Akutní myeloidní leukemie (AML) je onemocnění především staršího věku s mediánem při stanovení diagnózy 69 let. Bohužel výsledky intenzivní léčby u nemocných starších 60 let s AML, tedy pacientů s nejvyššími riziky rozvoje AML, jsou neuspokojivé. Důvodem těchto neuspokojivých výsledků jsou zvýšená rizika mortality při samotné intenzivní chemoterapii u starších nemocných a dále samotná biologická podstata AML ve vyšším věku, která je spojena se zvýšenou rezistencí k intenzivní léčbě a s vyšším rizikem relapsu. Možnost provedení alogenní transplantace krvetvorných buněk po redukované přípravě v rámci konzolidační léčby za současně se zlepšující podpůrné péče v potransplantačním období může dle publikovaných výsledků zlepšit nepříznivou prognózu AML u nemocných starších 60 let. Limitací uvedeného postupu je fakt, že alogenní transplantaci v současné době nemůže podstoupit většina starších pacientů kurativně léčených pro AML z důvodu nedosažení kontroly základního onemocnění nebo z důvodu neuspokojivého celkového stavu neumožňujícího provedení transplantace. Zavedení nových léčebných postupů do léčby AML a další zlepšení samotných transplantačních postupů může v budoucnu potenciálně zvýšit možnost zařazení transplantace do léčby pacientů starších 60 let s AML., Acute myeloid leukemia (AML) is disease of older age with the median of age at diagnosis 69 years. Unfortunately results of intensive treatment of patients older than 60 years with AML, thus patients with high-risk of AML development, are unsatisfactory. The reasons for this unsatisfactory results are higher risks of treatment-related mortality among older patients treated with intensive chemotherapy and biological characteristics of AML in older age with higher risk of AML resistance to intensive treatment and higher risk of AML relapse. According to published data reduced-intensity allogeneic transplantation as part of consolidation treatment and advancement in supportive care after transplantation can improve outcome of patients older than 60 years with AML. The limitation of this strategy is a fact that majority of older patients cannot undergo allogeneic transplantation due to unsatisfactory control of AML or due to diminished performance status which does not alow transplant procedure. New approaches to AML treatment and future improvements in transplant procedure could potentially increase the role of allogeneic transplantation in treatment of patients older than 60 years with AML., Michal Karas, and Lit.: 22
Lokální chemoprofylaxe neinvazivních nádorů močového měchýře je používána více než 40 let. Využíváno je několik agens, mezi nejčastější patří mitomycin C (MMC) a epirubicin. Názor na léčebná schémata není jednotný. Avšak na podkladě EBM je zřejmé, že adjuvantní instilace cytotoxických látek do močového měchýře prodlužuje dobu beznádorového intervalu. V práci předkládáme dlouhodobé zkušenosti s intravezikální chemoprofylaxí povrchových nádorů močového měchýře., Local chemoprophylaxis of noninvasive bladder tumors has been used for more than 40 years. Osteosarkom (OS) je nejčastější zhoubný nádor skeletu, představuje asi 35 % primárních kostních nádorů. Úspěšná léčba osteosarkomu vyžaduje úzkou spolupráci odborníků multidisciplinárního týmu, kam patří onkolog, chirurg, patolog a radiolog. Nejlepší péči zajišťují specializovaná centra s odpovídajícími zkušenostmi a možností komplexní péče. Léčebný režim musí zahrnovat radikální odstranění primárního nádoru, případně metastáz v kombinaci se systémovou chemoterapií. Z cytostatik je to zejména doxorubicin, vysoko-dávkovaný metotrexát, cisplatina a ifosfamid. Cílem rozdělení chemoterapie na část předoperační a pooperační je redukce nádorové masy a lepší ohraničení tumoru vůči okolním strukturám. Tím je usnadněna chirurgická radikalita s možností náhrady postižené části skeletu bez mutilujícího výkonu. Důležitá je také znalost histopatologické odpovědi na předoperační