V našem kazuistickém sdělení je popsán mikroskopicky verifikovaný případ IgG4 asociované pankreatitidy s přidruženým postižením žlučových cest u pacienta léčeného ve Fakultní nemocnici Hradec Králové. Karcinom prostaty patří mezi nejčastější nádorová onemocnění u mužů. V časném a lokalizovaném stadiu je léčitelný, určité procento pacientů však relabuje nebo se diagnóza stanoví až v pokročilejších stadiích, kde jsou léčebné možnosti limitované. Imunoterapie je kromě klasické hormonální terapie, radioterapie a chemoterapie další perspektivní léčebnou modalitou v těchto stadiích. Vznik nádorového onemocnění je mnohastupňový proces komplexní povahy, v němž samotné nádorové buňky představují heterogenní populaci zasazenou do kontextu mikroprostředí nenádorový ch buněk a tkání tvořících složitý nádorový „ekosystém“. Imunitní systém je schopen kontrolovat růst nádorových buněk v raných stadiích vzniku nádoru. S pokročilým růstem nádoru jsou imunitní mechanizmy víceméně paralyzovány a mnohdy na proliferaci nádorových buněk mají paradoxně podpůrný účinek. Aby byla protinádorová terapie úspěšná, musí zohledňovat všechny komplexní aspekty interakce imunitního systému a nádorových buněk včetně mikroprostředí. V článku jsou probrány současné možnosti imunoterapie karcinomu prostaty preparáty, které postoupily do III. fáze klinických studií., Prostate cancer is among the most common cancers in men. It is treatable in the early and localized stage; however, a proportion of patients relapse or the diagnosis is made only in more advanced stages when the therapeutic options are limited. In addition to classic hormonal therapy, radiotherapy, and chemotherapy, immunotherapy is another promising treatment modality in these stages. The development of cancer disease is a multistage process of a comprehensive nature wherein the tumour cells themselves represent a very heterogeneous population put into the context of a microenvironment of non-tumour cells and tissues, thus forming a complex tumour "ecosystem". The immune system is capable of controlling tumour cell growth in the early stages of tumour development. With advanced tumour growth, immune mechanisms become more or less paralysed and, in fact, not infrequently have a promoting effect on tumour cell proliferation. In order to be successful, anticancer therapy must take into consideration all the comprehensive aspects of interaction of the immune system and tumour cells, including the microenvironment. The presence of a broad spectrum of autoantibodies in Sjögren's syndrome (SjS) patients is the result of abnormal B-cell regulation that can be at least partially explained by abnormal BAFF/BAFFR regulation. The objective of this study was to determine both membrane and intracellular expression of BAFF/BAFFR in monocytes and B-cells in peripheral blood of 19 primary Sjögren's syndrome patients and 20 healthy controls using flow cytometry. We also measured sBAFF in serum. Compared to healthy controls, both surface and intracellular expression of BAFF was significantly increased in monocytes and B-cells of SjS patients. Also serum sBAFF level was elevated. Expression of BAFFR on B-cells of SjS patients was surprisingly decreased, but there was no clear increase or decrease within monocytes. Our results indicate that activated monocytes communicate with B-cells via BAFF and BAFFR, so that B-cells are stimulated, but BAFF is also produced to stimulate cells in autocrine way. The decrease of BAFFR expression in SjS patients suggests that there is the mechanism that attempts to take over in order to balance the high level of BAFF. and J. Krejsek, M. Koláčková, I. Úvod: Kombinace monoklonální antiCD20 protilátky rituximabu s chemoterapií je vysoce účinná v léčbě chronické lymfocytární leukemie (CLL). Hlavním nežádoucím účinkem rituximabu jsou infuzní reakce, např. syndrom uvolnění cytokinů (cytokine release syndrome – CRS), který zpravidla doprovází první podání rituximabu. Cílem naší retrospektivní analýzy bylo zhodnotit četnost výskytu infuzních reakcí v rutinní klinické praxi, jejich vztah k parametrům nádorové nálože a zjistit možnou spojitost infuzních reakcí s účinností léčby. Dále byla posouzena bezpečnost rychlé infuze s rituximabem u nemocných s CLL. Charakteristika souboru: Hodnoceno bylo 108 nemocných s CLL léčených chemoimunoterapií s rituximabem od května roku 2005 do března roku 2011 v našem centru. V rámci 1. linie (n = 66, 71 % mužů, věkový medián 63 let) byly použity tyto léčebné režimy: fludarabin, cyklofosfamid, rituximab – FCR (n = 43) a nízkodávkovaný FCR (n = 13); 10 nemocných bylo léčeno jinými protokoly. 