Objective: To prevent the possible functional complications after gastrectomy, to improve the quality of life and the process of early rehabilitation. Material and Methods: The study included 165 patients with gastric cancer of stage III (T3N12M0), which, depending on the method of recovery divided into 2 groups: Group 1 n = 80 patients who, after an extensive gastrectomy had reservoir formed, II group n = 85 patients who had gastrectomy performed without forming a reservoir. Results: There were no incidence of reflux esophagitis and dumping syndrome observed in the main group, while in the control group, these complications were respectively 61.2% and 25.8%. Weight gain in the main group was observed in 83.7% of patients, in the control group in 4.7%. Assessment of physical activity on the Karnovsky scale in the study group was 85.0%, while 62.0% in the control group. Within 3 months, physical rehabilitation was observed in 90.0% of patients of the main group, while in the control group it was 25.7%. Threeyear survival rate in the main group was 71.4+0.4%, while in the control group, 57.2+0.3% (P> 0.05). Conclusions: Creation of a stomach replacing reservoir after gastrectomy is adequate way in ridding patients from possible complications. Cíl: Cílem studie bylo zjistit, jaký je výskyt obezity u dětí ve školním věku a zmapovat faktory, které se na vzniku obezity u dětí podílí. Metodika: Výzkumný soubor tvořilo 228 ţáků 4. a 9. tříd navštěvujících základní školy v městě Ostrava. Průzkumné šetření probíhalo pomocí dotazníkového šetření a měření antropometrických parametrů dětí. Dotazník byl zaměřen na stravovací zvyklosti dětí a fyzickou aktivitu. Měřením se zjišťovaly parametry dětí: tělesná hmotnost, tělesná výška, obvod pasu a paţe. Poté probíhalo měření koţních řas. Šetření probíhalo v období od listopadu 2011 do dubna 2012. Výsledky: Na základě hodnot BMI bylo zjištěno, ţe se obezita vyskytuje z celkového počtu 228 dětí u 12,28 % dětí školního věku a 9,21 % dětí má nadměrnou hmotnost. Z celkového zkoumaného vzorku dětí bylo zjištěno, ţe je 16,67 % obézních chlapců a 8,33 % obézních dívek. Nadměrnou hmotnost má 12,04 % chlapců a 6,67 % dívek. Vyšší procento obézních dětí bylo zastoupeno u dětí 4. tříd. Z průzkumu vyplynulo, ţe v průměru mají vyšší zastoupení tělesného tuku ve všech věkových kategoriích dívky neţ chlapci. Šetřením bylo zjištěno, ţe děti mají nedostatek pohybové aktivity. Nejčastější činností dětí ve volném čase je sledování televize a počítačové hry. Závěr: Prevalence obezity se zvyšuje nejen u dospělých, ale stále častěji u dětské populace. Obezita se stává celosvětovým problémem, který přináší do ţivota dětí mnoho problémů nejen po stránce fyzické, ale také psychické. Dětský věk je důleţitým obdobím pro včasné zahájení boje proti obezitě. Prevence zaujímá nejdůleţitější místo v předcházení obezity u dětí a problémům, které jim přináší., Aim: To determine the prevalence of obesity among school children and to map the factors contributing to the development of childhood obesity. Methods: The research sample consisted of 228 grade 4 and 9 pupils from primary schools in the city of Ostrava, Czech Republic. The survey was conducted using a questionnaire and anthropometric measurements. The questionnaire was concerned with children’s eating habits and physical activity. The following parameters were measured: body weight, body height, waist and arm circumference. Skinfold thickness was also assessed. The study was carried out between November 2011 and April 2012. Results: Based on BMI values, obesity was found in 12.28% of the 228 school children. A total of 9.21% of children were overweight. In the sample, there were 16.67% of obese boys and 8.33% of obese girls. Moreover, 12.04% of boys and 6.67% of girls suffered from overweight. The percentage of obese children was higher in the grade 4 group. The survey showed that, on average, girls had a higher proportion of body fat than boys in all age categories. The research also revealed a lack of physical activity in school children. The most frequent leisure activities were watching TV or playing computer games. Conclusion: The prevalence of obesity is increasing not only in the adult population but also in children. Obesity is becoming a global issue that causes many problems in the life of children, both physical and mental. Childhood is the right time to start a fight against obesity. Cieľ: Cieľom práce bolo