Úvod: Rozsah nádoru a kvalita jeho chirurgického odstranění patří k hlavním prognostickým faktorům v léčbě karcinomu rekta. Počet pooperačních lokálních recidiv závisí na úplnosti odstranění mezorekta – TME (total mesorectal excision) – a dosažení bezpečných resekčních okrajů bez nádoru. Provedení a kvalita operace záleží na předoperační diagnostice a detekci rizikových míst na rektu a mezorektu, na chirurgické schopnosti výkon správně provést a na patologickém vyšetření, hodnotícím úplnost odstranění nádoru v parametrech resekčních okrajů včetně cirkumferentního (CRO) a kompletnost mezorektální excize. Cílem práce bylo zavést a standardizovat metodu hodnocení kvality chirurgického výkonu − mezorektální excize – v léčbě karcinomu rekta hodnocením resekčních okrajů včetně CRO a kompletnosti excize. Metody: Studie měla dvě části. V první, retrospektivní části multicentrické studie byly v sestavě 288 pacientů analyzovány jednotlivé dílčí parametry diagnostiky, operací a histologického vyšetření karcinomu rekta, identifikována kritická místa a připraven jednotný protokol sledování. Ve druhé, prospektivní části studie byla v sestavě 600 pacientů tato data sledována parametricky a byla hodnocena kvalita TME ve vztahu k onkologickému výsledku léčby. Výsledky: V předoperační diagnostice se zvýšil podíl pacientů, kterým byl proveden restaging po neoadjuvantní léčbě z 60,0 % na 81,7 %. Podíl resekátů, u nichž nebyla hodnocena kvalita excize mezorekta, se snížil z 52,9 % v retrospektivní části na 22,8 % v části prospektivní. Podíl kompletních excizí mezorekta se zvýšil z 22,6 % na 26,0 %, podíl téměř kompletních excizí se zvýšil z 10,1 % v retrospektivní části na 24,0 % v prospektivní části studie. Závěr: Zavedení parametrického sledování do rutinní praxe zvýšilo kvalitu předléčebné a předoperační diagnostiky, vyšetření resekátu patologem a vlivem zpětnovazebné informace také kvalitu chirurgického výkonu., Introduction: Tumour size and the quality of its complete surgical removal are the main prognostic factors in rectal cancer treatment. The number of postoperative local recurrences depends on whether the mesorectum has been completely removed – total mesorectal excision (TME) – and whether tumour-free resection margins have been achieved. The surgery itself and its quality depend on the accuracy of preoperative diagnosis and detection of risk areas in the rectum and mesorectum, on the surgeon’s skills, and finally on pathological assessment evaluating whether complete tumour excision has been accomplished including circumferential margins of the tumour, and whether mesorectal excision is complete. The aim of our study was to implement and standardize a new method of evaluation of the quality of the surgical procedure – TME – in rectal cancer treatment using an assessment of its circumferential margins (CRO) and completeness of the excision. Methods: The study consisted of two parts. The first, multi-centre retrospective phase with 288 patients analysed individual partial parameters of the diagnosis, operations and histological examinations of the rectal cancer. Critical points were identified and a unified follow-up protocol was prepared. In the second, prospective part of this study 600 patients were monitored parametrically focusing on the quality of the TME and its effect on the oncological treatment results. Results: The proportion of patients with restaging following neoadjuvant therapy increased from 60.0% to 81.7% based on preoperative diagnosis. The number of specimens missing an assessment of the mesorectal excision quality decreased from 52.9% in the retrospective part of to the study to 22.8% in the prospective part. The proportion of actually complete TMEs rose from 22.6% to 26.0%, and that of nearly complete TMEs from 10.1% to 24.0%. Conclusion: The introduction of parametric monitoring into routine clinical practice improved the quality of pre-treatment and preoperative diagnosis, examination of the tissue specimen, and consequently improved quality of the surgical procedure was achieved., and J. Úvod: Důležitou součástí diagnostického algoritmu neuropatické bolesti jsou jednoduché dotazníkové nástroje založené na přítomnosti typických popisných charakteristik (tzv. deskriptorů). V českém jazykovém prostředí