V loňském roce vyšel v Časopise zdravotnického práva a bioetiky článek Skalpel a duše (č. 1/2013, autorka Andrea Baršová), který se zabýval proměnami vztahu společnosti a práva k transsexualitě. O několik měsíců později přinesla světová média zprávu o „uznání třetího pohlaví“ v Německu. Novinka s obsahem citovaného článku souvisí. Stejná linie myšlení, která vedla k upuštění od nucených zásahů do těla u transsexuálů, má zde svoji paralelu v přístupu k intersexuálním lidem. Předkládaný příspěvek shrnuje dosavadní přístupy k intersexualitě a seznamuje s nejnovějším vývojem v Německu, zemi s výrazným hnutím intersexuálních lidí, a na mezinárodní úrovni. Malá změna v německém právním řádu, a sice možnost neuvést v matriční knize narození pohlaví dítěte, má zatím k uznání jiného pohlaví daleko. Zpráva Německé etické rady z roku 2012, která německé zákonodárce ke změně vedla, a probuzený zájem mezinárodních lidskoprávních organizací o toto téma jsou však zřetelnými signály nastupující paradigmatické změny v přístupu k intersexuálním lidem v naší kultuře., A year ago, the article by Andrea Baršová Skalpel a duše (Scalpel and the Soul), which addressed recent transformation of societal and legal attitudes towards transsexuals, was published by the Journal of Medical Law and Bioethics (1/2013). A few months later, media worldwide released news on the recognition of third sex in Germany. This development is linked to the published article. The same line of thinking, which led to abandonment of enforced medical interferences in the bodies of transsexuals (in order to recognise their gender change), has here its parallel in current attitudes towards intersex people. This article sums up existing approaches to intersex individuals and discusses newest developments in Germany, a country with strong intersex movement, and on the international level. A small change in the German law, namely an option to leave open the entry for sex in birth registers, is far from legal recognition of other sex as such. Alternativa je ve zdravé společnosti všudypřítomná a je zárukou diverzity, jež je obdobně jako v ekosystémech zárukou rozvoje a trvalé udržitelnos ti celku. Teprve modernizace zplodila masovou kulturu a masovou medicínu. Zatímco v mnoha společenských oblastech došlo od konce 60. let minulého století k legitimizaci alter nativních přístupů (umění, hudba, ekologie, zemědělství, energetika, pedagogika, peněžnictví), v medicíně je diskuze o alternativách odlišná a tendence k monopolizaci vědecké biomedicíny jako jediné správné medicíny extrémní. To je vysvětleno civilizačním procesem medicinalizace živ ota a existencí nesmírně složitého a sofistikovaného medicínsko-průmyslového komplexu, k terý obtížně dešifrovatelným způsobem propojuje rozmanité zájmy a potřeby lékařského škol ství, klinické praxe na jedné straně, masmedií a producentů léků, léčiv a zdravotnických technologií a pomůcek na straně druhé. Diskuze o alternativě v medicíně přesto probíhá a s ílí. Psychosomatický přístup je jednou z mnoha rozumných alternativ, nikoliv však jedinou. Dilema nestojí mezi medicínou, jež ignoruje mysl člověka a tou, jež jí přikládá někdy až příliš velký význam tváří v tvář genetickým faktorům či zátěži životního prostředí. Dalšími výz namnými alternativami je sociálně ekologický model zdraví a zdravotnictví, spočívajíc í na zdravých životních podmínkách a vztazích či komplexní alternativní léčebné systémy, jež jdou svým kosmologickým a spirituálním pojetím nad rámec psychosomatického. Psychické prož ívání je jen jednou z mnoha opomíjených determinant zdraví a jakákoli absolutizace či zjedn odušování vztahů mezi myslí a tělesnou úrovní by mohla být kontraproduktivní. Bylo by chyb né se domnívat, že zázračné řešení se nalézá v psychosomatice a že volba je buď- anebo. T rvale udržitelná medicína musí být pluralitní a integrativní, a jako taková bude bez d iskriminace, devalvování a nadřazování využívat všechny rozumné způsoby pomoci nemocným a to z důvodů etických, ekologických i ekonomických., Plurality is the fundament of healthy societies and diversity can be viewed as a symptom of growth and sustainability similarly to ecosystems. Only the modern civilization resulted in mass culture and mass medicine which is