Autoimunitní encefalitidy jsou akutní či subakutní monofázická nebo progresivní zánětlivá onemocnění CNS podmíněná autoimunitními mechanizmy (působením patogenních autoprotilátek nebo autoagresivních efektorových buněk). Mezi autoimunitní encefalitidy lze řadit klasické paraneoplastické syndromy s postižením CNS, limbické encefalitidy, anti‑NMDAR encefalitidu, Morvanův syndrom, Rasmussenovu encefalitidu, onemocnění z okruhu Stiff‑Person Syndromu včetně progresivní encefalomyelitidy s rigiditou a myoklonem, akutní autoimunitně podmíněné extrapyramidové poruchy hybnosti, encephalitis lethargica, neurologické syndromy se zvýšenou senzitivitou vůči glutenu a další. V posledních letech prodělala tato oblast neuroimunologie prudký rozvoj. Byly popsány nové syndromy, upřesněny klinické projevy a etiopatogenetické mechanizmy některých již známých chorob a identifikována řada nových typů protilátek. Tato minimonografie si klade za cíl podat ucelený přehled současných znalostí v oblasti autoimunitních encefalitid a umožnit tak klinikům snazší orientaci v této oblasti neurologie., Autoimmune encephalitis is a term used to describe a group of acute or subacute monophasic or progressive inflammatory CNS disorders with autoimmune pathogenesis (i.e. induced by pathogenic autoantibodies or autoreactive effector cells). This group includes such diverse conditions as paraneoplastic CNS syndromes, limbic encephalitis, anti-NMDAR encephalitis, Morvan’s syndrome, Rasmussen encephalitis, stiff-person syndrome spectrum disorders including progressive encephalomyelitis with rigidity and myoclonus, autoimmune movement disorders, encephalitis lethargica, neurological syndromes with increased sensitivity to gluten, etc. This area of neuroimmunology has evolved dynamically during the past 10 years. Many new syndromes have been described, understanding of pathogenesis of the already known syndromes has improved and new antibodies were identified. This review aims to provide an overview of current knowledge in the field of autoimmune encephalitis in order to help clinicians navigate the rough waters of this exciting area of neurology. Předkládaný text sumarizuje přístup k pacientovi s akutním závrativým stavem. Hlavním cílem prvního vyšetření je rozhodnout, zda jde o pacienta se závažným onemocněním – rozvíjející se cévní mozkovou příhodou (CMP) nebo o pacienta s vestibulární neuronitidou. Toto rozhodnutí je nutné ve většině případů učinit pod časovým tlakem a s vědomím, že v akutní fázi CMP nemusí neuroradiologické vyšetření poskytnout potřebné informace. Pečlivě provedené klinické vyšetření vestibulárního a okulomotorického systému postačuje k rozhodnutí, o jaký typ závrativé poruchy se jedná. Při sumarizaci klinických nálezů a stanovení pracovní diagnózy je nutné vycházet ze základní znalosti funkce vestibulárního systému, reflexních okruhů, do kterých je zapojen, a též principu organizace multisenzorického řízení rovnováhy., The following text summarizes management of patients with sudden vertigo. The main aim of the initial examination is to decide whether we are facing a patient with life-threatening disease, e.g. stroke, or a benign disease such as vestibular neuritis. This decision is usually made under a time pressure and knowing that neuroradiology examination may not provide sufficient information. Clinical examination of the vestibular and oculomotor function is sufficient to make the initial decision. Vestibular examination requires basic knowledge of the vestibular system function and reflex circuits involved in the organization and control of the multisensoric balance system. Isidor Soyka (1850–1889) byl první průkopník vědecké hygieny v českých zemích. Vystudoval lékařství a krátce se věnoval patologické anatomii. Jako první popsal primární amyloid. Po stáži u Maxe von Pettenkofera v Mnichově byl r. 1884 jmenován prvním profesorem hygieny a založil první hygienický ústav (při německé lékařské fakultě v Praze) v Čechách. Je autorem mnoha odborných prací z oblasti komunální hygieny a bakteriologie, výrazně se zasloužil o asanaci Prahy i dalších měst. Národnostní a náboženská netolerance své doby dovedla tohoto průkopníka naší vědecké hygieny k dobrovolnému odchodu ze života. Jako doplněk k článku je zpracována Soykova bibliografie., Isidor Soyka (1850–1889) was a pioneer of scientific methods in hygiene in Bohemia. He studied medicine and then shortly worked in the field of pathological anatomy. He was the first person to describe primary amyloidosis. After his studies under Max von Pettenkofer in Munich, he was appointed professor of hygiene and founded the first Hygiene Institute (as a part of the German Faculty of Medicine in Prague) in Bohemia. He published many works specializing in hygiene and bacteriology and greatly contributed to sanitation projects in Prague and other cities. Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. Neuromyelitis optica (NMO), také známá jako Devicova choroba, je devastující autoimunitní onemocnění centrálního nervového systému klinicky charakterizované záněty zrakového nervu, míchy a méně často mozkového kmene. Objev protilátek proti akvaporinu‑4 (NMO‑IgG/AQP4-IgG) definitivně odlišil tuto chorobu od roztroušené sklerózy. Testování AQP4-IgG je klíčové v diferenciálně diagnostickém procesu, ve kterém se dále neobejdeme bez magnetické rezonance mozku a míchy. Typickým nálezem jsou míšní ložiska přesahující délku tří míšních segmentů. Změny na magnetické rezonanci mozku diagnózu NMO nevylučují, jsou naopak relativně časté a mají poměrně typický charakter. Důležité informace přináší i optická koherentní tomografie, která kvantifikuje postižení sítnice a zrakového nervu. Neurologický deficit u této choroby je následkem relapsů onemocnění, a proto je zcela zásadní časné stanovení diagnózy a nasazení adekvátní terapie, která může změnit nepříznivou prognózu pacienta. Mezi hlavní léčebné strategie patří agresivní léčba relapsů (vysokodávkované steroidy či plazmaferéza) následované zahájením chronické terapie (mezi léky první volby dnes řadíme azathioprin a rituximab)., Neuromyelitis optica (NMO), also known as Devic‘s disease, is a devastating autoimmune disorder of the central nervous system and is clinically characterized by inflammation of the optic nerve, spinal cord, and, less frequently, brainstem. The discovery of antibodies against aquaporin‑4 (NMO‑IgG/AQP4-IgG) has definitively distinguished this disease from multiple sclerosis. AQP4-IgG testing is crucial in differential diagnosis that also includes magnetic resonance imaging of the brain and spinal cord. Typically, spinal cord lesions exceeding the length of three vertebral segments are found. Changes in magnetic resonance imaging of the brain do not exclude the diagnosis of NMO; by contrast, they are relatively common and are fairly typical. Optical coherence tomography provides important information, and quantifies the involvement of the retina and optic nerve. Neurologic deficit in this disease is a result of disease relapse, and it is therefore crucial to establish the diagnosis early and initiate an appropriate treatment that can change the unfavorable prognosis. The main therapeutic strategies include aggressive treatment of relapses (with high‑dose corticosteroids or plasmapheresis), followed by chronic therapy (first choice drugs include azathioprine and rituximab). Mezi nejnáročnější součásti klinického neurologického vyšetření patří vyšetření citlivosti vyžadující detailní edukaci pacienta a jeho následnou spolupráci. Při správné metodice vyšetření a jeho exaktním hodnocení lze často odhalit i diskrétní poruchy citlivosti, kterých si pacient nemusí být vědom nebo není schopen je validně popsat. Našim cílem je podat přehled nejčastěji používaných metod vyšetření běžně hodnocených senzitivních a algických modalit (taktilní, vibrační a termické čití, termoalgické čití, hodnocení hluboké tlakové bolesti a dále mechanická algická citlivost a hodnocení dynamické mechanické alodynie), včetně interpretace jejich nálezů a využitelných normativních dat (pokud jsou dostupná), a to od nejjednodušších kvalitativních a semikvantitativních metod po složitější metody kvantitativní. Kvalitativní či semikvantitativní metody jsou v běžné klinické praxi využívány podstatně častěji. Jejich výhodou je rychlost a metodická nenáročnost testování a široká dostupnost používaných pomůcek. Nevýhodou je naopak skutečnost, že poskytují pouze orientační informaci o citlivosti pro danou modalitu a spolehlivě detekují většinou až pokročilé poruchy senzitivity. Absence kvantifikace limituje možnosti těchto metod v longitudinálním sledování pacientů i při srovnání nálezů mezi pacienty či jejich skupinami. Kvantitativní metody souhrnně označované termínem kvantitativní testování senzitivity (Quantitative Sensory Testing, QST) jsou časově a metodicky náročnější a vyžadují použití specializovaných pomůcek. Jejich předností jsou kvantifikované výstupy s lepší diagnostickou validitou, které potenciálně diagnostikují i klinicky méně vyjádřené abnormity a jsou lépe využitelné např. pro longitudinální sledování. Tyto metody se častěji uplatňují v klinických studiích a jsou nezastupitelné v diagnostice neuropatie tenkých vláken., Sensory examination represents one of the most complicated parts of clinical neurological examination that requires detailed instructions to patients and their attentive cooperation. Using correct methodology of testing and precise evaluation of the results, sensory tests frequently disclose even very fine sensory abnormities which patients may not be able to describe or even may not be aware of. The aim of this paper is to provide an overview of methods most frequently used to evaluate common sensory and pain modalities (touch, vibration and thermal sensation, thermoalgic sensation, deep pain pressure, mechanical detection threshold and sensitivity, evaluation of dynamic mechanical allodynia), including interpretation of the findings and recommended normal values (if applicable). In each of the tested modalities, most commonly used methods are described from the easiest qualitative and semiquantitative methods (usually more frequently used in clinical practice) up to more complicated quantitative tests. Main advantages of qualitative and semiquantitative methods include their low methodological and time burden and high availability of the equipment used. Quantitative sensory testing (QST) is more demanding and requires the use of specialized, not widely available, equipment. These methods, however, provide quantified results with better diagnostic validity, may disclose even less pronounced sensory abnormities and are more useful in long-term follow-up as well as in inter-individual comparisons, both at individual and group level.
