Cíl: Průzkumné šetření zjišťovalo schopnosti spolupráce nemocných při léčbě Warfarinem a dodrţování léčebného reţimu v průběhu dlouhodobé antikoagulační terapie Warfarinem. Konkrétně byla mapována a vyhodnocena úroveň edukace sestrou, informovanost a adherence k léčbě u pacientů léčených Warfarinem. Metodika: Průzkumný soubor tvořilo 108 pacientů evidovaných v ambulantním léčení interní a kardiologické ambulance při interním oddělení Nemocnice ve Frýdku-Místku. Průzkumné šetření probíhalo v prosinci roku 2010 a lednu roku 2011. Pro sběr dat byla vyuţita metoda dotazníkového šetření. Výsledky: Výsledky průzkumného šetření ukazují, ţe naprostá většina pacientů léčených Warfarinem se aktivně podílí na léčebném procesu. Nemocným se od zdravotníků dostává poučení o léčbě, sestry se na edukaci podílejí v 75 %. Většina dotázaných dochází na pravidelné kontroly a zná zpaměti uţívanou dávku léku. Čtvrtina nemocných však není schopna uvést krvácení jako hlavní riziko léčby. Převáţná většina respondentů si je vědoma skutečnosti, ţe účinek léčby můţe být ovlivněn sloţením přijímané potravy, méně neţ polovina respondentů zná interakci Warfarinu s jinými léky či doplňky stravy. Nemocní mají na zřeteli, ţe je nutno informovat kaţdého lékaře o této léčbě, jen polovina respondentů však informuje i lékárníka. Závěr: Adherence k léčbě ze strany pacienta je základem úspěšné a bezpečné terapie Warfarinem. Cílem všech zdravotníků by měla být snaha o kvalitní a efektivní komunikaci s nemocnými, snaha o dosaţení co nejvyšší úrovně spolupráce, ochoty a schopnosti pacientů řídit se pokyny zdravotníků. Tím se výrazně zvýší úspěšnost léčby a eliminuje se riziko moţných komplikací léčby., Aim: Exploratory investigation ascertained capabilities of cooperation of patients when treated with warfarin and adherence to treatment regimen during long term anticoagulant therapy by warfarin. Specifically was mapped and assessed the level of education by nurse, foreknowledge and adherence to treatment in patients treated with warfarin. Methods: The exploratory set consisted of 108 patients, who were registered in ambulant treatment of internal and cardiac ambulance at internal department of Hospital in Frydek-Mistek. Exploratory investigation took place in December 2010 and January 2011. For data collection was used method of questionnaire survey. Results: The results of questionnaire survey show, that absolute majority of patients treated with warfarin is actively participating in the treatment process. The patients receive from the health care providers education about the treatment, nurses are participating in education in 75%. Majority of respondents have regular check ups and knows by heart the dose of the drug. A quarter of patients is not able to state bleeding as the main risk of treatment. The vast majority of respondents are aware of the fact, that the treatment effect may be influenced by the composition of the diet, less than half of the respondents know the warfarin interactions with other medication or supplements. Patients have in mind that every doctor should be informed about this treatment, only half of the respondents, however, informs as well the pharmacist. Conclusion: Adherence to treatment by the patient is the foundation of a successful and safe warfarin therapy. The goal of all health professionals should be to aim for quality and effective communication with patients, effort to achieve the highest level of cooperation, willingness and ability of patients to follow medical instructions. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. Cieľ: Cieľom štúdie bolo identifikovať úroveň emočnej inteligencie (EI) u sestier a zistiť rozdiely v úrovni EI vzhľadom na vek, dĺžku praxe a vzdelanie sestier. Metodika: Výskumný súbor tvorili sestry (n = 280) poskytujúce starostlivosť pacientom štyroch nemocníc Trnavského kraja. Priemerný vek sestier bol 39,64 (SD = 9,44), dĺžka ich profesijnej praxe bola 18,7 (SD = 10,64). Na meranie úrovne EI boli použité dotazníky, ktoré reprezentujú súčasné prístupy chápania EI. Dotazník SITEMO (Situational Test of Emotional Understanding), zisťuje úroveň EI ako schopnosti, výsledkom je celkové skóre EI, ktoré je dané súčtom správnych odpovedí. Dotazník SEIS (Schutte EI Scale) zisťuje úroveň EI sestier ako črty. Sestry vyjadrovali mieru súhlasu resp. nesúhlasu prostredníctvom 5bodovej Likertovej škály. Pri spracovaní dát boli využité metódy deskriptívnej štatistiky a induktívnej štatistiky. Údaje boli spracované v štatistickom softvéri SPSS 15.0. Výsledky: Zistili sme štatisticky