Cíl práce: Analyzovat výsledky indukovaných porodů u primipar po ukončeném 41. týdnu těhotenství bez další zátěže. Typ studie: Retrospektivní klinická studie. Název a sídlo pracoviště: Gynekologicko-porodnická klinika 1. LF UK v Praze, Nemocnice Na Bulovce. Materiál: V letech 2011–2013 porodilo na Gynekologicko-porodnické klinice v Nemocnici Na Bulovce celkem 6 317 žen. U 203 prvorodiček byl porod indukován po ukončeném 41. týdnu těhotenství prostaglandinem dinoproston. Do kontrolní skupiny bylo zařazeno 291 primipar se spontánním začátkem porodu po termínu. Metodika: Porovnali jsme u obou skupin věk rodiček, celkovou délku porodu, celkovou krevní ztrátu, rozdíly ve způsobu ukončení porodu, výskyt hypotonie děložní a manuální vybavení placenty, porodní poranění, četnost epidurální analgezie a poporodní adaptaci novorozenců (Apgarové skóre a pH z pupečníkové krve novorozenců). Údaje byly retrospektivně získány ze zdravotnické dokumentace a statisticky zpracovány. Výsledky: Ve sledované skupině jsme zjistili signifikantně vyšší četnost ukončení porodu císařským řezem (21,2 % vs 11,0 %, p < 0,002), statisticky významně vyšší krevní ztrátu (341 ml vs 314 ml, p = 0,002) a četnost podání epidurální analgezie (25,1 % vs 11,7 %, p < 0,001). Další sledované parametry byly srovnatelné u obou skupin. Předpokládaný vliv věku nad 35 let na frekvenci císařského řezu nebyl v této studii prokázán. Závěr: U primipar s indukovaným porodem jsme zjistili výrazně vyšší frekvenci císařských řezů. Délka porodů a frekvence hypotonie děložní byly srovnatelné s kontrolní skupinou. Farmakologická indukce porodu prostaglandinem E2 z důvodu potermínové gravidity představuje u primipar silný rizikový faktor k ukončení císařským řezem., Objective: To analyse effect of induced labor in primiparous women after completed 41st gestation week without any other pathology. Design: Retrospective clinical study. Setting: Department of Gynaecology and Obstetrics, First Faculty of Medicine, Charles Univesity in Prague, Hospital Na Bulovce. Material: Altogether 6 317 women delivered at Hospital Na Bulovce in Prague between years 2011–2013. In 203 primiparous women after completed 41st gestation week, labor was induced by prostaglandin dinoproston. The control group consisted of 291 primiparae with spontaneus onset of labor after the term. Methods: We compared the groups in respect to patient´s age, parity, total duration of labor, total blood loss, frequency of different ways of delivery, frequency of analgesia, frequency of diverse delivery injuries, manual removal of placenta and uterine hypotonia, neonatal outcomes (Apgar score of newborns, and the pH of umbilical arterial and vein blood). Data were retrospectively obtained from the medical documentation and statistically processed. Results: We proved significantly higher frequency of Caesarean sections (21,2 % vs 11,0 %, p < 0,002), statistically significant higher blood loss (341 ml vs 314 ml, p = 0,002), and frequency of epidural analgesia applications (25,1 % vs 11,7 %, p < 0,001) in the study group. Other analysed parameters were similar in both groups. Expected effect of age more than 35 years on frequency of Caesarean section was not proved in the study group. Conclusion: We found a substantially higher frequency of Cesarean sections in primiparous women with induced labor. The duration of delivery and the frequency of uterine hypotonia were similar as in the control group. Anémie je poměrně časté onemocnění zejména u žen a to v těhotenství i mimo ně. Protože její příznaky mají široké spektrum – mohou zhoršovat kvalitu života až být život ohrožující, je třeba na tento zdravotní problém myslet a vyšetřovat ženy, aby se vyloučila přítomnost anémie, a pokud se potvrdí, že je pacientka anemická, adekvátně ji léčit. Klíčová slova: anémie – gynekologie – těhotenství – perinatologie, Anaemia is a common disease in pregnancy, even in non-pregnant women. Symptoms have a wide spectrum – may reduce the quality of life or even life-threatening directly. That is why it is necessary to examine and adequately treated. Antifosfolipidový syndrom (APS) je autoimunitně podmíněné onemocnění s celou řadou potenciálně život ohrožujících manifestací. Vyskytuje se samostatně v primární podobě nebo jako sekundární doprovázející řadu chorobných stavů (autoimunitní, nádorová onemocnění atd.). Diagnostická kritéria zahrnují jak klinickou, tak