Tato práce pojednává o některých klíčových aspektech diagnostických kritérií pro MPN podle klasifikace Světové zdravotnické organizace (WHO) z roku 2008, přičemž většina poznámek se omezuje na onemocnění polycythaemia vera (PV). Při analýze hlavních kritérií poznamenáváme, že existují pacienti s PV, jejichž erytrocytóza může být dokumentována pouze měřením objemu erytrocytární masy a nikoli měřením koncentrace hemoglobinu nebo hematokritu. Probíráme reprodukovatelnost histopatologie pro diagnostiku různých stadií MPN, existenci normálních hladin EPO u pacientů s PV, specifitu růstu endogenních erytroidních kolonií (EEC) pro diagnózu PV a naše současné znalosti týkající se cytogenetiky a molekulární biologie PV a jiných MPN a jejich patofyziologie. Rovněž analyzujeme různé prognostické ukazatele morbidity (trombóza) a mortality u PV – vysokou hladinu hematokritu, leukocytózu a procentuální zastoupení buněk s mutací JAK2V617F. V případě esenciální trombocytemie (ET) komentujeme rozlišení pravé ET a “časné PMF”. V závěru stručně shrnujeme obecně přijímaná doporučení European LeukemiaNet (ELN) pro léčbu Ph-negativní MPN na základě nové stratifikace rizika, na důkazech založené riziko kancerogeneze u hydroxyurey a racionální doporučení pro výběr konkrétních léků., In this paper, we discuss some critical parts of the World Health Organization (WHO) 2008 criteria for classification of MPNs classification, with most of the remarks limited to polycythemia vera (PV). When analyzing the major criteria, we note that there are PV patients whose erythrocytosis can only be documented by red cell mass measurements and not by hemoglobin concentration or hematocrit measurements. We discuss the reproducibility of histopathology for the diagnosis of different stages of MPNs, the existence of normal EPO levels in PV patients, the specificity of Endogenous erythroid colony (EEC) growth for PV diagnosis and our current knowledge of the cytogenetics and molecular biology of PV and other MPNs and their pathophysiology. We also analyze various prognostic morbidity (thrombosis) and mortality markers in PV – high hematocrit level, leukocytosis and percentage of cells with the JAK2V617F mutation. In essential thrombocythemia (ET), we comment on the distinction of true ET and “early PMF”. Objectives: The aim of this study was to elucidate the role of dopamine receptor D2 / ankyrin repeat and protein kinase domain containing 1 (DRD2/ANKK1) TaqIA allelic polymorphism in the HPVinduced cervical carcinogenesis. Methods: 1. Effect on the risk of cervical precancer: After an 8year followup, out of 214 women with persisting highrisk HPV infection, 102 developed highgrade cervical dysplasia or cervical intraepithelial neoplasia (CIN) grade III, while 112 did not. The subjects were genotyped for the DRD2/ANKK1 TaqIA polymorphism by PCRRFLP, and the allelic distributions were compared between groups with and without highgrade dysplasia. 2. Prognostic value: Two hundred and thirty nine women with cervical precancer/cancer were followed for 5 years. Complete remission was achieved at 182 women. To assess the prognostic value of the TaqIA polymorphism, genotype frequencies were compared between patients reaching and not reaching complete remission. Results: The frequency of A1/A1+A1/A2 genotypes was higher among women who developed highgrade cervical dysplasia (OR: 1.87, 95% CI: 1.053.33; p=0.034) than in the other group. Occurrence of the A1 allele was more frequent among women who did not reach complete remission (OR: 2.00, 95% CI: 1.073.74; p=0.030) than in women with complete remission. Conclusions: This is the first report on the possible involvement of DRD2/ANKK1 TaqIA polymorphism in cervical carcinogenesis. The A1 allele seems to increase the risk of cervical precancer, and it may also be associated with a worse prognosis in women with HPVinduced cervical cancer. Gastrointestinální stromální tumory (GIST) jako nejčastější mezenchymální tumory trávicího traktu vycházejí z Cajalových intersticiálních buněk. Ke konfirmaci morfologické diagnózy jsme použili panel imunohistochemických protilátek CD117, desmin, S100 a DOG1. Při hodnocení biologických vlastností jsme vycházeli z Miettinenovy klasifikace pro GIST, rozdělující tumory do tří skupin podle rizika agresivního chování. Klasifikace tumoru sleduje tři parametry: lokalizaci, velikost a mitotický index. Mitotický index je definován jako počet mitóz v nádorové tkáni na celkové ploše 5 mm2. Provedli jsme srovnání metod odběru tenkojehlové, navigované aspirační cytologie – EUS FNA a klešťové endoskopické biopsie či resekátu nádorové tkáně z pohledu patologa se zaměřením na stanovení všech potřebných morfologických a prognostických parametrů této neoplázie. Naše závěry a zkušenosti ukazují, že výsledky cytologického vyšetření jsou pro diagnostiku GIST srovnatelné s bioptickým vyšetřením. Hlavní limitací cytologické metody je nemožnost v 75 % případů stanovit mitotický index z důvodů malého množství nádorové tkáně. Tento nedostatek lze nahradit vyšetřením nukleárního proteinu asociovaného s buněčnou proliferací Ki-67, který se dle literárních zdrojů ukazuje jako významný prognostický parametr progrese onemocnění., Gastrointestinal stromal tumors (GIST) belong to the most common mesenchymal tumors of digestive