A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Úvod: Maligní mezenchymové nádory jsou v orofaciální oblasti daleko vzácnější než karcinomy. Představují pouze 1 % z celkového počtu malignit s více než 50 podtypy. Angiosakomy jsou vysoce maligní nádory vycházející z krevních nebo lymfatických cév. Jejich výskyt je nízký až raritní s nejistou prognózou. Z celkového počtu sarkomů tvoří angiosarkom 5,4 % a v oblasti hlavy a krku se vyskytuje nejčastěji. Rámcový staging a grading se odvíjí dle FNCLC a NCI. Cíl práce: Cílem sdělení je upozornit na vzácné maligní onemocnění v oblasti hlavy a krku, které může být diagnosticky opomenuto či nerozpoznáno s fatálními následky pro nemocného. Materiál, metodika, výsledky: Autoři popisují klinické projevy onemocnění, diagnostiku a následnou terapii angiosarkomu mandibuly u zdravotně vysoce rizikového pacienta. V diagnostice tohoto vzácného onemocnění byly vedle histopatologického obrazu včetně imunohistochemického vyšetření využity i onkologické markery. Terapie byla zaměřena na chirurgickou léčbu a radioterapii. Závěr: Publikované zkušenosti s léčbou malých souborů nemocných v odborné literatuře neumožňují stanovit optimální terapeutický postup. Průměrná doba přežití je rok a půl., Introduction: Malignant mesenchymal tumours of the orofacial region are extremely rare and comprise only 1% of all malignant orofacial tumours with more than 50 subtypes. Angiosarcomas are highly malignant neoplasms that originate from blood or lymphatic vessels. Angiosarcomas form 5,4 % of all sarcomas and in the head and neck region are the most frequent type of sarcoma. Aim: The aim of this case report is to describe a rare malignant neoplasm in the head and neck topography which can be diagnostic difficulty and may be missed or unrecognised with fatal consequences for the patient. Materials, methods, results: The authors describe the clinical manifestations of the disease, the diagnosis and therapy of mandibular angiosarcoma in a seriously ill patient. The diagnosis was made on the basis of the histopathological findings and immunohistochemistry. The following treatment was radical surgery with subsequent radiotherapy and 18 months later, the 84 year old female patient remains alive and apparently healthy. Conclusions: Angiosarcoma has currently been literary described only in small number of patients. Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. Nádory hlavy a krku patří celosvětově mezi karcinomy se vzrůstající incidencí. Hlavním etiologickým faktorem podmiňujícím jejich vznik je kouření, abúzus alkoholu a v poslední době potvrzený HPV. Dvě třetiny pacientů přicházejí v pokročilém stadiu a až třetina pacientů má četné komorbidity. V jejich terapii je nejdůležitější multidiciplinární přístup. Standardem léčby je dle NCCN konkomitantní chemoradioterapie s platinovým derivátem (kategorie 1), nebo radioterapie konkomitantně s biologickou léčbou cetuximabem, nebo chirurgický přístup a dále dle histologie adjuvantní radioterapie či chemoradioterapie (1). V poslední době je zvažována v indikaci léčby také pozitivita HPV. Jedná se o pacienty s lepší prognózou, ale zatím nebyla stanovena standardní terapie. Cetuximab je monoklonální chimerická IgG1 protilátka, která cíleně blokuje extracelulární část EGFR. V České republice je zaregistrován mimo jiné pro léčbu pacientů s lokálně pokročilými nádory hlavy a krku konkomitantně s radioterapií dle Bonnerovy studie fáze III publikované v r. 2006 v N Engl J Med (2), která prokázala pro kombinaci radioterapie a cetuximabu proti radioterapii samotné signifikantní navýšení lokoregionální kontroly 24 měsíců versus 14,9 měsíců, s mediánem OS 54,0 měsíců versus 29,3 měsíců, bez signifikantního navýšení morbidity a toxicity., Worldwide, head and neck tumours are among cancers with an increasing incidence. The main aetiological factors determining their development are smoking, alcohol abuse, and recently detected HPV. Two thirds of patients present in an advanced stage and up to a third