42 nemocných (76 % mužů, věkový medián 65 let) podstoupilo 2. linii: rituximab-dexametazon (n = 18), FCR/nízkodávkované FCR (n = 14) a 10 nemocných jiný režim. Součástí premedikace před infuzí rituximabu byl metylprednisolon 80 mg i.v., paracetamol 1 000 mg p. o. a bisulepin 1 mg i. v., všichni nemocní měli nitrožilní hydrataci minimálně 2 000 ml/den a alopurinol 300–600 mg/den. V 1. cyklu byl rituximab podáván frakcionovaně v celkové dávce 375 mg/m2 a od 2. cyklu formou rychlé infuze v dávce 500 mg/m2. Výsledky: K rozvoji infuzní reakce po rituximabu došlo u 32 % nemocných v 1. linii a 19 % v 2. linii. Jednalo se převážně o mírné reakce a k závažné toxicitě (stupeň III/IV dle NCI CTCAE – National Cancer Institute – The Common Terminology Criteria for Adverse Events) došlo výjimečně (3 % nemocných v 1. linii a 2 % v 2. linii). Reakce se nejčastěji projevila třesavkou, zimnicí, dušností, horečkou či hypotenzí. U všech nemocných bylo možné dokončit infuzi rituximabu v plánované dávce ve stejný den. Hodnocení léčebné odezvy (overall response rate – ORR) neprokázalo statisticky významné rozdíly mezi nemocnými s reakcí či bez infuzní reakce. Nemocní s infuzní toxicitou měli vyšší absolutní počet lymfocytů (1. linie, 87 vs 56 × 109/l, p = 0,21; 2. linie, 101 vs 14 × 109/l, p = 0,043). Při mediánu sledování 36 měsíců nebyl prokázán statisticky významný rozdíl v PFS (doba přežití bez progrese – progression-free survival) a OS (celkové přežití – overall survival) mezi oběma skupinami. Závěry: Výskyt infuzních reakcí u nemocných s CLL léčených rituximabem je poměrně běžný (32 %), avšak při pečlivé premedikaci jsou reakce velmi dobře zvladatelné a těžká forma je vzácná. Potvrdili jsme velmi dobrou snášenlivost rychlé infuze rituximabu. Naše data neprokázala statisticky významnou souvislost mezi výskytem infuzních reakcí a efektem léčby., Background and Aims: Rituximab in combination with chemotherapy is an effective treatment of patients (pts) with chronic lymphocytic leukemia (CLL). The most frequent adverse event of rituximab is infusion-related toxicity, e.g. cytokine-release syndrome that occurs usually during the first infusion. However, there is scarce data on feasibility and tolerability of rituximab infusions in CLL outside clinical trials. Therefore, we performed a single-center retrospective analysis of the frequency of rituximab infusion-related adverse events during the first- and the second line CLL treatment administered in the routine practice. We also analyzed its relation to parameters of tumor load and possible association with treatment efficacy. The safety of rapid infusion of rituximab in CLL pts was also evaluated. Patients and Methods: We analyzed 108 pts with CLL treated with rituximab-containing regimens between March 2005 and May 2011 at our institution. The most common first-line regimens (n = 66, 47 males, median age 63 years) were FCR (43 pts) and low-dose FCR (13 pts); 10 pts were treated by other protocols. Forty-two pts (32 males, median age, 65 years) underwent second line treatment: 18 pts rituximab-dexamethasone, 7 pts FCR, 7 pts low-dose FCR and 10 pts other regimens. Intravenous hydration (2 000 ml daily on days 0 and 1 of cycle), allopurinol 300–600 mg p. o. daily and premedication with methylprednisolone 80 mg i. v., acetaminophen 1 000 mg p. o. and bisulepine 1 mg i. v. were administered before rituximab infusion. Rituximab was given by fractionated infusion (100 mg for 2 hours, then if tolerated well, the rest of the dose with infusion rate escalation from 100 mg/hour up to 400 mg/hour) at the dose of 375 mg/m2 in the first cycle. Subsequent doses (500 mg/m2) were administered by rapid-infusion protocol. Results: Rituximab infusion-related toxicity