Význam dnešního medicínského práva neustále stoupá, ale oblasti transplantologie není věnován dostatek pozornosti. Samotná nevědomost laické veřejnosti není tolik alarmující jako letitý nezájem a ignorace odborné veřejnosti našimi zákonodárci. Zákon č. 285/2002 Sb., o darování, odběrech a transplantacích tkání a orgánů a o změně některých zákonů nabyl, podle části osmé tohoto zákona, účinnosti dnem 1. září 2002. Je však nutné si uvědomit, že v takovýchto chvílích se nejedná pouze o něj samotného, a to ať už se vyskytne v roli čekatele, nebo případného dárce., Over the last years, the importance of the medical law has been increasing, however to the issue of transplantations among public has not been paid the sufficient attention. But this ignorance is not as alarming as a longtime lack of interest of Czech lawmakers. The Act No. 285/2002 – concerning a donation, removal and transplantation of the tissue and organs (Transplantation Act) – came into being on September 1, 2002. Since that time, physicians have referred to the contradictory provision of § 11, section 4 and also to the fact that this act is not seen as a contribution to a better clearance but rather as an increasing physicians’ uncertainty. Until now, lawmakers have not adjusted its wording, even the form of the international agreement, which would help us to solve this situation. There is no sufficient number of suitable organs and tissues for transplantations. In case of an organs removal from deceased foreigners, Czech Transplantation Centers are sued for law-breaking, although there are candidates for transplantation on a waiting list. It would be considered as an unlawful conduct even in those cases, when family ask for taking suitable organs from their deceased relatives, so that physicians are forced to reject their request. When patients wait for the surgical intervention and donor’s relatives agree with transplantation, medical facilities face the dilemma of following the law but at the expense of bioethics and human rights. This acting would be in a contradiction of the medical ethics and acts, such as the Constitution of Czech Republic and the Charter of Fundamental Rights and Basic Freedoms. In the context of the international law based on the Convention for the Protection of Human Rights and Dignity of the Human Being with regard to the Application of Biology and Medicine – Convention on Human Rights and Biomedicine, there would be an overlap of their principles which Czech Republic is obliged to follow from October 1, 2001. There is no doubt that physicians are not able to save each patient, but it is important to support people to be interested in medical and law issues. I presume that it would be appropriate to take action to change the Transplantation Act as soon as it is possible. From my point of view, there are two ways to solve this situation: to abandon the § 11, section 4 (perhaps the most acceptable), or to enter into the international treaty, which regulates this topic. Transplantology is a very promising field of medicine, but it is exploitable in many ways. Whether it is an issue of taking organs from deceased foreigner, or well-known “transplant tourism” and organ trade, it is necessary to have pillars in the legislative. A healthy person will not admit that this issue might be concerning him in any way. It is important to be aware that each of us can unexpectedly be in the role of a candidate or a donor., Alena Jedličková, and Literatura 15
Zjednodušeně řečeno, bylo třeba, aby se všichni mohli zase volně nadýchnout., In the presented casuistry there is given a description of a family therapist’s work with a family, where the mother suffers from backache and the child (3.5) is chronically ill suffering from cough of unknown origin. The author describes in detail the course of six sessions, during which the family atmosphere was gradually changing. The conflicts that significantly burdened the couple, had their roots in the past of both families and in wrong untold assumptions on both sides. Narrative therapy with respect to the structure and gender balance, which the author learned in the four-year training in the family therapy institute of psychosomatic disorders in Liberec, was the main tool of treatment. Treatment cannot be directly focused on the child’s symptom. It is a symptom of the whole family system. Basically, it was necessary that everyone could breathe freely again., and Sušická L.
