Rachels je autor zastávající novodobou koncepci utilitaristické morálky. Ve své publikaci The End of Life velmi podrobně probírá všechny důležité momenty filosofické a etické diskuse o eutanazii a vytyčuje tak kontury území, na němž se jak zastánci jeho závěrů, tak i jejich odpůrci, musí pohybovat. Hovoří o posvátnosti lidského života, rozebírá různé formy eutanazie, kriticky hodnotí některé důležité distinkce, široce a někdy nekriticky užívané v lékařské etice: rozdíl mezi usmrcením (killing) a ponecháním zemřít (letting die), mezi řádnými a mimořádnými léčebnými prostředky, mezi aktivní a pasivní eutanazií. Důležitým prvkem Rachelsovy reflexe je i diskuse o důležitosti úmyslu (intence) v etickém vyhodnocování lidských skutků., This contribution is the second of a series of articles devoted to the ethical evaluation of euthanasia. It discusses the problems of euthanasia on the background of Rachel´s famous book The End of Life. Rachels as an author is holding a modern concept of utilitarian morality. In his publication The End of Life he discusses in detail all the important moments of philosophical and ethical debate about euthanasia. In this way he also outlines the contours of the territory for his proponents as well as their opponents for recent discussion on euthanasia and end of life decisions. He talks about the problems of conception of the sanctity of human life, discusses the various forms of euthanasia, critically evaluates some important distinctions between terminology in medical ethics: the difference between killing (killing) and allowing to die (letting die), between ordinary and extraordinary means of treatment, the active and passive euthanasia. Tento příspěvek je první z řady článků věnovaných etického zhodnocení euthanasie. Pojednává o preferenčním utilitarismu Petera Singera a ukazuje, jakým způsobem se v rámci tohoto etického systému euthanasie ukazuje jako eticky oprávněná volba. Článek je rozdělený do dvou hlavních částí. V první části jsou předloženy základní principy utilitarismu a uvádí se důležité rozlišení mezi lidskými živočichy a osobami (založené na schopnosti vyšších kognitivních funkcí). Druhá hlavní část článku se podrobně věnuje vyjasnění základních forem euthanasie (vyžádaná, nevyžádaná, nedobrovolná) a argumentuje, že mezi pasivní a aktivní euthanasií neexistuje žádný morálně významný rozdíl. Článek končí konstatováním, že aktivní euthanasie je morálně lepší volbou než pasivní euthanasie., This contribution is the first of a series of articles devoted to the ethical evaluation of euthanasia. It discusses the preference utilitarianism of Peter Singer and shows that euthanasia can be a morally permissible choice. The article is divided into two main parts. In the first part the basic tenets of utilitarianism are set forth, the important distinction between human animals and persons is made (based on the capacity for higher cognitive functions). Few studies concerning the occurrence of cow’s milk allergy with the use of double-blind, placebo controlled food challenge test in adolescents and adult patients suffering from atopic dermatitis exist. Aim: To evaluate the occurrence of cow’s milk allergy in adolescents and adults suffering from atopic dermatitis. Method: Altogether 179 persons suffering from atopic dermatitis were included in the study: 51 men and 128 women entered the study with the average age of 26.2 (s.d. 9.5 years). Complete dermatological and allergological examinations were performed. Results: The positive results in specificIgE and in skin prick tests were recorded in 12% of patients. According to the open exposure tests and double-blind, placebo controlled food challenge tests these patients are only sensitized to cow’s milk without clinical symptoms of allergy. Double-blind, placebo controlled food challenge test confirmed food allergy to cow milk only in one patient (worsening of atopic dermatitis), the oral allergy syndrome was observed in another one patient, occurrence of this allergy was altogether 1.1%. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. The aim of this review is to summarize main aspects of the book Evropská úmluva o lidských právech written by Kmec, J., Kosař, D., Kratochvíl, J., Bobek, M., published in 2012 in Prague. As the title suggests the book itself is focused on Convention for the protection of human rights and fundamental freedom and relating questions, e.g. application to the European court of human rights, case proceeding and case - law. This review briefly informs about the authors, however it ́s mainly concentrated on th e publication ́s content. Cíl: Prezentovat scintigrafický nález u 15leté dívky s Ewingovým sarkomem. Metoda: Patnáctiletá pacientka s nevýznamnou osobní i rodinnou anamnézou byla přijata na dětské oddělení spádové nemocnice pro zhoršující se únavu, nechutenství, váhový úbytek (10 kg), bolesti kloubů a kostí, vysoké laboratorní zánětlivé parametry (CRP 160 mg/l, FW 120/128) a anémii. Byla provedena třífázová scintigrafie skeletu k vyloučení