validovať definujúce charakteristiky ošetrovateľskej diagnózy Deficit objemu telových tekutín vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré definujúce charakteristiky sú sestrami povaţované za hlavné a ktoré za vedľajšie. Metodika: Na hodnotenie definujúcich charakteristík ošetrovateľskej diagnózy Deficit objemu telových tekutín bol zvolený Fehringov model validity diagnostického obsahu (DCV model). Definujúce charakteristiky hodnotili sestry, ktoré získali minimálne 4 body podľa modifikovaných Fehringových kritérií. Súbor tvorilo 116 slovenských sestier – expertov. Definujúce charakteristiky boli sestrami hodnotené na Likertovej škále od 1 do 5 (1 – ţiadna významnosť, 2 – malá významnosť, 3 – stredná významnosť, 4 – veľká významnosť, 5 – najväčšia významnosť). Zber údajov prebiehal v období máj aţ október 2010. Výsledky: Za hlavné definujúce charakteristiky bolo súborom slovenských sestier – expertov zo 17 charakteristík označených 6 charakteristík: zníţený turgor koţe, zníţená diuréza, suché sliznice, zníţené napätie jazyka, zníţená venózna náplň, zníţený krvný tlak. Za vedľajšie definujúce charakteristiky boli označené nasledujúce charakteristiky: zvýšená merná hmotnosť moču, suchá pokoţka, zvýšený hematokrit, slabosť, smäd, oslabený pulz, zníţený objem pulzu (nitkovitý pulz), zvýšená pulzová frekvencia, zmeny v psychickom stave, zvýšená telesná teplota. Charakteristika „náhla strata hmotnosti (s výnimkou tretieho priestoru)“ dosiahla váţené skóre menej ako 0,5. Záver: Súbor slovenských sestier za významné znaky pre určenie ošetrovateľskej diagnózy Deficit objemu telových tekutín povaţuje 6 znakov., Aim: The aim of this study is to validate the defining characteristics of a nursing diagnosis of Deficient Fluid Volume by considering a selected sample of Slovak nurse – experts. In other words, to ascertain which defining characteristics are rated by these nurses as major or minor. Methods: Fehring’s Diagnostic Content Validation model (DCV model) was used for validation of this nursing diagnosis. Slovak nurses who met the modified criteria (4 points) recommended by Fehring comprised the sample of experts - a total of 116 Slovak nurses – experts. Defining characteristics were rated by the nurses using Likert’s 5-point scale (1 – not at all characteristic, 2 – very little characteristic, 3 – somewhat characteristic, 4 – considerably characteristic, 5 – very characteristic). Data collection occurred between May and October 2010. Results: Out of 17 defining characteristics of a nursing diagnosis of Deficient Fluid Volume, the Slovak nurses’ sample classified six cues as major: decreased skin turgor, decreased urine output, dry mucous membranes, decreased tongue turgor, decreased venous filling, and decreased blood pressure. The following cues were classified as minor defining characteristics: increased urine concentration, dry skin, elevated hematocrit, weakness, thirst, decreased pulse volume and pressure, increased pulse rate, changes in mental status, increased body temperature. The weighted score of the defining characteristic „sudden weight loss (except in the third space) was less than 0.5. Hlavní cíl: Zjistit vliv procvičování ošetřovatelského procesu podle OPT modelu klinické rozvahy na erudici studentů oboru všeobecná sestra při využívání pojmů Aliance 3N. Východiska: Motivem k realizaci studie bylo dílo autorek OPT modelu klinické rozvahy Pesut a Herman a výzkumné články k OPT modelu autorů Bartlett et al., Bland et al. a Kautz et al. Metodika: Aplikován byl design přirozeného experimentu u dvou skupin studentů s použitím pretestu a posttestu. Výzkumné nástroje byly nově pro tuto studii připraveny na podkladě publikovaných článků Herman a Bartlett. Jednalo se o pracovní protokoly pro studenty a nástroj k hodnocení erudice klinické rozvahy. Proběhla obsahová validizace protokolů a u hodnotícího nástroje byla ověřena reliabilita. Sběr dat proběhl u souboru šedesáti tří studentů formou záznamů do pracovních protokolů. Data, extrahovaná z protokolů, byla zpracována na hladině signifi kance 0,05 pomocí Studentova párového t-testu a Wilcoxonova párového testu. Výsledky: Medián celkového skóre erudice klinické rozvahy byl, při maximálním dosažitelném počtu 29 bodů, u studentů prezenční formy v pretestu 18,9 a v posttestu 19,5 bodů. U kombinované formy byl před a po cvičení 18,0 a 17,0 bodů. Závěry: U prezenční formy studia byla erudice klinické rozvahy s pojmy Aliance 3N