významný rozdiel (p < 0,05) v úrovni EI sestier vzhľadom k ich veku. Pri ostatných sociodemografických znakoch (dĺžka profesijnej praxe, vzdelanie) nebol zaznamenaný štatisticky významný rozdiel (p > 0,05). Sestry s Mgr. stupňom vzdelania dosiahli najlepší výsledok v priemernom poradí mier EI v oboch metodikách. Záver: Zistené výsledky umožňujú upriamiť pozornosť na rozvoj EI u sestier prostredníctvom možností vzdelávacích inštitúcií. Môžeme podporovať zmeny, ktoré budú prispievať ku zvýšenej kvalite poskytovanej ošetrovateľskej starostlivosti. Využívanie EI je nutnou a zároveň modernou požiadavkou ošetrovateľstva., Aim: The aim of this study was to identify the level of emotional intelligence (EI) of nurses and to identify differences in the level of EI in relation to their age, length of work experience and education. Methods: The sample consisted of nurses (n = 280) providing care to patients in four hospitals in the Trnava Region. The mean age of the nurses was 39.64 years (SD = 9.44) and the length of their work experience was 18.7 years (SD = 10.64). The level of EI was measured using questionnaires representing current approaches to understanding of EI. Emotional intelligence as an ability was determined by the Situational Test of Emotional Understanding (SIT-EMO) questionnaire, yielding the overall emotional intelligence score given by the sum of the correct answers. EI as a feature, on the other hand, was assessed by the Schutte EI Scale (SEIS) in which the nurses expressed the extent to which they agree or disagree with each statement on a five-point Likert scale. To analyze the data, methods of descriptive statistics and inductive statistics were used. The data were processed using the SPSS 15.0 statistical software. Results: A statistically significant difference (p < 0.05) was found between the level of nurses’ EI level and their age. In terms of other social demographic data (length of work experience, education), a statistically significant difference (p > 0.05) was not found, although the nurses with a master’s degree ranked highest as for their mean level of EI, using both methods. Conclusion: The results enable to focus attention on the development of nurses’ EI through educational institutions. Changes may be encouraged that will contribute to an increase in the quality of nursing care provided. Cíl: Hlavním cílem výzkumného šetření bylo zjistit, které faktory ovlivňují pouţívání ošetřovatelských diagnóz v klinické praxi. Dalším cílem bylo zjistit rozdíly v hodnocení faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi podle věku, vzdělání a typu pracoviště. Metodika: Pro získání dat byl sestaven nestandardizovaný dotazník zaměřený na zjištění faktorů ovlivňujících pouţívání ošetřovatelských diagnóz. Výzkumný soubor tvořilo 253 registrovaných všeobecných sester z 9 zdravotnických zařízení v České republice. Sestry hodnotili na Likertově škále od 1 do 5, jak jednotlivé faktory ovlivňují pouţívání ošetřovatelské diagnostiky v praxi. Výzkumné šetření probíhalo od června 2011 do listopadu 2011. Výsledky: Podle našeho výzkumného souboru nejvíce ovlivňují pouţívání ošetřovatelských diagnóz v praxi následující faktory: počet pacientů na 1 sestru, administrativní zátěţ, syndrom vyhoření, nedostatek času, rozdíly v teorii a praxi a nedostatek personálu. Statisticky významné rozdíly v hodnocení faktorů podle věku byly zjištěny v hodnocení 2 faktorů: rozdíly v teorii a praxi, nedostatek motivace. Podle dosaţeného vzdělání byly statisticky významné rozdíly zjištěny v hodnocení 4 faktorů: nejednotnost ošetřovatelských diagnóz, terminologie diagnóz, rozdíly v teorii a praxi, rutinní práce. V závislosti na typu pracoviště byly statisticky významné rozdíly nalezeny v hodnocení 5 faktorů: terminologie diagnóz, rozdíly v teorii a praxi, nedostatek praktických zkušeností s ošetřovatelskou diagnostikou, rutinní práce, syndrom vyhoření. Závěr: Rozeznání faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi a zlepšení institucionální podpory můţe ulehčit implementaci ošetřovatelské diagnostiky v klinické praxi, Aim: The main aim of the study was to identify which factors influenced the use of nursing diagnoses in clinical practice. Another aim was to ascertain whether age, education and type of workplace affected the relative importance accorded to these factors. Methods: A non-standardized questionnaire on factors influencing the use of nursing diagnoses was administered to a sample consisting of 243 registered general nurses from 9 healthcare facilities in the Czech Republic. Using