Motivation. Our previous study showed differences in the atherosclerosis phenotype between Lithuanian and Swedish men that could be influenced by complementary factors, namely oxidation processes and/or oxidative stress. The goal of this study was to evaluate the mainstream biological pathways inducing and maintaining the atherosclerotic process by analyzing genetic biomarkers particularly in inflammatory and metabolic pathways where the main focus is laid on the oxidation process. Methods. There were 32 families recruited for the study and clinical as well as biochemical analyses were performed. For genetic analysis 150 SNPs in 89 genes were selected in order to construct a microarray based on Arrayed Primer Extension (APEX) genotyping technology. Genotyping was carried out in 28 families and transmission disequilibrium test (TDT), siblingTDT (STDT), and combined analysis were performed. Results. Clinical and biochemical analysis revealed that probands with premature CAD were more likely to have diabetes mellitus, arterial hypertension, dyslipidemia and were male with high body mass index. Genetic analysis showed six SNPs statistically significantly associated with the atherosclerosis phenotype in the candidate genes ITGA2, IL1B, ALOX5A, OR13G1, MMP9 and NFKB1. These genes belong to different biological pathways: trombocyte adhesion and vessel damage, inflammation response, cholesterol and lypoxygenase metabolic pathway and nutrition. Conclusions. Od objavenia mimobunkovej fetálnej DNA v krvi matky uplynulo už 16 rokov. Avšak až v priebehu posledných 5 rokoch prebieha čoraz viac výskumov za účelom stanovenia jej presnej charakteristiky, koncentrácie a molekulárnej veľkosti, ktoré sa neskôr premietli do jednotlivých testov. Tie sú ale dostupné len v posledných 2 rokoch. Pri aktuálnom trende preferovania neinvazívnych metód prenatálnej diagnostiky a ich posúvania do obdobia prvého trimestra sa stanovenie mimobunkovej fetálnej DNA v krvi matky ukazuje ako veľmi perspektívnou metódou diagnostiky chromozómových aberácií u plodov. Jej širšie uplatnenie je limitované malým počtom štúdií na nerizikových skupinách pacientok a v nemalej miere aj ekonomickou náročnosťou vyšetrenia. Aj napriek tomu sa však táto metóda čoraz viac rozširuje zo Spojených Štátov Amerických do celého sveta, čo znižuje náklady na ňu, a stáva sa dostupnejšou pre čoraz väčší počet pacientok. Ukazuje sa, že táto metóda by v rámci podrobnejšieho vyšetrovania u rizikových pacientok mohla perspektívne minimalizovať riziko fetálnych strát, ktoré sú závažnou komplikáciou invazívnych metód prenatálnej diagnostiky., It has been 16 years since there has been a discovery of cell-free fetal DNA (cffDNA) in mother‘s blood. However in the last 5 years there has been more researching done in the purpose of its specific diagnosis, concentration and molecular size, which have later on reflected into particular tests. These have been available only in the past 2 years. Within the actual trend in preference of non-invasive methods of prenatal diagnosis and its shifting into first trimester the examination of mother’s blood has been showing as a very perspective method for early cell-free fetal DNA discovering of chromosome aberration diagnosis of fetus. Its wider enforcement is unfortunatelly limited by a small amount of studies within non-risk pregnancy groups of patients as well as its higher financial cost. Despite of this information this method is extending further more from United States of America into whole world which lowers its costs and its becoming more accesible for a wider amount of patients. Background: Despite the fact that the genetic basis of schizophrenia has been intensively studied for more than two decades, our contemporary knowledge in this field is rather fractional, and a substantial part of it is still missing. The aim of this review article is to sum up the data coming from genome‑wide association genetic studies in schizophrenia, and indicate prospective directions of further scientific endeavour. Methods: We searched the National Human Genome Research