The significance of intracavitary, intraorgan, hypertension syndrome identification as a novel uniform (model) pattern of a pathological process is substantiated in the in the article based on currently available literature sources and clinical practice experience. A conformity analysis of the considered pathological syndromes and model pathological processes criteria is presented with reference to data on etiology, pathogenesis, anatomical and some ontogenetic suppositions and stipulations. Diabetes 1. typu (DM1) vzniká jako následek autoimunitní destrukce beta buněk pankreatu. Vliv diabetu na celkový výsledek těhotenství je poměrně dobře prostudován. Mnohem méně se toho však ví o vlivu diabetu matky na vyvíjející se imunitní systém plodu. Riziko rozvoje DM1 u dětí diabetických matek (pokud onemocněly před těhotenstvím) je nižší než např. u dětí diabetických otců. Tato diskrepance podporuje teorii, že by mohlo docházet k určitému navození imunotolerance u dětí DM1 matek. Z tohoto důvodu jsme si pro náš výzkum vybrali T regulační buňky (Tregs), které hrají důležitou roli v imunoregulaci a patogenezi mnoha autoimunitních onemocnění. Studie Tregs pupečníkové krve DM1 matek jsou navíc ojedinělé. Pomocí průtokové cytometrie jsme analyzovali zastoupení Tregs (definovaných jako CD45+ CD3+ CD4+ CD25+ CD127(low/-)) ve vzorcích pupečníkové krve od 17 matek s DM1, 17 s gestačním diabetem a 42 zdravých kontrol. Signifikantní rozdíl jsme nalezli jen v zastoupení Tregs z Th lymfocytů, kde v pupečníkové krvi zdravých matek činil tento poměr 9,25 % a u matek s DM1 6,73 % (p=0,043). Ostatní rozdíly nedosáhly signifikance. Pokud tedy dochází k navození specifické imunotolerance, je tato patrně podmíněna jinak než změnou počtu Tregs. Pokračujeme proto ve funkční analýze Tregs s cílem nalézt vysvětlení pro nižší výskyt diabetu u dětí DM1 matek ve srovnání s ostatními prvostupňovými příbuznými DM1 pacientů. Pochopení tohoto jevu by mohlo přispět např. i k vytvoření imunointervenční terapie DM1., Type 1 diabetes (T1D) develops due to autoimmune pancreatic beta cells destruction. The effect of diabetes on pregnancy outcome is well documented. However, much less is known about the effect of maternal diabetes on the developing foetal immune system. The risk of T1D development in a child of T1D mother who was diagnosed prior pregnancy is lower than e.g. in a child of T1D father. This discrepancy supports the theory that there is some immunoregulatory influence on her baby. For this reason we decided to study T regulatory cells (Tregs) which are important in immunoregulation and for autoimmune diseases pathogenesis. Moreover Tregs cord blood (CB) studies of T1D mothers are limited. Tregs defined as CD45+ CD3+ CD4+ CD25+ CD127(low/-) in CB samples were investigated in our study by flow cytometry and 17 T1D mothers, 17 gestation diabetes mothers and 42 healthy controls were enrolled. We found just that CB from babies of T1D mothers contained 6,73 % Tregs/total Th-lymfocytes in comparison to 9,25 % in controls (p = 0,043). Other differences were not significant. If an establishment of specific immunological tolerance in foetus of T1D mother really exists, it seems to be rather due to other factors than just to changes in Tregs count (which was moreover found to be decreased). We are currently focused on explanation why children of T1D mothers suffer from diabetes less frequently than other first degree relatives by using functional analysis of Tregs. The reactivating and therapeutic efficacy of two combinations of oximes (HI‑6 + trimedoxime and HI‑6 + K203) was compared with the effectiveness of antidotal treatment involving single oxime (HI‑6, trimedoxime, K203) using in vivo methods. In vivo determined percentage of reactivation of cyclosarin‑inhibited blood and tissue acetylcholinesterase in poisoned rats showed that the reactivating efficacy of both combinations of oximes is slightly higher than the reactivating efficacy of the most effective individual oxime in blood, diaphragm as well as in brain. Moreover, both combinations of oximes were found to be slightly more efficacious in the reduction of acute lethal toxic effects in cyclosarin‑poisoned mice than the antidotal treatment involving single oxime. This article presents the author's technique and experience in the treatment of the flaccid "unhappy buttock" form with his surgical procedure of buttock lift by suture, without incision scars. The author first presented this new operation technique on a national level at the 2nd Annual Meeting of the National Bulgarian Society for Aesthetic Surgery and Aesthetic Medicine in Sofia on March 18, 1994 [1] and internationally at many scientific meetings over the world [2, 3, 4 ]. The result is a visual change in the buttock position to a higher one, which elongates the lower limbs and changes the proportions between lower and upper half of the body. The aim of this study is to describe a miniinvasive procedure of beautification of the buttock form without scars by creating a lifting effect on the buttock's subcutaneous tissue, using a suture that takes the inferiorly positioned deep fibrose tissue and fixes it upwards to the sacrocutaneous fascia, discovered by the author. Aesthetic and technical considerations required properly sculpting the buttocks into a higher position, demonstrating nicely rounded form. Preoperative shape is discussed and patient evaluations, operative techniques, postoperative management and results after 4 years of experience are emphasized. 