Likert´s 5-point scale, the nurses rated the importance of several factors in influencing their use of nursing diagnoses in practice. The research was conducted between June 2011 and November 2011. Results: Based on our sample, there are several factors which influence the use of nursing diagnoses in practice. Some of them are: number of patients per nurse, administrative strain, burnout syndrome, lack of time, discrepancies between theory and practice, lack of staff. Statistically significant age-related differences in rating factors were found for the following: discrepancies between theory and practice, lack of motivation. Statistically significant differences in ratings relative to education were found for four factors: disunity of nursing diagnoses, diagnostic terminology, discrepancies between theory and practice, workplace routine. Based on type of workplace, statistically significant differences in ratings were found for five factors: diagnostic terminology, discrepancies between theory and practice, lack of practical experience in nursing diagnostics, workplace routine, burnout syndrome. Cíl: Cílem studie bylo zjistit, jak rodinní pečovatelé pečující o blízkou osobu v České republice hodnotí definující charakteristiky ošetřovatelské diagnózy Přetížení pečovatele. Metodika: Na klinickou validizaci ošetřovatelské diagnózy Přetížení pečovatele byl použit „Na pacienta zaměřený Fehringův model klinické diagnostické validity“. Výzkum u rodinných pečovatelů probíhal v měsících srpen a září 2010 v Agentuře domácí péče Victoria na území města Šumperk. Do výzkumného souboru bylo vybráno 50 rodinných pečovatelů, kteří pečovali o své blízké v domácím prostředí déle než 2 roky. Souhlas se zařazením do výzkumu vyjádřilo 23 rodinných pečovatelů, co je 46 %. Výsledky: Rodinní příslušníci za hlavní definující charakteristiky Přetížení pečovatele označili celkem 8 charakteristik: obavy z možné institucionalizace péče příjemce; obavy, jak vyřešit péči o člena rodiny, když nebudu schopen tuto péči vykonávat; obavy o zdraví příjemce péče v budoucnosti; obavy související se schopností pečovatele poskytovat péči v budoucnosti; konflikt v rodině; nejistota ze změny vztahu s příjemcem péče; smutek ze změny vztahu s příjemcem péče; starost o členy rodiny. Za vedlejší bylo označeno 10 definujících charakteristik: obtíže v dokončování požadovaných úkolů, obtíže se sledováním příjemce péče v průběhu nemoci, obtíže ve vykonávání požadovaných úkolů, vysoký krevní tlak, kardiovaskulární onemocnění, somatizace, frustrace, depresivní nálada, porušený spánek, stres. Závěr: Pro skupinu vybraných pečovatelů nebyly všechny definující charakteristiky uvedeny v klasifikačním systému NANDA International významné., Aim: The aim of the study was to find out how the family caregivers caring for their close relatives in the Czech Republic evaluate defining characteristics of the nursing diagnosis Caregiver Role Strain. Methods: Fehring´s patient – focused clinical diagnostic validity model was used to clinical validation of the nursing diagnosis Caregiver Role Strain. The research was ongoing during the months August and September 2010 at Home Care Agency Victoria in region of city Šumperk. Fifty family caregivers, who take care for their family members more than 2 years, were chosen to the research sample. Twenty three caregivers (46 %) agreed with their including to the research. Results: Caregivers classified in total the following eight defining characteristics as major ones: apprehension about possible institutionalization of care receiver, apprehension about care receiver´s care if caregiver unable to provide care, apprehension about the future regarding care receiver´s health, apprehension about the future regarding caregiver´s ability to provide care, family conflict, uncertainty regarding changes relationship with care receiver, grief regarding changed relationship with care receiver, concerns about family members. Caregivers classified ten defining characteristics as minor ones: difficulty completing required tasks, difficulty watching care receiver go through the illness, difficulty performing required tasks, hypertension, cardiovascular disease, somatization, frustration, feeling depressed, disturbed sleep, stress. Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. Cieľ: Cieľom práce bolo validizovať defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré defi nujúce charakteristiky sú sestrami považované za hlavné a ktoré za vedľajšie. Metódy: Na hodnotenie defi nujúcich charakteristík ošetrovateľskej diagnózy Defi cit vedomostí bol zvolený Fehringov model validity diagnostického obsahu. Výsledky: Za hlavné defi nujúce charakteristiky boli označené: „slovné vyjadrenie problému“ a „chybné, nepresné dodržiavanie inštrukcií“. Za vedľajšie defi nujúce charakteristiky boli označené: „chybné, nepresné výsledky testu“ a „neprimerané správanie“. Charakteristika „prehnané správanie“ dosiahla vážené skóre menej ako 0,5. Záver: Nie všetky defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí uvedené v klasifi kačnom systéme NANDA-International sú pre vybraný súbor sestier rovnako významné pre jej stanovenie., Aim: The aim of the study was to validate the nursing diagnosis Deficient Knowledge by a chosen sample of Slovak nurses – experts, i. e. to find out which defining characteristics are considered major and minor. Methods: The Fehring’s Diagnostic Content Validity Model was used to evaluate the defi ning characteristics of nursing diagnosis Deficient Knowledge. Results: Th e following defi ning characteristics were marked as major: “verbalization of the problem”, “inaccurate compliance with instructions”. The following characteristics were marked as minor: “poor and inaccurate performance in a test” and “inappropriate behaviour”. The defining characteristic “exaggerated behaviour” had the weighted ratio less than 0.5. Cieľ: Cieľom práce bolo zistiť, aký vplyv má bolesť na vybrané dimenzie ţivota pacientov v domácom prostredí, medzi ktoré patrí nálada, vzťah k iným ľuďom, radosť zo ţivota, spánok. Metodika: Súbor tvorilo 144 respondentov (91 ţien a 53 muţov) v podmienkach domáceho prostredia vo veku od 29 do 97 rokov v siedmich samosprávnych krajoch Slovenska. Zber údajov prebiehal v období november 2008 aţ január 2009. V rámci realizácie zberu údajov bol pouţitý štandardizovaný dotazník BPI (Brief Pain Inventory). Výsledky boli vyhodnotené jednorozmernou deskriptívnou štatistikou. Výsledky: Prítomnosť bolesti v skúmanej vzorke negatívne ovplyvňuje náladu, spánok a radosť zo ţivota viac u muţov ako u ţien. U ţien viac ovplyvňuje bolesť vzťah k iným ľuďom ako u muţov. Štatistickým spracovaním nebola zistená signifikantná súvislosť medzi premennými: nálada – pohlavie (p = 0,387), vzťah k iným ľuďom – pohlavie (p = 0,876), spánok – pohlavie (p = 0,065), radosť zo ţivota – pohlavie (p = 0,238). Záver: Bolesť výrazne ovplyvňuje vybrané dimenzie ţivota pacientov v domácom prostredí, čo vedie k širokej škále negatívnych emocionálnych reakcií a prejavov, ktoré môţu zintenzívňovať bolesť. Pravidelné hodnotenie vplyvu bolesti na jednotlivé dimenzie ţivota pacientov prispieva veľkou mierou taktieţ ku kvalitnej komunikácií medzi sestrou pracujúcou v domácom prostredí a pacientom zaloţenou na vzájomnej dôvere., Aim: The aim of our work was to determine the impact on pain in selected domains of life of patients in home care in terms of their mood, relationship to other people, joy of life and sleep. Methods: 144 respondents (91 women and 53 men aged from 29 to 97) in home care were randomly selected to make up the sample in 7 Slovak municipalities. The data collection was gathered from November 2008 to January 2009 using brief Pain Inventory. The results were evaluated by one-dimensional descriptive statistics. Results: The presence of pain among the respondents negatively influences the spirits, sleep and joy of life; more among men than women. Pain affects relationship to other people more among women than men. Statistical analysis showed no significant correlation between variables: spirits - sex (p = 0.387), relationship to other people – sex (p = 0.876), sleep – sex (p = 0.065), the joy of life - sex (p = 0.238). Conclusion: Pain has a significant impact in selected domains of patient`s life in home care leading to a wide range of negative emotions and behavior that could intensify the pain. Cíl: Cílem výzkumu bylo zjistit kvalitu života seniorů s demencí pohledem rodinných příslušníků. Dále pak porovnat vnímání kvality života seniorů s demencí rodinnými příslušníky v závislosti na prostředí, ve kterém senior žije. Metodika: Byl proveden kvalitativní výzkum, metodou sběru dat byl nestandardizovaný rozhovor. Výzkumu se zúčastnilo 9 rodinných příslušníků pacientů s demencí. Na základě analýzy dat byly stanoveny 4 kategorie: 1. změny v psychické oblasti, 2. změny kognitivních funkcí, 3. změny v oblasti aktivit denního života a instrumentálních všedních činností a 4. celkové vnímání kvality života. Výsledky: Z pohledu rodinných příslušníků nejvíce zhoršuje kvalitu života seniorů s demencí ztráta