Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. BACKGROUND: In the cholesteatoma surgery ossicles can be replaced to reconstruct middle ear function. It is important that these ossicles are free of squamous epithelium, to prevent residual disease. This study focuses on the histological findings of the malleus and incus harvested during cholesteatoma surgery. MATERIALS AND METHODS: Eighty middle ears ossicles were examined in vivo and histologically to consider the relationship of cholesteatoma to ossicles, grade of bone destruction and invasion of cholesteatoma to deeper layers of bone. RESULTS: Serious ossicular destruction was observed more frequently in incus compared to malleus (p=0.0065). Difference of ossicles destruction between children and adults was not significant (p=0.3032). Deep invasion of cholesteatoma into the vascular spaces or inner core of the bone was not observed. CONCLUSIONS: Autograft ossicles from cholesteatomatous ears should not necessarily be rejected for reconstruction of the ossicular chain. The risk of high-grade lesions and carcinoma is known to correlate with specific high-risk HPV genotypes. The distribution of HPV types varies between countries and little is known about HPV distribution in our country. Therefore, the purpose of this study was to determine the range and frequency of HPV genotypes in studied group of women in Montenegro. HPV genotypes were determined using the method of enzyme restriction of PCR products amplified with group-specific primers MY09/MY11 and restricted with seven different restriction endonucleases. Out of the total number of women HPV infection was found in 1/5 of participants (20%). Genotyping performed in HPV DNA positive women shows that the HPV genotype 16 is dominant and present in more than 1/3 of the participants (36.8%). The second most frequent HPV infection is with HPV genotype 58 and it is found in 10.5% of participants. HPV 31 and HPV 6 infections are present in 7.9% of women, while infections with other genotypes were demonstrated individually by 2.6%. Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %. Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Úvod: Karcinom prsu je nejrozšířenějším zhoubným nádorem u žen. Nejčastěji se toto onemocnění vyskytuje u žen mezi 60. a 70. rokem života, nicméně v posledních letech dochází k nárůstu počtu pacientek i v mladších věkových skupinách. Incidence karcinomu prsu má celosvětově vzestupný charakter, zejména ve Spojených státech a západní Evropě. Jeho mortalita má však zejména díky screeningovému programu a pokročilým léčebným metodám spíše mírně sestupnou tendenci. Metody: Do souboru byly zařazeny pacientky s karcinomem prsu, které podstoupily operaci pro tuto diagnózu na I. chirurgické klinice FN Olomouc v letech 2008– 2012. U každé pacientky byl vypočítán věk v době stanovení diagnózy, respektive v době operace. Pacientky byly v každém roce rozděleny do věkových skupin po 10 letech. Současně byla v každém roce vytvořena podskupina pacientek s karcinomem prsu mladších 45 let. Byl porovnán počet pacientek v odpovídajících skupinách, resp. podskupinách, v jednotlivých letech. V každém roce byl rovněž stanoven průměrný věk a medián pacientek. Ke zhodnocení přínosu mamárního screeningu bylo porovnáno zastoupení stadií operovaných tumorů v jednotlivých sledovaných letech. Výsledky byly statisticky zpracovány a vyhodnoceny. Výsledky: V letech 2008–2012 bylo operováno na I. chirurgické klinice FN Olomouc celkem 980 pacientek ve věku od 20 do 88 let, z toho 101 pacientek mladších 45 let. Průměrný věk pacientek byl 59 let a medián 60 let. V jednotlivých letech se celkový počet pacientek zvýšil ze 153 v roce 2008 až na 240 v roce 2012. V podskupině pacientek do 45 let nedošlo ve sledovaných letech k signifikantnímu nárůstu počtu nemocných. V roce 2012 bylo diagnostikováno více pacientek ve stadiu II a méně pacientek ve stadiu III v porovnání s rokem 2010. Závěr: Analýzou našeho souboru byla potvrzena vzestupná tendence ve výskytu karcinomu prsu. Nebyl prokázán statisticky významný nárůst počtu pacientek v tzv. předscreeningovém věku, tedy mladších 45 let. Zároveň byl zjištěn častější záchyt časnějších stadií onemocnění, zejména v posledních třech sledovaných letech., Introduction: Breast cancer is the most common malignancy in women. It affects mostly women between the ages of 60 and 70; however, in the past years, the number of younger female patients has been increasing. The incidence of breast cancer has been rising worldwide, especially in the United States and Western