Competencies develop and changes throughout a persons life, they can gain or lose, going through various age stages. Their development does not end in youth, but continue on through the adult life. The ability to thing and reflect those thoughts specifically come forward in the center of structure of competency, which grows at the same time as the individual matures. One of the competency types is health competency. Health competency is a relatively new concept; it is not sufficiently researched. Aim of the study was to determine the factors of an adult individual health competency. 827 respondents participated in the study, in the processing of data was used SPSS. Was used factor analysis, analysis of variance with ANOVA and KruskalWallis test and Pearsons correlation. It was found that health competency is affected by several factors. These are: health education, health behavior, and the value of the environment. Each of the sets was distributed to key factors. In this article, the results of the morphological study of the cerebral arteries in the patients with the hemorrhagic ischemic stroke on the background of metabolic syndrome were submitted. We established that under hemorrhagic stroke on the background of metabolic syndrome one could observe the atherosclerotic damages in the form of plaques, hyalinosis of vessels walls, destructive and necrotic changes of the middle coat of vessel wall, which are, in our opinion, the main reason in morphogenesis of hemorrhagic stroke. The atherosclerotic affection of the cerebral vessels with the lipidosis and fibrotic plaques in the vessel wall, which caused in disorganization of the vessel wall in the form intimae proliferation, structural reconstruction of the internal elastic membrane which resulted in narrowing of the vessel lumen and development of ischemia is the morphological basis for the ischemic stroke with the metabolic syndrome. The Purpose of this study is to demonstrate the effectiveness of combined treatment for III AB stage of nonsmallcell lung cancer. Nonsmallcell lung cancer spread form treatment is disputable. Operation is not final solution. The combined treatment is being tested (in clinical trials) in this study. This article argues that a combination of treatment is a better option than that of a specific treatment. Objective: to analyze main clinical-statistical indices of spine and spinal cord injuries (SCI) and system of organization of medical aid to the victims. We conducted a cohort retrospective study of materials of medical institutions of Tashkent city. Medical records of patients and records of forensic examinations were analyzed. 242 cases of spinal cord injuries were studied. Over the last 10 years in Tashkent the frequency of SCI have been tend to increase, males and working-age people are dominated. In 60.9% of cases the injury was associated, in 36.2% - isolated, and in 2.9% - combined. Fallings from height and road traffic accidents were the main reasons for getting SCI. Lesions at cervical level registered in 45% of cases, thoracic level - 27.3%, at lumbar level - 27.7%, respectively. According to ASIA/IMSOP, full injuries were noted in 40.1% of cases, incomplete injures were in 59.9%. Mediterranean spotted fever (MSF) in Bulgaria shows increasing severity, especially in patients over sixty years. As T lymphocytes are the primary effector cells in MSF and are altered by the ageing process we aimed to specify the changes in their numbers in elderly patients. We performed a clinicoepidemiological study, haematological and biochemical analysis of 132 patients of 60+ and 30 patients between 19 and 57 years with MSF. We investigated the cell immunity of 20 patients of 60+ using immunofluocytometry. The control group consisted of 10 younger patients and 10 healthy individuals of 60+. MSF showed more unfavourable course in the elderly.We found T-lymphocytes depletion in all patients with MSF. Activated T lymphocytes were increased in elderly patients with MSF. We found significant differences between the number of activated T lymphocytes in elderly patients and healthy elderly persons. The observed T-lymphocytes depletion in MSF corresponded to the disease severity. Východiska: Pohybový projev zdravého organismu je charakterizován vysokou mírou variability provedení pohybového úkonu. Za situace vzniklé poruchy řízení, zejména v důsledku neurologických onemocnění, dochází v různé míře k fixnímu pohybového projevu, projevujícímu se omezením schopnosti pružné reakce na změnu zevních či vnitřních podmínek, působících v průběhu provádění cíleně zaměřené pohybové akce. Cíl: Zjistit rozdíly ve svalové aktivitě a variabilitě svalových souher akrálních a proximálních svalů během dosahu a úchopu u zdravých jedinců a u pacientů s hemiparézou. Metody: Experimentální soubor tvořilo 24 probandů po prodělané ischemické cévní mozkové příhodě (iktu) s pravostrannou hemiparézou. Kontrolní soubor tvořilo 30 zdravých probandů. Svalová aktivita byla zaznamenána pomocí povrchové elektromyografie v průběhu dosahu, úchopu a oddálení válcového a kulového předmětu. Rozdíly v hodnotách plochy pod křivkou záznamu jednotlivých svalů mezi experimentálním a kontrolním souborem