Institute’s Catalog of genome‑wide association studies for schizophrenia to identify all papers related to this topic. In consequence, we looked up the possible relevancy of these findings for etiology and pathogenesis of schizophrenia using the computer gene and PubMed databases. Results: Eighteen genome‑wide association studies in schizophrenia have been published till now, referring to fifty‑seven genes supposedly involved into schizophrenia’s etiopathogenesis. Most of these genes are related to neurodevelopment, neuroendocrinology, and immunology. Fyziologická gravidita je sprevádzaná zmenami v lokálnej a systémovej koagulácii a fibrinolýze. Hemostáza zasahuje do procesu implantácie blastocysty a placentácie. Udržiava optimálne prekrvenie placenty, zabezpečuje rýchle a adekvátne zrážanie počas pôrodu. Mnohé koagulačné proteíny sú exprimované na začiatku embryonálneho vývoja a hrajú úlohu mimo koaguláciu v proliferácii a diferenciácii buniek. Mechanizmus regulácie v placente nie je dostatočne chápaný. Poruchy hemostázy sa podieľajú na peripartálnom krvácaní, gestačnom tromboembolizme a považujú sa za rizikový faktor neúspešnej reprodukcie a tehotenských komplikácií., Physiological pregnancy is accompanied by changes in local and systemic coagulation and fibrinolysis. Haemostasis interferes with the implantation of the blastocyst and placentation. Maintain optimal blood flow placenta, ensures prompt and adequate clotting during the labour. Many coagulation proteins are expressed early in embryonic development and play roles outside of coagulation in cells proliferation and differentiation. Mechanism regulating haemostasis within the placenta remain poorly understood. The results of morphological ivestigation of prostate tissue after vaporization prostatectomy by Laser Dornier Medilas D UroBeam and transurethral prostate resection in patients with benigh prostatic hyperplasia is shown. We found that the successive use laser Dornier Medilas D Urobeam at power 175-250 W and transurethral resectionof prostate provides diminishing of area of coagulative necrosis which prevents formation of massive scab. Cíl: cílem našeho šetření bylo vyhodnotit moţnou přítomnost bolesti u klientů s demencí, kteří byli přeloţeni na gerontopsychiatrické lůţko psychiatrické léčebny v Kroměříţi z jiného zdravotnického zařízení. Metodika: studium překladové ošetřovatelské dokumentace z odesílajících zdravotnických zařízení, škála pro hodnocení bolesti u pacientů s demencí (PAINAD). Soubor: klienti přeloţení do psychiatrické léčebny v Kroměříţi, obojího pohlaví, starší 65 let, pro demenci jakéhokoliv typu. Tíţe demence byla středně těţká a těţká (bylo hodnoceno při přijetí na gerontopsychiatrické lůţko celkovým skóre dosaţeným v Mini Mental State Examination - MMSE - v rozmezí 0 - 17). Výsledky: pomocí škály PAINAD byla zachycena bolest u 26,7 % přeloţených klientů se středně těţkou nebo těţkou demencí. Bolest však byla řádně zadokumentována v překladových ošetřovatelských zprávách jen u 8,96 % klientů. Téměř třetina ošetřovatelské překladové dokumentace (29,9 %) měla zcela nevyplněný, nebo nedostatečně vyplněný formulář pro hodnocení bolesti. Téměř pětina (19,4 %) ošetřovatelských překladových zpráv fyzicky vůbec neobsahovala ţádný takový formulář. Závěr: přítomnost nepodchycené nebo nedostatečně vyhodnocené bolesti u klientů s kognitivními poruchami je závaţným problémem moderního ošetřovatelství. Nutná je potřeba neustálé edukace zdravotnického personálu, kvalitního vedení ošetřovatelské dokumentace, ale také potřeba důslednějšího systému kontrol z řad vedoucích pracovníků., Aim: to evaluate presence of pain in clients of both sexes, older than 65, who have been admitted to Mental hospital Kroměříţ due to dementia of any type. Methods: study of nursing documentation from dispatching health care facilities, PAINAD scale (Pain assessment in advanced dementia). Severity of dementia was moderate of severe (this was evaluated by Mini-Mental State Examination total score during admission process to our hospital, scores ranged from 0 to 17). Results: pain was