- Motivation. Our previous study showed differences in the atherosclerosis phenotype between Lithuanian and Swedish men that could be influenced by complementary factors, namely oxidation processes and/or oxidative stress. The goal of this study was to evaluate the mainstream biological pathways inducing and maintaining the atherosclerotic process by analyzing genetic biomarkers particularly in inflammatory and metabolic pathways where the main focus is laid on the oxidation process. Methods. There were 32 families recruited for the study and clinical as well as biochemical analyses were performed. For genetic analysis 150 SNPs in 89 genes were selected in order to construct a microarray based on Arrayed Primer Extension (APEX) genotyping technology. Genotyping was carried out in 28 families and transmission disequilibrium test (TDT), siblingTDT (STDT), and combined analysis were performed. Results. Clinical and biochemical analysis revealed that probands with premature CAD were more likely to have diabetes mellitus, arterial hypertension, dyslipidemia and were male with high body mass index. Genetic analysis showed six SNPs statistically significantly associated with the atherosclerosis phenotype in the candidate genes ITGA2, IL1B, ALOX5A, OR13G1, MMP9 and NFKB1. These genes belong to different biological pathways: trombocyte adhesion and vessel damage, inflammation response, cholesterol and lypoxygenase metabolic pathway and nutrition. Conclusions. - Od objavenia mimobunkovej fetálnej DNA v krvi matky uplynulo už 16 rokov. Avšak až v priebehu posledných 5 rokoch prebieha čoraz viac výskumov za účelom stanovenia jej presnej charakteristiky, koncentrácie a molekulárnej veľkosti, ktoré sa neskôr premietli do jednotlivých testov. Tie sú ale dostupné len v posledných 2 rokoch. Pri aktuálnom trende preferovania neinvazívnych metód prenatálnej diagnostiky a ich posúvania do obdobia prvého trimestra sa stanovenie mimobunkovej fetálnej DNA v krvi matky ukazuje ako veľmi perspektívnou metódou diagnostiky chromozómových aberácií u plodov. Jej širšie uplatnenie je limitované malým počtom štúdií na nerizikových skupinách pacientok a v nemalej miere aj ekonomickou náročnosťou vyšetrenia. Aj napriek tomu sa však táto metóda čoraz viac rozširuje zo Spojených Štátov Amerických do celého sveta, čo znižuje náklady na ňu, a stáva sa dostupnejšou pre čoraz väčší počet pacientok. Ukazuje sa, že táto metóda by v rámci podrobnejšieho vyšetrovania u rizikových pacientok mohla perspektívne minimalizovať riziko fetálnych strát, ktoré sú závažnou komplikáciou invazívnych metód prenatálnej diagnostiky., It has been 16 years since there has been a discovery of cell-free fetal DNA (cffDNA) in mother‘s blood. However in the last 5 years there has been more researching done in the purpose of its specific diagnosis, concentration and molecular size, which have later on reflected into particular tests. These have been available only in the past 2 years. Within the actual trend in preference of non-invasive methods of prenatal diagnosis and its shifting into first trimester the examination of mother’s blood has been showing as a very perspective method for early cell-free fetal DNA discovering of chromosome aberration diagnosis of fetus. Its wider enforcement is unfortunatelly limited by a small amount of studies within non-risk pregnancy groups of patients as well as its higher financial cost. Despite of this information this method is extending further more from United States of America into whole world which lowers its costs and its becoming more accesible for a wider amount of patients. Molekulárně-genetické testování hlavních predispozičních genů je u žen s karcinomem prsu a vaječníků, které splňují indikační kritéria, součástí klinické praxe. Terapie hereditární formy onemocnění se v současnosti neliší od léčby sporadického karcinomu prsu. Prognóza pacientek s karcinomem prsu s mutací v BRCA1, BRCA2 genech se může odlišovat od prognózy pacientek se