Pancreatic schwannoma is a rare neoplasm. By 2012, there were only 47 cases reported on pancreatic schwannoma the English literature to the best of our knowledge. Hereby, we report a 37-year-old Caucasian male with a cystic schwannoma in the head of the pancreas. The patient, complaining of vague abdominal pain and pruritus underwent a computed tomography (CT) scan of the abdomen. The CT scan of the abdomen revealed a cystic lesion in the head of the pancreas. Initially, a cystic mucinous tumor of the pancreas was suspected. Whipple’s procedure was undertaken. The histopathologic evaluation of the specimen revealed a pancreatic schwannoma. The patient did well postoperatively. Even rarely seen, pancreatic schwannoma should be taken into account in the differential diagnosis of pancreatic lesions., Bulent Aytac, Ahmet Karamercan, Mehmet Eren Yuksel, Gamze Kulduk, and Literatura
The topic of the paper is: how to set up PBSP into one to one session format within an ordinary clinical psychology practice. I reflect my own professional experience with the method I have been using for years in its basic group form. However, recent developents of PBSP technology have facilitated it‘s use also in individual psychoteprapy. In the first decades of PBSP developement, the process of therapy was running on ahaed fast, often dramatically, due to body work that had been carried out immediately in special exercises. The current approach has become much slower, subtle and safe and therefore it fits better for the individual setting conditions. However, the basic question stayed remained: how to introduce A METHOD into dyad therapist – client?? In terms of PBSP – a very peculiar method – a great deal of active collaboration is required on a part of a client to apprehend its special characteristics as well as special slang., Siřínek J., and Literatura
These cases are concerning very controversial ethical aspects of biomedical developments in the field of medical techniques of artificial procreation and reshape the content (meaning) of Article 8 of European Convention of Human Rights. This paper provides short overview of cases of European Court of Human Rights involving “right to access to assisted reproduction” and focuses especially on the latest developments in this field (case S.H. and Others v. Austria - Application no 57813/00) . Although the technology of medically assisted procreation had been available in Europe for several decades, many of the issues to which it gave rise, remained the subject of debate. There is no consensus towards the use of IVF treatment in general and even less clear are trends in some particular questions (f.e. allowing gamete donation). These unsolved problems give to raise serious doubts about the possibility of the Court provide “final decisions” in these matters., Tomáš Doležal, and Literatura 10
Acenocoumarol is a vitamin K antagonist that is used for the treatment of acquired and congenital, both arterial and venous, thrombotic diseases. Its use is complicated by the narrow therapeutic range. Bleeding following oral anticoagulation, despite rare, remains the major complication. Most cases of hemorrhagic episodes usually require short hospitalization and transfusion, while surgical drainage of the hematoma is not recommended. However, in cases that conservative treatment isn’t successful, surgical intervention remains an option. We present a case of severe spontaneous bleeding of the rectus abdominis muscle which was successfully managed surgically., Orestis Ioannidis, George Paraskevas, Anastasios Kotronis, Stavros Chatzopoulos, Athina Konstantara, Nikolaos Papadimitriou, Apostolos Makrantonakis, Emmanouil Kakoutis, and Literatura 15
Sústreďuje sa na zásadnú úlohu „virtuálnej kolonoskopie“ pomocou trojdimenzionálnej transvaginálnej sonografie (3D-TVUS) pri hodnotení rozsahu ochorenia., Deep infiltrating endometriosis (DIE) is a chronic progressive estrogen-dependent disease that can also affect structures of the gastrointestinal tract. The disease is usually asymptomatic in long-term, respectively, is manifested with very nonspecific symptoms. Its late detection can lead to irreversible changes in the individual structures. This case report describes a rare case of extensive deep infiltrating endometriosis of posterior pelvic compartment, which resulted into the state of obstructive ileus. It focuses on the essential role of „virtual colonoscopy“ using three-dimensional transvaginal sonography (3D-TVUS) in evaluating the extent of the disease., Silvia Vernerová, Róbert Dankovčík, Svetlana Jánošová, Pavel Bartoš, Martin Trhlík, Anna Šebová, and Literatura
Jde snad o posun poruchy příjmu potravy do mladších ročníků?