tract deriving from Cajal interstitial cells. A panel of immunohistochemical antibodies CD117, desmin, S100 and DOG1 is used to confirm the morphological diagnosis. When evaluating the biological characteristics we used a Miettinen classification for GIST which divides tumors into three groups according to a risk of aggressive behaviour. This classification observes three parameters – tumor localisation, its size and mitotic index. Mitotic index is defined as a number of mitoses in the tumor tissue on the overall area of 5 mm2. We performed a comparison of sampling methods – biopsy forceps, resection versus endoscopic ultrasound-guided fine-needle aspiration cytology (EUS FNA) from the point of view of a pathologist, focusing on the assessment of all necessary morphological and prognostic parameters of neoplasia. The study conclusions and experience show that when diagnosing GISTs, results of cytological examination of a sample using EUS FNA are comparable to those using forceps endoscopic biopsy. The main limitation of EUS FNA method is that in 75 % of cases it is impossible to state the mitotic index due to insufficient amount of tumor tissue. Vrozená genetická predispozice je příčinou vzniku nádorového onemocnění u 5–10 % pacientek s karcinomem prsu. Molekulárně-genetické testování hlavních predispozičních genů je u žen s karcinomem prsu a vaječníků, které splňují indikační kritéria, součástí klinické praxe. Terapie hereditární formy onemocnění se v současnosti neliší od léčby sporadického karcinomu prsu. Prognóza pacientek s karcinomem prsu s mutací v BRCA1, BRCA2 genech se může odlišovat od prognózy pacientek se sporadickým karcinomem s ohledem na možnosti systémové terapie a zvýšené riziko vzniku dalšího onkologického onemocnění. Obsahem článku je shrnutí současných znalostí o funkcích BRCA1 a BRCA2 i dalších predispozičních genů; srovnání klinicko-patologických charakteristik hereditárního karcinomu s BRCA mutací a tzv. „BRCAness“ sporadické formy karcinomu prsu a vyhodnocení rozdílné efektivity onkologické terapie u této specifické podskupiny nemocných., Hereditary predisposition causes neoplastic disease in 5–10% of patients with breast cancer. Molecular- genetic testing of high- penetrant genes is standard of clinical care about patients with breast and ovarian cancer, who meet the criteria for testing. Therapy of hereditary mutation associated breast cancer is not different from sporadic breast cancer therapy nowadays. Prognosis of patients with BRCA1, BRCA2 mutation associated breast cancer may be different from sporadic breast cancer patients due to the existing systemic treatment options and higher risk of other malignant disease. Úvod: Biklonální gamapatie nejistého významu (BGUS) je méně obvyklé onemocnění charakterizované přítomností dvou monoklonálních gradientů a relativně příznivým biologickým chováním. Cíl: Cílem práce bylo posouzení možného přínosu či naopak limitací obvyklých parametrů používaných u monoklonální gamapatie nejistého významu (MGUS) ve skupině jedinců s BGUS. Metody: Analyzovaná sestava čítala 18 vzorků sér od jedinců s BGUS (9x IgG–IgA, 7x IgG–IgM a 2x IgG–IgG typ). Pro analýzu byl použit turbidimetr SPA Plus a stanovované parametry zahrnovaly sérové hladiny volných lehkých řetězců imunoglobulinu (FreeLite™), hladiny polyklonálních imunoglobulinů a hladiny párů těžkých/lehkých řetězců imunoglobulinu (HevyLite ™). Výsledky a závěr: Pilotní analýza a zjištěné výsledky potvrzují skutečnost, že u velké části BGUS dochází k ovlivnění některých parametrů užívaných ve stratifikaci a sledování jedinců s MGUS, zejména v případě odlišnosti lehkých řetězců v obou molekulách monoklonálních imunoglobulinů. Jako velmi perspektivní se jeví analýza párů těžkých/lehkých řetězců imunoglobulinu (HLC), zohledňující možnost izotypové suprese alternativního páru s ovlivněním HLC indexu. Avšak v podmínkách běžné klinické praxe je zřejmé, že zásadní pro určení prognózy BGUS bude nutná izolace a analýza klonálních populací buněk na molekulárně-cytogenetické úrovni., Introduction: Biclonal gammopathy of undetermined significance (BGUS) is a less frequent disease characterized by the presence of two monoclonal gradients and a relatively favourable biological development. Objective: The objective was to assess potential benefits and/or limitations of laboratory parameters typically used with monoclonal gammopathy of undetermined significance (MGUS) when applied to a group of BGUS patients. Methods: The analysed batch comprised 18 serum samples from BGUS patients (9x IgG–IgA, 7x IgG–IgM a 2x IgG–IgG type). The assays were performed with a SPA Plus turbidimeter. Serum levels of polyclonal immunoglobulins, free light immunoglobulin chains (FreeLite™), and pairs of heavy/light immunoglobulin chains (HevyLite™) were assayed. Results and conclusion: The results of the pilot analyses confirm that a significant portion of BGUS patients exhibits changes of certain parameters typically applied to the stratification and monitoring of MGUS patients, particularly when the light chains in both molecules of monoclonal immunoglobulins differ. The analysis of heavy/light immunoglobulin chain pairs (HLC) appears very promising, indicating the possibility of an isotype suppression of the alternative pair affecting the HLC index.