po procvičování zlepšena, ale rozdíl nebyl statisticky signifi kantní. U kombinované formy studia prokázal Wilcoxonův párový test signifi kantní zhoršení. Využití NANDA diagnóz bylo u obou skupin subjektů explicitní, zásadní nedostatky byly shledány ve volbě optimálních NOC a NIC., Main aim: To determine the effect of practicing nursing process by OPT model of clinical reasoning on the expertise of students of General Nursing study in the use of concepts Alliance 3N. Background: The motive for the study was the work of the OPT model of clinical reasoning authors Pesut and Herman and research articles on the OPT model, the authors Bartlett et al., Bland et al. and Kautz et al. Methods: Applied design was a natural experiment on two groups of students using pretest and posttest. Research instruments for this study were new and prepared on the basis Herman and Bartlett’s published articles. Th ese were working protocols for students and a tool to assess erudition of clinical reasoning. Content validation was carried out and evaluation tool was tested on reliability. The data were collected in a group of sixty-three students in the form of records to work protocols. The data extracted from the protocols were processed at 0.05 level of signifi cance using Student’s paired t -test and Wilcoxon paired test. Results: The median of total score sheet clinical erudition was at full-time students in the pretest (before practicing) 18.9 points aft er exercise 19.5 (posttest). The median of total score before and aft er exercise was 18.0 and 17.0 points at part-time study students. Conclusions: For full-time students’ erudition in clinical reasoning with 3N Alliance terms aft er exercises has improved, but the diff erence was not statistically signifi cant. For part-time study students Wilcoxon paired test showed a signifi cant deterioration in student achievement. Background Patent ductus arteriosus (PDA) is common in very premature infants. Pharmacological closure of PDA with indomethacin, a prostaglandin inhibitor, has remained the mainstay of treatment in premature infants over the last three decades. Intravenous ibuprofen was recently shown to be as effective and to have fewer adverse reaction in preterm infants. If equally effective, then oral ibuprofen for PDA closure would have several important advantages over the intravenous route. This study was designed to assess the efficacy and safety of oral ibuprofen and intravenous ibuprofen for the early pharmacological treatment of PDA in LBW preterm infants with respiratory distress syndrome. Methods A randomized, single-blinded, controlled study was performed on premature neonates at the neonatal care unit of the University Hospital for Obstetrics and Gynecology”Koco Gliozheni”, Tirana, Albania, from January 2010 to December 2012. The study enrolled 68 preterm infants with gestational age between 28-32 weeks, birth weight ≤ 2000 g, postnatal age 48-96 h, and had echocardiographically confirmed significant PDA. The preterm infants received either intravenous or oral ibuprofen randomly as an initial dose of 10 mg/kg, followed by 5 mg/kg at 24 and 48 h. After the first dose of treatment in both groups, echocardiographic evaluation was performed, to determine the need for a second or third dose. The rate of ductal closure, adverse effects, complications, and the patient’s clinical course were recorded. Results All patients were born after 28 until 32 weeks’ gestation. 36 patients were treated with oral ibuprofen and 32 with intravenous ibuprofen in this period. After the first course of the treatment, the PDA closed in 30 (83.3%) of the patients assigned to the oral ibuprofen group versus 23 (71.8%) of those enrolled in the intravenous ibuprofen group (p = 0.355). There was no difference between treatment groups in demographics or baseline renal function. In the evaluation of renal tolerance, none of the patients had oliguria. There were no significant differences with respect to complications during the stay. Conclusions In low birth weight infants, the rate of early ductal closure with oral ibuprofen is at least as good as with the intravenous route. Adolescent hallux valgus (HV) is a progressive deformity of adolescent age consisting of metatarsus primus varus and hallux valgus. It has a high recurrence rate after conventional surgical correction. Ten feet in nine patients (two males, seven females) were treated surgically with the Peterson