1032 female patients, and 26 male patients aged 1862 years, with ptosis and cellulite on the buttocks were treated since 1993 on an outpatient basis by the "Serdev suture technique without visible scars". Important instrumentarium is a long, curved, elastic needle and Polycon semielastic Bulgarian antimicrobial polycaproamide long term (in 2 years) absorbable surgical threads Polycon, produced in Bulgaria. This operation has been performed either alone or after ultrasonic assisted liposculpture (UAL) that reduces the amount of fat and heaviness. All patients reported a high degree of satisfaction. A stable improvement in the buttock position and form was observed for the period described. In the postoperative period the complication rate was minimal and resolved in the first 45 days post operative period. The skin puncture in the perianal zone makes antibiotic prophylaxis obligatory as well as a strict follow up for the first 7 days. Some pain in the sitting position was observed for at least 5 to 10 days, but all other social and professional duties and activities were possible. Autorky vycházejí z teze, že validita je nezbytnou komponentou bádání a má významný dopad na věrohodnost výzkumu diagnostických pojmů NANDA International. Předkládají názor, že k posílení vědecké úrovně validizace diagnostiky NANDA International bude prioritní realizovat studie s užitím konstruktové validity., The article aims to give information about types of validity and validation studies of diagnostic NANDA International, emphasize their importance for the development of nursing science and point out the researchers who deal with this issue. Cílem práce je hodnocení vývoje úhrad nemocniční péče v ČR po roce 1998 ve vztahu k alokační efektivitě finančních zdrojů, rozdělených mezi různé typy českých nemocnic podle jejich velikosti, funkcí a rozsahu poskytovaných služeb s využitím statistických dat o činnosti a hospodaření těchto nemocnic. Vývoj úhrad je hodnocen v kontextu tržně zaměřených reforem zdravotnictví v ČR. Malé a střední nemocnice se v ČR staly v posledních dvou letech cílem další redukce jejich činnosti (lůžka, pracovníci, oddělení, rušení celých nemocnic) v rámci aktivit veřejných zdravotních pojišťoven (nástrojem jsou smlouvy pojišťoven s nemocnicemi). Předchozí restrukturalizace lůžkové péče v 90. letech byla v souladu s platnou právní úpravou řízena veřejnou správou. Současný reformní proces probíhá bez výchozích analýz a hodnocení ze strany veřejné správy na centrální a regionální úrovni. Veřejná a odborná komunikace tohoto procesu jsou nedostačující, vede k různým protestům ze strany dotče - ných subjektů. Není naplněn zákonem stanovený postup. U velkých nemocnic, zajišťujících poskytování zdravotní péče, výzkum a výuku byl zjištěn rozdíl mezi jejich výnosy a náklady na lůžkovou péči, uvedených v jejich výkazech. Jde o poměrně vysoké finanční částky (ročně v mld Kč). Tento rozdíl je zapotřebí objasnit cestou nezávislé veřejné kontroly. Práce upozorňuje na potřebu rozšířit současnou technokratickou argumentaci organizačních změn nemocniční péče, založenou na primárním neoliberálně směrovaném záměru inančních úspor a omezení produkce ve veřejném sektoru služeb. Doporučeným východiskem je obnova demokratických mechanismů se zohledněním funkcí a cílů zdravotnického systému v souladu s veřejnými zájmy a lidskými právy se vztahem ke zdraví., The aim of the work is to assess the development of hospital payment in the Czech Republic after 1998 in relation to the allocative efficiency of financial resources distributed among different types of Czech hospitals according to their size, function, and scope of services, using statistical data of their activities. Small and medium-sized hospitals in the Czech Republic were during the last two years challenged to further reduction of their activities (beds, staff, departments, hospitals complete interference) in the framework of their contracts with public health insurance. Previous restructuring of inpatient care in the 90s, was in accordance with the valid legislation. The current process takes place without cooperative baseline analysis and evaluation of the public administration at central and regional level. Jak název napovídá, kniha se zabývá chronologickým vývojem problematiky profesního pochybení lékařů v USA v minulém století. Tato recenze stručně informuje o autorovi, nicméně její podstatná část se zabývá obsahem publikace. Recenze se zabývá popisem jednotlivých kapitol ve stejném pořadí, jak jsou uvedeny v knize. Zdůrazněny jsou hlavní historické milníky problematiky profesního pochybení lékařů. V závěru je recenzentem provedeno zhodnocení publikace a zamyšlení nad případnou aplikací americké právní doktríny týkající se profesního pochybení lékaře v českém právním systému., The aim of this review is to summarize main aspects of the book Unhealed wounds: medical malpractice in the twentieth century written by Neal C. Hogan, published in 2003 in New York. As the title suggests the book itself is focused on chronological