laboratorní složku. Mezi klinické manifestace patří výskyt trombóz arteriálních či žilních a dále komplikace v těhotenství. Laboratorně musí být opakovaně prokázána přítomnost antifosfolipidových protilátek v minimálním odstupu 12 týdnů. Klinická manifestace APS může být velmi pestrá a často vyžaduje multidisciplinární přístup. Obávanou, ale vzácnou komplikací je tzv. katastrofický APS spojený s vysokou morbiditou a mortalitou. Včasná diagnóza spolu s terapií výrazně ovlivňuje prognózu pacientů s APS. APS occurs in a primary form with no associated conditions, or in a secondary form associated with various other illnesses (autoimmune, neoplastic and others). APS is diagnosed according to clinical and laboratory criteria. It is characterized by recurrent arterial or venous thrombosis and/or pregnancy‑related complications and laboratory findings of antiphospholipid antibodies in a minimum time interval of 12 weeks between individual laboratory tests. Clinical manifestations of APS can vary and a multidisciplinary approach is needed. Catastrophic APS is a very serious and life‑threatening condition associated with high mortality and morbidity, although it is rare. Cíl studie: zhodnocení efektivity cerkláže dle McDonalda ve Fakultní nemocnici Ostrava v období 1/2007 – 3/2014. Typ studie: retrospektivní studie Název a sídlo pracoviště: Porodnicko–gynekologická klinika, Fakultní nemocnice Ostrava Metodika: retrospektivní analýza provedených cerkláží ve Fakultní nemocnici Ostrava a rozbor výsledků takto ošetřených gravidit. Kritériem úspěšnosti byl stanoven porod po ukončeném 34. týdnu gravidity. Závěr: ve sledovaném období bylo indikováno devět cerkláží – 7 terapeutických a 2 záchranné. Výkony byly provedeny mezi 18. a 25. týdnem gravidity. Tři pacientky již měly v minulosti cerkláž provedenu. 57 % takto ošetřených těhotných porodilo po ukončeném 34. týdnu gravidity. Pouze v jednom případě (11,1 %) byla cerkláž neúspěšná a těhotenství skončilo potratem. Průměrná doba mezi provedením cerkláže a porodem činila 69 dnů., Objective: To evaluate the efficacy of cerclage according to McDonald at the University Hospital Ostrava in the period 1/2007 - 3/2014. Design: A retrospective study Obstetrics and Gynecology: Obstetrics - Gynecology, University Hospital Ostrava Methods: A retrospective analysis of cerclage performed at the University Hospital Ostrava and analysis of the results thus treated pregnancies. The criterion for success was determined after childbirth completed 34 weeks of preg - nancy. Conclusion: In the reporting period, nine indicated cerclage - 7 of therapeutic and 2 emergency. The procedures were performed between the 18th and 25th week of pregnancy. Three patients have had in the past cerclage performed. 57% of the treated pregnant women gave birth after 34 completed weeks of gestation. Only in one case (11.1 %) cerclage was unsuccessful and the pregnancy ended in a miscarriage. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Trend odkládání reprodukce vede k vyššímu procentu plánovaných početí. To umožňuje prekoncepční intervenci. Nejlépe dokumentován je význam podávání folátů, které snižují výskyt defektů neurální trubice, vrozených srdečních vad, vrozených vad uropoetického traktu, plodů s nižší porodní váhou, patologií placenty a spontánních potratů. Suplementaci foláty minimálně měsíc před otěhotněním a v 1. trimestru doporučuje 17 německých odborných společností. Metylfolát [(6S)-5methyltetrahydrofolát] je biologicky aktivní folát, který není závislý na aktivitě MTHFR. K dosud jediné dostupné kalciové soli (označované jako 3. generace folátu) nově přibývá glukosaminová sůl metylfolátu (4. generace folátu), charakterizovaná vyšší stabilitou, rozpustností a biologickou dostupností., The tendency of reproduction delay leads to a higher rate of planned conception. This allows preconception intervention. The importance of folate administration is well documented. Folates reduce the incidence of neural tube defects, congenital heart defects, congenital anomalies of urinary tract, foetuses with lower birth weight, placental pathology and spontaneous abortion. Seventeen German professional societies recommend folate supplementation for at least one month before pregnancy and in the first trimester. Methylfolate [(6S) -5methyltetrahydrofolic acid] is the biologically active folate which is not dependent on the activity of MTHFR.