zánětlivého postižení sakroiliakálního skloubení (SIS) vpravo. Pacientce byla dle hmotnosti (59,5 kg) podána aktivita 750 MBq 99mTc-metylendifosfonátu (MDP). Za 10 minut po intravenózní aplikaci radiofarmaka byla provedena 2. fáze třífázové kostní scintigrafie zaměřená na oblast sakroiliakálního skloubení. S odstupem tří hodin byla vyšetřena 3. fáze (kostní). Vyšetření bylo provedeno na dvouhlavé gamakameře AXIS firmy PICKER se semikvantitativním vyhodnocením na počítači. Gamakamera byla osazena kolimátory pro nízké energie s vysokým rozlišením a paralelními otvory. Výsledky: Třífázová scintigrafie skeletu metodou celotělové scintigrafie zjistila ve 3. fázi výrazně skvrnitě zvýšené vychytávání v axiálním skeletu, v kalvě okcipitálně ve střední čáře, v obou kyčelních kloubech a v levé fibule. Diferenciálně diagnosticky nález naznačoval možnost hematologické malignity, proto byla pacientka přeložena na onkohematologické oddělení dětské kliniky FN Olomouc. Zde bylo doplněno vyšetření skeletu pomocí magnetické rezonance (MR), plnohodnotné vyšetření kostní dřeně a trepanobiopsie. Nález podporoval diagnózu Ewingův sarkom, respektive Ewingův sarkom - primitivní neuroektodermový nádor (ES/PNET) s nejasným primárním ložiskem. Pacientka byla předána k další péči na pracoviště Kliniky dětské onkologie do Brna. Zde byl nález uzavřen jako Ewingův sarkom/PNET s primárním ložiskem na levé fibule a s nálezem mnohočetných metastáz ve skeletu a kostní dřeni, T2N0M1, klinické stádium Enneking III. Závěr: Kazuistika poukazuje na význam vysoce senzitivních nukleárně medicínských vyšetření při složité diagnostice onemocnění nejen dětského věku a na jejich přínos z hlediska urychlení dalšího vyšetřovacího postupu a léčby., x, Jana Páterová, Jaromír Bernátek, Blanka Kadllčíková, and Literatura
Průmyslové emise obsahují široké spektrum cizorodých látek ohrožujících zdraví člověka i zvířat, včetně endokrinních modulátorů (endocrine disruptors), které interferují se signálními systémy buněk. Tato skupina xenobiotik poškozuje endokrinní systémy člověka i zvířat nepříznivým vlivem na jejich zdraví, nejen současné populace, ale potenciálně i generací příštích. První z této skupiny cizorodých látek byly identifikovány ty, které ovlivňovaly signální dráhy estrogenů. Dnes je již prokázanou zkušeností, že řada těchto látek nepříznivě působí také na signální dráhy steroidních hormonů a hormonů štítné žlázy. Endokrinní modulátory mohou působit na několika různých etážích signálních drah, včetně alterace činnosti žláz s vnitřní sekrecí, ve kterých tyto hormony vznikají, ovlivněním regulačních enzymů spojených s homeostázou příslušných hladin hormonů nebo modifikací transkripce hormonální odpovědi indukované hormony v jejich cílových orgánech či tkáních. Následující řádky jsou stručnou informací o současném stavu výzkumu v oblasti endokrinních modulátorů a nastíněním nejdůležitějších šancí a problémů výzkumu v předmětné oblasti., Industrial emissions contain a wide spectrum of xenobiotics threatening human and animal health including endocrine disruptors which interfere with signalling systems of cells. This group of xenobiotics cause damage to health not only to present generation but constitutes a potential risk to health of generations to come. The first group of that xenobiotics discovered were those influencinng signalling pathways of estrogens. Up today it has been proven experience that some of the family of xenobiotics have adverse effects on signalling pathways of steroid hormons and thyroideal ones. Endocrine disruptors can act on several etages of the signalling pathways including alterations at the level of endocrine glands producing hormons, by influencing homeostase regulating enzymes or by the modification of transcription of responses induced by hormones themselves in target organs or tissues. Cíl: Cílem studie bylo provést analýzu expozičních parametrů skiagrafického a skiaskopického módu, a porozumět tak fungování expoziční automatiky angiografického systému, který je využíván pro intervenční kardiologické výkony. Dále bylo cílem zhodnotit vliv přídavné filtrace na dávku pacientům. Metoda: Studie byla provedena pro systém Axiom Artis MP se zesilovačem obrazu. Do analýzy byly zahrnuty expoziční parametry plynoucí z funkce expoziční automatiky pro několik skiagrafických módů používaných pro dospělé pacienty, ale stejně tak pro módy používané v pediatrii. Jednalo se hlavně o analýzu následujících parametrů: napětí rentgenky, proud rentgenky, šířka pulzu a přídavná filtrace. Výsledky: Hlavní rozdíl mezi expozičními parametry pro SG a SS režim byl zjištěn pro proud rentgenky a šířku pulzu, menší rozdíl pak pro přídavnou filtraci. Pro AEC platí, že s rostoucí tloušťkou fantomu se zvyšuje nejprve proud rentgenky, šířka pulzu a napětí zůstává konstantní. Při dalším zvětšení tloušťky fantomu narůstá proud a šířka pulzu, napětí je udržováno na konstantní hodnotě. Při ještě větší tloušťce fantomu se zvyšuje i použité napětí a současně se zmenšuje přídavná filtrace. Závěr: Expoziční