Nádory hlavy a krku patří celosvětově mezi karcinomy se vzrůstající incidencí. Hlavním etiologickým faktorem podmiňujícím jejich vznik je kouření, abúzus alkoholu a v poslední době potvrzený HPV. Dvě třetiny pacientů přicházejí v pokročilém stadiu a až třetina pacientů má četné komorbidity. V jejich terapii je nejdůležitější multidiciplinární přístup. Standardem léčby je dle NCCN konkomitantní chemoradioterapie s platinovým derivátem (kategorie 1), nebo radioterapie konkomitantně s biologickou léčbou cetuximabem, nebo chirurgický přístup a dále dle histologie adjuvantní radioterapie či chemoradioterapie (1). V poslední době je zvažována v indikaci léčby také pozitivita HPV. Jedná se o pacienty s lepší prognózou, ale zatím nebyla stanovena standardní terapie. Cetuximab je monoklonální chimerická IgG1 protilátka, která cíleně blokuje extracelulární část EGFR. V České republice je zaregistrován mimo jiné pro léčbu pacientů s lokálně pokročilými nádory hlavy a krku konkomitantně s radioterapií dle Bonnerovy studie fáze III publikované v r. 2006 v N Engl J Med (2), která prokázala pro kombinaci radioterapie a cetuximabu proti radioterapii samotné signifikantní navýšení lokoregionální kontroly 24 měsíců versus 14,9 měsíců, s mediánem OS 54,0 měsíců versus 29,3 měsíců, bez signifikantního navýšení morbidity a toxicity., Worldwide, head and neck tumours are among cancers with an increasing incidence. The main aetiological factors determining their development are smoking, alcohol abuse, and recently detected HPV. Two thirds of patients present in an advanced stage and up to a third of them have numerous comorbidities. A multidisciplinary approach is of utmost importance in the treatment. The standard treatment according to NCCN is concomitant chemoradiotherapy with a platinum derivative (category 1) or radiotherapy concomitantly with biological therapy with cetuximab, or surgical approach, followed by, according to histology, adjuvant radiotherapy or chemoradiotherapy (1). Recently, HPV positivity has also been taken into consideration concerning indications for treatment. This applies to patients with a better prognosis, but no standard treatment has been determined so far. Cetuximab is a chimeric IgG1 monoclonal antibody that targets the extracellular domain of the EGFR. Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. Vemurafenib je malá molekula, která má schopnost inhibovat mutovanou BRAF proteinkinázu. Na základě provedených klinických studií BRIM-2 a BRIM-3, kdy vemurafenib dokázal prodloužit celkové přežití (OS) a dobu do progrese onemocnění (PFS), byl zaregistrována v EU k léčbě v monoterapii u dospělých pacientů s neresekovatelným nebo metastazujícím melanomem s pozitivní mutací V600 genu BRAF., Metastatic melanoma is an aggressive disease. Cíl: Posouzení efektivity biopsie sentinelové uzliny a na ní navazující terapie karcinomu prsu po implementaci metodiky do klinické praxe. Soubor a metodika: V průběhu let 2006-2010 bylo sledováno 94 pacientek s karcinomem prsu po biopsii sentinelové uzliny, průměrného věku 60,6 roku, 100 % ve stádiu do T2 v TNM klasifikaci (85 % T1, 10 % T2, 5 % TIS). Biopsie sentinelové uzliny byla provedena s periareolární aplikací radiofarmaka s většími koloidními částicemi. Pozitivní sentinelové lymfatické uzliny byly nalezeny u 27 % pacientek, z nichž se ve 23 % jednalo o mikrometastatické postižení nebo izolované tumorózní buňky. V případě pozitivity sentinelové uzliny byla indikována exenterace axily. U 79 % z celého souboru pacientek byla provedena radioterapie prsu a výběrově i axily. Následně byly pacientky léčeny podle standardního protokolu a sledovány v onkologické ambulanci Nemocnice Pelhřimov. Průměrná doba sledování byla v období vyhodnocování souboru 3,9 roku. Byly sledovány běžně používané parametry v hodnocení péče o onkologické pacienty. Výsledky: Lokoregionální rekurence byla 1 %, axilární rekurence 0 %, vzdálená rekurence 0 %, přežití bez nemoci dosaženo v 99 % a celkové přežití v 99 % případů. Závěr: Operace řízená výsledky biopsie sentinelové uzliny s periareolární aplikací radiofarmaka u pacientek s karcinomem prsu v kombinaci s radioterapií se jeví jako velmi efektivní, splňující požadavky na lokoregionální kontrolu nemoci a při uplatnění standardního léčebného protokolu zabezpečuje dobré celkové výsledky onkologicko-chirurgické léčby., x, Milan Šimánek, Václav Abrahám, Jaromír Kořínek, and Literatura
INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Cíl studie: Na základě soudobých studií podat přehled o hlavních biologických funkcích lipoproteinů o vysoké hustotě (HDL), zejména ve vztahu k reverznímu transportu cholesterolu a k jejich protizánětlivému působení. Popsat pravděpodobné mechanismy vzniku dysfunkčních prozánětlivých HDL a uvést některé parametry asociované s tímto procesem. Podat přehled o laboratorních metodách stanovení kvantitativních, strukturních a funkčních vlastností HDL, včetně moderních metod založených na hmotové spektrometrii. Typ studie: přehledový článek Závěr: V případě systémového zánětu a/nebo oxidačního stresu dochází ke vzniku dysfunkčních HDL akumulujících oxidanty. Probíhá strukturní modifikace apolipoproteinu AI, což vede mimo jiné k inhibici reverzního transportu cholesterolu a tvorbě prozánětlivých HDL. Na ztrátě příznivých vlastností HDL se podílí modifikace proteinové i lipidové složky HDL. Izolované měření