sporadickým karcinomem s ohledem na možnosti systémové terapie a zvýšené riziko vzniku dalšího onkologického onemocnění. Obsahem článku je shrnutí současných znalostí o funkcích BRCA1 a BRCA2 i dalších predispozičních genů; srovnání klinicko-patologických charakteristik hereditárního karcinomu s BRCA mutací a tzv. „BRCAness“ sporadické formy karcinomu prsu a vyhodnocení rozdílné efektivity onkologické terapie u této specifické podskupiny nemocných., Hereditary predisposition causes neoplastic disease in 5–10% of patients with breast cancer. Molecular- genetic testing of high- penetrant genes is standard of clinical care about patients with breast and ovarian cancer, who meet the criteria for testing. Therapy of hereditary mutation associated breast cancer is not different from sporadic breast cancer therapy nowadays. Prognosis of patients with BRCA1, BRCA2 mutation associated breast cancer may be different from sporadic breast cancer patients due to the existing systemic treatment options and higher risk of other malignant disease. - Nádory štítné žlázy jsou nejčastějšími neoplaziemi endokrinního systému. Nejvíce zastoupeným typem nádorů štítné žlázy je papilární karcinom, u kterého jsou nalézány bodové mutace genů BRAF a RAS a RET/PTC přeskupení. Nejčastějšími molekulárně-genetickými změnami u folikulárního karcinomu jsou mutace RAS a PAX8-PPARγ přeskupení. Bodové mutace v RET proto-onkogenu jsou klíčové pro vývoj medulárního karcinomu štítné žlázy, kde se u familiární formy vyskytují zárodečné mutace a u sporadické formy mutace somatické. U nejagresivnějšího anaplastického karcinomu štítné žlázy jsou detekovány mutace BRAF a RAS a dále mutace v genech TP53 a CTNNB1. Díky zjištění genetických příčin vzniku nádorů štítné žlázy začaly být tyto geny a jejich mutace zkoumány jako nadějné terapeutické cíle. Mnoho látek ukázalo protinádorový efekt v preklinických studiích. Nejslibnějšími tyrozinkinázovými inhibitory jsou Vandetanib, Sunitinib a Sorafenib, které jsou zkoumány na tkáňových kulturách, zvířecích modelech a v klinických studiích u metastatických stadií karcinomů štítné žlázy., Thyroid cancer is the most frequent neoplasm of endocrine system. The major type of thyroid tumor is papillary thyroid carcinoma, in which point mutations in the BRAF and RAS genes and RET/PTC rearrangements are found. The most frequent molecular genetic changes in follicular thyroid carcinoma are mutations in the RAS genes and PAX8-PPARγ rearrangemets. Point mutations in the RET proto-oncogene are crucial for development of medullary thyroid carcinoma, where germ-line and somatic mutations occur in familial and sporadic form, respectively. Mutations in the BRAF and RAS genes and moreover mutations in the TP53 and CTNNB1 genes are also detected in the most aggressive anaplastic thyroid carcinoma. Thanks to the finding of genetic causes of thyroid cancer, these genes and their mutations begin to be investigated as promising therapeutic targets. Many compounds show anti-tumor effect in preclinical studies. 246. Genome-wide association studies in schizophrenia, and potential etiological and functional implications of their results

- Background: Despite the fact that the genetic basis of schizophrenia has been intensively studied for more than two decades, our contemporary knowledge in this field is rather fractional, and a substantial part of it is still missing. The aim of this review article is to sum up the data coming from genome‑wide association genetic studies in schizophrenia, and indicate prospective directions of further scientific endeavour. Methods: We searched the National Human Genome Research Institute’s Catalog of genome‑wide association studies for schizophrenia to identify all papers related to this topic. In consequence, we looked up the possible relevancy of these findings for etiology and pathogenesis of schizophrenia using the computer gene and PubMed databases. Results: Eighteen genome‑wide association studies in schizophrenia have been published till now, referring to fifty‑seven genes supposedly involved into schizophrenia’s etiopathogenesis. Most of these genes are related to neurodevelopment, neuroendocrinology, and immunology. - Cíl: Stanovit podíl četnosLi leze šlachy dlouhé hlavy bicepsu (LHBT) na dysfunkci ramenního kloubu. Ověřit předpokládaný vztah leze LHBT k dalším typům současného poškození struktur ramenního kloubu. Posoudit vztah leze LHBT k impingement syndromu. Metodika: Retrospektivně byla hodnocena leze šlachy dlouhé hlavy bicepsu v souboru 79 pacientů, který tvořilo 55 mužů (41,8 ? 