detected in 26.7% of clients with moderate or severe dementia by means of PAINAD scale, compared to 8.96% of clients with pain which has been reported in nursing documentation. Almost in one third of nursing documentation (29.9%) no data or incomplete data about pain were found. Almost one fifth (19.4%) of nursing documentation have not contained any tool for pain screening. Concusion: undiagnosed pain in cognitively impaired clients is a severe problem of nursing care. Cíl: Cílem studie bylo zjistit, jak rodinní pečovatelé pečující o blízkou osobu v České republice hodnotí definující charakteristiky ošetřovatelské diagnózy Přetížení pečovatele. Metodika: Na klinickou validizaci ošetřovatelské diagnózy Přetížení pečovatele byl použit „Na pacienta zaměřený Fehringův model klinické diagnostické validity“. Výzkum u rodinných pečovatelů probíhal v měsících srpen a září 2010 v Agentuře domácí péče Victoria na území města Šumperk. Do výzkumného souboru bylo vybráno 50 rodinných pečovatelů, kteří pečovali o své blízké v domácím prostředí déle než 2 roky. Souhlas se zařazením do výzkumu vyjádřilo 23 rodinných pečovatelů, co je 46 %. Výsledky: Rodinní příslušníci za hlavní definující charakteristiky Přetížení pečovatele označili celkem 8 charakteristik: obavy z možné institucionalizace péče příjemce; obavy, jak vyřešit péči o člena rodiny, když nebudu schopen tuto péči vykonávat; obavy o zdraví příjemce péče v budoucnosti; obavy související se schopností pečovatele poskytovat péči v budoucnosti; konflikt v rodině; nejistota ze změny vztahu s příjemcem péče; smutek ze změny vztahu s příjemcem péče; starost o členy rodiny. Za vedlejší bylo označeno 10 definujících charakteristik: obtíže v dokončování požadovaných úkolů, obtíže se sledováním příjemce péče v průběhu nemoci, obtíže ve vykonávání požadovaných úkolů, vysoký krevní tlak, kardiovaskulární onemocnění, somatizace, frustrace, depresivní nálada, porušený spánek, stres. Závěr: Pro skupinu vybraných pečovatelů nebyly všechny definující charakteristiky uvedeny v klasifikačním systému NANDA International významné., Aim: The aim of the study was to find out how the family caregivers caring for their close relatives in the Czech Republic evaluate defining characteristics of the nursing diagnosis Caregiver Role Strain. Methods: Fehring´s patient – focused clinical diagnostic validity model was used to clinical validation of the nursing diagnosis Caregiver Role Strain. The research was ongoing during the months August and September 2010 at Home Care Agency Victoria in region of city Šumperk. Fifty family caregivers, who take care for their family members more than 2 years, were chosen to the research sample. Twenty three caregivers (46 %) agreed with their including to the research. Results: Caregivers classified in total the following eight defining characteristics as major ones: apprehension about possible institutionalization of care receiver, apprehension about care receiver´s care if caregiver unable to provide care, apprehension about the future regarding care receiver´s health, apprehension about the future regarding caregiver´s ability to provide care, family conflict, uncertainty regarding changes relationship with care receiver, grief regarding changed relationship with care receiver, concerns about family members. Caregivers classified ten defining characteristics as minor ones: difficulty completing required tasks, difficulty watching care receiver go through the illness, difficulty performing required tasks, hypertension, cardiovascular disease, somatization, frustration, feeling depressed, disturbed sleep, stress. Autorky příspěvku seznamují s dílčími výsledky vlastní sociometricky zaměřené metody, kterou aplikovaly ve třídách druhého stupně jedné ZŠ v České republice. V příspěvku je věnována pozornost hlavně těm výstupům, které se vztahují ke skupince dětí s nadváhou a obezitou. Pro adolescentní děti je prostředí školy a školní třídy důležitou součástí sociální reality, která značně ovlivňuje utváření jejich sebedůvěry, sebevědomí, identity a z těchto hledisek ovlivňuje po