Background: To evaluate the clinical usefulness of serum levels of soluble form of endoglin in stage III colorectal adenocarcinomas (CRC) patients for detection of recurrence. Methods: The case-control study consisted of 80 stage III CRC patients who underwent surgery with curative intent and 70 age-and sex-matched healthy volunteers. Serum levels of soluble form of endoglin (sol-end) were measured in both groups. Also, predictive factors of recurrence were evaluated using multivariate analyses. Results: Serum levels of sol-end in stage III CRC patients were significantly higher than those in controls. There was not a significant association between serum levels of sol-end and clinicopathological features in CRC patients. Multivariate regression analysis showed the LN R (hazard ratio, 2.54; 95% CI , 1.46–4.34; p < 0.001), to be significant independent factors to estimate local recurrence in stage III CRC patients. Primární nádory mozku jsou heterogenním onemocněním. Nejzávažnější diagnózu představuje glioblastoma multiforme, který tvoří 75 % z high grade gliomů. Jedná se o agresivní nádory s omezenými terapeutickými možnostmi a špatnou prognózou. Medián přežití od stanovení diagnózy je bez léčby 4,6 měsíce, s léčbou kolem 12 měsíců. Zlatým standardem terapie je v dnešní době radikální operace, chemoradioterapie a následně adjuvantní chemoterapie temozolomidem. V naší práci jsme se zaměřili na přínos přidání temozolomidu ke standardní radioterapii v porovnání s jeho toxicitou., Primary brain tumors are a heterogeneous disease. The most important is glioblastoma multiforme, which constitutes 75 % of the high grade gliomas. These are aggressive tumors with limited therapeutic options and poor prognosis. Median survival from diagnosis is 4,6 months without treatment in the management of around 12 months. The gold standard therapy today is radical surgery, chemoradiotherapy followed by adjuvant chemotherapy with temozolomide. Natalizumab (monoklonální protilátka) je v současné době indikován jako lék druhé volby u relabující-remitující formy roztroušené sklerózy (RR-RS). Závažným a potenciálně fatálním nežádoucím účinkem této léčby je progresivní multifokální leukoencefalopatie (PML), která patří k vzácným oportunním infekcím mozku. Je vyvolána JC virem (JCV). Dochází k progredujícím zánětům bílé hmoty v mnoho četných lokalizacích mozku. Vysoké riziko vzniku PML zahrnuje pozitivitu JCV v krvi, předchozí imunosupresivní léčbu a délku léčby natalizumabem více než 2 roky. Jedním z nejdůležitějších diagnostických metod je MRI, neboť může určit postižení mozku ve velmi raných fázích nemoci. Prezentujeme nálezy 47leté pacientky s RR-RS, která byla pro progresi choroby poslední 3 roky léčena natalizumabem s velmi dobrým klinickým efektem. V posledním roce léčby se u pacientky vyskytl epileptický záchvat, v pozdějších fázích pak rychle probíhající deteriorace kognitivních funkcí a těžká kvadruparéza. V likvoru byl pozitivní nález kopií JCV, na MR byla diagnostikována splývavá ložiska frontálně, charakteristická pro PML. I přes okamžité vysazení léčby natalizumabem a následné zaléčení opakovanými plazmaferézami onemocnění PML progredovalo a pacientka zemřela za 2 měsíce od definitivního stanovení diagnózy. PML je závažná komplikace léčby roztroušené sklerózy natalizumabem. Důležité je včasné rozpoznání příznaků této nemoci. Proto je nezbytné sledovat tyto pacienty podrobně klinicky a včas vyšetřit MR (zejména flair), aby se po stanovení diagnózy a zaléčení nemoci předešlo fatálním komplikacím., Natalizumab (monoclonal antibody) is currently indicated as a second line treatment for relapse-remitting multiple sclerosis (RR-MS). Serious and potential fatal side effect of this treatment is progressive multifocal leukoencefalopathy (PML), which belongs to rare brain infections caused by JC virus (JCV). There is a progressive inflammation of the brain white matter in multiple localizations. A high risk of developing PML includes serum JCV positivity, previous immunosuppressive treatment and duration of natalizumab treatment of more than two years. Magnetic resonance is one of the most important diagnostic methods because it can identify brain damage in the early stage of the disease. We present 47-years-old female with RR-MS which has