chemoterapii, která ovlivňuje intenzitu pooperační léčby. Dosud není dostatečně vyřešena léčba metastatického onemocnění a případné recidivy. Zlepšení výsledků je možno očekávat až po zavedení nových léčebných přípravků a modalit., Osteosarcoma (OS) is the most common malignant tumour of the skeleton accounting for approximately 35 % of primary bone tumours. Successful treatment of osteosarcoma requires a close cooperation between the specialists of a multidisciplinary team consisting of an oncologist, surgeon, pathologist and radiologist. The best care is provided by specialized centres with appropriate experience in which comprehensive care is available. The therapeutic regimen must include radical removal of the primary tumour and/or metastases in combination with systemic chemotherapy. The cytostatic drugs primarily used include doxorubicin, high-dose methotrexate, cisplatin and ifosfamide. The goal of dividing chemotherapy into preoperative and postoperative parts is a reduction in the tumour mass and improved delineation of the tumour from the surrounding structures. This improves the surgical radicality with the affected part of the skeleton being replaced without the need for a mutilating procedure. Also of importance is the knowledge of the histopathological response to preoperative chemotherapy which affects the intensity of postoperative treatment. The management of metastatic disease and possible recurrence has not been addressed adequately as yet. Hodgkinův lymfom (HL) je jedním z nejlépe léčitelných nádorových onemocnění. Moderními léčebnými strategiemi je vyléčeno 85–95 % pacientů. Do popředí začaly vystupovat pozdní následky léčby, především kardiotoxicita a sekundární malignity, na které 15 let po skončené terapii umírá více pacientů než na HL. Cílem výzkumu je najít takovou terapii, která by zajistila maximální kontrolu tumoru při minimalizaci akutní a dlouhodobé toxicity. German Hodgkin Study Group (GHSG) prokázala ve studii HD10 pro počáteční stadia HL možnost redukce chemoterapie i radioterapie, bez snížení účinnosti léčby. Studie pro pokročilá stadia HD9 prokázala výborné výsledky chemoterapie BEACOPP eskalovaný oproti konvenční terapii ABVD, ale s vyšší akutní a dlouhodobou toxicitou. Cílem dalších studií GHSG pro pokročilá stadia (HD12, HD15 a nyní aktivní studie HD18) je snížit intenzitu chemoterapie a posoudit, zda je nutná následná radioterapie. Pozitivní výsledky přináší hodnocení studie HD15, kde je testován význam FDG – PET při indikaci radioterapie. Současná studie HD18 sleduje pomocí FDG – PET i časnou odpověď tumoru po 2 cyklech chemoterapie. Další naděje jsou vkládány do nových biologických léků, které cílí přímo na receptory maligních buněk (monoklonální protilátky, inhibitory)., Hodgkin lymphoma (HL) is one of the best curable malignant diseases. Modern therapeutic strategies can cure 85–95 % of patients. Late effects have become increasingly important, especially cardiotoxicity and second tumors, that cause more deaths than HL 15 years after completion of treatment. The goal of the research is to find the therapy that maximizes tumor control and minimizes acute and longterm toxicity. German Hodgkin Study Group (GHSG) proved the possibility to reduce chemotherapy and radiotherapy without reducing the effectiveness of treatment in the HD10 study for early stages of HL. HD9 study for advanced stages demonstrated excellent results of BEACOPP escalated chemotherapy compared to the conventional therapy ABVD, but BEACOPP escalated induced higher acute and long-term toxicity. The goal of subsequent GHSG studies for advanced stages (HD12, HD15 and ongoing active HD18 study) is to reduce the intensity of chemotherapy and to evaluate the significance of subsequent radiotherapy. The HD15 study tests the significance of FDG-PET for indication of radiotherapy. Current HD18 study tests by FDG-PET the early response of tumor after 2 cycles of chemotherapy.