paměti, která je vyřazuje z běžného života, dále ztráta schopnosti orientace v prostoru a čase a ztráta schopnosti vykonávat činnosti běžného denního života. Kvalitu života snižují také smyslové poruchy, které pacienty uvádějí do ještě větší sociální izolace a mohou v nich podporovat nárůst paranoidity. Rodinní příslušníci hodnotili kvalitu života svých blízkých v domácím prostředí výrazně lépe v porovnání s kvalitou života pacientů s demencí umístěných v domově pro seniory. V domácím prostředí měli senioři více podnětů, byli více aktivizováni a vtahováni do rodinného a společenského života. Po nástupu do institucionální péče došlo k zhoršení příznaků onemocnění a postupnému vyřazení nemocných z aktivizačních programů. Závěr: Důležitou součástí zvýšení kvality života pacientů s demencí je podpora rodinných pečovatelů, aby nemocní mohli zůstat co nejdéle ve svém přirozeném prostředí., Objective: The aim of the research was to determine the quality of life of seniors with dementia from the perspective of family members. Furthermore, to compare the perceptions of quality of life of seniors with dementia by family members depending on the environment in which the senior lives. Methodology: A qualitative research was performed, for the method of data collection was used non-standardized interview. Research was attended by nine family members of patients with dementia. Based on analysis of data there were determined four categories: 1. changes in the psychological field, 2. changes in cognitive function, 3. changes in the area of activities of daily living and instrumental activities of everyday life and 4. overall perception of quality of life. Results: From the perspective of family members most deteriorating quality of life of seniors with dementia is the memory loss, which excludes them from normal life, as well as loss of ability to orientation in space and time, and loss of ability to perform normal activities of daily life. The sensory disturbance also reduces the quality of life, which leads patients into a greater social isolation and this may promote the growth of the paranoia. Family members rated the quality of life of their loved ones at home as significantly better in comparison with the quality of life of patients with dementia placed in the home for the elderly. In a domestic environment have seniors more stimulations, are more activated, and are more drawn into the family and social life. After coming into institutional care occurs worsening of symptoms and the gradual removal of patients from activation programs. Cieľ: Cieľom práce bolo zistiť, aká je kvalita života žien s karcinómom maternice vo vzťahu k veku a dosiahnutému vzdelaniu. Metodika: Súbor tvorilo 300 pacientiek, u ktorých bolo diagnostikované nádorové ochorenie maternice a bola u nich vykonaná hysterektómia. Výskum bol realizovaný prostredníctvom štandardizovaného dotazníka Quality of Life in Adult Cancer Survivors (QLASC). Výsledky: Ženy vo veku nad 50 rokov mali lepšie prežívanie kvality života ako ženy do 50 rokov. Priaznivejšie výsledky u pacientiek vo veku do 50 rokov sa preukázali len v doméne benefit z choroby. Vo vzťahu k vzdelaniu sme podľa priemerného skóre zistili v ôsmich doménach priaznivejšie výsledky u žien s vyšším vzdelaním. Zaujímavé bolo aj zistenie, že v doménach negatívne emócie a distres z opakovania mali ženy so stredoškolským vzdelaním bez maturity lepšie prežívanie faktorov kvality života než ženy so stredoškolským vzdelaním s maturitou. Záver: Na prežívanie kvality života žien s nádorovým ochorením maternice po hysterektómii vplýva biologický vek a stupeň dosiahnutého vzdelania., Aim: The aim of the paper was to ascertain the quality of life of women with uterine cancer in relation to their age and educational level. Methods: The sample consisted of 300 patients who were diagnosed with uterine cancer and underwent hysterectomy. The research was using a standardized questionnaire - Quality of Life in Adult Cancer Survivors (QLASC). Results: Women aged over 50 years reported better quality of life than women aged under 50 years. More favorable results recorded in patients under 50 years of age were only shown in the domain of Benefit of disease. In relation to educational attainment, more favorable results were recorded in women with higher education based on the average scores in eight domains. Interesting was the finding that in the domains Negative emotions and Distress of recurrence, women with lower education were experiencing better quality of life than women with higher education.