been successfully treated by natalizumab for three years with very good clinical outcome. In last year of treatment, patient experienced a seizure and in later stages progressive deterioration of cognitive function and severe quadriplegia. Positive JCV was found in cerebrospinal fluid. MR presented diffuse hyperintensive lesions located in frontal lobes. Despite immediate discontinuation of natalizumab treatment and subsequent daily plasma-exchange PML disease progressed and patient died within two months of the final diagnosis. PML is a serious complication of natalizumab treatment in multiple sclerosis. It is important to recognize early signs and symptoms of this disease. V naší kazuistice prezentujeme případ 11leté pacientky s akutní bolestí bederní páteře, pánve a proximální části levého stehna. Třífázová scintigrafie skeletu s nálezem zvýšené akumulace 99mTc-hydroxydifosfonátu v levém sakroiliakálním skloubení napomohla k diagnóze levostranné septické sakroileitidy. Hemokultivace byla pozitivní, etiologicky byl zjištěn Staphylococcus aureus. Parenterální antibiotická terapie vedla k poklesu zánětlivých parametrů, snížení bolestivosti. Pro zformování abscesu mezi levou lopatou kosti kyčelní a musculus iliacus byla provedena v celkové anestézii jeho evakuace. Multiple sclerosis (MS) is the most common cause of nontraumatic neurological disability in Europe and North America. Growth factor expression could participate in the repair process of the demyelinating disease. Among growth factors, brain derived neurotrophic factors (BDNF) has been demonstrated to play an important role in neuronal and axonal survival. In the central nervous system (CNS ), neurons are the main source of BDNF. Another potential source are activated astrocytes, which are present in inflamed areas in the CNS as shown in MS. In this study, total protein concentration (TPC) and BDNF levels in the cerebrospinal fluid (CS F) samples from the patients with MS (n = 48) and control subjects (n = 53) were measured using a Bio-Rad protein assay and enzyme linked immunosorbent assay (ELISA). No significant change in the CS F TPC of patients with MS was seen as compared to normal CS F. The presence of BDNF in the CS F samples was shown by Western blot. Using ELISA , it was shown that the level of BDNF in the MS CS F is higher than in normal CS F. It is concluded that BDNF is a constant component of human CS F. We present a 73 year old female with intractable pruritus and nonspecific cutaneous rash for a period of 9 months. She had recieved symptomatic therapy with no improvement. A complete examination revealed axillary and abdominal lymphadenopathy. A biopsy confirmed the diagnosis of Hodgkins lymphoma with Langerhans cell histiocytosis. She received 5 cycles of chemotherapy with resolution of pruritus and reduction in axillary and abdominal lymphadenopathy. The patient presented 6 months later with relapse and succumbed to the illness. Simultaneous occurrence of Langerhans cell histiocytosis and Hodgkins lymphoma may lead to misdiagnosis. Účinná léčba nádorových onemocnění často způsobuje trvalou ztrátu reprodukčních schopností pacienta. Jsou představeny moderní ukazatele ovariální rezervy, které mohou v běžné klinické praxi významně pomoci při zhodnocení míry rizika poškození gonadálních buněk v důsledku onkologické léčby. Práce dále popisuje zavedené metody reprodukční ochrany – kryokonzervace embryí a oocytů, kryokonzervace ovariální tkáně včetně možnosti její autotrasplantace a metody ochrany ovariální funkce aplikací analog gonadoliberinu. V diskuzi je pozornost věnována zejména výhodám a limitům popsaných metod ovariální protekce. V závěru jsou formulována stručná doporučení pro konzultace s pacientkami v adolescentním a reprodukčním věku před onkologickou léčbou, která významně ohrožuje jeho fertilitu., Effective treatment of cancer often causes patients irreversible damage of reproductive abilities. Modern markers ovarian reserves are introduced. These clinical examinations can effectively help to evaluate the risk of irreversible damage of gonadal cells done by oncology disease or its curative treatment. The paper describes current oncofertility techiques implemented to clinical practice – embryo and oocyte banking, ovarian tissue