13,1 let) a 24 žen (50,4 ? 13 let) odeslaných v období 2011-2012 k vyšetření magnetickou rezonancí (MR) pro dysfunkci ramenního kloubu. Nepřímou MR artrografií jsme vyšetřili 88,6% pacientů, ostatních 11,4 % vyšetření bylo provedeno nativní MR. Pacienti byli vyšetřeni na přístroji Signa HDxT 1,5T (GE Healthcare, Milwaukee) v sekvencích: TI FS ve třech rovinách, MERGE axiálně, STIR koronálně. U nepřímé MR artrografie jsme intravenózně aplikovali 1 molární gadoliniovou kontrastní látku v dávce 0,1 ml/kg. K posouzení závislostí lézí ramenního kloubu jsme použili test rozdílů relativních četností. Nulová hypotéza byla zamítána na hladině statistické významností p < 0,05. Výsledky: Ve sledovaném souboru byl vysoký výskyt leze dlouhé šlachy bicepsu, která byla prokázána u 38 % pacientů (36,4 % mužů a 41,7% žen). Leze šlachy dlouhé hlavy bicepsu se vyskytovala u 43,9 % případů subakromiálního impingementu, u dalších typů impingement syndromů byl současný výskyt statisticky významně nižší: 16,7 % u subkorakoideálního a 6,7 % u posterosuperiorního impingementu. Závěr: Ze studie vyplývající výsledky vysokého podílu leze LHBT na dysfunkci ramenního kloubu nás při hodnocení poruch ramenního kloubu nabádají ke zvýšené pozornosti při hodnocení této struktury. Pozornost je nutné věnovat i posouzení možnosti současné leze LHBT a dalších sdružených patologií ramenního kloubu., Aim: To determine the frequency of the impact of lesions of the long head of the biceps tendon (LHBT) on dysfunctions of the shoulder joint. To examine the supposed relationship of the lesions of the LHBT to other types of simultaneous impairment of structures of the shoulder joint. To assess the relationship of the lesions of the LHBT to the impingement syndrome. Method: The lesions of the long head of the biceps tendon were retrospectively evaluated in a set of 79 patients, which consisted of 55 males (41.8 ? 13.1 years) and 24 females (50.4 ? 13 years), who, in the period 2011-2012, underwent magnetic resonance imaging (MRI) for dysfunctions of the shoulder joint. We used indirect MR arthrography to examine 88.6% of the patients who were examined; the other 11.4% of the patients were examined using native MR examination. The patients were examined using HDxT 1.5T Signa (GE Healthcare, Milwaukee) in the following sequences: TI FS in three planes, MERGE axial, STIR coronal. For the indirect MR arthrography we applied the 1 mol gadolinium contrast agent intravenously (dosage 0.1 ml/kg body weight). To assess the dependence of the shoulder joint lesions, we applied the software test of differences in relative frequencies. The null hypothesis was rejected at the level of statistical significance of p < 0.05. Results. In the set of patients, there was a high incidence of lesions of the long head of the biceps tendon, which were found in 38% of the patients (36.4% of the men and 41.7% of the women). Lesions of the long head of the biceps tendon were observed in 43.9% of the cases of subacromial impingement, while for other types of impingement syndromes the coincidence was statistically significantly lower: 16.7% for subcoracoid impingement and 6.7% for posterosuperior impingement. Conclusion. The study shows that the impact of lesions of the LHBT on the dysfunction of the shoulder joint is relatively high, which suggests that greater attention should be paid to the evaluation of this structure when