occurred in 32 % pts (n = 21) during the first line treatment and 19% pts (n = 8) during the second line treatment. Adverse events were predominantly mild and NCI CTCAE grade III/IV occurred rarely (3% in the first line, 2% in the second line). Infusion toxicity manifested predominantly as rigors, chills, fever and hypotension. All patients with adverse events could finish rituximab infusion as initially planned on the same day. Treatment response analysis did not demonstrate statistically significant differences between patients with and without rituximab infusion toxicity. Patients who developed rituximab infusion toxicity had higher absolute lymphocyte count (first line, 87 vs 56 × 109/l, p = 0.21; second line, 101 vs 14 × 109/l, p = 0.043). At the median follow-up of 36 months, there were no statistically significant differences in PFS or OS in both cohorts. Conclusions: Rituximab infusion-related toxicity in pts with CLL is relatively frequent (32%). However, occurrence of infusion-related symptoms can be reduced by proper premedication and severe adverse events are uncommon. In our experience, all patients were able to receive the planned dose of rituximab. Subsequent doses of rituximab could be safely administered by rapid-infusion protocol. Cíl: Sémontův repoziční manévr se v praxi užívá k léčbě pacientů s benigním paroxyzmálním polohovým vertigem zadního polokruhového kanálku. Cílem studie bylo zhodnotit interindividuální a intraindividuální variabilitu při provádění tohoto manévru a definovat parametry, které jeho provedení nejvíce ovlivňují. Metody: Tři zkušení terapeuti provedli manévr oboustranně u 10 zdravých jedinců. Inerciální měřicí jednotka složená z akcelerometru a gyroskopu získávala informace o trajektorii pohybu hlavy v prostoru a její rychlosti. Úhlové změny byly zaznamenány v souřadnicovém referenčním systému. Z odchylek od uvažované roviny polokruhového kanálku byla vyhodnocena data o přesnosti provedení manévru. Výsledky: Parametry ovlivňujícími přesnost manévru jsou výška probanda (p = 0,0252), fáze pohybu (p < 0,0001) a dále terapeut a strana pohybu, přičemž tyto faktory vykazují vliv především ve své interakci. Byl hodnocen vztah fáze pohybu k výšce probandů (p = 0,0130), terapeuta k fázi pohybu (p = 0,0001), terapeuta k výšce probandů (p < 0,0252). Největší zaznamenaný rozsah odchylky od senzorické roviny byl v rozmezí –37,17° až 31° se standardní odchylkou 16,6°. Závěry: Získaná data poukazují na překvapivě vysokou variabilitu při provádění Sémontova manévru ať v porovnání mezi jednotlivými měřeními u jednoho terapeuta nebo v porovnání mezi terapeuty. Analýza vlivu těchto odchylek na úspěšnost terapie benigního polohového paroxyzmálního vertiga a bližší analýza ovlivňujících parametrů by měly být předmětem dalšího výzkumu., Aim: Sémont liberatory manoeuvre is used in the treatment of posterior semicircular canal in patients with benign paroxysmal positional vertigo. The aim of the study was to determine the inter-individual and intra-individual variability in the processing of the manoeuvre and to define the parameters that affect it the most. Methods: Three experienced therapists applied the manoeuvre bilaterally in 10 healthy probands. Inertial measurement unit consisting of the accelerometer and gyroscope recorded the trajectory and speed. Angle changes were placed in the coordinate reference system. Accuracy of the manoeuvre was evaluated based on the deviations from the projected sensory plane. Results: Parameters affecting the accuracy of the manoeuvre included height of a proband (p = 0.0252), phase of the movement (p < 0.0001), therapist and the side of the movement. The effect of these factors is the most pronounced when combined. We assessed interactions of the phase of the movement and the height of the probands (p = 0.0130), the therapist and the phase of movement (p = 0.0001), the therapist and the height of the probands (p < 0.0252). The largest magnitude of deviation from the sensory plane was in the range of –37.17° to 31° with a standard deviation of 16.6°. Conclusions: The data highlight high variability in the implementation of the Sémont liberatory manoeuvre, whether measurements by a single therapist or inter-individually between the therapists are compared. Analysis of the impact of these deviations on therapeutic efficacy in patients with benign paroxysmal positional vertigo and detailed analysis of influencing parameters should be the subject of further research. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Carnosine is a dipeptide formed of the amino acids β-alanine and histidine. Only a limited number of studies have examined the effects of carnosine on sympathetic nerve activation and anxiety. The present study was undertaken to determine the dose-related effects of carnosine on anxiety in the elevated T-maze test (ETM) with electrodermal activity (EDA). Carnosine was injected in three groups of rats with doses of 10 (low dose), 100 (medium dose) and 1000 (high dose) mg/kg i.p. Physiological saline was injected in the sham group. The anxiety scores of the rats were measured with ETM 20 minutes after injection. Then, SCL was measured. The decreased number of entries into the open arm (NEOA), the percentage of time spent in the open arm (% TSOA) and higher EDA [shown by skin conductance level (SCL)] indicate higher anxiety. The NEOA and % TSOA were lower in the high-dose group than in the other groups. SCL was lower in the medium-dose carnosine group than in the high-dose carnosine and sham groups. SCL was higher in the high-dose group than in the medium-dose and sham groups. Our results suggest that high-dose carnosine produced anxiety-like effects as assessed in the SCL and ETM. Cíl: Ukázat na možnosti a bezpečnost IRE v léčbě nádorů. Metodika: Uvádíme soubor 23 nemocných s adenokarcinomem pankreatu, dvou nemocných se solitární jaterní metastázou kolorektálního karcinomu a jednoho adenokarcinomu solitární ledviny. Všichni byli léčeni pomocí IRE perkutánně pod CT kontrolou nebo při laparotomii s US asistencí. Výsledky: IRE je bezpečná metoda, 30denní mortalita nulová. Umožňuje destrukci nádorů v lokalitách, kde ostatní ablační metody selhávají. U neresekabilních nádorů pankreatu však v naší sestavě nepřinesla významné prodloužení života. Průměrná doba přežití ve skupině nádorů pankreatu je 10 měsíců. Závěr: IRE je bezpečná a užitečná ablační metoda zvláště pak nádorů umístěných u kritických struktur., Aim: To show the safety and clinical usefulness of IRE. Methods: A total of 23 pancreatic adeno-carcinomas, two liver mets of colorectal carcinoma and 1 renal adeno carcinoma have undergone IRE percutaneously under CT control or during laparotomy with US guidance. Results: IRE is safe method, 30 days mortality 0. There is no significant prolongation of survival in the group of unresectable pancreatic cancer. Survival in the group of pancreatic cancer is 10 months. Úvod: Z klinického hlediska je zjevné, že karcinom rekta a karcinom kolon jsou ve svém průběhu a léčbě odlišné nosologické jednotky. Cílem bylo analyzovat a objasnit rozdíly mezi chováním jaterních metastáz karcinomu kolon a karcinomu rekta. Studie těchto faktorů je důležitá pro stanovení přesné prognózy a indikace jejich co nejefektivnější terapie a léčby karcinomu kolon a karcinomu rekta jako systémového onemocnění. Metoda: Do našeho souboru bylo zařazeno 223 pacientů s metastatickým postižením jater kolorektálním karcinomem, kteří byli operováni na Chirurgické klinice LF UK a FN Plzeň od 1. 1. 2006 do 31. 1. 2012. Z celkového počtu 223 jedinců bylo 145 (65 %) mužů a 78 (35 %) žen. Jednalo se celkem o 275 výkonů. Resekční výkon byl proveden u 177 pacientů a ošetření pomocí radiofrekvenční ablace (RFA) v celkem 98 případech. Soubor byl rozdělen do kategorií podle lokalizace primárního tumoru na C (kolon) čítající 58 pacientů, S (c. sigmoideum) čítající 61 pacientů a R (rektum) čítající celkem 101 pacientů. Analýza významnosti jednotlivých studovaných parametrů (věk, pohlaví, TNM klasifikace, grading, typ operačního výkonu) byla provedena pomocí ANOVA testu. Celkové přežití (OS), bezpříznakové přežití (DFI) nebo období bez známek onemocnění (NED) bylo hodnoceno za pomoci Kaplan-Meierových křivek, které byly porovnávány pomocí Log-Rank a Wilcoxon testů. Výsledky: Z hlediska porovnávání primárního origa jaterních