však není k dispozici validovaná verze žádného z mezinárodně akceptovaných dotazníků. Cíl: Cílem práce bylo vytvoření české jazykové verze jednoho z nejpoužívanějších dotazníků zaměřených na diagnostiku neuropatické bolesti a charakteristiku jednotlivých jejích subtypů, a to tzv. NPSI (Neuropathic Pain Symptom Inventory) a její validace. Soubory a metodika: Dotazník byl přeložen metodou „forward‑backward translation“. Pro potřeby validace byla touto českou jazykovou verzí následně vyšetřena skupina pacientů s neuropatickou bolestí periferního (n = 66) nebo centrálního (n = 30) původu (v rámci polyneuropatie či roztroušené sklerózy) a skupina nemocných s bolestí nociceptivní v důsledku pokročilé artrózy kyčelního nebo kolenního kloubu (n = 70). Za účelem hodnocení opakovatelnosti testu byl u části pacientů (n = 19) dotazník administrován dvakrát s odstupem přibližně tří hod. Výsledky: Srovnání jednotlivých hodnocených položek, dílčích škál i celkového NPSI skóre pomocí ROC analýzy prokázalo vysokou schopnost diskriminace mezi neuropatickou a nociceptivní bolestí (p < 0,001), ale nízkou schopnost odlišení neuropatické bolesti centrálního a periferního původu (p > 0,05). Opakovaná administrace testu potvrdila vysokou opakovatelnost testu (ICC, Intraclass Correlation Coefficient = 0,86 pro sumární skóre). Pomocí clusterové analýzy bylo identifikováno šest odlišných symptomových profilů. Závěr: Vytvořená česká verze dotazníku NPSI je schopna spolehlivě diskriminovat neuropatickou a nociceptivní bolest a identifikovat odlišné symptomové profily bolesti při akceptovatelné opakovatelnosti testu. Dotazník lze tedy doporučit jako vhodný a snadno použitelný nástroj v diagnostickém algoritmu neuropatické bolesti, využitelný i pro longitudinální sledování., Introduction: Simple questionnaires based on so called “neuropathic pain descriptors” are the most frequently used screening tools in the diagnosis of neuropathic pain, especially by non‑specialists. However, no validated Czech version of any of the internationally accepted nueropathic pain questionnaires is available. Aim: The aim of this study was to develop and validate a Czech version of the Neuropathic Pain Symptom Inventory (NPSI), a questionnaire that allows the user to characterize subtypes of neuropathic pain, is sensitive to change, and has discriminatory power. Patients and methods: The NPSI was translated into the NPSIcz using the forward‑backward translation method and validated in a group of patients suffering from neuropathic pain of peripheral (n = 66) or central origin (n = 30) due to polyneuropathy or multiple sclerosis, and in a group of individuals suffering from nociceptive pain (due to severe osteoarthritis of the knee or hip joint; n = 70). The NPSIcz was repeated within three hours in 19 individuals to determine test‑retest reliability. Results: The NPSIcz showed good diagnostic validity in discriminating between patients with neuropathic and nociceptive pain (p < 0.001) but low ability to discriminate between patients with central and peripheral neuropathic pain (p > 0.05). Repeated administration confirmed acceptable test‑retest reliability (intra‑class correlation coefficient of 0.86 for the NPSIcz sum score). Cluster analysis identified six different pain profiles. Conclusions: The NPSIcz is able to discriminate between neuropathic and nociceptive pain, to identify different pain profiles, and has acceptable test‑retest reliability. It can, therefore, be recommended as an easy‑to‑administer questionnaire suitable for the diagnosis of neuropathic pain, including long‑term follow up. Cílem studie bylo prozkoumat vztah mezi sociální oporou, socioekonomickými determinantami a průběhem onkologické léčby. Byl sledován výskyt komplikací souvisejících s toxicitou protinádorové terapie, výskyt viróz během 6 měsíců po operaci a pooperační komplikace. Ve studii participovalo 139 pacientek s karcinomem prsu (medián věku 65 let), sociální opora (SO) a socioekonomický status (SES) byly hodnoceny na základě strukturovaného interview. SES byl odhadován na základě kategorií ekonomického příjmu a dosaženého vzdělání. Ze zdravotnické dokumentace byly do analýz zařazeny údaje o výskytu obecně léčebných komplikací, komplikací po operaci a o výskytu infekčních onemocnění