INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. Úvod: Biliární ileus představuje 1−4 % mechanických obstrukcí gastrointestinálního traktu, ale u pacientů nad 65 let již 25 % nestrangulačních mechanických ileů tenkého střeva. Objemný kámen žlučníku obvykle vytvoří tlakovou nekrózu, protlačí se vzniklou biliodigestivní píštělí a obturuje gastrointestinální trakt. Kazuistiky: Mezi daty 1. 1. 2000 a 31. 1. 2015 bylo na Chirurgické klinice FN v Plzni operováno 35 pacientů pro poruchy pasáže s nálezem biliárního ileu. Průměrný věk pacientů v době operace byl 79 let (medián 78 let, min. 58 let, max. 92 let). Aerobilie na rentgenovém nativním snímku břicha byla přítomna z 27 pořízených snímků v 7 případech (26 %). Předoperačně byla provedena dvakrát kolonoskopie a devětkrát ezofagogastroduodenoskopie. Z operačních výkonů byla 21x provedena extrakce kamene z enterotomie, třikrát extrakce z gastrotomie, sedmkrát resekce tenkých kliček, jedenkrát Hartmannova resekce, jedenkrát resekce ileoascendentoanastomózy a jedenkrát extrakce konkrementu s následnou duodenojejunální anastomózou a s cholecystektomií v jedné době. Jedenkrát byla provedena ileotransverzoanastomóza. Pooperační 30denní mortalita byla 14,3 %. Pooperační morbidita dle klasifikace Clavien-Dindo činila u I. stupně 22,8 %, u II. stupně 5,7 %, u III. stupně 11,5 % a u IV. stupně 11,5 %. K recidivě biliárního ileu došlo u 8,8 % pacientů. Primární ošetření píštěle a cholecystektomie byla provedena v jednom případě. Závěr: Rozhodnutí o ošetření cholecystoenterální píštěle a cholecystektomii v první době bude vždy záležet na celkovém stavu pacienta a místním nálezu v oblasti žlučníku. Při ponechání píštěle je nutné počítat s rizikem recidivy ileu. Při operaci je nutné též zrevidovat celé střevo k vyloučení přítomnosti dalších konkrementů a snížit tak riziko časné recidivy. However, the rate of non-strangulated small bowel obstructions reaches as much as 25% in patients over 65 years of age. Usually, a pressure necrosis is created by a large gallstone that passes through the developed biliodigestive fistula, subsequently obturating the gastrointestinal tract. Case reports: 35 patients underwent a surgical procedure at the Department of Surgery, Teaching Hospital Pilsen for biliary ileus from January 1, 2000 to January 31, 2015. Mean age was 79 years (median 78 years; min. 58 years; max. 92 years). Aerobilia was visible in abdominal X-ray scans in 7 cases (26%) of 27 acquired images. Preoperatively, colonoscopy was done 2 times and esophagogastroduodenoscopy 9 times. Regarding surgical procedures, enterolithotomy was done in 21 cases, extraction of the gallstone from gastrotomy was done 3 times, small bowel resection 7 times, Hartmann resection 1 time, resection of ileoascendentoanastomosis 1 time, and extraction of the gallstone from duodenotomy with duodenojejunoanastomosis and cholecystectomy was done in 1 case. Ileotransversoanastomosis was performed in 1 case. Postoperative 30-day mortality was 14.3%. Postoperative morbidity according Clavien-Dindo was 22.8% for grade 1, 5.7% for grade 2, 11.5% for grade 3, and 11.5% for grade 4. A recurrence was observed in 8.8%. Primary treatment of the fistula and cholecystectomy were done in 1 case. Conclusion: The indication of primary biliodigestive fistula treatment always depends on the general condition of the patient and on the local finding in the gallbladder area. Residual fistula increases the risk of reccurence, but primary treatment is associated with higher mortality. Terapie akutní fáze ischemické cévní mozkové příhody (CMP) je zaměřena jednak bezprostředně na omezení rozsahu poškození pomocí co možná nejčasněji zahájené reperfuzní strategie a dále nastavení léčby zajišťující farmakologickou sekundární prevenci recidivy onemocnění. Klíčový předpoklad správné a efektivní sekundární prevence představuje včasné stanovení příčiny