byly statisticky vyhodnoceny pomocí T-testu pro nezávislé skupiny. Vztah mezi aktivitou svalů předloktí a svalů pletence ramenního byl hodnocen pomocí Spearmanova korelačního koeficientu pro neparametrické hodnoty. Výsledky: Byly zjištěny významné rozdíly v míře a počtu statisticky významných korelací akrálních a pletencových svalů. U souboru probandů s hemiparézou byla zjištěna statisticky významně nižší aktivita m. pectoralis major a naopak vyšší aktivita m. trapezius. Současně převažovala vysoká korelace m. pectoralis major a extenzorů zápěstí a prstů, na rozdíl od souboru zdravých probandů, kdy se vzájemné korelace akrálních a proximálních svalových skupin objevovaly nahodile, v menší míře a četnosti. Závěry: Výsledky ukazují na snížení variability svalové aktivity u pacientů s hemiparézou v průběhu úchopové funkce. Náhradou nedostatečné funkčnosti akrálních svalových skupin se stává relativně fixní „ko-aktivace“ s vybranými proximálními svalovými skupinami., Background: Movement of a healthy organism is characterized by a high degree of variability performance of a movement task. In a situation of a motor control lesion, mainly due to neurological disease, occurs in varying degrees to a fixed musculoskeletal manifestation. ere are manifesting in a limiting responsiveness to changing external or internal conditions during the task oriented physical action. Objective: To determine differences in muscle activity and variability between a muscle synergy of acral and proximal muscles during reach and grip in healthy subjects and stroke patients. Methods: e experimental group consisted of 24 probands aer ischemic stroke (ictus) with right-sided hemiparesis. e control group consisted of 30 healthy subjects. Muscle activity was recorded using surface electromyography during the performation of selected types of grips of cylindric and spheric objects. e differences in the area under the curve record of individual muscles between the experimental and control group were statistically evaluated using the t-test for independent groups. e relationship between the activity of the muscles of the forearm and shoulder girdle muscles was assessed using Spearman‘s correlation coefficient for non-parametric values . Results: ere were the significant differences in the size and number of statistically significant correlations distal and proximal arm muscles. In stroke group was found significantly lower activity m. pectoralis major and higher activity m. trapezius. At the same time the prevailing high correlation m. pectoralis major and extensor wrist and fingers, unlike the group of healthy subjectss, when the correlation distal and proximal muscle groups appeared at random, in a lesser extent and frequency. Conclusions: e results indicate a reduction in variability of muscle activity during grasp function of stroke patients. Ultrazvukové vyšetrenie maternice po pôrode zahŕňa niektoré špecifiká, ktoré vyplývajú z rýchlo sa meniacich rozmerov maternice, ale aj compliance pacientky vo včasnom šestonedelí. Pri riešení popôrodných komplikácií je potrebné poznať normálny ultrazvukový nález, dynamické zmeny maternice v priebehu šestonedelia, aby sme vedeli lepšie rozpoznať patologický priebeh od normálneho priebehu šestonedelia. Klinický význam ultrazvukového posúdenia veľkosti maternice ako aj oblasti jazvy po cisárskom reze v priebehu šestonedelia doteraz nie je dostatočne popísaný. Prehľadový článok rozoberá špecifiká ultrazvukovej diagnostiky po vaginálnom pôrode a cisárskom reze. In dealing with postpartum complications is necessary to know the normal ultrasound findings, the dynamic changes in the uterus during the puerperium, so we can better recognize the pathological process during the normal puerperium. The clinical significance of ultrasound assessment of uterine size and the scar after Cesarean section during the puerperium have not been yet sufficiently described. Pozadie: Článok sa zaoberá vybranými aspektmi ústretovosti českého zdravotníckeho systému z pohľadu slovenských imigrantov so zameraním na dostupnosť zdravotnej starostlivosti a komunikáciu medzi pacientom a lekárom. Metódy: Kvalitatívna štúdia bola realizovaná prostredníctvom rozhovorov u 20 Slovákov žijúcich v Českej republike. Výsledky: Najväčšie prekážky pri čerpaní zdravotnej starostlivosti vidia respondenti v nedostatočnom počte registračných miest u lekárov primárnej starostlivosti. Naopak respondenti boli spokojní s prístupom českých lekárov. Závery: Pravdepodobným dôvodom ťažkostí slovenských imigrantov pri registrácii u lekárov primárnej starostlivosti je pracovná preťaženosť lekárov., Background: The article deals with selected aspects of health system responsiveness from the perspective of Slovak immigrants in the Czech Republic. It focuses on access to health care and communication between patient and physician. Methods: A qualitative study was conducted through interviews with 20 Slovaks living in the Czech Republic. Results: The biggest obstacles to health care receiving consist, according to respondents, in inadequate number of registration places at primary care physicians. However, respondents were satisfied with physician`s approach in the Czech