metastáz kolorektálního karcinomu bez ohledu na jejich ošetření (resekce i RFA) z naší studie vyplývá, že metastázy karcinomu rekta mají statisticky významně časnější recidivu (kratší NED/DFI). V případě ostatních faktorů byl pro prognózu časné recidivy statisticky významný lokálně pokročilý nález u primárního nádoru kolon a sigmoidea, dále podstoupená R2 resekce jaterních metastáz a pozitivita uzlinových metastáz v případě primárního nádoru kolon a sigmoidea. Dále jsme prokázali, že u nemocných s primárním postižením rekta nemá na DFI po resekci metastáz jater vliv pozitivita uzlinových metastáz ani lokální pokročilost primárního nádoru. Další studované faktory (časový odstup diagnózy orgánových metastáz od primární operace, grading, pohlaví či věk) nebyly pro prognózu OS i DFI (souhrnně u kolorektálního karcinomu) prokázány jako statisticky významné. Závěr: Z naší studie je zřejmé, že u karcinomu rekta a karcinomu kolon lze ve vztahu k jejich jaterním metastázám předpokládat různé chování se specifickými prognostickými faktory. Tyto rozdíly nejsou dosud plně objasněny a vyžadují další zkoumání a rozdělení, a to nejen na základě histopatologických, imunohistochemických a klinických faktorů, ale i molekulárně biologických parametrů. Klíčová slova: metastázy karcinomu kolon − metastázy karcinomu rekta − prognostické faktory − celkové přežití – jaterní metastázy, Introduction: From the clinical point of view, rectal cancer and colon cancer are clearly different nosological units in their progress and treatment. The aim of this study was to analyse and clarify the differences between the behaviour of liver metastases from colon and rectal cancer. The study of these factors is important for determining an accurate prognosis and indication of the most effective surgical therapy and oncologic treatment of colon and rectal cancer as a systemic disease. Method: 223 patients with metastatic disease of colorectal carcinoma operated at the Department of Surgery, University Hospital in Pilsen between January 1, 2006 and January 31, 2012 were included in our study. The group of patients comprised 145 men (65%) and 117 women (35%). 275 operations were performed. Resection was done in 177 patients and radiofrequency ablation (RFA) in the total of 98 cases. Our sample was divided into 3 categories according to the location of the primary tumor to C (colon), comprising 58 patients, S (c. sigmoideum) in 61 patients, and R (rectum), comprising 101 patients. Significance analysis of the studied factors (age, gender, staging [TNM classification], grading, presence of mucinous carcinoma, type of operation) was performed using ANOVA test. Overall survival (OS), disease-free interval (DFI) or no evidence of disease (NED) were estimated using Kaplan-Meier curves, which were compared with the log-rank and Wilcoxon tests. Results: As regards the comparison of primary origin of colorectal metastases in liver regardless of their treatment (resection and RFA), our study indicated that rectal liver metastases showed a significantly earlier recurrence than colon liver metastases (shorter NED/DFI). Among other factors, a locally advanced finding, further R2 resection of liver metastases and positivity of lymph node metastases were statistically significant for the prognosis of an early recurrence of the primary colon and sigmoid tumor. Furthermore, we proved that in patients with primary rectal carcinoma, DFI (after the resection of liver metastases) was not influenced by the positivity of lymph node metastases of primary tumor or locally advanced primary tumor. The other factors studied (time from diagnosis of organ metastases to primary operation, grading, sex or age) were not shown to be statistically significant for the prognosis of OS and DFI (colorectal cancer in total). Conclusion: As proven by our study, rectal cancer and colon cancer are two different nosological units with specific prognostic factors with respect to their liver metastases. These differences have not been fully understood yet and require further exploration and classification based not only on histopathological, immunohistochemical and clinical factors, but also on molecular