parametry skiagrafického módu se odlišují od parametrů skiaskopického módu. Skiaskopický mód pracuje s nižším proudem a vyšší filtrací, což znamená snížení dávky na kůži pacientů, ale současně je to spojeno s méně kvalitním obrazem. Použití větší přídavné filtrace není možné pro obézní pacienty kvůli nadměrnému zatížení rentgenky., Aim: The aim of this work was to analyse exposure parameters of cine mode and fluoroscopy mode and therefore to understand the automatic exposure control of an angiography system used for interventional cardiology procedures. The influence of the additional filtration on dose to patients was assessed. Methods: The study was performed on the angiography system Axiom Artis MP with an image intensifier. Exposure parameters selected by the automatic exposure control of many cine modes for adult patients as well as for pediatrie patients were analysed. Mainly it included tube voltage, tube current, pulse width and additional filtration. Results: The main difference between cine mode and fluoroscopy mode was in used tube current and pulse width, smaller difference was between additional filtration. With increasing thickness of the phantom, the AEC makes the tube current increase, pulse width and voltage is kept constant. With still increasing phantom width, current and pulse width increase, voltage is kept constant. Then tube current, pulse width and also voltage increase. With voltage increased, the additional filtration is lowered. Conclusion: The exposure parameters of cine mode were a little bit different from parameters of fluoroscopy mode. Thicker additional filtration significantly lowers the skin dose to patients but cannot be used for obese patients due to excessive X-ray tube loading., Lucie Súkupová, and Literatura
Introduction: Oral Lichen planus (OLP) is chronic inflammatory oral mucosal disease of unknown etiology. Basement membrane damage and T‑cell migration in OLP may be mediated by matrix metalloproteinases (MMPs). We examined the expression of matrix metalloproteinase 9 to support this hypothesis. Materials and methods: The study population consisted of 71 patients with OLP and 10 control patients with oral fibromas. Indirect immunohistochemistry was used for detection of MMP 9 expression (polyclonal rabbit anti‑human MMP antibody). Results: In all cases of OLP, the MMP‑9 expression was seen mainly in the area of lymphocytic inflammatory infiltrate in the lamina propria including lymphocytes within the overlying epithelium. In addition, it was observed in the epithelial keratinocytes, particularly in the stratum basale and stratum spinosum with occasional positivity in the superficial layer. Fibroblasts and endothelium of small vessels in the lamina propria showed MMP9 expression as well. In all cases of oral mucosal fibromas, the MMP‑9 expression was seen only in fibroblasts and in endothelium of small vessels with occasional positivity within the overlying epithelium. Extrakardiální patologie u CT srdce prováděného z jakékoliv indikace nalézáme u 10 až 60 % pacientů v závislosti na populaci, průměru zobrazované oblasti (FOV), podání kontrastní látky, dále na tom, zda vyšetření interpretuje radiolog nebo kardiolog a zda je v dané geografické oblasti praktikována defenzivní medicína. U vyšetření provedených se širokým FOV nalézáme oproti úzkému FOV extrakardiální patologii u dalších zhruba 10% pacientů. Přibližně 19-26% z těchto nálezů vyžaduje další vyšetření. Výhodou identifikace extrakardiálních nálezů je možnost stanovení alternativní diagnózy, která vysvětluje obtíže pacienta (např. pleuritida, hiátová hernie). Hlavní nevýhodou je nízká výtěžnost, finanční náklady a radiační zátěž při následných vyšetřeních. Na druhou stranu u značné části pacientů není i potenciálně významný nález podroben dalšímu vyšetření. Otázkou zůstává, zda riziko přehlédnutí extrakardiální patologie (riziko sporu) je větší než potenciální benefit pro pacienta - to je však společné téma i pro další vyšetření (např. CT páteře)., Non-cardiac pathology in CT imaging of the heart can be found in 10-60% of patients depending on the population, field of view (FOV), administration of contrast material, practicing of defensive medicine, and whether the examination is interpreted by a radiologist or a cardiologist. When large FOV is used, non-cardiac findings can be found in additional 10% of patients compared to small FOV. Around 19-26% of secondary findings require further workup. Identification of non-cardiac findings has its merits: alternative diagnosis that may explain patients complaints (eg. pleurisy, hiatal hernia). Its major drawbacks include low yield of clinically significant pathology, cost and radiation burden of subsequent examinations. On the other hand, in many patients even a potentially significant finding is not subject to further workup. The vexed question whether the risk of overlooking a non-cardiac pathology (risk of litigation) is greater than the potential benefit for a patient is a topic common with other examinations (eg.