řadu kritických let kvalitu jejich života. Některé klinicky zaměřené zahraniční publikace tuto realitu reflektují a okolnostem života dětí s nadváhou a obezitou ve školním prostředí věnují pozornost (Kiess et al., 2009; Pallan et al., 2012; Taylor et al., 2005; Skidmore, Yarnell, 2004). Vzhledem k malému počtu dětí s nadváhou a obezitou byly výsledky v diskusi a v závěrečné části příspěvku shrnuty jen popisným způsobem., The authors of this article acquaint with the partial results of their own sociometric focused method, that they applied on the second stage of one elementary school in the Czech Republic. Attention is paid mainly to those outcomes, that relate to a group of children with overweight and obesity. For adolescent children are school environment and school classes important part of social reality, that greatly affects the formation of self-confidence, self-esteem, identity and affects many aspects of their quality of life during these crucial years. Even some clinically oriented foreign publications reflect this reality and pay attention to these circumstances of these children´s life (Kiess et al., 2009; Pallan et al., 2012; Taylor et al., 2005; Reilly et al., 2012; Skidmore, Yarnell, 2004). Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. Cíl: Cílem studie bylo zjistit spokojenost pacientů s poskytovanou ošetřovatelskou péčí na vybraných odděleních dvou nemocnic Moravskoslezského kraje. Porovnat spokojenost pacientů podle zdravotnického zařízení a oddělení, kde byli hospitalizováni. Zjistit, zda existuje souvislost mezi demografi ckými a klinickými charakteristikami pacientů a jejich spokojeností s ošetřovatelskou péčí. Metodika: Zkoumaný soubor deskriptivní korelační studie tvořilo 507 pacientů hospitalizovaných v období od února do března 2011 na 13 lůžkových odděleních v nemocnicích Karviná-Ráj a Orlová. Ke sběru dat byl použit standardizovaný dotazník Patient Satisfaction Scale (PSS), který měří spokojenost pacientů v jedenácti položkách třech oblastí – technické/racionální aspekty, podávání informací a oblast interakce a podpory. Výsledky: Pacienti byli nejvíce spokojeni s technickými aspekty ošetřovatelské péče a nejméně s poskytováním informací. Demografi cké charakteristiky pacientů na jejich spokojenost zásadní vliv neměly. Pouze v jedné nemocnici (NsP Karviná) byli muži ve všech faktorech spokojenější než ženy a naopak nejmenší spokojenost udávali pacienti, kteří popisovali svůj zdravotní stav jako uspokojivý. V NsP Orlová udávali vyšší spokojenost s podporou a interakcí pacienti hospitalizovaní delší dobu. Závěr: Výsledky studie neprokázaly statisticky významné rozdíly v hodnocení spokojenosti pacientů mezi nemocnicemi ani mezi sledovanými odděleními., Aims: The aim of this study was to determine patient satisfaction with nursing care provided in selected wards of two hospitals of Moravian-Silesian region, to determine whether there is a connection between demographic and clinical characteristics of patients and their satisfaction and to compare patient satisfaction according to health facility and department, where they were hospitalized. Methods: The sample of descriptive correlation study consisted of 507 patients hospitalized in the period from February to March 2011 on 13 inpatient wards in hospitals Karvina-Raj and Orlova. Data were collected using a standardized questionnaire Patient Satisfaction Scale (PSS), which measures patient satisfaction in eleven items of three areas – technical/rational aspects, provision of information and area of interaction and support. Results: Patients were most satisfi ed with the technical aspects of nursing care and least with provision of information. Demographic characteristics of the patients did not have a major impact on their satisfaction. Only in one hospital (NsP Karvina) were men in all factors happier than women and on the contrary the smallest satisfaction reported patients who described their health status as satisfactory. In NsP Orlova they reported higher satisfaction with the support and interaction patients hospitalized longer period of time.
