N‑metyl‑D‑aspartát receptorová (NMDAR) encefalitida je nedávno definované autoimunitní postižení centrálního nervového systému (CNS). Podstatou neurologických potíží je porucha synaptického přenosu v důsledku internalizace NMDA receptorů po navázání autoprotilátek. V následujícím sdělení přinášíme nejnovější informace o onemocnění a jeho diagnostice. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. Předkládáme popis případu nemocného s familiární formou amyloidové polyneuropatie. Mladý nemocný s postupně progredujícími projevy léze periferních nervů byl zprvu diagnostikován a léčen pro chronickou zánětlivou demyelinizační polyneuropatii. Pro následné komplikace bylo provedeno endoskopické vyšetření zažívacího traktu s bioptickým průkazem amyloidových depozit. Následné komplexní vyšetření potvrdilo hereditární formu transtyretinové amyloidové polyneuropatie na podkladě raritní mutace v genu pro transtyretin na pozici Val50Ala. Anamnesticky, matka nemocného byla léčena pro amyloidózu s neuropatickými projevy. Léčba pacienta preparátem tafamidis meglumine vedla k zabránění další progrese onemocnění a následně byla provedena ortotopní transplantace jater. V současnosti dochází ke klinickému zlepšování stavu nemocného s postupnou regresí motorických a senzitivních neuropatických projevů., We present a case of a patient with familial amyloid polyneuropathy. A young patient with gradually progressing symptoms of peripheral neuropathy was initially diagnosed and treated for chronic inflammatory demyelinating polyneuropathy. Subsequent complications resulted in endoscopic examination of the digestive tract. Biopsy showed evidence of amyloid deposits. Subsequent comprehensive examination confirmed hereditary form of amyloid polyneuropathy on the basis of a rare mutation in the transthyretin gene in Val50Ala position. Family history revealed that the mother of the patient was treated for amyloidosis with neuropathic symptoms. Treatment with tafamidis meglumine prevented further disease progression, and was followed by orthotopic liver transplantation. At present, clinical condition of the patient gradually improves with regression of motor and sensory neuropathic symptoms. Autoři na dvou předložených kazuistikách představují širokou diferenciální diagnostiku granulomatózních procesů hepatobiliárního traktu. Izolovaný granulomatózní zánět v této oblasti nacházíme velmi zřídka. Diferenciální diagnostika těchto procesů zasahuje od infekční etiologie až po systémová onemocnění či chemickou iritaci. Granulomy často imitují maligní tumory v jaterní krajině, a chirurg tak může být lékařem, který dané onemocnění odhalí jako první při resekčním výkonu či cholecystektomii., The authors present two case reports to demonstrate wide differential diagnosis of granulomatous disorders of the hepatobiliary tract. Intramurální hematom jícnu je vzácně se vyskytující diagnóza. Prezentuje se retrosternální bolestí, dysfagií či hematemezou. Je způsoben barotraumatem či přímým poraněním jícnu, nebo se může objevit spontánně bez zjevné příčiny, což je častější u pacientů užívajících antiagregační či antikoagulační preparáty. Ve většině případů je léčba konzervativní, v ojedinělých případech endoskopická, endovaskulární či chirurgická. Tento článek prezentuje kazuistiku pacientky se spontánním hematomem jícnu řešeným konzervativně a diskuzi o dalších možnostech průběhu a léčby onemocnění., Intramural hematoma of the oesophagus is a rare diagnosis presenting with retrosternal pain, dysphagia and haematemesis. It is caused by barotrauma or direct injury to the esophagus, or it may occur spontaneously without apparent cause, which is more common in patients taking antiplatelet or anticoagulant medications. In most cases, treatment is conservative. Rarely, endoscopic, endovascular or surgical treatment is effective. Incidence primárního lymfomu mléčné žlázy představuje méně než 0,5% z celkového počtu maligních onemocnění prsu. Tím patří do skupiny vzácně se vyskytujících malignit, jejichž četnost je menší než 5 %, z celkového počtu zhoubných nádorů. Vyskytuje se ve formě primárního onemocnění nebo v rámci systémového onemocnění. Klinicky se postižení prsu primárním lymfomem manifestuje jako hmatná bulka. Mamografický, ultrazvukový i obraz na magnetické rezonanci není zcela specifický, většinou se lymfom projevuje jako ohraničená, hypervaskularizovaná expanze s tendencí k rychlému růstu. Přestože je výskyt lymfomu vzácný, je potřeba na tuto klinickou jednotku myslet. Včasná diagnóza a následně zahájená léčba, která se zásadně liší od terapie karcinomu prsu, velmi často vede k remisi onemocnění., Incidence of primary lymphoma of the breast is less than 0.5% of all malignant breast lesions. Therefore it belongs