HDL cholesterolu nemusí u některých syndromů bezvýhradně korelovat s mírou kardiovaskulárního rizika a v některých situacích je vhodné stanovit markery mapující strukturu a funkci HDL., Objective: On the basis of recent studies to make an overview concerning the crucial biological functions of high density lipoproteins (HDL), with emphasis on the role in reverse cholesterol transport and their antiinflammatory traits. The aim was to describe probable mechanisms of dysfunctional proinflammatory HDL formation with introduction some of associated parameters. To present laboratory methods for determination of quantitative, structural and functional qualities of HDL, including advanced mass spectrometry techniques. Study design: review Conclusion: In the case of systemic inflammation and/or oxidative stress, the formation of dysfunctional HDL accumulating oxidants takes place, including apo AI structural modifications. This process can lead to the inhibition of reverse cholesterol transport and proinflammatory HDL generation, among others. Loss of beneficial qualities results from modification of both lipid and protein components of HDL. Kvalita výživy ve stáří ovlivňuje zásadním způsobem celkový stav organizmu a významné deficity mohou přispívat k multimorbiditě starších nemocných. Včasnou detekcí malnutrice ve stáří se můžeme vyhnout zhoršení funkční výkonnosti, závažným zdravotním komplikacím, zvýšené nemocnosti, úmrtnosti a zhoršení kvality života. Vyšší věk je spojen s častějším výskytem poruch výživy, především s malnutricí. Prevalence malnutrice u hospitalizovaných geriatrických pacientů se nachází v rozmezí mezi 30–60 % a u dlouhodobě institucionalizovaných dosahuje 52–85 %. Nepřesné posuzování a sledování nutričního stavu seniorů ve zdravotnických zařízeních zdravotnickým personálem může mít zásadní vliv na vznik malnutrice během hospitalizace. Cílem příspěvku je zhodnotit stav výživy seniorů na gerontopsychiatrickém oddělení a posoudit vliv faktorů jako deprese, úroveň kognitivního stavu a soběstačnosti na vznik malnutrice., The quality of nutrition in old age affects significantly the overall condition of the body, and significant deficits may contribute to multimorbidity in elderly patients. Early detection of malnutrition in old age can avoid worsening functional performance, serious medical complications, increased morbidity, mortality and impaired quality of life. Increasing age is associated with a greater incidence of nutritional disorders, especially with malnutrition. The prevalence of undernutrition in hospitalized geriatric patients is in the range between 30 to 60% and in the long-term institutionalized it reaches 52–85%. Inaccurate assessment and monitoring of nutritional status of older people in healthcare facilities healthcare professionals can have a significant influence on the occurrence of malnutrition during hospitalization. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. V posledních letech se udály významné změny v léčbě chronické lymfocytární leukemie (CLL). K pokroku došlo kombinací konvenční chemoterapie s monoklonálními protilátkami. Režim FCR (fludarabin, cyklofosfamid a rituximab) je nyní zlatým standardem v léčbě mladších a fyzicky zdatných pacientů. Starší a komorbidní nemocní, kteří tvoří většinu CLL populace, nejsou zpravidla schopni podstoupit intenzivní léčbu z důvodu vysokého rizika nepřijatelných nežádoucích účinků. U této skupiny nemocných lze tedy dosud považovat chlorambucil jako hlavní léčebnou volbu. Nízkodávkované fludarabinové režimy stejně jako kombinace chlorambucilu s monoklonálními anti-CD20 protilátkami jsou nyní zkoušeny v randomizovaných klinických studiích. Další léčebné možnosti, u kterých zatím chybí podrobnější data pro skupinu starších a komorbidních nemocných a které musí být přísně individualizovány, představují použití bendamustinu v 1. linii, alemtuzumabu, vysokodávkovaných glukokortikoidů či ofatumumabu v léčbě relapsu/refrakterní CLL. Přidružené choroby, tělesná výkonnost a biologický věk jsou důležitými prognostickými faktory, které ovlivňují rozhodování o volbě léčby. Článek poskytuje přehled současných a budoucích léčebných možností u starších a komorbidních pacientů s CLL., The management of chronic lymphocytic leukemia (CLL) has recently undergone major changes. The progress was achieved by combining conventional chemotherapy with monoclonal antibodies. Chemoimmunotherapy, FCR (fludarabine, cyclophosphamide, rituximab) is currently established as a gold standard in younger and physically fit patients. However, elderly and/or comorbid patients, who represent the majority of CLL population, may not benefit from such an intensive approach due to high risk of unacceptable toxicity. Chlorambucil therefore remains the preferred treatment of choice in these patients. Dose-reduced fludarabine-based combinations as well as protocols containing chlorambucil with anti-CD20 monoclonal antibodies are currently evaluated in randomized clinical trials. Further treatment options include bendamustine in 1st line and high-dose glucocorticoids, alemtuzumab or ofatumumab in refractory CLL; however, experience in older/comorbid patients is still limited and the use of these agents has to be strictly tailored. Comorbidity, performance status, biological age are important prognostic factors that influence decision-making process of treament choice.
