Úvod: Pruritus ani je definován jako kožní onemocnění charakterizované svěděním a/nebo pálením v perianální oblasti. Postihuje 1–5 % populace. Muži jsou postiženi častěji než ženy v poměru 4:1. Projevuje se neodolatelnou touhou po škrábání v perianální oblasti. Dělí se na dva podtypy: primární (idiopatický) a sekundární. Primární pruritus je takový, u kterého se nepodaří najít žádnou příčinu svědění. Sekundární pruritus má zjevnou kauzální příčinu. Cílem práce je nabídnout čtenáři komplexní pohled na možné příčiny, diagnostické postupy a možnosti léčby tohoto pro pacienty nepříjemného a obtěžujícího onemocnění. Metody: Pomocí databází PubMed a MEDLINE jsme prozkoumali literaturu se zaměřením na články týkající se análního pruritu. Nejprve bylo zadáno klíčové slovo „Pruritus ani“ bez omezení. Následně jsme výběr limitovali časovým obdobím 5 let a 10 let, dále na vyhledání článků v anglickém jazyce, německém jazyce a v českém jazyce a v poslední řadě na výběr přehledových článků (review), klinických studií a ostatních. Výsledky: Celkem bez omezení bylo nalezeno 574 článků, z toho 45 přehledových a 25 klinických studií. Článků v anglickém jazyce bylo 437, z toho 40 přehledových, v německém jazyce 44, z toho 1 přehledový. Po zadání časového omezení bylo v posledních 5 letech nalezeno celkem 33 článků, z toho 6 přehledových a 4 klinické studie, v posledních 10 letech celkem 66 článků, z toho 14 přehledových a 10 klinických studií. Ve většině ostatních článků z celkového počtu byl anální pruritus zmiňován jen okrajově jako součást článků zaměřených na jinou tématiku. V českém písemnictví nebyl nalezen žádný souhrnný článek týkající se tohoto tématu. Závěr: Pruritus ani je běžné onemocnění s množstvím příčin, a proto účinná léčba může být zpočátku nedostatečná. Terapie je zaměřena na základní příčinu, pokud je nalezena. Musí být brána v úvahu široká diferenciální diagnostika a přehodnocení terapie je prvořadé. Při absenci jasné sekundární příčiny je empirická léčba zaměřena na zlepšení anální hygieny, změnu životního stylu, odstranění běžných dráždidel a na ochranu perianální kůže. It occurs in 1−5% of the population. Men are affected more frequently than women, in the ratio 4:1. It is accompanied by an irresistible desire to scratch in the perianal area. Pruritus ani is divided into two subtypes: primary (idiopathic) and secondary. In idiopathic (primary) pruritus it is not possible to detect any other cause of itching. Secondary pruritus has an obvious causal origin. The aim of this paper is to offer a complex overview of possible causes, diagnostic procedures and treatment possibilities of this unpleasant and annoying disease. Methods: We have researched available publications using PubMed and MEDLINE databases, focusing on articles on anal pruritus. At first the key word “Pruritus ani” was put in without any restrictions. Subsequently, we limited the selection by the time period of 5 years and 10 years; then we looked up articles in English, German and Czech languages, and finally review articles, clinical trials and others. Results: 574 articles were found without entering any restrictions; 45 of them were review articles and 25 clinical trials. 437 articles were in the English language and 40 of them were review articles. 44 were in the German language and 1 of them was a review article. A total of 33 articles were found with a 5-year time limit. 6 of them were review articles and 4 were clinical trials. 66 articles from the last 10 years were found. 