- BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. Therefore, elective neck dissection or radiotherapy of the neck should be considered in patients with high risk of occult metastases or micrometastases. and P. Čelakovský, D. Kalfeřt, K. - AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). - Dna je zánětlivé, metabolicky podmíněné onemocnění indukované krystaly. Prevalence dny stále vzrůstá. V klinické praxi je často špatně diagnostikována a terapie především akutního záchvatu není adekvátní. V první části publikace jsou rozebírány diagnostické možnosti dnavé artritidy. Především je zdůrazněna výhodnost analýzy synoviálního výpotku a přímého průkazu krystalů v polarizačním mikroskopu. Při nedostupnosti materiálu na krystalografickou analýzu je nutné použít kombinaci klinických kritérií tak, jak např. uvádí ve svých doporučeních Evropská liga proti revmatizmu (EULAR). V druhé části je rozebírána terapie dny, která je rozdělena na období asymptomatické hyperurikemie, akutního dnavého záchvatu, interkritického období a chronické tofózní dny. Asymptomatickou hyperurikemii léčíme pouze při opakovaně vyšších urikemiích než 540 ?mol/l nebo při přítomnosti dalších rizikových faktorů a komorbidit. Pro léčbu akutního dnavého záchvatu je důležitější včasné zahájení léčby než výběr preparátu. Alternativou jsou NSA, kolchicin nebo glukokortikoidy. Nově registrovaným lékem pro léčbu refrakterního akutního zánětu je inhibitor IL1 kanakinumab. Při léčbě hyperurikemie se uplatňují režimová opatření, dietní opatření, abstinence a hypourikemická léčba. K dispozici jsou inhibitory xantinoxidázy alopurinol a febuxostat, který je vhodnější u pacientů se středně závažnou renální insuficiencí. Novým lékem pro léčbu těžké refrakterní tofózní dny je peglotikáza. Klíčové slova: dnavá artritida – kolchicin – nesteroidní antirevmatika, Gout is an inflammatory, metabolically conditioned crystal-induced disease. Prevalence of gout is on the increase. In clinical practice it is frequently wrongly diagnosed and the therapy of acute attacks in particular is not adequate. The first part of the publication discusses diagnostic possibilities of gouty arthritis. First of all the advantage of the analysis of synovial exudate and of direct evidence of crystals in the polarization microscope is emphasized. If the material for crystallographic analysis is not available, it is necessary to use a combination of clinical criteria as specified e.g. in the recommendations of the European League Against Rheumatism (EULAR). The second part focuses on the therapy of gout which is divided into the periods of asymptomatic hyperuricemia, acute gouty attack, intercritical and chronic tophaceous gout. Asymptomatic hyperuricemia is only treated when uricemia greater than 540 ?mol/l occur repeatedly, or when other risk factors and comorbidities are present. In the treatment of acute gouty attack its early start is more important than the choice of a preparation. Alternatives are NSA, colchicine or glucocorticoids. A newly registered medicine for the treatment of refractory acute inflammation is the IL-1 inhibitor canakinumab. The treatment of hyperuricemia involves regimen and diet measures, abstinence and hypouricemic therapy. Available are the xanthine oxidase inhibitors, allopurinol and febuxostat; the latter is better suited for patients with moderate renal insufficiency. - Po prvním záchvatu je nutné určit, zda se jednalo o záchvat epileptický nebo neepileptický, a v případě záchvatu epileptického pátrat po možné akutní příčině. Odlišení záchvatů neepileptických a také identifikace záchvatů epileptických provokovaných (akutních symptomatických) je z hlediska stanovení dalšího postupu, ať již diagnostického či terapeutického, zcela zásadní. Pro odlišení jednotlivých typů záchvatů jsou rozhodující údaje v anamnéze se zaměřením na detailní popis průběhu záchvatu a okolností jeho vzniku, poté i výsledky pomocných vyšetření. Akutní symptomatické záchvaty představují až 40 % všech prvních epileptických záchvatů a vznikají v těsné časové souvislosti s probíhajícím postižením centrálního nervového systému (CNS) různé povahy. Jejich příčina je jasně identifikovatelná – nejčastěji se jedná o akutní cévní mozkové příhody, kraniocerebrální poranění, zánětlivá onemocnění CNS, významné jsou ale i vlivy primárně extracerebrální – toxické a metabolické poruchy, užívání léků nebo alkoholu. Pokud jde o první epileptický záchvat neprovokovaný nebo reflexní, je možné jej v případech přetrvávající predispozice s vysokým (> 60 %) rizikem opakování dle nově přijaté definice považovat za epilepsii, případně i zahájit dlouhodobou léčbu antiepileptiky. Po prvním neprovokovaném záchvatu by mělo být vždy doplněno vyšetření mozku magnetickou rezonancí v adekvátním protokolu. Z hlediska posouzení rizika opakování záchvatu má kromě anamnestických údajů význam i případná abnormita v EEG či odchylky v objektivním nálezu svědčící pro postižení CNS. Zahájení léčby je po prvním záchvatu indikováno při vysokém riziku opakování či při zvýšené pravděpodobnosti závažných komplikací dalšího záchvatu., Following a first seizure, it is important to establish whether this was an epileptic or non-epileptic seizure, and to ascertain possible cause of an epileptic seizure. With respect to further management, diagnostic or therapeutic, it is crucial to differentiate non-epileptic from epileptic seizures and to identify provoked epileptic seizures (acute symptomatic). Patient history with a detailed description of the course of a seizure and circumstances under which it happened is instrumental in discriminating between the different types of seizures. Complementary investigations can further support the diagnostic process. Acute symptomatic seizures represent up to 40% of all single epileptic seizures and occur