incorporated in a complex health care system. While in many areas of society a discourse on alternative so lutions became increasingly legitimate since the late 60s of the last century (alternative art, music, ecology, agriculture, energy, education, finance), the discussion on alternative solutions i n medicine and health care is much less visible and the overall monopolization of health care by th e scientific biomedicine prevails. This is explained by medicalization of life and by the impa ct of a highly complex and sophisticated medico- industrial complex, which merges the intere sts and needs of medicine (medical education, clinical practice and research) on one h and, with the interests of drug and technology producers, on the other. Though, the discussion on alternative approaches to health and disease exists and grows further. Psychosomatic approach is one of reasonable alternatives, but not the only one. The dilemma is not between medicine that ignores the human mind and that which integrates mind and body. Other possible approaches to health and disease are represented by e.g. social ecological model of health and health c are, which is based on healthy living conditions, fair opportunities and relationships or alternative healing systems which go beyond the psychosomatic perspective. Psychological domain is one of many neglected determinants of health. Psychosomatic medicine and psychosomatic ap proach per se cannot save the biomedicine. Sustainable medicine must be pluralist ic, integrative and not-discriminating in using all reasonable means to help the sick. Moderní lékařské diagnostické metody umožňují zjištění vážného postižení plodu. Přesto dochází k narozením těžce malformovaných jedinců, neschopných bezprostředně nebo do budoucna uvědoměle využívat faktického stavu své biologické existence. Prezentuje se názor, že u novorozenců s takto vážnými malformacemi by mělo být přípustné upustit od úsilí o záchranu života i při naději na přežití, jsou-li malformace tak závažné, že vylučují do budoucna možnost smysluplné a uvědomělé lidské existence. Mělo by však jít o mimořádnou situaci, které je třeba předcházet především zabezpečením takové úrovně prenatální péče a zdravotnické výchovy, aby se prakticky možnost donošení a porodu těžce malformovaného jedince vyloučila., Modern medical diagnostic methods allow the detection of serious birth defects. Yet there are born severely malformed individuals who are unable (immediately or in the future) to use consciously actual state of their biological existence. There is opinion that it should be possible to abandon the effort to save lives (even when there is a chance of survival) when the malformations of the neonates are so severe that they exclude the future possibility for meaningful and conscious human existence. Pro klasifikaci prekanceróz vulvy se používá termín vulvární intraepiteliální neoplázie (VIN). Histologicky lze rozlišit dva základní typy VIN: VIN obvyklého typu (u-VIN) a VIN diferencovaného typu (d-VIN). Rozsah a intenzita dysplastických změn se v případě u-VIN vyjadřuje třístupňovým gradingem (u-VIN I, II a III). Prekancerózní potenciál d-VIN odpovídá vždy karcinomu in situ, proto se u této diagnostické jednotky grading neprovádí. Etiologicky je u-VIN spojena s infekcí vysoce rizikovými typy lidského papillomaviru (HPV), zatímco d-VIN je HPV negativní a rozvíjí se v terénu chronických vulvárních dermatóz typu lichen sclerosus a lichen simplex chronicus. u-VIN má tendenci k multifokalitě a k dlouhodobému recidivujícímu průběhu před transformací v invazivní dlaždicobuněčný karcinom. d-VIN se naopak vyskytuje spíše solitárně a má sklon k rychlé stromální invazi. u-VIN typicky postihuje ženy premenopauzálního věku s vyšší incidencí ostatních prekancerózních lézí dolního ženského pohlavního systému včetně perinea a anu. Pacientky s d-VIN bývají postmenopauzální ženy a není u nich pozorována asociace s výše zmíněnými dyspláziemi ostatních anatomických lokalizací. Nízká diagnostická reproducibilita kategorie u-VIN I, pochyby o prekancerózním potenciálu u-VIN I a problematické rozlišení u-VIN II a u-VIN III vedly k revizi současného diagnostického schématu. Podstatou změn je zrušení gradingu u-VIN se sloučením kategorií u-VIN