14 of them were review articles and 10 were clinical trials. In most of the other articles among the total number of articles found, pruritus ani was mentioned only marginally in articles focused on different topics. We have not found any summary articles on this topic in Czech publications. Conclusion: Pruritus ani is a common disease with a number of causes; therefore, effective treatment may be insufficient in the initial stages. The therapy is focused on the primary cause, if found. Broad differential diagnosis options need to be taken into consideration, and reevaluation of the therapy is a priority. When no obvious secondary cause is found, the empiric treatment is focused on an improvement of hygiene and change in the life style, removal of common irritators, and protection of perianal skin. Poranění anorekta, s výjimkou iatrogenních poškození, jsou vzácná. Vzhledem k široké škále příčin a rozsahu poranění vyžaduje diagnostika a léčba těchto poranění individuální přístup ke každému pacientovi. Dle rozsahu poranění je třeba zvolit vhodnou léčebnou strategii tak, abychom úspěšně ošetřili toto poranění a zároveň co nejvíce eliminovali četné komplikace těchto poranění, zvláště pak poruchy kontinence. Autoři prezentují poranění hráze a anorekta s poraněním svěračů u staršího pacienta po pádu ze žebříku., Anorectal injuries, with the exception of iatrogenic damage, are rare. Considering the extensive range of causes and potential extent of damage, the diagnosis and treatment of these injuries requires an individual approach to every patient. Based on the extent of damage, the best way of treatment strategy has to be selected for successful treatment of the primary injury and elimination of frequent complications, especially fecal incontinence. Úvod: Adjuvantní radioterapie je nedílnou součástí současné léčby celé řady maligních tumorů. Kromě benefitů ve smyslu snížení rizika recidivy však vede také k celé řadě nežádoucích účinků, z nichž jedním z nejzávažnějších je vznik sekundárních malignit. Kazuistika: Autor popisuje kazuistiku 77leté ženy, která prodělala v roce 2005 hysterektomii s oboustrannou adnexektomií a pánevní lymfadenektomií pro karcinom dělohy pT1bN0M0, G1. Následovala ještě adjuvantní zevní radioterapie dávkou 45Gy + brachyterapie 3x4Gy. Poté byla dispenzarizována gynekologickou klinikou, odkud byla odeslána v lednu 2015 pro rychle rostoucí tumor kůže a podkoží v hypogastriu v místě původního ozáření. Byla indikována k jeho excizi s podezřením na metastázu původního karcinomu dělohy, následné histologické vyšetření preparátu však odhalilo, že se jedná o angiosarkom. I přes negativní resekční okraje došlo k brzké recidivě s generalizací tumoru. Závěr: Angiosarkom jako následek ozáření bývá v současnosti popisován stále častěji. V drtivé většině případů se jedná o následek adjuvantní terapie po operaci karcinomu prsu. Jeho prognóza je velmi nepříznivá a terapeutické možnosti omezené. Základem léčby zůstává chirurgická excize s negativními okraji. Je třeba na tuto diagnózu myslet a pacienty podstupující radioterapii dlouhodobě dispenzarizovat., Introduction: Adjuvant radiotherapy is an integral part of the current treatment of many malignant tumours. However, this mode of treatment does not provide only benefits in the sense of lowering the risks of recurrence, but it is also associated with many undesirable side effects, from which one of the most serious is the development of secondary malignancies. Case report: The author describes a case report of 77 years old woman who underwent hysterectomy with bilateral adnexectomy and pelvic lymphadenectomy for uterine carcinoma pT1bN0M0, G1 in 2005. Adjuvant external radiotherapy with the dose 45Gy and brachytherapy 3x4Gy followed. Subsequently the patient was followed at the department of gynecology, which referred her in January 2015 with a quickly growing tumour of the skin and subdermal tissue in the hypogastric area, where previous radiation had been applied. Its excision was indicated with the diagnosis of suspected uterine carcinoma metastasis, but subsequent histological evaluation confirmed angiosarcoma. Despite negative resection margins the tumour quickly recurred and disseminated. Conclusion: Angiosarcoma as a side effect of irradiation is described increasingly more often nowadays. The vast majority of cases are related to the treatment of breast carcinoma. Its prognosis is very poor and therapeutic possibilities are limited. Surgical excision with negative margins remains the mainstay of the treatment.