examining dysfunctions of the shoulder joint. - Úvod: Od konce 80. let minulého století jsou organizovány deskriptivní studie na lékařských fakultách po celém světě. Jejich výsledky poskytují údaje o znalostech, postojích a kuřáckém chování budoucích lékařů a ostatních zdravotnických pracovníků jako výchozí báze pro cílenou výuku o problematice kouření. Metodika: Pomocí dotazníků mezinárodní studie Světové zdravotnické organizace (WHO) a Center pro kontrolu nemocí a prevence (CDC) Global Health Professions Student Survey (GHPSS) bylo po složení státní rigorózní zkoušky vyšetřeno 220 mediků 6. ročníku od září 2011 do června 2012. Výsledky: Téměř čtvrtina studentů nikdy nezkoušela kouřit cigarety, 5,5 % studentů se pokládá za kuřáky, v posledním měsíci pravidelně kouřilo 2,8 % mediků. Ojedinělé zkušenosti s kouřením ostatních druhů tabákových výrobků udává 45 %, s kouřením vodní dýmky 80 % studentů. Expozice pasivnímu kouření je velmi častá ve veřejných prostorách, téměř 40 % studentů popisovalo nedodržování zákazu kouření v prostorách fakulty a klinik. Studenti posledního ročníku LF MU jsou významně lépe informováni o specifické úloze lékařů a ostatních zdravotníků v motivaci kuřáků, aby zanechali kouření, a tuto roli plně akceptují. Praktické dovednosti při poradenství o zanechání kouření u hospitalizovaných pacientů získala během pregraduálního studia více než třetina posluchačů. Závěr: Trend pregraduální výuky mediků nastavený na Lékařské fakultě Masarykovy univerzity v Brně v 90. letech úspěšně pokračuje; naprostá většina absolventů LF MU považuje nekouření za samozřejmost a je si vědoma i specifické úlohy lékařů v omezování kuřácké epidemie. Určité rezervy jsou v nedodržování zákazu kouření na Lékařské fakultě MU a v nutnosti rozšíření poradenské praxe studentů během pregraduálního studia., Backround: From the end of the 1980s worldwide descriptive studies concerned about medical students have been organized. Their results gave data about knowledge, attitudes and smoking behaviour among future physicians and other health professionals as a basis for their future target education about smoking. Methods: From September 2011 to June 2012 220 medical students attending the 6th year filled the World Health Organization (WHO) and Centres for Disease Control and Prevention (CDC) Global Health Professions Student Survey (GHPSS) questionnaire at the end of their State examinations. Results: Almost one quarter of students never tried to smoke cigarettes, 5.5% of students reported themselves as smokers, 2.8% of students smoked regularly during the last month. Single attempts with smoking other tobacco products described 45% and the same frequency of attempts with water pipe reported 80% of students. They are often exposed to passive smoking in public places, almost 40% of students described non-compliance with the ban of smoking within the faculty premises and clinics. Students in the last year of studies are significantly better informed about the specific role of physicians and health professionals in the motivations of smokers to quit and fully accept it. During their pre-gradual studies more than one third of students have obtained practical skills with advice and support patients with smoking cessation. Conclusions: The trend of pre-gradual medical study developed at Medical Faculty, Masaryk University, Brno in the 1990s continues successfully: the majority of graduated students accept non-smoking as an obvious part of life style and understand the specific role of health professionals in the reduction of the smoking epidemic. Haemostasis interferes with the implantation of the blastocyst and placentation. Maintain optimal blood flow placenta, ensures prompt and adequate clotting during the labour. Many coagulation proteins are expressed early in embryonic development and play roles outside of coagulation in cells proliferation and differentiation. Mechanism regulating haemostasis within the placenta remain poorly understood.