development of medical malpractice in the USA in previous century. This review briefly informs about the author but it´s mainly concentrated on the publication´s content. Paper itself descripts component chapters in the same order as it is written in the book. Break points of malpractice history are highlighted. Cieľ: Obsahová validácia ošetrovateľskej diagnózy Chronická zmätenosť a identifi kácia defi nujúcich charakteristík v podmienkach slovenskej a českej ošetrovateľskej praxe. Metódy: Na validáciu bol použitý retrospektívny Diagnostic Content Validity Model (DCV Model) podľa Fehringa. Za expertov boli považované sestry, ktoré získali minimálne 4 body podľa modifi kovaných Fehringových kritérií. Súbor expertov tvorilo 144 slovenských a 107 českých sestier. Výsledky: Za hlavné defi nujúce charakteristiky (vážené skóre VS viac ako 0,75) považovali zhodne slovenskí aj českí experti: narušená interpretácia podnetov a reakcia na ne, progresívna kognitívna porucha, klinicky podložená organická porucha mozgu, zmeny v časopriestorovej orientácii a dlhotrvajúca kognitívna porucha, ktoré sú totožné s charakteristikou syndrómu demencie. Závery: Experti nepovažovali všetky defi nujúce charakteristiky za rovnako významné. Určitý podiel zohralo zloženie samotného súboru expertov, ktorý naplnil iba spodnú hranicu kritérií na experta (6 bodov), nekompatibilita NANDA-I Taxonómie II so Zoznamom sesterských diagnóz platným v SR a absencia štandardizovanej ošetrovateľskej terminológie v Českej republike., Aim: Content validation of the nursing diagnosis Chronic confusion and identification of the defining characteristics in Slovak and Czech nursing practices. Method: Fehring’s retrospective Diagnostic Content Validity Model (DCV Model) was used. Experts were nurses who obtained minimum four points according to modified Fehring’s criteria. The group of respondents comprised of 144 Slovak and 107 Czech nurses. Results: Slovak and Czech experts alike considered the following characteristics to be defining (weighted score higher than 0.75): altered interpretation and altered response to stimuli, progressive cognitive impairment, clinical evidence of organic impairment, changes in spatiotemporal orientation and long-standing cognitive impairment, which are congruent with the characteristic features of dementia syndrome. Conclusion: Experts did not regard all the defining characteristics to be of the same importance. This was due to the composite of respondents, who only reached bottom level of criteria for an expert (6 points), incompatibility of NANDA-I Taxonomy II with the List of nursing diagnosis used in the Slovak Republic and lastly the absence of standardised nursing terminology in the Czech Republic., Martina Tomagová, Ivana Bóriková, and Literatura 28
Cíl: Cílem šetření bylo zjistit vliv kognitivní výkonnosti seniorů s demencí na jejich soběstačnost. Metodika: Výběrový soubor byl sestaven ze 106 seniorů (30 mužů a 76 žen, průměrný věk 83,25), u kterých byla potvrzena diagnóza demence. Statistická analýza byla provedena softwarem SPSS verze 10.0. Testování měřících nástrojů Mini Mental State Examination (MMSE) a Dotazníku soběstačnosti (DAD-CZ) probíhalo u seniorů žijících ve třech sociálních zařízeních. Pro testování vztahů a závislosti byl použit ANOVA test a Spearmanův korelační koeficient. Statistické testy byly hodnoceny na hladině významnosti α = 0,05. Výsledky: Naše studie potvrdila signifikantní vztahy mezi dotazníky MMSE a DAD (rs = 0,7619, p = 0,001) a současně byly zjištěny statisticky významné rozdíly v hodnocení soběstačnosti v závislosti na věku seniorů. Věk, vzdělání a pohlaví neměly vliv na výsledné hodnoty testu MMSE. Závěr: Výskyt kognitivních poruch je u starších osob poměrně častý. Problematika hodnocení soběstačnosti a kognitivních funkcí proto nesmí být opomíjenou oblastí v péči o seniory ani v zařízeních dlouhodobé péče. Pozdní detekce kognitivních poruch zhoršuje soběstačnost seniorů, která velmi často vede k rostoucí izolaci a větší závislosti a v konečném důsledku může vést k snížení kvality života., Aim: The aim of the investigation was to determine the influence of cognitive efficiency of the elderly with dementia on their self-sufficiency. Methods: The sample consisted of 106 elderly (30 men and 76 women, average age 83,25), with confirmed dementia. Statistical analysis was performed by software SPSS version 10.0. Testing of measuring tools Mini Mental State Examination (MMSE) and Self-sufficiency questionnaire (DAD-CZ) took place in the elderly living in three social settings. To test the relationship and dependencies the ANOVA test and Spearman correlation coefficient were used. Statistical tests were evaluated at a significance level α = 0,05. Results: Our study confirmed the significant relationship between MMSE and DAD (rs = 0,7619, p = 0,001) and also were found statistically significant differences in self-evaluation depending on the age of elderly. Age, education and gender had no impact on the resulting value of the MMSE test. Conclusion: The incidence of cognitive impairment in the elderly is relatively common. The problematics with self-sufficiency and cognitive functions should not therefore be neglected area in caring for the elderly even in the long-term-care facilities.