Republic. Bor a jeho sloučeniny byly zařazeny díky Nařízení Evropské komise č. 1907/2006 (REACH) do skupiny látek negativně ovlivňující reprodukci a vývoj plodu. To vyvolalo potřebu shrnout současné poznatky o jeho toxicitě a užití v medicíně během těhotenství a laktace. Akutní i chronická toxicita sloučenin boru je nízká. Otravy byly popsány jen ve výjimečných případech. Reprodukční toxicita zahrnuje v pokuse na zvířeti poškození spermatogenese a možnost vzniku vrozených vývojových vad obvykle za situace, kdy matka vykazuje známky toxického poškození. Práce rovněž shrnuje publikované klinické studie o vlivu kyseliny borité na vývoj plodu. Dle publikovaných výsledků studií nedosahuje expozice u člověka takových hodnot, aby k poškození reprodukce a plodu došlo. Protože užití jakéhokoli léku v těhotenství a laktaci musí být opodstatněné a profit musí převládat nad rizikem, shrnuje práce i studie, které se zabývají účinností kyseliny borité ve srovnání s jinými antimykotiky. Pro léčbu mykóz lze užít lokálně působící nystatin, případně klotrimazol, nebo mikonazol, výjimečně perorálně flukonazol v nízkých dávkách. Účinnost kyseliny borité je srovnatelná, případně vyšší, při léčbě rezistentních kmenů. Výhodou je i její nízká cena. Na závěr je třeba zdůraznit, že užití jakéhokoli přípravku během 1. trimestru by mělo být podloženo naléhavou potřebou léčby., The European Union Regulation REACH (No. 1907/2006) classified of boron and its compounds in the group adversely affecting reproduction and fetal development raised the need to to summarize findings about its toxicity and use in medicine during pregnancy and lactation. Acute and chronic toxicity of boron compounds is low. Intoxications were reported only in exceptional circumstances. In animals, reproductive toxicity includes the damage to spermatogenesis and the possibility of congenital defects usually in doses causing maternal toxicity. The work also summarizes the published clinical studies on the effect of boric acid on fetal development. According to the published results of studies, the human exposure does not reach of such values that the reproduction and fetal damage occurred. Since the use of any drug in pregnancy and lactation must be justified and profit must prevail over the risk, the work summarizes studies on the effect of boric acid as compared to other antifungal agents. Nystatin, clotrimazole, or miconazole, fluconazole can be used topically for the treatment of fungal infections, applied p.o. exceptionally in low doses, only. The effectiveness of boric acid is comparable or higher in resistant strains. Low price is an advantage. Cieľ: Cieľom štúdie realizovanej v rámci projektu podporeného APVV (Agentúra na podporu výskumu a vývoja) bola obsahová validácia ošetrovateľskej diagnózy Narušená pamäť a identifikácia definujúcich charakteristík v podmienkach českej a slovenskej praxe. Metodika: Na validáciu sme vybrali Diagnostic Content Validity Model (model DCV) podľa Fehringa. Za expertov sme považovali sestry, ktoré získali minimálne 4 body podľa modifikovaných Fehringových kritérií. Experti posudzovali významnosť definujúcich charakteristík ošetrovateľskej diagnózy v pripravenom validačnom hárku. Obsahoval 10 definujúcich charakteristík ošetrovateľskej diagnózy, 4 položky z nástroja MMSE (Folsteinov test, Mini-Mental State Examination) a 2 neutrálne položky. Súbor expertov tvorilo 94 českých a 154 slovenských sestier. Výsledky: Za hlavné definujúce charakteristiky (vážené skóre, VS viac ako 0,75) považovali experti tie, ktoré súvisia s jednotlivými fázami pamäti (recepcia, retencia, reprodukcia). V českom a slovenskom súbore expertov boli výsledky rovnaké a sú porovnateľné so zisteniami iných autorov. Záver: Experti nepovažovali všetky definujúce charakteristiky za rovnako významné, podobne ako v iných štúdiách v rámci projektu APVV. Určitý podiel zohralo zloženie samotného súboru expertov, ktorý naplnil iba spodnú hranicu kritérií na experta (6 bodov) a absencia štandardizovanej ošetrovateľskej terminológie v Českej a Slovenskej republike., Aim: The aim of this study (supported by project Slovak Research and Development Agency, SRDA) was to examine the content validity of nursing diagnosis Impaired Memory and to identify the defining characteristics in Czech and Slovak clinical practice. Methodology: Fehring’s Diagnostic Content Validity Model was used. Experts were nurses who achieve minimally the total number of 4 points according to modified Fehring’s criteria. Experts measured importance of diagnostic characteristics of Impaired Memory in validation form. It contained 10 defining characteristics of nursing diagnosis, 2 neutral characteristics and 4 items from MMSE (Mini-Mental State Examination) tool. Respondents were 94 Czech and 154 Slovak nurses. Results: Experts identically considered as main defining characteristics (weighted score more than 0,75), which consistent with memory phases (encoding, storage, retrieval). In Czech and Slovak data of respondents were results almost identical and are comparable with results another authors. Conclusion: Experts didn’t consider all defining characteristics as uniformly important, similarly as another SRDA studies.