Newman bunion procedure, with a minimum follow-up of one year. During the final follow-up all these patients had no complaints of pain, joint stiffness or limping. Even though the patients had some mild loss of range of movements at the MTP joints 4–6° compared to preoperative value, it did not cause any functional impairment and all were satisfied with the final outcome. The double ostetomy for treatment of hallux valgus is technically precise procedure, provides excellent correction and stability and has low rate of recurrence of deformity. Cíl: Příspěvek seznamuje s prvním klinickým ověřením zkrácené české verze celosvětově nejrozšířenějšího dotazníku, který umožňuje diagnostikovat pacientovo pojetí nemoci – dotazníku IPQ-R (Moss-Morrisová et al., 2002). Studie si vytkla tři cíle: zhodnotit pacientovo pojetí nemoci na homogenním vzorku osob, zhodnotit interpretace výsledků sestrami včetně stanovení edukačních potřeb pacientů, zjistit názor sester na oprávněné použití dotazníku v rámci ošetřovatelské anamnézy. Metody: Zkrácená verze dotazníku IPQ-R-CZ (Brief) má 20 položek a má srovnatelné psychometrické parametry původní „dlouhé“ verze IPQ-R-CZ. Zjišťuje pět proměnných, které charakterizují pacientovo pojetí nemoci. Sledovaný soubor sestával ze 100 pacientů s převážně kardiovaskulárním onemocněním a pěti sester, které dotazníky administrovaly a podle klíče vyhodnocovaly. Výsledky: Klinické ověření ukázalo, že pro sestry bylo snadné na základě výsledků určit konkrétní oblast edukační potřeby. Závěr: Dotazník by mohl po dalších úpravách a ověřování na základě faktorové analýzy doplnit ošetřovatelskou anamnézu a stát se podkladem edukačního procesu u vybraných pacientů., Aim: The report gives information about the first clinical verification of a shortened Czech version of the most widely used questionnaire which enables diagnosing the patient’s perception of illness – questionnaire IPQ-R (Moss-Morris et al., 2002). The study had three aims: to evaluate patient’s perception of illness in a homogenous sample, to evaluate interpretations of the results by nurses including the determination of educational needs of patients, to find out the nurses opinion on justified use of the questionnaire in nursing anamnesis. Methods: Shortened questionnaire IPQ-R-CZ (Brief) contains 20 items and comparable psychometric parameters of the original “long” version IPQ-R-CZ. It features fi ve variables which characterize patient’s perception of illness. The observed sample comprised 100 patients with mostly cardiovascular disease and five nurses who administrated the questionnaires and evaluated them using a key. Results: The clinical verifi cation showed that it was easy for the nurses to identify specific domains of educational needs based on results. Cíl: Cílem tohoto příspěvku je prezentovat výsledky pilotního šetření testování dotazníku PNAP „Hodnocení potřeb pacientů v paliativní péči“ provedeného na onkologické klinice Fakultní nemocnice v Ostravě. Dále pak prezentovat zjištěnou důležitost bio-psycho-sociálně-spirituálních potřeb pacientů s ukončenou onkologickou léčbou. Metodika: Výzkumný soubor tvořilo 52 pacientů onkologické kliniky FN Ostrava, u nichž byla ukončena onkologická léčba v důsledku léčebně neovlivnitelné progrese onkologického onemocnění. Pro hodnocení potřeb byl použit dotazník - Hodnocení potřeb pacientů v paliativní péči (PNAP), který se skládá ze 42 položek sdružených do 5 domén. Výsledky: Za nejdůležitější považovali onkologičtí pacienti potřeby v doméně autonomie a fyzické potřeby. Nejméně byli respondenti spokojeni se saturací psychických a spirituálních potřeb. Pacienti mladší 60 let považovali sociální potřeby za signifikantně důležitější a současně byli méně spokojeni s jejich saturací (p<0,05). Ženy považovaly za důležitější psychické a spirituální potřeby. Se saturací těchto potřeb byly však více spokojeny (p<0,05) než muži. Z jednotlivých položek považovali respondenti nejčastěji za důležité: řešení bolesti, únavy, dušnosti, vyrovnat se s nemocí, komunikaci se zdravotníky a jejich podporu, rozhodování, respekt a úctu při ošetřování, vnitřní klid, naději a smysl života. Za nedostatečně saturované potřeby uvedli únavu, bolest, vyrovnanost s nemocí, strach ze závislosti, pokračování v obvyklých aktivitách, pozitivní náhled na život a naději. Závěr: Dotazník PNAP vychází z holistického pojetí péče. Zjištěné nedostatečně saturované potřeby by se měly stát předmětem