biological parameters. Video prezentuje naši techniku provádění LESS nefrektomie (NE). Materiál: Od 8/2011 do 10/2014 jsme provedli 262 laparoskopických NE, z toho 62 LESS (23,7 %), které jsou dále hodnoceny. Celkem 26 mužů a 36 žen, 33 vlevo, 29 vpravo. Průměrné BMI 26,9±3,9 (16–33,2). Většina pomocí Quadport+®, pouze osm pomocí GelPoint®. Z nich 45 pro nádor, 17 pro nenádorové onemocnění. Indikace pro LESS nebyla dána žádnými striktními kritérii, záleželo na subjektivním hodnocení proveditelnosti operujícím týmem. Operace prováděli čtyři operatéři. Jednalo se zejména o neobézní pacienty s méně pokročilým nádorem. Popis metody: Video prezentuje typický případ. Poloha na boku, transumbilikální minilaparotomie, multikanálový port (Quadport+®). Kamera 10 mm přímohledná, jeden dvakrát zahnutý grasper, pečetící nástroj. Uvolněn dolní pól ledviny, ureter a s ním celý hilus ledviny elevovány vláknem zavedeným přes stěnu břišní. Hilus přerušen en bloc staplerem. Douvolněna ledvina a odstraněna původní minilaparotomií v sáčku (Endocatch®). Minilaparotomie uzavřena bez drénování. Výsledky: U 27 LESS-NE (43,5 %) byl užit transumbilikální přístup (u posledních 19 jen takto), u zbylých pararektální incize. Přídatný port užit ve 27,4 % (17) – 2/33 (6,1 %) vlevo, 15/29 (51,7 %) vpravo. U tří z nich (4,8 %) ještě další port (= konverze na LNE). U dvou (3,2 %) provedena otevřená konverze. Průměrný čas operace byl 96,7±40,0 (28– 230). U nejzkušenějšího operatéra (34 výkonů) to bylo 73,4±20,0 (28–104). Hilové cévy byly přerušeny u 14 (22,6 %) separovaně pomocí uzamykatelných klipů, u 48 (77,4 %) en bloc staplerem. Dimise 5,9±4,8 (2–40) pooperační den. Komplikace dle Claviena 3 x 2, 2 x 3b (operační revize pro lézi tenké kličky zavzaté do stehu minilaparotomie resp. krvácení z jater) a 1 x 4a (pooperační kardiopulmonální resuscitace). Závěr: LESS NE prováděnou zkušeným laparoskopistou je relativně bezpečná a efektivní metoda nefrektomie pro benigní i maligní onemocnění u vybraných případů (BMI<33 a méně pokročilé tumory)., The video presents our technique of LESS nephrectomy. Material: From 8/2011 to 10/2014 we performed 262 mini-invasive nephrectomies including 62 LESS procedures (23.7 %) that are evaluated below. There were 26 men, 36 women, 33 left sided and 29 right sided. The mean BMI was 26.9±3.9 (16–33.2). We used mostly Quadport+® and GelPoint® in 8 cases. There were 45 tumours and 17 benign masses. We had no strict indication criteria for LESS, it depended on subjective evaluation of the feasibility of surgical team. Procedures were performed by 4 surgeons. Nonobese patients with less advanced tumours were included. Method: This video presents a typical case. The surgery begins in a fl ank position using transumbilical minilaparotomy and multichannel port (Quadport+ ®). A 10 mm 0° video camera, one pre-bent grasper and some sealing devices were used. The lower pole of the kidney is deliberated, the ureter and hilum are identifi ed and elevated by using a thread that is introduced through the abdominal wall. The renal hilum is divided en bloc with a stapler. The kidney is totally deliberated without any drainage. Results: We used the transumbilical approach in 27 LESS (43.5 %) nephrectomies (the last 19 only with this procedure) and a pararectal incision in the rest. An additional port was used in 27.4 % (17) – 2/33 (6.1 %) on the left, 15/29 (51.7 %) on the right. In 3 cases of them (4.8 %), another additional port (conventional laparoscopic nephrectomy) was necessary. There were 2 conversions (3.2 %). The mean operation time (OT) was 96.7±40.0 (28–230) minutes. The most experienced surgeon had mean OT 73.4±20.0 (28–104). Hilar vessels were divided separately in 14 (22.6 %) cases using lockable clips, and 48 (77.4 %) cases en bloc with a stapler. The mean hospitalization time was 5.9±4.8 (2–40) days. Complications according to the Clavien-Dindo classifi cation were 3 times grade II and 2 times grade IIIb (revisions for lesion of small intestine sutured to the minilaparotomy and liver bleeding) and one grade IVa (postoperative cardiopulmonary resuscitation). Conclusion: In selected cases (BMI<33 and less advanced tumours).
