V letech 1959–2006 bylo u českých mužů evidováno 81 884 nových karcinomů prostaty (KP) s nárůstem z 12,3 na 96,1 na 100 tis. mužů. Úmrtnost se zvýšila z 21,3 na 27,2 na 100 tis. mužů. Podíl KP ze všech nádorů se zvýšil u incidence ze 7,1 % na 13,2 %, u mortality z 6,7 % na 8,9 %. V letech 1989–2005 vzrostla prevalence KP z 98 na 408,5 na 100 tisíc mužů a dosáhla téměř 20 500 žijících. Z 11 183 nemocných s vícečetnými KP bylo evidováno 4 608 primárních a 6 575 následných případů, představujících 17,4 % ze všech evidovaných KP v letech 1976–2005. Z primárních KP bylo léčeno 89,7 % duplicit a 10,3 % multiplicit. Počet primárních KP vzrostl na 282 v roce 1996 a pak klesal, následných KP trvale rostl na 705 v roce 2005. Téměř polovina KP se vyskytla ve věku 70–79 let a třetina ve věku 50–69 let. Poměr 1 115 (21,3 %) synchronních a 4 121 (78,7 %) metachronních následných novotvarů byl 1:3,7 s průměrnou dobou vzniku následného novotvaru 4,1 roku. Z 5 236 následných novotvarů po primárních KP byly nepočetnější nádory kůže 28,6 %, trávicí 26,7 %, močové 17,8 % a respirační 10,9 % soustavy. Z 5 613 novotvarů před KP jako druhou diagnózou, bylo 35,3 % nádorů kůže, 21 % trávicí, 19,6 % močové a 8,7 % respirační soustavy. Z nemocných s primárním KP k 17. 10. 2007 žilo 22,4 % a zemřelo 77,6 %, s následným KP žilo 31,6 % a zemřelo 68,4 % mužů. Vzhledem k výskytu KP ve spojení s dalšími novotvary není u české populace zatím dostatečná intervence ke zdravému životnímu stylu také vzhledem k očekávanému počtu 42 tisíc mužů, žijících s KP v roce 2015., In Czech men there were registered 81,884 new prostate cancers (PC) during 1959–2006, increased from 12.3 to 96.1 per 100,000 men. The average time between the primary and subsequent neoplasms was 4.1 years. The most frequent from 5,236 subsequent neoplasms after primary PC there were 28,6 % cancers of skin, 26,7 % gastrointestinal cancers, 17,8 % of urinary and 10,9 % in respiratory tract. The most frequent from 5,613 neoplasms before PC as the second diagnosis there were 35,3 % cancers of skin, 21 % gastrointestinal, 19,6 % urinary and 8,7 % respiratory tract. From patients with primary PC survived 22,4 % and died 77,6 %, from subsequent PC survived 31,6 % and died 68,4 % men up to 17. 10. 2007. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. U deprese ve vyšším věku hraje menší roli heredita, významnější jsou biologické faktory včetně nacházených strukturálních změn CNS a psychosociální stres. Udává se, že 1?4 % starších lidí trpí depresivní poruchou. Deprese v pozdním věku je charakterizována podobnými příznaky jako u mladších pacientů, jsou tu však některé významné rozdíly (nemocný si spontánně nestěžuje na depresivní náladu, častější je přetrvávající anhedonie, úbytek hmotnosti, psychotické příznaky a suicidální jednání). Z hlediska diferenciální diagnózy jsou důležité sekundární deprese. V posledních letech pozorujeme nárůst dokonaných sebevražd ve starší věkové kategorii, což zřejmě souvisí i se stárnutím populace. Ve věkové skupině nad 75 roků je výskyt 4krát vyšší než v mladší věkové kategorii. Dokonané sebevraždy ve vyšším věku se nicméně podílejí jen v 1 % na celkové mortalitě u mužů a na 0,7 % mortality žen, naproti tomu v mladším věku se podílejí 20 % u mužů a v 13,5 % u žen. Obecně lze sebevraždám předcházet vzděláváním lékařů a omezením přístupu ke smrtícím prostředkům. V praxi je u starší populace důležitá včasná diagnostika, léčba a identifikace rizikových a protektivních faktorů. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Demografické stárnutí se dotýká oblasti ekonomické, sociální i zdravotněpolitické. Zdravotní a funkční stav starých lidí charakterizuje vysoký podíl osob s různým stupněm závislosti. Jedním z hlavních úkolů a priorit zdravotní politiky ČR by proto mělo být zajištění dostupné a kvalitní zdravotní a zdravotně-sociální péče o dlouhodobě nemocné a nesoběstačné seniory. Prodlužování lidského života bez zajištění jeho dostatečné kvality, tedy zachování soběstačnosti, kontaktu se světem a plnohodnotného prožívání, nastoluje naléhavé otázky ceny života, lidské důstojnosti a s nimi spojené potřeby zajištění kvalitní rehabilitační a ošetřovatelské péče. Článek se zabývá praktickými problémy dostupnosti a financování dlouhodobé zdravotně-sociální péče o seniory i seniorského bydlení, terénními službami a problematikou podpory pečujících rodin. Upozorňuje na nejzávažnější problémy péče o seniory, tedy na ageismus v naší společnosti a na neřešené problémy dostupnosti a kvality péče o staré lidi, s nimiž se setkávají odborníci z oblasti geriatrie, poskytovatelé i samotní uživatelé zdravotnických a sociálních služeb a jejich rodiny. Jde o systémové problémy, kdy jsou opomíjeny základní principy zachování spravedlnosti a sociální rovnosti (equity) v péči o staré lidi., Proportion of older people in the Czech population is quickly growing up and the consequences of this fact can be observed in all other fields of social life. Demographic ageing has its economic, social and health implications. For the health and functional status of elderly people the high portion of dependency in various stages is characteristic. Accessible and high-quality health and social care for people with long-term illness and dependency should be one of the priorities of the national health policy. Prolonging the human life without assurance of its quality which means self-sufficiency, contact with the world around and experiencing a full life brings fundamental questions of the value of the human life and human dignity in connection with the need to assure high-quality physiotherapy and nursing care. The article is concerned with the practical problems of accessibility and financing of the long-term care for seniors, with the community services and caring families. The subject of liability for damage to client caused during performance of social services in residential establishments becomes increasingly relevant. As opposed to the liability for damage caused during performance of health services, this subject is rather underappreciated by general members of the legal profession. For this reason RILSA produced a study „Legal aspects of liability for damage caused to a client while providing a social-health care in residential establishment for social services“ that aimed to find out the level of knowledge among the providers of residential social services in the area of liability for damage. To find out the level of legal knowledge of providers of residential social services, a question form was distributed randomly in the Czech Republic. We found out, that providers of residential social services know about the existence of liability for damage and that they are insured. But in the case of significant incurred damage or in the case of more damages within one year the insurance coverage limit might not be sufficient.