ošetřovatelských intervencí., Aim: The aim of this contribution is to present the results of pilot research into a testing of the PNAP questionnaire “Patient needs assessment in palliative care“ implemented at the oncological clinic of the University Hospital Ostrava, as well as to present an established significance of bio-psycho-social-spiritual needs of patients with terminated oncological treatment. Methods: The sample was made up of 52 patients of the oncological clinic of the University Hospital Ostrava, whose oncological treatment was terminated as a result of an incurable progression of oncological disease. For needs assessment the questionnaire Patient needs assessment in palliative care (PNAP) was used, consisting of 42 items clustered into five domains. Results: Oncological patients considered the needs in the domains of autonomy and physical needs as the most important. They were the least satisfied with the saturation of psychological and spiritual needs. Patients under 60 years considered social needs as significantly more important and at the same time they were less satisfied with their saturation (p<0.05). Women considered psychological and spiritual needs as more important. However, they were more satisfied with the saturation of these needs than men (p<0.05). The following individual items were considered the most often as important: treating the pain, fatigue, dyspnoea, coming to terms with an illness, communication with health workers and their support, decision making, respect and dignity while being treated, inner peace, hope and the meaning of life. Insufficiently saturated needs were reported: fatigue, pain, reconciliation with an illness, fear from dependence, continuation in common activities, positive opinion of life and hope. Conclusion: The questionnaire PNAP is based on a holistic conception of care. According to our research, Uzbek individuals with Parkinson’s disease have high prevalence of 0/0 polymorphisms of GSTT1 and GSTM1 genes, as well as combinations of GSTT1 (0/0)/GSTM1/(0/0) genotypes. These mutations are associated with an earlier debut of the disease, its mixed form, and rapid rate of progression. Metodika: Senzitivní vyhledávací strategií byly vyhledány publikované informace k ošetřovatelské diagnóze dle NANDA International Porucha polykání (00103), k jejímu diagnostikování a k výskytu této diagnózy u pacientů. K vyhledání relevantních dat byly využity elektronické databáze: Bibliographia Medica Čechoslovaca, MEDLINE, Springer, ProQuest a vyhledávač Google. Výsledky: V odborných publikacích se autoři velmi často věnují problematice porušeného polykání a jejímu diagnostikování, nikoli však pomocí ošetřovatelské diagnostiky dle NANDA International. Ošetřovatelská diagnóza Porucha polykání (00103), diagnostikována prostřednictvím ošetřovatelské diagnostiky dle NANDA International, se vyskytovala ve velkém zastoupení u pacientů na jednotkách intenzivní péče, u pacientů s onkologickým onemocněním a u pacientů s onemocněním jícnu. Závěr: V ošetřovatelství je porucha polykání častým problémem, který lze diagnostikovat prostřednictvím klasifikačního systému NANDA International. Další výzkumy by mohly přispět ke zjištění, zda diagnostika poruchy polykání pomocí NANDA International je vhodná pro použití v klinické praxi., Aim: The aim of the study was to search for findings related to NANDA International nursing diagnosis Impaired Swallowing (00103), focusing on the incidence of this nursing diagnosis in different groups of patients and its establishment using the NANDA International classification system. Methods: Published information concerning NANDA International nursing diagnosis Impaired Swallowing (00103), its establishment and incidence in patients were found by a sensitive search strategy. The following electronic databases were searched for relevant information: Bibliographia Medica Čechoslovaca, MEDLINE, Springer and ProQuest, together with Google. Results: Although paper authors frequently mention impaired swallowing, it is rarely diagnosed using the NANDA International classification system. Nursing diagnosis Impaired Swallowing (00103) as diagnosed according to NANDA International was largely present in intensive care unit patients, oncology patients and those with oesophageal disease. Conclusion: Impaired swallowing is a frequent condition in nursing which can be diagnosed using the NANDA International classification system.