ischemického iktu, protože riziko recidivy iktu je nejvyšší v prvních týdnech po prodělané CMP. Nedílnou součástí managementu pacienta s ischemickým iktem je tedy vyloučení potenciálních kardiálních a cévních zdrojů embolizace do mozkové cirkulace pomocí zobrazovacích metod. Tyto embolizační zdroje jsou kategorizovány na vysoce rizikové stavy a stavy spojené se středním nebo nejasným rizikem., Treatment of acute phase of ischemic stroke is aimed to limit the amount of damage using reperfusion strategy and subsequently administration of treatment providing pharmacological secondary prevention of recurrence. The early aetiology diagnosis of stroke is crucial for effective secondary prevention as the highest risk of recurrence is within few weeks after stroke. Exclusion of cardiac and vascular source of embolism by imaging methods is an important part in management of patients with ischemic stroke. Úvod: Naše výsledky byly zhodnoceny za účelem posouzení bezpečnosti a efektivity endoskopické endonazální techniky v léčbě kraniofaryngeomů. <p align="">Materiál a metody: Od roku 2008 bylo endoskopicky endonazálně operováno14 pacientů s typickým supraselárním extraventrikulárním kraniofaryngeomem. Soubor se skládá z 11 mužů a 3 žen (věk 17–60 roků, průměrný věk 38 let). Jedenáct pacientů mělo předoperační deficit zorného pole v rozsahu od malého deficitu ve vnějším kvadrantu až k slepotě jednoho oka a těžkému deficitu druhého oka. Byla použita endoskopická endonazální technika čtyř rukou s peroperační 3,0 T MR. <p align="">Výsledky: V sedmi případech bylo dosaženo radikální resekce. Ve čtyřech případech byla provedena resekce subtotální, ve dvou případech resekce parciální. V jednom případě se jednalo o vypuštění kraniofaryngeomové cysty. V resekci po iMR bylo pokračováno ve třech případech. K úpravě zorného pole došlo v pěti případech. Pooperační diabetes insipidus se nově objevil ve třech případech. Reoperace pro únik mozkomíšního moku byla nezbytná ve třech případech. <p align="">Závěr: Endoskopické techniky v léčbě kraniofaryngeomu jsou bezpečné. Nicméně jsou spojeny s vyšším výskytem pooperačního úniku mozkomíšního moku ve srovnání s transkraniálními přístupy., Introduction: We reviewed our results in order to evaluate safety of endoscopic endonasal technique in the treatment of craniopharyngiomas. <p align="">Material and methods: Since 2008, endoscopic endonasal approach was used in 14 patients with typical suprasellar extraventricular craniopharyngioma. This cohort consisted of 11 males and three females (age 17 to 60 years, average age 38 years). Eleven patients had preoperative visual field deficit ranging from small deficit in the outer quadrant to blindness on one eye and a severe deficit of the second eye. Endoscopic endonasal four hands technique was used with intraoperative 3.0 T MRI. <p align="">Results: In seven cases, the radical resection was achieved. Subtotal resection was performed in four cases. The resection was partial in two cases. In one case, a cyst was drained. Resection after iMRI was performed in three cases. Visual field deficit improved in five cases. Postoperative diabetes insipidus developed in three cases. Reoperation for CSF leakage was necessary in three cases. <p align="">Conclusion: Endoscopic technique in the treatment of craniopharyngioma is safe. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. Cíl: Určení vztahu vybraných ukazatelů ke střednímu tlaku v plicnici (mPAP). Posouzení přítomnosti plicní hypertenze (PH) na základě RTG a HRCT obrazů. Metodika: 53 nemocných s chronickým plicním onemocněním podstoupilo pravostrannou srdeční katetrizaci, RTG hrudníku a HRCT plic. U 30 nemocných byla PH potvrzena. Skiagramy i HRCT byly hodnoceny třemi radiology, kteří se kromě jednotlivých nálezů vyjadřovali k přítomnosti PH. Byl vyhodnocen vztah relevantních parametrů k mPAP a shoda radiologů s objektivní přítomností PH. Výsledky: Nejsilnější vazba k mPAP byla na RTG hrudníku nalezena u poměru TI/ LHB (r = 0,256650, p = 0,02048) a šíře truncus intermedius (r = 0,188897, p = 0,023362). U HRCT vyšetření jsme nalezli významnou vazbu mezi hodnotou mPAP a průměrem kmene plicnice (r = 0,396894, p = 0,000001) a indexem PA/AA (r = 0, 382652, p = 0,000002). Hodnota rozhodovací meze pro PH u průměru kmene plicnice byla 31,2 mm se specificitou 80,769 % a senzitivitou 68,182 %. Shoda radiologů s mPAP při hodnocení RTG byla 58,49 %, 54,72 % a 64,15 %, při hodnocení HRCT byla 70,83 %, 60,42 % a 62,50 %. Závěr: Existuje vztah mezi šíří centrálních úseků plicních tepen a mPAP. Při pozitivitě několika specifických nálezů lze vyjádřit podezření na PH, při jejich absenci lze PH vyloučit s velkou pravděpodobností., Aim: To evaluate relationship between selected parameters and mean pulmonary artery pressure (mPAP). To assess prediction of pulmonary hypertension (PH) on chest radiographs (CXR) and High-resolution CT (HRCT) of lungs. Methods: 53 individuals with chronic lung disease underwent right heart cathetrisation, CXR and HRCT. 30 patients had PH confirmed. CXRs and HRCTs were reviewed by three radiologist who besides evaluation of findings stated whether PH is present. Relation between relevant parameters and mPAP and agreement between radiologists and real presence of PH were assessed. Results: The strongest correlation on CXR was found between mPAP and ratio of right descending pulmonary artery width and the left main bronchus (r = 0.256650, p = 0.02048) and right descending branch of pulmonary artery width (r = 0.188897, p = 0.023362). The strongest correlation on CXR was found between mPAP and pulmonary artery diameter (r = 0.396894, p = 0.000001) and PA/AA ratio (r = 0.382652, p = 0.000002. The cut-off value for PH for PA width was 31,2 mm with specificity 80.769% and sensitivity 68,182%. Agreement between radiologists and value of mPAP was in CXRS 58.49%, 54.72% and 64.15%, in HRCT of the lungs 70.83%, 60.42% and 62.50%. Conclusion: Width of pulmonary arteries and mPAP are related. V práci zabývající se hormonální terapií pacientek s karcinomem prsu se nabízí přehled péče, která je těmto pacientkám poskytována gynekologem, od prevence a diagnostiky přes monitoraci hormonální terapie první linie. Jsou rozebrány příčiny stoupající incidence karcinomu prsu a historicky nastíněn rozvoj hormonální terapie pacientek. Hlavní část práce je věnována vlivu tamoxifenu na ženský organizmus, zejména na genitální trakt, a rozebrány jsou nežádoucí účinky v gynekologické oblasti, hlavně endometria. Uvedena je retrospektivní studie provedená na Gyn. por. klinice 3. LF UK zaměřená na detekci patologických nálezů endometria pod vlivem tamoxifenu. Je vypracován a uveden návrh algoritmu monitorace pacientek s Ca mammae s dělohou léčených tamoxifenem gynekologem a zmíněna jsou i úskalí této péče. Dalším problémem, který práce řeší, je kontroverzní možnost hormonální substituční terapie klimakterického syndromu u pacientek s karcinomem prsu., Author describes hormonal treatment of breast cancer pacients and review gynaecological care from prevention and diagnostics to monitoring of the first line hormonal treatment. Reasons of increasing breast cancer incidence are discussed and historical overview of hormonal treatment posibilities in pacients with breast cancer is summarized. The main part of the article characterizes influence of tamoxifen into woman organism, especially in her genital tract (endometrium changes) and side effects are analysed. The retrospective trial (Dept. Obst. and Gyn. 3-rd Medical Faculty) focusing pathological changes of endometrium under tamoxifen influence is mentioned. Design of monitoring algorithm of breast cancer patients with intact uterus under treatment of tamoxifen is elaborated. Complications and difficulties of this monitoring are specified.
