- V průběhu posledních let dochází k nárůstu počtu imunosuprimovaných nemocných a tím i k vzestupu invazivních mykotických onemocnění. Terapie hlubokých mykóz je mnohdy obtížná, mortalita a morbidita těchto infekčních komplikací jsou vysoké. Stále lepší dostupnost nových antimykotik rozšiřuje naše léčebné možnosti a zlepšuje výsledky terapie těchto invazivních mykotických infekcí. Předkládaná práce podává informace o využití jednotlivých antimykotik v léčbě invazivních mykóz, shrnuje léčbu invazivní aspergilózy (IA), invazivní kandidózy (IC) a empirickou antimykotickou terapii. Doporučení vychází ze závěrů prezentovaných Europen Conference on Infection in Leukemia (ECIL)., The frequency of invasive fungal infections has significantly increased with the rise in at-risk populations of patients in the last years. Management of deep fungal infection is difficult and the morbidity and mortality of these infections are very high. New antifungal agents provide the managment options and improve therapeutic outcomes of these infections. This article informs about the role of available antifungal agents in the management of invasive fungal infections and reviews empirical antifungal treatmnet and the treatment of invasive aspergillus and candida infections. - The aim of our study was to determine the effect of selected cytostatics on a human ovarian cancer cell line A2780 as a model system for ovarian cancer treatment. This cell line is considered cisplatin-sensitive. Panel of tested cytostatics included cisplatin, paclitaxel, carboplatin, gemcitabine, topotecan and etoposide. These cytostatics have a different mechanism of action. To evaluate cytotoxic potential of the tested compounds, the methods measuring various toxicological endpoints were employed including morphological studies, MTT assay, dynamic monitoring of cell proliferation with xCELLigence, cell cycle analysis, caspase 3 activity and expression of proteins involved in cell cycle regulation and cell death. - Brachial Plexus is formed by the union of the anterior rami of cervical 5, 6, 7, 8 and thoracic 1 nerves. These nerves unite and divide to form the key nerves innervating the upper limb. Variations in the course of these nerves are clinically important to anesthetists, neurologists and orthopedicians. We report bilateral variations in the arterial and neural structures in the upper limb of a 65 year old cadaver. The muscles of the arm on one side were innervated by the median nerve with absence ofmusculocutaneous. While on the other side the musculocutaneous nerve contributed to the formation of the median nerve. There was a presence of high bifurcation of brachial artery on both sides. Knowledge of such variations in the innervations of muscles and the arterial supply of the limbs are important to remember before performing any reconstructive procedures or interventions on the limb. and V. - Unilateral otosclerosis combined with avascular necrosis of stapes crura is a rare entity. It should be considered in a case of high grade otosclerosis. Symptoms are the same as in patients who suffer from common otosclerosis. Patients complain on progressive hearing loss and tinnitus. The diagnosis is made clinically by conventional audiologic evaluation and radiologically by x-ray mastoid Schuller's view and CT scan. HRCT scan makes visible all parts of ossicular chain and gives surgeon some information about ossicular chain damage. - Významný úspech liečby s inhibítormi tyrozínkinázy (TKI) u chorých s Philadelphia chromozóm (Ph) pozitívnou chronickou myelocytovou leukémiou (CML) dramaticky zmenil prognózu tohto ochorenia. Aj napriek tomu, že prvolíniová liečba CML použitím imatinibu znamená významný benefit pre väčšinu chorých, ukazuje sa, že 25–35 % pacientov potrebuje v priebehu 5 rokov zmenu liečby z dôvodu rezistencie alebo zlyhania liečby. Najčastejším mechanizmom zodpovedným za rezistenciu proti imatinibu u pacientov s CML je rozvoj mutácií v BCR-ABL kinázovej doméne. Tieto mutácie vyvolávajú rôznu mieru rezistencie proti imatinibu a zatiaľ čo niektoré z nich môžu reagovať na zvýšené dávky imatinibu, iné sú rezistentné proti nilotinibu a ďalšie vykazujú zvýšenú mieru rezistencie proti dasatinibu. Práca je prehľad liečebných možností v primárnej liečbe chronickej fázy CML a pri jej zlyhaní. Ukazuje sa, že pokroky v molekulových metódach umožňujú lepšie porozumieť podstatu choroby, zvážiť benefit a riziko terapie, individualizovať terapeutický prístup a prispôsobiť liečbu CML včas tak, aby sa minimalizovalo riziko progresie do akcelerovaných fáz., The phenomenal success of therapy with tyrosine kinase inhibitors (TKI) in Philadelphia chromosome (Ph) – positive chronic myeloid leukemia (CML) has dramatically changed the prognosis of this disease. Despite the fact that first-line therapy of CML using imatinib represents revolutionized the treatment of this disease, it became clear that during 5 years 25–35 % of the patients require change in the therapy due to the development of imatinib resistance or failure. The most frequent mechanism responsible for imatinib resistance is development of mutation in BCR-ABL kinase domain. Mutations cause different level of imatinib resistance and while some of them can be overcome by increased dose of imatinib, others seem to be resistant to nilotinib and others are more resistant to dasatinib. This review is focused on primary treatment and treatment after imatinib failure. It has been shown recently that advances in molecular methods enable to better understand disease itself, weight benefit to risk ratio of the therapy, individualize therapeutic approach and eventually adjust CML therapy earlier in order to minimize the risk of CML progression to accelerated phase. resistance, new drugs., Elena Tóthová, Adriana Kafková, and Literatura 28
