Introduction: Obstruction of the appendiceal lumen is the primary cause of appendicitis. The most common causes of luminal obstruction are fecaliths and lymphoid follicle hyperplasia. Additionally, bacterial infections or enteric and systemic viral diseases can cause a reaction of the lymphoid follicle. Case presentation: An 11-year-old boy with active phase of chickenpox presented on our Pediatric surgery emergency department under the impression of acute appendicitis. An appendectomy was performed on the same day. An inflamed and edematous retrocecal appendix was removed during surgery. Histological investigation of the appendix revealed transmural acute inflammation, with diffuse proliferation of inflammatory cells, with characteristic intranuclear inclusion surrounded by a clear halo. The PCR analysis of peripheral blood and appendix tissue specimen revealed positive VZV DNA. Plicní aspergilóza patří mezi zánětlivá onemocnění způsobená houbami rodu Aspergillus. V závislosti na stavu imunitního systému a plicního parenchymu nemocného existují invazivní, semi-invazivní a neinvazivní formy onemocnění. V diagnostice tohoto onemocnění hrají hlavní úlohu prostý rentgenový snímek hrudníku a HRCT plic. The problem of prophylaxis of incompetence of bronchial stump and bronchial fistula is many sided. Notwithstanding the development of new technologies, perfection of surgical technique, its frequency still remains on high level that persistently requests the search of new ways for resolving this problem, which preserved its topicality till nowadays. We have found that: The most efficiency of BFS is achieved during treatment of bronchial fistulas with diameter up to 3 mm and with effectiveness at 83.2% patients after rightside PE and at 92.3% patients after leftside PE. In case of BPF with diameter more than 3 mm the number of satisfactory results of BFS reaches only 33.3%. but, it is necessary to mention that among patients with BPF sized over 3 mm after leftside PE the effectiveness of BFS significantly exceeds as general index (50.0% against 33.3%) so after rightside PE (50.0% against 25.0%)., F. Nádory hlavy a krku patří celosvětově mezi karcinomy se vzrůstající incidencí. Hlavním etiologickým faktorem podmiňujícím jejich vznik je kouření, abúzus alkoholu a v poslední době potvrzený HPV. Dvě třetiny pacientů přicházejí v pokročilém stadiu a až třetina pacientů má četné komorbidity. V jejich terapii je nejdůležitější multidiciplinární přístup. Standardem léčby je dle NCCN konkomitantní chemoradioterapie s platinovým derivátem (kategorie 1), nebo radioterapie konkomitantně s biologickou léčbou cetuximabem, nebo chirurgický přístup a dále dle histologie adjuvantní radioterapie či chemoradioterapie (1). V poslední době je zvažována v indikaci léčby také pozitivita HPV. Jedná se o pacienty s lepší prognózou, ale zatím nebyla stanovena standardní terapie. Cetuximab je monoklonální chimerická IgG1 protilátka, která cíleně blokuje extracelulární část EGFR. V České republice je zaregistrován mimo jiné pro léčbu pacientů s lokálně pokročilými nádory hlavy a krku konkomitantně s radioterapií dle Bonnerovy studie fáze III publikované v r. 2006 v N Engl J Med (2), která prokázala pro kombinaci radioterapie a cetuximabu proti radioterapii samotné signifikantní navýšení lokoregionální kontroly 24 měsíců versus 14,9 měsíců, s mediánem OS 54,0 měsíců versus 29,3 měsíců, bez signifikantního navýšení morbidity a toxicity., Worldwide, head and neck tumours are among cancers with an increasing incidence. The main aetiological factors determining their development are smoking, alcohol abuse, and recently detected HPV. Two thirds of patients present in an advanced stage and up to a third of them have numerous comorbidities. A multidisciplinary approach is of utmost importance in the treatment. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. Karcinóm žalúdka je smrteľná choroba a je stále druhá najčastejšia príčina úmrtí na rakovinu na celom svete. Chirurgický zákrok s kompletnou resekciou tumoru, ktorá má kuratívny potenciál, zostáva hlavnou terapiou pre skoré štádiá. Bohužiaľ, väčšina pacientov s rakovinou žalúdka je diagnostikovaná v pokročilom štádiu, čím paliatívna systémová liečba zostáva jedinou liečebnou možnosťou. Najčastejšia kombinácia chemoterapie ako liečby prvej línie na liečbu pokročilého karcinómu žalúdka (PKŽ) zahŕňa zlúčeniny platiny a fluoropyrimidínov. Fluoropyrimidíny sú chrbtovou kosťou chemoterapeutických režimov pre liečbu karcinómu žalúdka. Surgical resection remains the main treatment for early stages with complete resection having the potential for a cure. Unfortunately, most patients with gastric cancer are diagnosed in advanced stages, rendering palliative systemic therapy as the only choice of treatment. The most common chemotherapy combination as a first-line treatment in advanced gastric cancer (AGC) includes a platinum compound and fluoropyrimidine. Fluoropyrimidines have been the backbone in the chemotherapy regimens for the treatment of gastric cancer. Teysuno® (S-1) is a peroral fourth-generation fluoropyrimidine that combines tegafur, which is a prodrug of 5-fluorouracil (5-FU), and two biochemical modulators: 5-chloro-2,4-dihydroxypyridine, a powerful but reversible inhibitor of dihydropyrimidine dehydrogenase that prevents 5-FU degradation, and potassium oxonate, which reduces gastrointestinal (GI) toxicity by inhibition of 5-FU phosphorylation in the GI mucosa. Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. Vemurafenib je malá molekula, která má schopnost inhibovat mutovanou BRAF proteinkinázu. Na základě provedených klinických studií BRIM-2 a BRIM-3, kdy vemurafenib dokázal prodloužit celkové přežití (OS) a dobu do progrese onemocnění (PFS), byl zaregistrována v EU k léčbě v monoterapii u dospělých pacientů s neresekovatelným nebo metastazujícím melanomem s pozitivní mutací V600 genu BRAF., Metastatic melanoma is an aggressive disease. Riziko CNS progrese u vysoce agresivních lymfomů (lymfoblastový a Burkittův lymfom) dosahovalo v historických souborech až 30%, podařilo se jej však výrazně snížit pomocí intratekální profylaktické terapie a inkorporací systémových cytostatik prostupujících hematoencefalickou bariéru do léčebných protokolů. U ostatních agresivních lymfomů je riziko CNS progrese nižší (pod 10 % v recentních studiích), avšak u vybrané skupiny nemocných se může i zde pohybovat kolem 25–30 %. Bohužel, na rozdíl od lymfoblastového a Burkittova lymfomu se u jiných agresivních histologií nepodařilo profylaktické úspěchy zopakovat. Tento článek se zabývá incidencí, rizikovými faktory a možnostmi profylaxe CNS relapsu u imunokompetentních pacientů s difuzním velkobuněčným B-lymfomem, lymfomem z buněk pláště, periferními T-lymfomy a primárním mediastinálním B-lymfomem.