II a u-VIN III a dále zrušení u-VIN I jako diagnostické jednotky. V revidované klasifikaci prekanceróz vulvy proto termín u-VIN reprezentuje high grade dysplastické léze asociované s HPV (dříve u-VIN II a u-VIN III) a d-VIN i nadále zůstává označením pro HPV negativní high grade prekancerózu., Classification of vulvar precancerous lesions is based on the concept of vulvar intraepithelial neoplasia (VIN). Two basic types of VIN are recognized histologically: VIN of the usual type (u-VIN) and VIN of the differentiated type (d-VIN). The three grade evaluation system is used to assess the intesity of dysplastic changes in u-VIN (u-VIN I, II and III). The precancerous potential of d-VIN corresponds to carcinoma in situ and therefore grading is not applied. u-VIN is etiologically linked to HPV infection, whereas d-VIN is HPV negative and emerges in the environment of chronic vulvar dermatoses (lichen sclerosus and lichen simplex chronicus). u-VIN has a tendency to multifocality and prolonged recurrent progression to the invasive squamous cell carcinoma. On the other hand, d-VIN represents rather a solitary lesion with a propensity to the rapid stromal invasion. u-VIN usually affects premenopausal women with a higher incidence of other precancerous lesions of the lower female genital tract including the perineum and anal area. Patients diagnosed with d-VIN are of postmenopausal age without any association with aforementioned dysplastic lesions of other anatomic locations. The low diagnostic reproducibility of the u-VIN I category, doubts about the precancerous potential of u-VIN I and the problematic distinction between u-VIN II and u-VIN III resulted in the modification of the current terminology. The grading of u-VIN was abandoned, u-VIN II and u-VIN III categories were merged and u-VIN I was removed from the classification scheme. The revised u-VIN category therefore represents high grade dysplastic lesions associated with HPV infection (former u-VIN II and u-VIN III categories) and the term d-VIN is still reserved for the HPV negative high grade vulvar precancerosis., and P. The purpose of this study was to investigate the content neurospecific markers protein S-100 and neuroenolaza in blood serum and tear fluid of patients with ocular ischemic syndrome. Material and methods. We observed 43 patients aged 57 to 79 years, mean age 67.3 ± 2.7 years. Control group consisted of 11 volunteers without ophthalmic symptoms. The main group consisted of 32 patients with OIS. The neurospecific proteins S100 and NSE were investigated in blood serum and tear fluid. Results. The study found that in patients of the control group the content of protein were within the normal range: S -100 in the tear fluid – 0,0662 ± 0,00335 mkg/l, in the blood serum 0,0508 ± 0,00241 mkg/l. In patients of the main group the indicators of protein in the tear fluid were elevated in all patients - 3,12 ± 0,246 mkg/l ( p<0.005). The normal evels in blood serum of marker S-100 was in 30 patients - 0,0589 ± 0,00303 mkg/l, while, in 2 patients protein S-100 were raised and averaged 0,2175±0,00725 mkg/l. It was found that in patients of the control group content of protein NSE in the tear fluid and blood serum were within normal values - 15,86 ± 0,148 Ng/ml, 15,60 ± 0,202 Ng/ml respectively. In the main group the amount of protein NSE tended to increase in the tear fluid in 23 patients and averaged 33,012 ± 3,2626 Ng/ml (p<0.005), a significant decrease the quantity of protein was observed in 9 patients, which amounted to 5,166 ± 0,8301 Ng/ml. At normal levels in the blood serum protein NSE detected in 30 patients and averaged 14,48 ± 0,263 Ng/ml, whereas, in 2 patients there was a significant increase of content of protein NSE and was 27,47 ± 3,068 Ng/ml. Conclusions. In a review of the book " Medical law and ethics " by Jonathan Herring, I made it my goal to familiarize the reader with the issues of medical law and ethics, which author discuss in his work . The book should be a valuable source of knowledge for both, experts and laymen, because the author dealt with many topics from different angles, that raise a number of legal and ethical issues (the issue of euthanasia , research on humans , cloning , sterilization , etc. .).
