The author reported clinical characteristic features and risk factors for acute intestinal infections in 225 children (140 boys and 85 girls, mean age 1.5 years old). 90 (40%) patients aged up to 12 months, 101 (44.9%) were from 1 to 2 years old, and 34 (15.1%) were more than 2 years old. Most children were hospitalized in the first week of illness and had moderate severe course of disease. Severe forms of illness reliably more often were noted in children being on artificial feeding. Toxicdystrophic condition, severe forms and perinatal encephalopathy were prevailed in boys. Factorial analysis revealed 20 most significant factors with factorization fullness 70.8%. The most significant factors were F1( factor of hyperthermia duration) with 8.5% effect, F2 (factor of pain syndrome) 5.8%, F3 (factor of normal enzymatic activity of colon bacillus) 4.9%, F4 (etiological factor) 4.6%, as well as F5( factor of opportunistic flora of the intestine) 4.4%, respectively., Mukarram Shadjalilova, and Literatura
Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. An eight-month-old male child was admitted with weakness and swelling in the feet. Paraparesis and bilateral lower extremity edema were present in the neurological examination. Thoracic MRI showed an intradural intramedullary mass 61 × 11 mm in size in the T5–T10 levels. Laminotomy between the T5–T10 vertebrae was performed. A mass with smooth borders was separated from most of the neural tissue. In the postoperative MRI, we observed a contrast enhancing area, considered a residual fragment, only 5 × 4 mm in size. Histopathological properties were compatible with the intermixed subtype of ganglioneuroblastoma. Only a limited number of thoracic cord Ganglioneuroblastoma reports have been previously published. Although very rare in children and young adults, ganglioneuroblastoma should be included in the differential diagnosis of thoracic cord tumors. It is difficult to obtain a preoperative diagnosis with clinical features and radiological investigations. Diagnosis depends on histopathological examinations. Curative treatment should be in the form of a complete resection of the tumor. Infantilní hemangiomy jsou nejčastějšími benigními vaskulárními nádory novorozeneckého a kojeneckého věku. Jedná se o nádory s velmi dobrou prognózou. Většina infantilních hemangiomů spontánně zcela regreduje v prvních deseti letech života. Jen deset procent infantilních hemangiomů vyžaduje nějakou terapii. V současné době je preferováno použití b-blokátoru propranololu. Ostatní typy benigních cévních nádorů je zapotřebí diferenciálně diagnosticky odlišit pro zahájení vhodné terapie a určení prognózy. Předložený článek shrnuje základní poznatky o hemangiomech u novorozenců. V závěru článku je zmíněna krátká kazuistika, kdy byla léčba rozsáhlého hemangiomu zahajována po konzultacích na našem pracovišti za kontroly kardiology., Infantile hemangiomas are the most common benign vascular tumors of neonatal and infancy. These are tumors with a good prognosis. Most infantile hemangiomas spontaneously regress completely in the first decade of life. Only ten percent of infantile hemangiomas require some therapy. Currently it is preferred to use b-blocker propranolol. Other types of benign vascular tumors are needed to distinguish the differential diagnosis for initiation of appropriate therapy and prognosis. V předložené kazuistice je zachycena práce rodinného terapeuta s rodinou, kde matka trpí bolestmi zad a dítě (3,5) je chronicky nemocné. Trpí kašlem nejasného původu. Autorka podrobně popisuje průběh šesti sezení, během nichž se v rodině postupně měnila atmosféra. Konflikty, které pár značně zatěžovaly, měly své kořeny v minulosti obou rodin a v mylných nevyslovovaných předpokladech na obou stranách. Narativní terapie s respektem ke struktuře a genderové vyváženosti, kterou se autorka učila v rámci čtyřletého výcviku v institutu rodinné terapie psychosomatických poruch v Liberci, byla zde hlavním nástrojem léčby. Léčba se nemůže zaměřit přímo na symptom dítěte. Jde o symptom celého rodinného systému. Zjednodušeně řečeno, bylo třeba, aby se všichni mohli zase volně nadýchnout., In the presented casuistry there is given a description of a family therapist’s work with a family, where the mother suffers from backache and the child (3.5) is chronically ill suffering from cough of unknown origin. The author describes in detail the course of six sessions, during which the family atmosphere was gradually changing. The conflicts that significantly burdened the couple, had their roots in the past of both families and in wrong untold assumptions on both sides. Narrative therapy with respect to the structure and gender balance, which the author learned in the four-year training in the family therapy institute of psychosomatic disorders in Liberec, was the main tool of treatment.
