karotické stenózy prochází neustálým vývojem. Zejména v posledních letech můžeme zaznamenat velmi výrazné změny v názorech jak na podkladě výsledků metaanalýz starších důkazů, tak důkazů nových. Významným faktorem je také rychlý rozvoj nových endovaskulárních terapeutických metod. V reakci na tyto změny byly řadou odborných společností vydány doporučené postupy zaměřené na diagnostiku a léčbu karotické stenózy. Přestože jsou podloženy prakticky stejnými medicínskými důkazy, ne vždy se ve svých doporučeních shodují. Cílem práce bylo sumarizovat aktuální doporučení, která se týkají diagnostiky a léčby karotické stenózy. Diagnostika a intervenční terapie u asymptomatických a symptomatických pacientů byla zpracována odděleně, neboť se jedná o 2 velmi rozdílné skupiny. Podrobně byla zpracována také doporučení, která se týkají volby typu intervence a rozhodování mezi karotickou endarterektomií a karotickým stentingem., Diagnosis and treatment of carotid stenosis is under incessant development. Especially in last years, we have noticed significant changes in opinions based on the results of meta-analyses of old evidence and the new evidence as well. Important factor involved is the fast development of new endovascular therapies. In reaction to these changes, several medical societies published their guidelines for the management of carotid stenosis. Even though, all of them are based practically on the same published medical evidence, they are not always similar in recommendations. The aim of our work was to summarize actual recommendations regarding management of carotid stenosis. The management of asymptomatic and symptomatic patients is being discussed separately where appropriate because these two groups of patients are very different. Tranzitorní ischemická ataka (TIA) a minor stroke patří mezi akutní cévní onemocnění mozku, jež sama o sobě nemají ve většině případů invalidizující dopad na pacienta. Jejich podcenění však může vést k recidivě cévní mozkové příhody (CMP) s fatálními následky. Cílem tohoto přehledového článku je upozornit na hlavní problémy současné klinické praxe v České republice a navrhnout racionální algoritmus péče o pacienty s TIA/minor stroke. Článek podává informace o nové definici TIA, která se opírá o fakt, že až u 50 % pacientů po TIA je na magnetické rezonanci (MR) přítomen infarkt, o stratifikaci rizika a jejím využití při managementu pacientů, o významu zobrazovacích vyšetření a o možnostech terapie. Poukazuje na skutečnost, že stratifikace rizika není v České republice dostatečně používána, což vede k neadekvátnímu poměru mezi rizikem pacienta a zvoleným diagnosticko-terapeutickým postupem. Urgentní péči potřebují obzvláště pacienti s vysokým rizikem recidivy CMP – tj. pacienti s poruchou řeči nebo motoriky trvající ≥ 5 min, se skóre ABCD2 ≥ 4, s fibrilací síní a pacienti s infarktem na difuzně vážených sekvencích (DWI) MR či CT (počítačové tomografii) a s okluzí nebo významnou ≥ 50% stenózou extra-/intrakraniální velké mozkové tepny. Přehledový článek zdůrazňuje nutnost nejen okamžitého zahájení medikamentózní terapie, ale i provedení časné endarterektomie a shrnuje nové možnosti léčby, např. duální antiagregace, jejíž přínos byl v časné fázi po TIA a minor stroke nedávno dokumentován., Transient ischemic attack (TIA) and minor stroke are acute cerebrovascular diseases that are usually non-disabling. However, if not followed adequately, they may lead to potentially fatal stroke recurrence. The aim of this review is to summarize the main challenges of the current clinical practice (in the Czech Republic) and suggest a rational algorithm for TIA/minor stroke management. This review informs about the new TIA definition based on the fact that up to 50% of TIA patients have a brain infarction on MRI (magnetic resonance imaging), about risk stratification and its use in patients management, about the importance of imaging studies and about the available treatment options. Risk stratification for TIA is underused in the Czech Republic and this affects risk-benefit ratios for the available management plans. In particular, follow-up of high-risk patients need urgent attention - patients with speech and/or motor symptoms lasting ≥ 5 min, with ABCD2 score ≥ 4, with atrial fibrillation, and patients with an infarction on DWI MRI or CT (computed tomography), and with an occlusion or significant ≥ 50% stenosis of a precerebral/cerebral large vessel. The review highlights the importance of an immediate medical intervention with early endarterectomy and provides an overview of new therapeutic options, e.g. dual antiplatelet therapy, the benefit of which has recently been documented for the first time. Endovaskulární terapie (EVT) je nejúčinnější metoda léčby akutního uzávěru velké tepny (arteria carotis interna, arteria cerebri media, arteria basilaris). Recentně publikované prospektivní randomizované kontrolované studie porovnávající kombinovanou EVT a medikamentózní léčbu (intravenózní trombolýzu; IVT) prokázaly superioritu kombinované EVT a IVT oproti samotné IVT v terapii akutních ischemických příhod v karotickém povodí. Cílem naší práce je podat přehled sedmi studií publikovaných v letech 2014–2015. V druhé části pojednáváme o faktorech, které ovlivňují výsledný klinický stav: věk, komorbidity, hyperglykemie při příjmu, známky časné ischemie na CT hodnocené pomocí Alberta Stroke Program Early CT Score (ASPECTS), absence kolaterálního zásobení, doba do dosažení rekanalizace a použití celkové anestezie. Úspěch EVT závisí na správném výběru vhodných pacientů., The endovascular therapy (EVT) is the most efficacious treatment for recanalisation of large vessel artery occlusion. Recently published prospective randomized controlled trials (RCT) demonstrated better outcomes with combined EVT and intravenous thrombolysis (IVT) versus IVT alone in the treatment of proximal arterial occlusion in the anterior cerebral circulation. In this article seven of these RCT studies published in 2014 - 2015 are reviewed. In the latter part of the article, the factors influencing final clinical outcome are discussed: age, comorbidities, hyperglycemia on admission, signs of early ischemia as evaluaed by Alberta Stroke Program Early CT Score (ASPECTS), absence of collateral supply, time before recanalisation occurs and application general anesthesia. The careful selection of patients indicated for EVT is of utmost importance for the succes of therapy.