Europe. Breast carcinoma mortality, on the other hand, has shown a slight decrease due to early screening programmes and advanced treatment methods. Methods: We included patients who had undergone surgery for breast carcinoma in the 1st Department of Surgery at Teaching Hospital in Olomouc between 1 January 2008 and 31 December 2012. In each patient, her age at the time of diagnosis/surgery was calculated. All patients were divided into 10-year age groups. At the same time, a sub-group of breast cancer patients younger than 45 years was created. We compared the numbers of patients in the respective groups and sub-groups in every year. The mean age and the median of age were also calculated. To evaluate the benefit of mammary screening, we compared the staging of operated tumours in the different years studied. The results were statistically processed and evaluated. Results: The total number of 980 patients underwent surgery for breast carcinoma at our department between 2008 and 2012 with age ranging from 20 to 88 years. 101 of them were younger than 45 years. The mean age of the patients was 59 years, the median was 60 years. The total number of patients increased from 153 in 2008 to 240 in 2012. There was no significant increase in the number of patients younger than 45 years. There were more patients diagnosed with stage II carcinoma and fewer patients with stage III carcinoma in 2012 than in 2010. Conclusion: The analysis of our group of patients confirmed the increasing tendencies of breast cancer incidence in total. We did not prove a statistically significant increase in the number of patients in pre-screening age (i.e., younger than 45 years). Zkoušky verbální fluence jsou jednoduché testy, které kladou nároky zejména na mentální flexibilitu, na schopnost rychle organizovat informace do sémanticky příbuzných skupin, na efektivní vybavovací strategie, a v neposlední řadě i na kvalitu pracovní paměti. Na základě výzkumů zobrazovacích a funkčních metod mozku se odborná veřejnost shoduje, že zatímco poškození frontálních laloků vykazuje spíše nekonzistentní obraz deficitu v oblasti lexikální verbální fluence, poškození temporálních oblastí bývá úžeji spojeno s deficity v oblasti verbální fluence sémantické. Rozhodli jsme se ověřit, nakolik lze v rámci běžné klinickopsychologické praxe využít tyto závěry k diferenciální diagnostice mezi jednotlivými neurologickými onemocněními. Zkoumaný soubor byl tvořen čtyřmi skupinami pacientů: pacienti s epilepsií frontálního laloku, pacienti s levostrannou temporální epilepsií, pacienti s pravostrannou temporální epilepsií a pacienti s Parkinsonovou nemocí, kdy v každé skupině bylo 25 subjektů. Na základě využití neparametrického Kruskalova‑Wallisova testu bylo zjištěno, že se skupiny pacientů statisticky významně nelišily v hodnotách sémantické ani lexikální verbální fluence., Verbal fluency tests are simple assessments evaluating patients’ mental flexibility, ability to organize information into semantically similar groups, effective recall strategies and quality of working memory. Based on the results of research into brain imaging and functional brain imaging methods, experts assume that, while frontal lobe lesions are linked to rather inconsistent performance patterns in phonemic verbal fluency, temporal lobe lesions are more related to deficits in semantic verbal fluency. The aim of our study was to verify validity of these findings in routine clinical psychological practice during differential diagnostic procedure in the area of specific neurological disorders. Four groups of patients were included in the study: patients with frontal lobe epilepsy, two groups of patients with temporal lobe epilepsy – left and right hemisphere, and patients with Parkinson’s disease. Each group consisted of 25 patients. Statistical analysis was performed using the nonparametric Kruskal-Wallis test. Lokální chemoprofylaxe neinvazivních nádorů močového měchýře je používána více než 40 let. Využíváno je několik agens, mezi nejčastější patří mitomycin C (MMC) a epirubicin. Názor na léčebná schémata není jednotný. Avšak na podkladě EBM je zřejmé, že adjuvantní instilace cytotoxických látek do močového měchýře prodlužuje dobu beznádorového intervalu. V práci předkládáme dlouhodobé zkušenosti s intravezikální chemoprofylaxí povrchových nádorů močového měchýře., Local chemoprophylaxis of noninvasive bladder tumors has been used for more than 40 years. Several agents are used, with the most common including mitomycin C (MMC) and epirubicin. Opinions on therapeutic strategies vary.