cryopreservation including further possibility of its orthotopic autotransplantation and administration of gonadoliberin analogues for ovarian protection. V současnosti neexistuje standardní léčba cílená na inhibici HER2 pro pacientky s metastatickým HER2-pozitivním karcinomem prsu po progresi na léčbě trastuzumabem a lapatinibem. V retrospektivních studiích se prokázalo, že trastuzumab si může v této situaci zachovávat terapeutickou aktivitu a být validní léčebnou možností pro pacientky v dobrém celkovém stavu., Currently there are no standard therapeutical options for patients with metastatic HER2-positive breast cancer after progression on trastuzumab and lapatinib. Retrospective studies have indicated promising activity of trastuzumab retreatment in this clinical situation. Cíl. Zhodnotit možnosti hybridního zobrazení a frekvenci používaných výkonů v detekci okultního muskuloskeletálního pyogenního zánětu jako příčiny sepse. Metoda. Retrospektivně byly zhodnocena hybridní zobrazení indikovaná během 3 let k detekci příčiny septického stavu nejasného původu, vyšetření byla vyhledána mezi všemi provedenými 18F-FDG-PET/CT a SPECT/CT. Výsledky. Bylo provedeno celkem 27 vyšetření 18F-FDG-PET/CT z indikace sepse nejasného původu, kdy v šesti případech byla nalezena spondylodiscitida, pyogenní artritida ve třech případech, svalový absces ve dvou případech a infekce endoprotézy ve čtyřech případech. Muskuloskeletální příčina sepse tvořila 55,5 % případů. Bylo provedeno celkem 16 vyšetření SPECT/CT, kdy v deseti případech byly použity techneciem značené fragmenty myších protilátek a v šesti případech leukocyty značené 99mTc-HMPAO. Ve dvou případech byla prokázána akutní spondylodiscitida, v jednom případě chronická spondylitida a v jednom případě mnohočetné abscesy svalů původem v infekci endoprotézy. Muskuloskeletální příčina tvořila 25 % nálezů. Závěr. Za dobu současného používání PET/CT a SPECT/CT v detekci okultních infekcí tvoří větší díl vyšetření pomocí 18F-FDG-PET/CT, neboť nabízí vícečetné výhody před zobrazeními leukocytárními., Aim. To asses a role and frequency of indications of hybrid imaging methods in the detection of occult pyogenous musculoskeletal inflammation as a cause of the septic state of unknown origin. Method. The retrospective evaluation of the hybrid imaging indicated due a sepsis of unknown origin during three years was performed. All procedures of 18F-FDG-PET/CT and the SPECT/CT using labeled leucocytes were enrolled into the assessment. Results. 27 procedures of 18F-FDG-PET/ CT were performed due to the searching of the cause in cases of the sepsis of unknown origin; the spondylodiscitis was found in 6 cases, pyogenous arthritis in 3 cases, muscular abscesses in 2 cases and 4 cases of pyogenous infection of implanted joint prosthesis. Muskuloskeletal inflammations formed 55,5% of all findings. 16 procedures of SPECT/CT were performed, 10 using 99mTc-labeled antigranulocyte antibodies, and 6 using 99mTc-HMPAO labeled leukocytes. In 2 cases, the acute spondylodiscitis was found, one case of chronic spon-dylitis and one case of soft tissue abscesses due to the infection of endoprosthesis were found. Muskuloskeletal inflammations formed 25% of all findings. Conclusion. Po dva roky trvajících bolestech bederní páteře s narůstajícími klaudikacemi dolních končetin byl u 61leté ženy diagnostikován rozsáhlý tumor v oblasti těla L3 a retroperitonea. Nádor byl totálně odstraněn a provedena náhrada těla L3 a stabilizace. Po operaci přetrvávaly výrazné bolesti, oslabení dolních končetin a instabilita stoje a chůze. Po léčebné rehabilitaci došlo k vymizení bolesti, ústupu nestability i parézy levé dolní končetiny. Vývoj pooperačního stavu dokumentuje stabilometrické vyšetření., After 2-year period of persistent pain of lumbar spine with increasing intensity of lower extremities claudication a vast tumor of L3 vertebra extending to the retroperitoneum was diagnosed in a 61-year old woman. The total L3 vertebrectomy as well as complete tumor removal has been performed and situation was reconstructed and stabilized. Pronounced pain, lower extremities paresis and instability persisted after the surgery. The rehabilitation therapy led to the relief of the pain improvement of standing and gait instability and paresis.
