Kožní metastázy se mohou objevit u jakéhokoliv maligního nádoru (1) a u více než poloviny jde histologicky o adenokarcinom. Metastázy adenokarcinomu ovaria do kůže a podkoží jsou vzácným nálezem, ale mohou však být první známkou onemocnění. Tvoří asi 4 % kožních metastáz u žen a bývají lokalizovány v pupku, na vulvě a na proximálních částech stehen. Uvádíme případ neobvyklého masivního rozsevu kožních metastáz, které byly prvním příznakem generalizace onemocnění., Cutaneous metastases may occur in any malignancy, (1) of which adenocarcinomas represent more than one half. Skin metastases of ovarian carcinoma are rarely seen in clinical practice, however they can be the first sign of the disease. Aspergillus sp. are ubiquitous mould infections and in most patients, the source is presumed to be air-borne infections during surgical procedures. Prevention of these infections requires special attention of ventilation systems in operating rooms. Post-operative aspergillosis occurs mainly in immunocompromised patients as well as those who receive corticosteroids temporarily. Cíl: Cílem sledování bylo zhodnotit efektivitu zavedených nefarmakologických aktivit na podporu a zachování kognitivních funkcí na ženském gerontopsychiatrickém oddělení psychiatrické léčebny v Kroměříži a zhodnotit spokojenost hospitalizovaných klientek s těmito aktivitami. Metodika: Na začátku a ke konci hospitalizace bylo použito škály Mini-Mental State Examination (MMSE) a testu kreslení hodin (CDT) k orientačnímu zhodnocení kognitivních funkcí a na závěr hospitalizace bylo použito řízeného rozhovoru. Výsledky: Ke konci hospitalizace nebylo prokázáno statisticky signifikantního zlepšení ve stavu kognitivních funkcí (krátké období pozorování je nejpravděpodobnějším důvodem). Klientky však hodnotí psychoterapeutické aktivity k ovlivnění kognitivních funkcí kladně. Závěr: Nefarmakologické postupy mají významné místo v léčbě demence a doplňují farmakologické i podpůrné léčebné postupy., Aim: Aim of the study was to evaluate the effectiveness of non-pharmacological approaches influencing cognitive functions that are used in routine practice in inpatient women psycho-geriatric ward in Mental hospital in Kromeriz. Furthermore the study tried to evaluate satisfaction with these activities in hospitalised clients. Methods: At the beginning and at the end of hospitalisation was used range of Mini-Mental State Examination (MMSE) and Clock Drawing Test (CDT) to make an indicative assessment of cognitive functions, and at the end of hospitalization was used a controlled interview. Results: At the end of hospitalisation there was no significant evidence of improving cognitive functions (short period of research is the most probable reason). Nevertheless, psychotherapeutic activities were evaluated by clients positively. Autoři předkládají přehledový článek shrnující současný pohled na diagnostiku a léčbu neuroendokrinních nádorů pankreatu. Zmiňují aktuální TNM klasifikaci těchto neoplazií. Ze zobrazovacích metod podtrhují důležitost CT, MRI, EUS a octreotidové scintigrafie. V léčbě zdůrazňují snahu o radikální (R0) resekci, která přináší nejlepší dlouhodobé výsledky. Připouštějí i možnost debulking resekcí a transplantace jater v případě resekabilního origa a neresekabilních jaterních metastáz. Poukazují na rozdílnou prognózu pankreatických neuroendokrinních nádorů a karcinomu pankreatu. Zdůrazňují nutnost mezioborové spolupráce, kterou dokládají dvěma kazuistikami pacientů s lokálně neresekabilním a metastatickým nádorem., The authors put forward a review article summarizing current opinion in diagnostics and therapy of pancreatic neuroendocrine tumors. They present actual TNM classification of these tumors highlighting CT, MRI, EUS and octreoscan as standard imaging methods. In the field of therapy, radical, R0 resection, which brings the best overall survival, is accented. They also accept the possibility of debulking resections and liver transplantation in the cases of local advanced and metastatic disease respectively. They point out the different prognosis of pancreatic neuroendocrine tumors and pancreatic cancer. Autoři popisují některé komplikace sekundárního lymfedému po onkologické léčbě u žen a jejich léčbu. Existují časté komplikace: infekční (erysipel, herpes zoster), progrese primárního nádoru a metastázy, hand-foot syndrom jako vedlejší efekt chemoterapie a relativně vzácné: duplicitní malignity (lymfangiosarkom u pacientů s lymfedémem), vícečetné malignity. Autoři zjistili autoimunitní tyreoiditidu s hypotyreózou u pacientky léčené pro postmastektomický lymfedém., The authors describe some complications of secondary lymphedema after malignant tumours treatement in women including their therapy. There are frequent complications: infections (recurrent erysipelas, herpes zoster), primary tumour progressions and metastases, hand-foot syndrome as a side-effect of chemotherapy and relatively rare complications e.g. second primary malignancy (lymphangiosarcoma in lymphedema patients) as well as multiple malignancies. According to our research, Uzbek individuals with Parkinson’s disease have high prevalence of 0/0 polymorphisms of GSTT1 and GSTM1 genes, as well as combinations