of them have numerous comorbidities. A multidisciplinary approach is of utmost importance in the treatment. The standard treatment according to NCCN is concomitant chemoradiotherapy with a platinum derivative (category 1) or radiotherapy concomitantly with biological therapy with cetuximab, or surgical approach, followed by, according to histology, adjuvant radiotherapy or chemoradiotherapy (1). Recently, HPV positivity has also been taken into consideration concerning indications for treatment. This applies to patients with a better prognosis, but no standard treatment has been determined so far. Cetuximab is a chimeric IgG1 monoclonal antibody that targets the extracellular domain of the EGFR. In the Czech Republic, it has been registered, among other things, for the treatment of patients with locally advanced head and neck tumours concomitantly with radiotherapy according to the phase III study by Bonner published in the New England Journal of Medicine in 2006 (2) in which the combination of radiotherapy and cetuximab versus radiotherapy alone was shown to significantly increase locoregional control to 24 months versus 14.9 months, with a median OS of 54.0 months versus 29.3 months, and with no significant increase in morbidity and toxicity., Markéta Pospíšková, Milan Kohoutek, and Literatura
Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. The present article describes two unrelated cases of progressive myoclonic epilepsy (PME) of the Lafora's disease and Unverricht-Lundborg types who were treated with topiramate (TPM) as add-on therapy for their myoclonus. After the initiation of topiramate therapy both cases responded with marked decrease in myoclonic seizure frequency and improvement of quality of life. Cíl: Prezentovat scintigrafický nález u 15leté dívky s Ewingovým sarkomem. Metoda: Patnáctiletá pacientka s nevýznamnou osobní i rodinnou anamnézou byla přijata na dětské oddělení spádové nemocnice pro zhoršující se únavu, nechutenství, váhový úbytek (10 kg), bolesti kloubů a kostí, vysoké laboratorní zánětlivé parametry (CRP 160 mg/l, FW 120/128) a anémii. Byla provedena třífázová scintigrafie skeletu k vyloučení zánětlivého postižení sakroiliakálního skloubení (SIS) vpravo. Pacientce byla dle hmotnosti (59,5 kg) podána aktivita 750 MBq 99mTc-metylendifosfonátu (MDP). Za 10 minut po intravenózní aplikaci radiofarmaka byla provedena 2. fáze třífázové kostní scintigrafie zaměřená na oblast sakroiliakálního skloubení. S odstupem tří hodin byla vyšetřena 3. fáze (kostní). Vyšetření bylo provedeno na dvouhlavé gamakameře AXIS firmy PICKER se semikvantitativním vyhodnocením na počítači. Gamakamera byla osazena kolimátory pro nízké energie s vysokým rozlišením a paralelními otvory. Výsledky: Třífázová scintigrafie skeletu metodou celotělové scintigrafie zjistila ve 3. fázi výrazně skvrnitě zvýšené vychytávání v axiálním skeletu, v kalvě okcipitálně ve střední čáře, v obou kyčelních kloubech a v levé fibule. Diferenciálně diagnosticky nález naznačoval možnost hematologické malignity, proto byla pacientka přeložena na onkohematologické oddělení dětské kliniky FN Olomouc. Zde bylo doplněno vyšetření skeletu pomocí magnetické rezonance (MR), plnohodnotné vyšetření kostní dřeně a trepanobiopsie. Nález podporoval diagnózu Ewingův sarkom, respektive Ewingův sarkom - primitivní neuroektodermový nádor (ES/PNET) s nejasným primárním ložiskem. Pacientka byla předána k další péči na pracoviště Kliniky dětské onkologie do Brna. Zde byl nález uzavřen jako Ewingův sarkom/PNET s primárním ložiskem na levé fibule a s nálezem mnohočetných metastáz ve skeletu a kostní dřeni, T2N0M1, klinické stádium Enneking III. Závěr: Kazuistika poukazuje na význam vysoce senzitivních nukleárně medicínských vyšetření při složité diagnostice onemocnění nejen dětského věku a na jejich přínos z hlediska urychlení dalšího vyšetřovacího postupu a léčby., x, Jana Páterová, Jaromír Bernátek, Blanka Kadllčíková, and Literatura
V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma.
