Introduction: Obstruction of the appendiceal lumen is the primary cause of appendicitis. The most common causes of luminal obstruction are fecaliths and lymphoid follicle hyperplasia. Additionally, bacterial infections or enteric and systemic viral diseases can cause a reaction of the lymphoid follicle. Case presentation: An 11-year-old boy with active phase of chickenpox presented on our Pediatric surgery emergency department under the impression of acute appendicitis. An appendectomy was performed on the same day. An inflamed and edematous retrocecal appendix was removed during surgery. Histological investigation of the appendix revealed transmural acute inflammation, with diffuse proliferation of inflammatory cells, with characteristic intranuclear inclusion surrounded by a clear halo. The PCR analysis of peripheral blood and appendix tissue specimen revealed positive VZV DNA. Plicní aspergilóza patří mezi zánětlivá onemocnění způsobená houbami rodu Aspergillus. V závislosti na stavu imunitního systému a plicního parenchymu nemocného existují invazivní, semi-invazivní a neinvazivní formy onemocnění. Autoři popisují případ 78leté ženy s masivní hemoptýzou, zdrojem které byla hypertrofická bronchiální tepna anomálně odstupující z a. thoracica interna. CT angiografie provedená na multidetektorovém CT přístroji ektopickou bronchiální tepnu zobrazila. Následná selektivní embolizace dané tepny akrylátovým lepidlem vedla k zástavě hemoptýzy. Nemocná neměla recidivu krvácení během 24měsíčního sledování. Anomální odstup bronchiální tepny není vzácný a proto je nutné na něj myslet zejména v případech, kdy není patrná žádná tepna zásobující bronchy v normální anatomické lokalizaci. CT angiografie provedená na multidetektorovém CT přístroji má vysokou úspěšnost v detekci odstupu bronchiálních tepen a slouží k navádění při jejich katetrizaci a embolizaci., The authors describe a case of a 78-year-old woman with massive hemoptysis, which was from to hypertrophic bronchial artery anomalously originating from the internal thoracic artery. CT angiography performed on multidetector CT shows anomalous origin of the right bronchial artery. The bronchial bleeding was treated with embolization. The patient had no recurrent bleeding during 24 month follow-up. Ectopic origin of bronchial arteries is not rare, therefore it is necessary to consider it especially in cases when no bronchial artery arises between the upper margin of the fifth thoracic vertebra and the lower margin of the sixth thoracic vertebra. Chemodektom, paragangliom glomus caroticum neboli tumor karotického glomu se vyskytuje v populaci s incidencí 1 : 30 000. Tumor je makroskopicky, mikroskopicky i biologicky velmi podobný adrenálnímu feochromocytomu. Významným znakem těchto tumorů je jejich bohatá vaskularizace. Patří mezi nejvíce prokrvené tumory vůbec. Histologicky se popisuje alveolární uspořádání nádorových buněk, na rozdíl od feochromocytomu, ostře od sebe ohraničených jemnými pruhy vaziva se širokými kapilárami a buňkami podobnými buňkám Schwannovým. Cytoplazma nádorových buněk hlavních je slabě eozinofilní a jemně granulovaná. Jádra buněk mohou být pleomorfní a hyperchromní, ale nutno konstatovat, že jaderná atypie není jednoznačnou známkou malignity. Biologicky se jedná o tumory převážně benigní. Klinicky se jedná o pomalu rostoucí, nebolestivý, tuhý a kulovitý útvar za úhlem dolní čelisti. This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. Cíl: Prezentovat zajímavý případ 44leté ženy s metastázou karcinomu prsu v axilární lymfatické uzlině vpravo s negativní mamografií, biopsií a ultrasonografií prsů, kdy 18F-FDG PET/CT odhalil multifokální karcinom prsu vpravo. Materiál a metoda: Pacientka vyhledala lékaře pro hmatnou rezistenci v pravé axile. Následovala exstirpace paketu lymfatických uzlin z pravé axily, histologické vyšetření prokázalo metastázu karcinomu prsu. Mamografie a ultrasonografie obou prsů byla s negativním výsledkem. Vzhledem k pozitivitě metastáz v lymfatických uzlinách v pravé axile následovala ještě biopsie z pravého prsu, také s negativním nálezem. V rámci pátrání po primárním ložisku karcinomu prsu pacientka absolvovala celotělové PET/CT zobrazení v rozsahu od baze lební do proximální třetiny stehen, a to 60 minut po intravenózní aplikaci radiofarmaka 18F-FDG o aktivitě 377 MBq (5 MBq/kg). Akviziční čas PET skenu byl 2,5 minuty na 15 cm projekci, celkem 7 projekcí. Kontrastní (i.v. + per os kontrast) CT sken byl proveden ve venózní fázi. Výsledky: PET/CT odhalil multiložiskový viabilní tumor v laterální části pravého prsu a viabilní metastázy v lymfatických uzlinách v pravé axile a infraklavikulárně vpravo. Závěr: 18F-FDG PET/CT správně detekoval multifokální karcinom prsu u pacientky s pozitivní metastázou tohoto karcinomu v lymfatické uzlině a neznámým primárním ložiskem, čímž zkrátil dobu diagnostického procesu a umožnil zvolit správnou léčbu., Aim: To present a rare case report of the multifocal breast cancer finding on 18F-FDG PET/CT in a 44-year-old woman with the positive right axillary breast metastases and negative mammography, ultrasound and right breast biopsy. Material and Method: The patient underwent 18F-FDG PET/CT for the positive right axillary lymph node breast cancer metastases and negative mammography, ultrasound a right breast biopsy. The whole body PET/CT was obtained 60 minutes after intravenous injection of 18F-FDG (377 MBq – 5 MBq/kg). The PET scan acquisition time was 2.5 minutes per bed position for whole body imaging from base of the skull to mid femur and seven bed positions were necessary. Contrast-enhanced CT (venous phase) provided both full CT evaluation (including intravenous and oral contrast) and PET attenuation correction. Results: The examination revealed PET/CT positive multifocal tumour in the right breast and regional metastases in the right axilla and infraclavicular region without distant metastases. The patient underwent the subsequent neoadjuvant chemotherapy, surgery and adjuvant radiotherapy and the complete remission was induced. An eight-month-old male child was admitted with weakness and swelling in the feet. Paraparesis and bilateral lower extremity edema were present in the neurological examination. Thoracic MRI showed an intradural intramedullary mass 61 × 11 mm in size in the T5–T10 levels. Laminotomy between the T5–T10 vertebrae was performed. A mass with smooth borders was separated from most of the neural tissue. In the postoperative MRI, we observed a contrast enhancing area, considered a residual fragment, only 5 × 4 mm in size. Histopathological properties were compatible with the intermixed subtype of ganglioneuroblastoma. Only a limited number of thoracic cord Ganglioneuroblastoma reports have been previously published. Although very rare in children and young adults, ganglioneuroblastoma should be included in the differential diagnosis of thoracic cord tumors. It is difficult to obtain a preoperative diagnosis with clinical features and radiological investigations. Diagnosis depends on histopathological examinations. Curative treatment should be in the form of a complete resection of the tumor.