in the group of rare malignant diseases, frequency of less than 5% of all malignant tumors. Lymphoma could be found as primary extranodal lymphoma or secondary to systemic involvment. The first clinical sign is usually a palpable mass or nodule. Its appearance on mammography, sonography and also magnetic resonance is nonspecific, usually a well circumscribed, hypervascularised lesion with fast growth tendency. Although breast lymphoma is rare it has to be considered in differencial diagnosis. Low-grade myofibroblastic sarcoma (LGMS) is a very rare, atypical myofibroblastic tumor with fibromatosis-like features with predilection mostly in head and neck region. LGMS occurs primarily in adult patients with a slight male predominance. Only few cases of LGMS affecting the larynx have been reported in literature to this date. We describe a case of low-grade myofibroblastic sarcoma of the larynx in a 40-year-old male patient. Neurologické postižení je poměrně vzácná komplikace hepatitidy E. Uvedená kazuistika popisuje 52letého imunokompetentního muže s aseptickou meningitidou a mnohočetným postižením periferních nervů na horních končetinách typu mononeuropatia multiplex, u kterého byla sérologickým vyšetřením prokázána akutní virová hepatitida E. Navíc byl virus hepatitidy E zjištěn polymerázovou řetězovou reakcí v séru i likvoru (genotyp 3f). Pacient byl léčen pouze symptomaticky, po půl roce přetrvával mírný neurologický deficit. Infekci virem hepatitidy E je vhodné zvažovat u pacientů se současným neurologickým a jaterním postižením., Neurological complications of acute hepatitis E are relatively rare. The case report describes mononeuropatia multiplex with aseptic meningitis in a 52‑year‑old immunocompetent man. Viral hepatitis E was confirmed serologically and by polymerase chain reaction from the blood and cerebrospinal fluid (genotype 3f). Symptomatic treatment was given. Residual neurological deficit persisted six months after the onset of the disease. Diagnostic of hepatitis E should be considered in patients with concurrent neurological symptoms and liver cytolysis. Oncocytic Schneiderian papilloma (OSP) is one of the three morphologically distinct tumors that arise from Schneiderian membrane (the others include exophytic papilloma and inverted papilloma). OSP almost always occurs unilaterally in the paranasal sinuses, usually in the maxillary sinus, ethmoid cells or sphenoid sinus. We report a case of a 64-year-old woman with OSP arising from the left frontal sinus. In the report herein, we describe an OSP originating in the region of frontal sinus, which, to the best of our knowledge, represents the first documented example in English literature of OSP developing in this anatomical site. and D. Skull-base osteomyelitis is a rare disease affecting predominantly patients with diabetes mellitus or with immune dysfunction. The major symptoms include intensive local pain and usually unilateral cranial neuropathy. Pseudomonas aeruginosa or Staphylococcus aureus are the leading causative agents. The diagnosis is confirmed with magnetic resonance imaging (MRI) or single-photon emission tomography (SPECT), eventually combined with computer tomography (SPECT/CT). Broad-spectrum antibiotic treatment with effective pharmacokinetics to bone structures over several weeks is needed. The authors present a case report of a 75-years-old woman with skull-base osteomyelitis who was erroneously treated for neuroborreliosis. The MRI showed an extensive destructive process of the skull-base affecting clivus, left mandible and the first 3 cervical vertebrae. The 14 days lasting treatment with meropenem and subsequent long-term combination treatment with rifampicin/co-trimoxazole led to remission of the disease., Osteomyelitida baze lební je vzácné onemocnění postihující především pacienty s diabetes mellitus 2. typu a s porušenými funkcemi imunitního systému. Klinické projevy zahrnují intenzivní lokální bolest a postižení různých hlavových nervů. Nejčastějším vyvolávajícím agens je Pseudomonas aeruginosa nebo Staphylococcus aureus. Diagnózu lze potvrdit provedením vyšetření hlavy magnetickou rezonancí (MR) nebo jednofotonovou emisní tomografií (SPECT), případně kombinovanou s počítačovou tomografií (SPECT/CT). Léčba vyžaduje několikatýdenní podávání širokospektrých antibiotik pronikajících do kosti. V předkládané kazuistice prezentují autoři případ 75leté ženy s osteomyelitidou baze lební, která byla mylně léčena pro neuroborreliózu. Pomocí MR byl prokázán rozsáhlý destruktivní proces celé baze lební s přestupem na clivus, na mandibulu vlevo a na první tři krční obratle. Léčba meropenemem po dobu 14 dní a dlouhodobá udržovací terapie kombinací rifampicin/kotrimoxazol vedla k remisi obtíží.
