Žilní a arteriální onemocnění zřejmě mohou mít řadu společných rizikových faktorů. Z hlediska patofyziologického se zřejmě jedná o podobný mechanizmus poškození cévní stěny, na jehož počátku jsou subklinické zánětlivé změny. Mechanizmus vzniku život ohrožujících trombotických příhod může být také podobný u obou skupin onemocnění, a to zejména selháním regulačních vlastností endotelu. Výsledky dosud publikovaných klinických a epidemiologických studií však zatím jednoznačné důkazy o společných faktorech vzniku a komplikací tepenných a žilních onemocnění nepřinesly. V našich studiích jsme zjistili častější výskyt nižšího poměru tlaků kotník/paže u žen s chronickým žilním onemocněním dolních končetin, nenalezli jsme však přímou souvislost mezi známkami preklinické aterosklerózy a výskytem žilních trombóz u pacientů s trombofilními stavy; u této skupiny však jsme však nalezli souvislost mezi tromboembolickými příhodami a hypertenzí. Tepenná i žilní onemocnění mohou tedy být příznivě ovlivněna již zavedenými léčebnými postupy zaměřenými na prevenci kardiovaskulárních onemocnění aterosklerotického původu. Důkazem možného společného ovlivnění tepenných a žilních onemocnění farmaky jsou výsledky rozsáhlé klinické studie, ve které došlo k významnému poklesu nejen kardiovaskulárních příhod, ale i tromboembolických žilních příhod při podávání rosuvastatinu. In available clinical and population based studies, however, unequivocal data are presented regarding association between arterial and venous diseases and their risk factors. In our studies, we found a higher prevalence of lower ankle brachial index in women with chronic venous disease of the lower extremities At the same time, nevertheless we found no strong evidence of a direct link between preclinical atherosclerosis and the occurrence of venous thrombosis in patients with thrombophilias; in the latter group, however, we found a link between hypertension and thromboembolic events. Arterial and venous disease may thus be favorably managed by already well-established and available tools used in prevention of atherosclerotic cardiovascular disease. Evidence of a possible impact of pharmacotherapy on both arterial and venous disease stems from a large clinical study in which treatment with hypolipemic drug, rosuvastatin, significantly decreased not only incidence of cardiovascular events but also of venous thromboembolic events. Úvod: PARADIGM HF srovnávala inhibitor angiotenzinového receptoru a inhibitor neprilysinu LCZ696 s enalaprilem u nemocných se srdečním selháním a sníženou ejekční frakcí. Metodika: Jednalo se o dvojitě slepou studii u 8 442 nemocných se srdečním selháním NYHA klasifikace II, III a IV a ejekční frakcí pod 40 %, kteří dostávali LCZ696 (2 × 200 mg) nebo enalapril (2 × 10 mg) přidané ke standardní medikaci. Primární cíl byl smíšený – kardiovaskulární mortalita a hospitalizace pro první srdeční selhání. Studie byla naplánována tak, aby odhalila rozdíl v kardiovaskulární mortalitě. Výsledky: Studie byla ukončena předčasně podle předepsaných pravidel při průměrné době sledování 27 měsíců pro jasný prospěch z léčby LCZ696. V době ukončení se primární cíl vyskytl u 914 nemocných (21,8 %) ve LCZ696 skupině a u 1 117 nemocných (26,5 %) ve skupině léčené enalaprilem (HR LCZ696 skupina, 0,80; 95% CI, 0,73–0,87; p < 0,001). Celkem 711 nemocných (17,0 %) léčených LCZ696 a 835 nemocných (19,8 %) léčených enalaprilem zemřelo (HR pro úmrtí z jakékoli příčiny 0,84; 95% CI, 0,76–0,93; p < 0,001); z těchto nemocných 558 (13,3 %) a 693 (16,5 %) zemřelo z kardiovaskulární příčiny (HR 0,80; 95% CI, 0,71–0,89; p < 0,001). LCZ696 ve srovnání s enalaprilem snižoval riziko hospitalizace pro srdeční selhání o 21 % (p < 0,001) a snižoval symptomy srdečního selhání (p = 0,001). Ve skupině léčené LCZ696 bylo více hypotenzí a nezávažných angioedémů, ale menší výskyt renálního selhání, hyperkalemie a kašle než ve skupině léčené enalaprilem. At the time of study closure, the primary outcome had occurred in 914 patients (21.8%) in the LCZ696 group and 1117 patients (26.5%) in the enalapril group (hazard ratio in the LCZ696 group, 0.80; 95% confidence interval [CI], 0.73 to 0.87; P < 0.001). Crohn's disease is a chronic immune-mediated intestinal inflammation targeted against a yet incompletely defined subset of commensal gut microbiota and occurs on the background of a genetic predisposition under the influence of environmental factors. Genome-wide association studies have identified about 70 genetic risk loci associated with Crohn's disease. The greatest risk for Crohn's disease represent polymorphisms affecting the CARD15 gene encoding nucleotide-binding oligomerization domain 2 (NOD2) which is an intracellular sensor for muramyl dipeptide, a peptidoglycan constituent of bacterial cell wall. The accumulated evidence suggests that gut microbiota represent an essential, perhaps a central factor in the induction and maintaining of Crohn's disease where dysregulation of normal co-evolved homeostatic relationships between intestinal microbiota and host mucosal immune system leads to intestinal inflammation. Taken together, these findings identify Crohn's disease as a syndrome of overlapping phenotypes that involves variable influences of genetic and environmental factors. A deeper understanding of different genetic abnormalities underlying Crohn's disease together with the identification of beneficial and harmful components of gut microbiota and their interactions are essential conditions for the categorization of Crohn's disease patients, which enable us to design more effective, preferably causative, individually tailored therapy. and L. Crohnova nemoc se často projevuje čistě jako zánětlivé onemocnění, ale u mnoha pacientů se vyvine v