Kvalita ţivota je ovlivněna zdravotním stavem, který zahrnuje fyzické, psychické a sociální charakteristiky. Největším zdravotním, ale i společenským problémem současné epochy nejsou nemoci akutní, ale nemoci dlouhodobé, chronické a trvalé. Výzkum se zaměřuje na oblast sexuality u pacientů s hypertenzí, s důrazem na holistické pojetí lidské osobnosti, hlubší vnímání a proţívání sexuálního ţivota, sexuálních potřeb pacientů. Cíl: Cílem kvalitativního šetření bylo zjistit, zda léčba hypertenze ovlivňuje sexuální potřeby pacientů. Metodika: Výzkumný soubor tvořilo 20 respondentů, výběr zkoumaného vzorku byl záměrný, byli to pacienti, kteří se léčili pro hypertenzi minimálně 1 rok ve Fakultní nemocnici Ostrava. Výzkum byl proveden pomocí kvalitativní metody, sběr dat byl proveden rozhovorem, který byl nahráván na diktafon v příjemném, nerušeném prostředí. Ve výzkumu byla pouţita metoda barvení textu s definovanými 4 kategoriemi a podkategoriemi. Výsledky: Z analýzy rozhovorů vyplynulo, ţe u některých pacientů antihypertenzní terapie negativně ovlivnila jejich sexuální potřeby. Byl zjištěn výskyt sexuálních dysfunkcí, u ţen se jednalo o sníţený sexuální apetit, sníţenou lubrikační schopnost, delší interval k vyvrcholení a u muţe byl objeven problém s erekcí. Závěr: V rámci subjektivní spokojenosti s vlastní sexualitou u pacientů, kteří se léčí s arteriální hypertenzí je důleţitá znalost vedlejších účinků a následná edukace s vyuţitím výsledků vědeckých studií domácích i zahraničních odborníků., The quality of life is influenced by health condition, which include physical, psychological and social features. The biggest, not only medical but also social problem nowadays are not acute diseases but long-term diseases, chronical and permanent. The research is concentrated on the sphere of sexuality among patients with hypertension, with stress on holistic concept of human being, deeper perception and experiencing of sexual life and sexual needs of patients. Aim: The aim of qualitative research was to find out how the treatment of hypertension influences sexual needs of patients. Methods: Researched sample was formed by 20 respondents. The choice of researched sample was intentional. They were patients who were treated for hypertension minimally one year in University Hospital Ostrava. The research was done by qualitative method, collection of datas was done by interview, which was recorded on voice recorder in a pleasant, by nothing disturbed place. In the research was used method of coloring the text with 4 defined categories and subcathegories. Results: From the analysis of interviews was clear that at some patients antihypertension therapy influenced their sexual needs in a negative way. There was found out the occurrence of sexual dysfunctions. Among women it was lower sexual appetite, lower lubricative ability, longer time for climax and among men was found the problem with erection. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. Předmětem této pilotní studie je verifi kace vlivu pozitivních emocí na motorické učení prostřednictvím vizuálního feedbacku formou hry ve virtuálním prostředí u pacientů po iktu. Pilotního měření se zúčastnili 3 pacienti po cévní mozkové příhodě, kteří absolvovali dvoutýdenní terapii formou hry ve virtuálním prostředí. U každého pacienta byla hodnocena před a po terapii posturální stabilita a hrubá motorika paretické horní končetiny pomocí standardizovaného klinického testování (Berg Balance Scale, Action Research Arm Test) i přístrojového vyšetření (dynamická počítačová posturografi e). Z výsledků hodnocení pacientů je zřejmý pozitivní efekt terapie na posturální stabilitu a koordinaci. Hru ve virtuálním prostředí považujeme za vhodný doplněk komplexního fyzioterapeutického přístupu u vybraných pacientů po cévní mozkové příhodě. Vzhledem k výsledkům naší pilotní studie jsou žádoucí další měření zabývající se touto problematikou., This pilot study aims to objectively verify the infl uence of positive emotions on motor learning through visual feedback in the form of games in a virtual environment in individuals aft er a stroke. In this pilot study, we evaluated three stroke patients. They completed a two-week intensive therapy by means of playing virtual games. Postural stability was assessed in every patient before and aft er the therapy, as well as gross motor skills of paretic upper limb by both clinical (Berg Balance Scale, Action Research Arm Test) and instrumental (Dynamic Computed Posturography) testing. The evaluation of patients showed signifi cant positive eff ect of game based therapy on postural stability and coordination. Therefore, playing games in a virtual environment might be considered a suitable addition to the complex of physiotherapeutic approach in patients aft er stroke. Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. The aim of the study was to establish the frequency of hypovitaminosis D in children with type 1 diabetes mellitus (T1D), its influence on biochemical and densitometric parameters and the relation to diabetic nephropathy. 58 children with T1D at the age 9–19 years were enrolled to the study. Vitamin D concentration less than 30 ng/ml was considered as insufficient. 37 children (63.79%) had vitamin D level under 30 ng/ml, from these 19 subjects (32.7%) had vitamin D level under 20 ng/ml and 2 subjects (3.44%) under 10 ng/ml. Children with vitamin D deficiency had significantly lower magnesium concentration and lower Z score of lumbar spine (−1.34 ± 1.24 vs. −0.30 ± 1.21, p = 0.01) compared to diabetics with sufficient vitamin D concentration. No significant difference was found in parameters calcium, phosphorus or glycosylated hemoglobin. Patients with diabetic nephropathy (n = 18) showed no significant difference in vitamin D, glycosylated hemoglobin or Z score of lumbar spine compared to the patients without nephropathy (n = 40). Subjects with nephropathy had significantly longer diabetes duration, significantly higher cholesterol and triacylglycerol concentration. Pro klasifikaci prekanceróz vulvy se používá termín vulvární intraepiteliální neoplázie (VIN). Histologicky lze rozlišit dva základní typy VIN: VIN obvyklého typu (u-VIN) a VIN diferencovaného typu (d-VIN). Rozsah a intenzita dysplastických změn se v případě u-VIN vyjadřuje třístupňovým gradingem (u-VIN I, II a III). Prekancerózní potenciál d-VIN odpovídá vždy karcinomu in situ, proto se u této diagnostické jednotky grading neprovádí. Etiologicky je u-VIN spojena s infekcí vysoce rizikovými typy lidského papillomaviru (HPV), zatímco d-VIN je HPV negativní a rozvíjí se v terénu chronických vulvárních dermatóz typu lichen sclerosus a lichen simplex chronicus. u-VIN má tendenci k multifokalitě a k dlouhodobému recidivujícímu průběhu před transformací v invazivní dlaždicobuněčný karcinom. d-VIN se naopak vyskytuje spíše solitárně a má sklon k rychlé stromální invazi. u-VIN typicky postihuje ženy premenopauzálního věku s vyšší incidencí ostatních prekancerózních lézí dolního ženského pohlavního systému včetně perinea a anu. Pacientky s d-VIN bývají postmenopauzální ženy a není u nich pozorována asociace s výše zmíněnými dyspláziemi ostatních anatomických lokalizací. Nízká diagnostická reproducibilita kategorie u-VIN I, pochyby o prekancerózním potenciálu u-VIN I a problematické rozlišení u-VIN II a u-VIN III vedly k revizi současného diagnostického schématu. Podstatou změn je zrušení gradingu u-VIN se sloučením kategorií u-VIN II a u-VIN III a dále zrušení u-VIN I jako diagnostické jednotky. V revidované klasifikaci prekanceróz vulvy proto termín u-VIN reprezentuje high grade dysplastické léze asociované s HPV (dříve u-VIN II a u-VIN III) a d-VIN i nadále zůstává označením pro HPV negativní high grade prekancerózu., Classification of vulvar precancerous lesions is based on the concept of vulvar intraepithelial neoplasia (VIN). Two basic types of VIN are recognized histologically: VIN of the usual type (u-VIN) and VIN of the differentiated type (d-VIN). The three grade evaluation system is used to assess the intesity of dysplastic changes in u-VIN (u-VIN I, II and III). The precancerous potential of d-VIN corresponds to carcinoma in situ and therefore grading is not applied. u-VIN is etiologically linked to HPV infection, whereas d-VIN is HPV negative and emerges in the environment of chronic vulvar dermatoses (lichen sclerosus and lichen simplex chronicus). u-VIN has a tendency to multifocality and prolonged recurrent progression to the invasive squamous cell carcinoma. On the other hand, d-VIN represents rather a solitary lesion with a propensity to the rapid stromal invasion. u-VIN usually affects premenopausal women with a higher incidence of other precancerous lesions of the lower female genital tract including the perineum and anal area. Patients diagnosed with d-VIN are of postmenopausal age without any association with aforementioned dysplastic lesions of other anatomic locations. The low diagnostic reproducibility of the u-VIN I category, doubts about the precancerous potential of u-VIN I and the problematic distinction between u-VIN II and u-VIN III resulted in the modification of the current terminology. The grading of u-VIN was abandoned, u-VIN II and u-VIN III categories were merged and u-VIN I was removed from the classification scheme. The revised u-VIN category therefore represents high grade dysplastic lesions associated with HPV infection (former u-VIN II and u-VIN III categories) and the term d-VIN is still reserved for the HPV negative high grade vulvar precancerosis., and P.