in close chronological proximity to an ongoing central nervous system injury of various origins. The source can easily be identified – most frequently this is an acute cerebrovascular event, craniocerebral injury, inflammatory CNS disorder, while primarily extracerebral effects are also important – toxic and metabolic disorders and drug or alcohol abuse. Unprovoked or reflex single epileptic seizures in patients with persisting predisposition and a high risk (> 60%) of relapse may, according to the new definition, be regarded as epilepsy and long-term antiepileptic treatment may be initiated. MRI under appropriate protocol should always be performed after the first unprovoked seizure. The risk of seizure recurrence can be estimated based on patient history as well as any EEG or physical examination abnormity that suggests an injury of the CNS. Antiepileptic treatment is indicated in patients with a single seizure if there is a high risk of recurrence or an increased likelihood of severe complications of further seizures. Hlavním cílem prvního vyšetření je rozhodnout, zda jde o pacienta se závažným onemocněním – rozvíjející se cévní mozkovou příhodou (CMP) nebo o pacienta s vestibulární neuronitidou. Toto rozhodnutí je nutné ve většině případů učinit pod časovým tlakem a s vědomím, že v akutní fázi CMP nemusí neuroradiologické vyšetření poskytnout potřebné informace. Pečlivě provedené klinické vyšetření vestibulárního a okulomotorického systému postačuje k rozhodnutí, o jaký typ závrativé poruchy se jedná. Při sumarizaci klinických nálezů a stanovení pracovní diagnózy je nutné vycházet ze základní znalosti funkce vestibulárního systému, reflexních okruhů, do kterých je zapojen, a též principu organizace multisenzorického řízení rovnováhy., The following text summarizes management of patients with sudden vertigo. The main aim of the initial examination is to decide whether we are facing a patient with life-threatening disease, e.g. stroke, or a benign disease such as vestibular neuritis. This decision is usually made under a time pressure and knowing that neuroradiology examination may not provide sufficient information. Clinical examination of the vestibular and oculomotor function is sufficient to make the initial decision. Vestibular examination requires basic knowledge of the vestibular system function and reflex circuits involved in the organization and control of the multisensoric balance system. - Syndróm polycystických ovárií (PCOS) je najčastejšou endokrinopatiou u žien fertilného veku a najčastejšou príčinou porúch menštruačného cyklu. Je charakterizovaný hyperandrogénnym stavom (klinickým alebo biochemickým) a ovariálnou dysfunkciou (anovuláciou alebo ultrasonografickým nálezom polycystických ovárií), čo sú zároveň kritériá pre jeho diagnózu podľa Androgen Excess and PCOS Society. Syndróm má mnohopočetné fenotypové prejavy, medzi ktoré patrí okrem uvedených charakteristík aj metabolický syndróm, predovšetkým obezita a inzulínová rezistencia. Diagnóza PCOS je v klinickej praxi pomerne náročná a stále platí, že je diagnózou per exclusionem, po vylúčení iných príčin hyperandrogénneho stavu a chronickej oligo-anovulácie. Vyžaduje si úzku spoluprácu gynekológa, endokrinológa a z hľadiska častých metabolických komplikácií aj internistu, diabetológa a prípadne aj kardiológa. Kľúčové slová: AES kritériá – diagnóza – diferenciálna diagnóza – syndróm polycystických ovárií, Polycystic ovary syndrome (PCOS) is the most frequent endocrinopathy among women of reproductive age and the most frequent cause of menstruation cycle disorders. It is marked by a hyperandrogenic state (clinical and/or biochemical) and ovulatory dysfunction (anovulation and/or ultrasonographic finding of polycystic ovaries), which are also criteria for its diagnosis according to Androgen Excess and PCOS Society. The syndrome has multiple phenotypic expressions, among them besides the above characteristics also a metabolic syndrome, primarily obesity and insulin resistance. Diagnosing of PCOS may be rather exacting in clinical practice and it remains to be a diagnosis per exclusionem, following elimination of other causes of hyperandrogenic state and chronic oligo-anovulation. It requires a close cooperation between a gynecologist and endocrinologist and with regard to frequent metabolic complications also with an internist, diabetologist and possibly cardiologist. - Diastematomyelie je vzácná vrozená vada patřící mezi spinální dysrafizmy a obvykle je asociována se syndromem míšního ukotvení. Diastematomyelie je tvořena dvěma polovinami rozštěpené míchy s vlastními durálními vaky, které jsou rozděleny osteokartilaginózním septem. Tato vada bývá diagnostikována především v dětském věku. U dospělých je vzácná a dominujícím příznakem je bolest. Autoři prezentují kazuistiku ženy ve věku 44 let s diastematomyelií, která si stěžovala na chronické bolesti zad a progredující slabost dolních končetin. U pacientky jsme provedli laminektomii, deliberaci míchy a plastiku durálního vaku. Po operaci pacientka zůstala bez neurologického deficitu a udávala výraznou úlevu od bolestí., Diastematomyelia is a rare congenital defect, a type of spinal dysraphisms, often associated with the tethered cord syndrome. Diastematomyelia is formed by the two halves of the split spinal cord that each has own dural sac separated by a rigid or fibrotic septum. This defect is mainly diagnosed in children. Pain is the dominant symptom in adult patients. The authors present a case of a woman aged 44 years with diastematomyelia who complained of chronic back pain and progressive weakness in her legs. In this patient, laminectomy was performed, the spinal cord released and the dural sac restored. After the surgery, the patient had no neurological deficits and reported significant pain relief.