komplikované onemocnění charakterizované tvorbou striktur nebo píštělí. Přesná etiopatogeneze tohoto závažného onemocnění není zcela jednoznačně objasněna, a to i přes veškeré úsilí výzkumu a přes poznání řady dílčích patogenetických mechanizmů. Mnoho studií naznačuje, že se zvýšeným rizikem rozvoje komplikované formy onemocnění souvisí zejména varianty v NOD2/CARD15 genu. Samotná přítomnost variant v NOD2 genu je pouze jen jedním z faktorů přispívajících k rozvoji tohoto onemocnění. U geneticky predisponovaných jedinců v kombinaci s působením faktorů prostředí dochází k porušení vrozené (např. porušená střevní bariéra, dysfunkce Panethových buněk) i adaptivní (např. nerovnováha efektorových a regulačních T lymfocytů a cytokinů, migrace a retence leukocytů) imunitní odpovědi vůči snížené pestrosti střevních komenzálů. Data dosud provedených metaanalýz neposkytují jednoznačné důkazy k podpoře top-down strategie terapie závislé pouze na jediné variantě v NOD2 genu, ale na druhé straně v případě vysoce rizikových pacientů se dvěma variantami v NOD2 genu data ukazují, že cílená a časná intenzivní terapie by mohla být prospěšná. Genetically predisposed individuals in combination with influence of environmental factors result in a disturbed innate (i.e., disturbed intestinal barrier, Paneth cell dysfunction) and adaptive (i.e., imbalance of effector and regulatory T cells and cytokines, migration and retention of leukocytes) immune response towards a diminished diversity of commensal microbiota. V tomto roce uplynulo 25 let od otevření Střediska komplexní terapie psychosomatických poruch v Liberci. Rekapitulujeme okolnosti vzniku, mapujeme teoretické zdroje, ze kterých tým pracoviště vyšel, a na základě autentických dokumentů ilustrujeme vývoj, kterým za čtvrt století na své cestě do neznáma tento projekt prošel. Ambulantní týmové pracoviště zaměřené na psychosomatické pacienty vzniklo na půdě všeobecného lékařství a představovalo neobvyklé řešení v systému zdravotní péče. Při pohledu zpátky se ukazuje, že shodou okolností byly vstupní parametry nastaveny tak šťastně, že tato „porucha“ v systému měla dostatečný potenciál k rozvoji v oblasti, se kterou u nás bylo málo zkušeností a která neměla téměř žádnou podporu oficiálních řídících struktur. Hlavní koncepce pracoviště vznikala v diskusích mezi lékaři a psychology během osmdesátých let, a v době otevření pracoviště byla prakticky hotova. Základní nastavení bio-psycho-sociálního přístupu k pacientům bylo založeno na teorii narativní terapie a rodinné psychosomatiky. Kolem ní se postupně sdružovala řada dalších terapeutických přístupů, gestalt-terapeutický přístup, rogersovský přístup, ericksonovský přístup, psychoanalytické směry, práce s tělem, meditativní směry terapie spojené s jógou, akupunktura a rehabilitace, podle aktuálního složení týmu. Pracovištěm prošlo za 25 let přes 50 pracovníků, terapeutů, lékařů, psychologů, fyzioterapeutů, středního zdravotnického personálu a administrativy. S výjimkou prvních asi tří let prochází pracovištěm stabilně kolem jednoho tisíce pacientů ročně. Lze tedy hovořit o zkušenosti s nejméně 20 000 pacienty trpícími komplikovaným psychosomatickým stonáním. S naším způsobem práce jsme se netajili, vyškolili jsme stovky zdravotnických pracovníků a pomohli jsme založit další podobná pracoviště. Speciálním tréninkem rodinné terapie psychosomatických poruch, akreditovaným pro zdravotnictví, prošlo na 150 lékařů, psychologů a dalších profesí. Další stovky odborníků prošly kurzem o rozhovoru nebo sebezkušenostními výcviky vedenými v gestalt-terapeutickém stylu., This year it has been 25 years since the Complex Therapy Centre of Psychosomatic Disorders in Liberec was opened. We are summarizing the circumstances of the formation, surveying theoretical resources on which the workplace team has been based, and we are illustrating the development on the basis of authentic documents which this project has gone through on its way into the unknown during a quarter of a century. The ambulatory team workplace focused on psychosomatic patients came into existence on the ground of general medicine and represented an unusual solution in the health care system. When looking back it appears that coincidentally, the input parameters were so well-chosen that this „failure“ in the system had sufficient potential for the development in the area with which we had had little experience, and that did not have almost any support of official governance structures. The main workplace concept emerged in the discussions between doctors and psychologists during the eighties, and it was virtually finished at the time of the opening of the workplace. The basic set-up of the bio-psycho-social approach to patients was based on the theory of narrative therapy and family psychosomatics. A number of other therapeutic approaches cropped gradually around it, gestalt therapeutic approach, Rogers’s approach, Erickson’s approach, psychoanalytic trends, work with the body, meditative therapy trends associated with meditation and yoga, acupuncture and rehabilitation, according to the current staff. 45 workers, therapists, doctors, psychologists, physiotherapists, middle medical and administrative personnel have worked in the workplace during 25 years. With the exception of the first three years, about one thousand patients come per year. Therefore it is possible to speak of experience with at least 20 thousand patients with complicated psychosomatic diseases. We have not kept our way of work secret; we have trained hundreds of health workers, and helped found other similar workplaces. 150 doctors, psychologists and other professions have participated in a special training of family therapy of psychosomatic disorders accredited for health care.
