V našem kazuistickém sdělení je popsán mikroskopicky verifikovaný případ IgG4 asociované pankreatitidy s přidruženým postižením žlučových cest u pacienta léčeného ve Fakultní nemocnici Hradec Králové. Mozkový absces vzniká přímým šířením hnisavého procesu z okolí, metastaticky nebo v souvislosti s kraniocerebrálním poraněním anebo operačním zákrokem. Léčba spočívá v kombinaci navigované punkce a evakuace abscesu s cílenou antibiotickou terapií. Méně častá je radikální exstirpace abscesu. Absces bývá doprovázen kolaterálním edémem vyžadujícím v některých případech antiedematózní terapii. Uvedená léčba by měla v současné době vést k úplnému vyléčení. Ve svém sdělení popisujeme ojedinělý případ nemocného se solitárním abscesem mozku, u kterého došlo po punkci k progresi kolaterálního edému s následnou klinickou deteriorací. Akutně provedená dekompresivní kraniektomie vedla k úplné úpravě klinického stavu. Po ústupu edému byla provedena kranioplastika. Abscesové ložisko po punkci a antibiotické terapii zcela vymizelo. Původcem abscesu byli peptostreptokokus a fusobakterium. Použití dekompresivní kraniektomie v terapii mozkového abscesu je výjimečné a v písemnictví je popisováno sporadicky., Brain abscess is caused by direct dissemination of inflammation from surrounding structures, remote infectious sources or is associated with brain trauma or neurosurgical procedures. Treatment includes surgical aspiration and intravenous antibiotics. Excision of the lesion is sometimes indicated. Antiedematous therapy is necessary in cases of brain oedema. At the present time, the majority of patients with brain abscesses survive and recover completely. Here we describe the case of a patient with brain abscess, whose condition deteriorated after aspiration of the lesion. The deterioration was due to progression of collateral brain oedema. After urgent decompressive craniectomy, the patient recovered completely. A cranioplasty was performed when the oedema disappeared. The abscess resolved completely following aspiration and antibiotic therapy. Fusobacterium and peptostreptococus were identified as the causative pathogens. Decompressive craniectomy has very rarely been applied in the treatment of brain abscesses and is reported sporadically. Ruptura aneuryzmatu Valsalvova sinu je vzácné, ale závažné srdeční onemocnění, které může ohrozit pacienta na životě. Včasná diagnostika a správná léčba může výrazně ovlivnit prognózu pacienta. V naší kazuistice popisujeme případ 44letého muže přijatého na Interní kardiologickou kliniku FN Brno pro typické bolesti na hrudi s nespecifickými změnami při elektrokardiografickém vyšetření a s pozitivním troponinem T. Vstupní transthorakální echokardiografické vyšetření odhalilo aneuryzma nekoronárního sinu a komunikaci mezi aortou a pravou síní, která byla potvrzena i CT angiograficky. Pacient byl poté odeslán do kardiochirurgického centra, kde byla následně provedena resekce aneuryzmatu a uzavřena komunikace mezi aortou a pravou síní záplatou z perikardu. Při vzniku aneuryzmatu nelze vyloučit podíl metabolického účinku kortikoidů, které pacient dlouhodobě užíval. Z uvedené kazuistiky plyne, že pečlivé klinické vyšetření a neinvazivní vyšetřovací metody zůstávají stále základním diagnostickým postupem. Early diagnosis and proper therapeutic treatment can significantly affect the patient’s prognosis. We describe the case of a 44‑year–old man admitted to the Cardiology Department of the University Hospital Brno with typical chest pain and non‑specific changes on electrocardiogram and positive troponin T. Transthoracic echocardiography revealed a non‑coronary sinus aneurysm and communication between the aorta and the right atrium, which was confirmed by computed tomography. The patient was referred to the cardiothoracic surgery centre and the aneurysm was resected. The communication between the aorta and the right atrium was closed via pericardial patch. The aneurysm formation can be caused by the metabolic effects of corticosteroids which the patient had been using for a long time. This case report suggests that careful clinical examination and noninvasive methods remain an essential diagnostic procedure. Ortotopická transplantace srdce je zavedenou metodou léčby pacientů v terminálním stadiu srdečního selhání, která zlepšuje přežívání těchto pacientů. Cenou za podávání imunosupresivní léčby je řada potenciálních rizik, jedněmi z nejzávažnějších jsou infekční komplikace. Kazuistika popisuje případ pacientky se srdečním selháním na podkladě primární amyloidózy, která podstoupila srdeční transplantaci. Po transplantaci byla v plánu kurativní hematoonkologická léčba, která však vzhledem ke kompletní remisi základního onemocnění nebyla uskutečněna. V časném potransplantačním období pacientka prodělala řadu komplikací, z nichž nejzávažnější byla invazivní plicní aspergilóza. Tato infekce byla úspěšně zaléčena vorikonazolem a pacientka je nyní 28 měsíců po srdeční transplantaci ve velmi dobrém klinickém stavu bez známek aktivní infekce., Orthotopic heart transplantation is an established method of treatment for patients with end‑stage heart failure, which improves the survival of these patients. The use of immunosuppressive therapy comes at the price of a number of potential risks, the most serious being infectious complications. Our case report describes a patient with heart failure on the basis of primary amyloidosis who underwent cardiac transplantation. Hemato‑oncological curative treatment was planned after transplantation, but has not been applied due to the complete remission of the primary disease. In the early post‑transplant period, the patient experienced a number of complications, the most serious being invasive pulmonary aspergillosis.