typu viróz do 6 měsíců po operaci. Statistická významnost rozdílů v kategoriálních proměnných mezi testovanými skupinami byla hodnocena Fisherovým přesným testem. Statisticky významné rozdíly mezi skupinami s a bez SO byly zjištěny u celého souboru pacientek v méně častém výskytu viróz u pacientek se SO (bez viróz 70,3 % vs 48,3 % bez SO). U pacientek se SO ve věku do 60 let byl zjištěn nižší výskyt pooperačních komplikací (56,7 % vs 0 %) i obecně nižší počet komplikací v souvislosti s onkologickou léčbou (více než 4 komplikace se projevily u 47,1 % se SO vs 100 % bez SO). U pacientek se SO ve stadiu nemoci II–III byl zjištěn nižší výskyt viróz (bez viróz 63,2 % vs 28,6 %) a nižší četnost komplikací (více než 4 komplikace 35,5 % vs 73,3 %). Statisticky významný vztah (P = 0,015) byl zjištěn mezi vzděláním a počtem komplikací u pacientek se stadiem II–III, u pacientek se základním a učňovským vzděláním byl vyšší podíl více než 4 komplikací (55,9 %) oproti skupině se středním a vyšším vzděláním (32,6 %). Zvýšený trend byl nalezen pro vztah mezi SES a výskytem komplikací souvisejících s onkologickou léčbou (P = 0,06) ve skupině pacientek s karcinomem prsu ve stadiu II–III.Zjistili jsme indicie pro vztah sociální opory a průběh onkologické léčby pro starší pacientky napříč klinickými stadii a pro vztah SES a výskyt léčebných komplikací pro starší ženy ve stadiu II–III. Sociální opora je velmi důležitý faktor pro zdraví pacientek, významnou pozitivní roli hraje také vyšší socioekonomický status a vzdělání. Klíčová slova: karcinom prsu – onkologická léčba – sociální opora – socioekonomický status, The objective of the study was to investigate the relationship between social support, socioeconomic determinants and oncological treatment progress in patients with complications due to the toxicity of antitumor therapy, viral diseases during 6 months following surgery, as well as postoperative complications. The study sample consisted of 139 female patients with breast cancer (age median: 65 years), and social support (SS) and socioeconomic status (SES) were evaluated based on a structured interview. SES was estimated based on income and education classification. The analyses performed included information from patients’ documentation, including data on the occurrence of general therapy complications, postoperative complications, and the occurrence of infectious viral diseases within the first 6 months following surgery. The level of statistical significance of the differences within category-specific variables between the analysed groups was evaluated with Fisher’s exact test. The differences in statistical significance between groups with and without SS were found within the entire group of patients, while viral diseases occurred less in patients with SS (no viral diseases 70.3% vs 48.3% without SS). Patients with SS and below 60 years of age experienced a lower occurrence of postoperative complications (56.7% vs 0%), as well as generally lower numbers of complications resulting from oncological treatment (more than 4 complications occurred in 47.1% of patients with SS vs 100% of patients without SS). Patients with SS in the Stage II–III experienced a lower occurrence of viral diseases (63.2% no virosis vs 28.6%) and a lower occurrence of complications (35.5% more than four complications vs 73.3%). A statistically significant relationship (P = 0.015) was determined between the education of patients and the number of complications in patients in Stage II–III: more than four complications occurred in patients with basic and vocational education (55.9%), compared to patients with secondary and higher education (32.6%). An increasing trend was determined for the relationship between SES and the occurrence of complications due to oncological treatment (P = 0.06) within the group of patients with Stage II–III breast cancer. We have found evidence supporting the idea of a relationship between social support and oncological treatment for older patients across clinical stages and for the relationship between SES and the occurrence of complications for older women in Stage II–III. Social support is a factor which is of great importance for patient health, while higher socioeconomic status and education play a significant role, too.