of GSTT1 (0/0)/GSTM1/(0/0) genotypes. These mutations are associated with an earlier debut of the disease, its mixed form, and rapid rate of progression. In the department of coloproctology of NORC MH RUz 17 patients with disseminated forms of colorectal cancer was made the study of oncogenes and complex treatment by 2 protocols using FOLFOX4 regime and FOLFIRI regime. In second protocol there used 2 sessions of endolymphatical polychemotherapy FOLFOX4 regime against EHFhyperthermia. All patients were performed additional investigations directed to study the presence of multiple drug resistance in them where definition of р53, bcl2 oncogene expression. In our observations we followed resistance to FOLFOX4 scheme in 4 patients, and to FOLFIRI scheme in 2 cases. The Purpose of the study: define the indications and contraindications to, combined operation in locally invasive tumor of the rectum. The Material and methods of the study: we have analysed results combined operation in cancer of the rectum with germination in genital organs, performed in department of coloproctology in National Oncological Scientific centre during 20052009 years. Under observation were 118 women at age from 21 to 68 years. This before 45 years 26 patients, from 46 to 59 years 54 patients, 60 and above years. The Results and their discussion: Postoperative complications suppurativeinflammatory character appeared beside 36 patients (30,5%), most of all after abdomeno perineal extirpation of rectum (35,5%) and abdominoanal resection of the rectum (33,3%). In lesser extend after front resection of the rectum and after operation Hartman (28,5% and 22,7%). The General lethality has formed 3,4%, have died after combined operation 4 patients from 118 operated patients. The Conclusion: thereby, brought data evident that such important factors, as frequency of the origin relapse (28%), 5year probability of survival (37,1%), under combined interference and operation of the standard volume in the cancer of the rectum practically the same., H. V letech 1959–2008 počet zhoubných novotvarů centrální nervové soustavy (ZN CNS) z dat registru nádorů ČR dosáhl 26 548, z toho 14 263 (53,7 %) u mužů a 12 285 (46,3 %) u žen, v roce 2008 představující u mužů 412 (8,1/100 tis.), u žen 418 (7,9/100 tis.) onemocnění. Jejich prevalence v letech 1989–2005 vzrostla z 1169 na 2 565 žijících případů s očekávaným počtem 3 430 v roce 2015. Z 962 vícečetných ZN CNS, evidovaných v letech 1976–2005, bylo 150 primárních onemocnění u mužů a 162 u žen, 327 následných ZN CNS u mužů a 323 u žen tj. 5,1 % ze všech nově hlášených u mužů a 6,2 % u žen. Za toto období vzrostl v pětiletých intervalech počet primárních případů dvakrát, následných téměř třicetkrát. U primárních a následných ZN CNS převažovaly ve věku do 49 let ženy nad muži, v 50–69 letech muži nad ženami. Ze 14 krajů se vyskytlo 47 % ZN CNS ve čtyřech krajích (Moravskoslezský, Jihomoravský, Praha, Středočeský). Pouze 6,2 % multiplicit bylo evidováno v časných a 7,4 % v pokročilých klinických stadiích; 86,4 % stadií nebylo nahlášeno. Průměrný interval mezi 150 primárními ZN CNS a 170 jinými následnými novotvary u mužů byl pět let, mezi 162 primárními a 170 následnými u žen byl šest let, ze kterých byly u obou pohlaví nejpočetnější následné karcinomy trávicího ústrojí a kůže. Před 327 následnými ZN CNS bylo u mužů 390 primárních novotvarů, u žen před 323 následnými 362 primárních, ze kterých byly nejpočetnější karcinomy kůže, rodidel, prsů, prostaty a trávicího ústrojí. V říjnu 2007 žilo v době analýzy z 962 ZN CNS jen 91 a zemřelo 871 nemocných s převahou žijících žen nad muži., The number of malignant neoplasms of central nervous system (MN CNS), based in the Czech Cancer Registry, reached 26,548 diseases, of which there were 14,263 (53.7 %) of males and 12,285 (46.3 %) of females in 1959–2008, presented 412 (8.1 per 100,000 males) and 418 (7.9 per 100,000 females) diseases in 2008. Their prevalence increased from 1,169 to 2,565 survived cases between 1989–2005, with 3,430 cases expected in 2015. A total of 962 registered multiple MN CNS there were 150 primary diseases in males and 162 in females, 327 subsequent MN CNS in males and 323 in females, i. e. 5.1 % in males and 6.2 % in females of all newly registered MN CNS in period 1976–2005. The primary and subsequent MN CNS prevailed in the age group till 49 years in females over males, in the age group 50–69 years in males over females. A total of 14 Czech regions there were distributed 47 % of all multiple MN CNS in the four regions (Northern and Southern Moravia, Prague, Central Bohemia). Only 6.2 % multiplicities were registered in early and 7.4 % in advanced clinical stages; 86.4 % stages were unregistered. The average interval between 150 primary MN CNS and 170 other subsequent neoplasms was five years in males, between 162 primary and 170 subsequent cases six years in females, of which the most frequent were following digestive and skin carcinomas in both sex. In 327 subsequent MN CNS preceded 390 primary cancers in males, in 323 subsequent were 362 primary cases in females, of which the most frequent were carcinomas of skin, female genital and breast, prostate and digestive tract.
