Hemangiom je nejčastější primární benigní nádor páteře typu hamartomu, který nikdy nemalignizuje. V drtivé většině případů se jedná o asymptomatické solitární léze hrudní páteře jakožto náhodné nálezy radiografických vyšetření. Pouze v 1 % případů se hemangiom obratle projeví klinicky a agresivita této benigní léze pak spočívá v expanzivním chování nádorové tkáně či v důsledku patologické fraktury obratle. Klinickými příznaky potom jsou především dorzalgie, v pokročilých případech pak známky kořenové nebo míšní komprese. Radiodiagnostika je poměrně snadná, protože hemangiom obratle vykazuje zcela typické grafické příznaky, pro potvrzení diagnózy agresivního hemangiomu obratle jsou stanovena grafická kritéria. Spektrum léčebných modalit je velmi široké od konzervativní léčby přes hodně využívanou vertebroplastiku po radikální chirurgickou léčbu. Komplementárně jsou často využívány embolizační výkony, alkoholová sklerotizace a lokální radioterapie. Agresivní hemangiom obratle je poměrně velmi vzácná diagnóza. Proto zatím neexistují velké soubory či klinické studie, podle kterých by bylo možné stanovit optimální terapeutický postup., Hemangioma is the most common primary benign hamartoma type spine tumor. Malignant degeneration has never been reported. In the majority of cases, this is an asymptomatic solitary lesion of the thoracic spine found incidentally during a radiographic examination. Only 1% of cases manifests clinically and this benign lesion is considered aggressive because of the expansive nature of the tumor and because it may cause pathological fracture of the vertebrae. Clinical symptoms then include dorsalgia and either root or spinal neurological symptoms. Radiological diagnosis is relatively easy because vertebral hemangioma is associated with quite typical graphic signs; graphic criteria to confirm the diagnosis of aggressive vertebral hemangioma have been clearly defined. The range of treatment options is very wide, from conservative treatment through frequently used vertebroplasty to radical surgical treatment. Embolization procedures, alcohol sclerotherapy and local radiotherapy are often used as complementary procedures. Aggressive vertebral hemangioma is a relatively rare diagnosis, so far there are no large cohorts or clinical studies from which it would be possible to determine an optimal therapeutic approach. Syndróm polycystických ovárií (PCOS) je najčastejšou endokrinopatiou u žien fertilného veku a najčastejšou príčinou porúch menštruačného cyklu. Je charakterizovaný hyperandrogénnym stavom (klinickým alebo biochemickým) a ovariálnou dysfunkciou (anovuláciou alebo ultrasonografickým nálezom polycystických ovárií), čo sú zároveň kritériá pre jeho diagnózu podľa Androgen Excess and PCOS Society. Syndróm má mnohopočetné fenotypové prejavy, medzi ktoré patrí okrem uvedených charakteristík aj metabolický syndróm, predovšetkým obezita a inzulínová rezistencia. Diagnóza PCOS je v klinickej praxi pomerne náročná a stále platí, že je diagnózou per exclusionem, po vylúčení iných príčin hyperandrogénneho stavu a chronickej oligo-anovulácie. Vyžaduje si úzku spoluprácu gynekológa, endokrinológa a z hľadiska častých metabolických komplikácií aj internistu, diabetológa a prípadne aj kardiológa. Kľúčové slová: AES kritériá – diagnóza – diferenciálna diagnóza – syndróm polycystických ovárií, Polycystic ovary syndrome (PCOS) is the most frequent endocrinopathy among women of reproductive age and the most frequent cause of menstruation cycle disorders. It is marked by a hyperandrogenic state (clinical and/or biochemical) and ovulatory dysfunction (anovulation and/or ultrasonographic finding of polycystic ovaries), which are also criteria for its diagnosis according to Androgen Excess and PCOS Society. The syndrome has multiple phenotypic expressions, among them besides the above characteristics also a metabolic syndrome, primarily obesity and insulin resistance. Diagnosing of PCOS may be rather exacting in clinical practice and it remains to be a diagnosis per exclusionem, following elimination of other causes of hyperandrogenic state and chronic oligo-anovulation. It requires a close cooperation between a gynecologist and endocrinologist and with regard to frequent metabolic complications also with an internist, diabetologist and possibly cardiologist. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. Ochorenia myelínu CNS sa súhrnne označujú ako leukoencefalopatie. Termín zahŕňa poruchy, pri ktorých zmeny v bielej hmote prevažujú alebo sú výlučné, pričom patofyziologický mechanizmus ani histopatologická báza nie sú rozhodujúce. Z tejto širokej skupiny vrodených a získaných porúch sa vyčleňuje skupina leukodystrofií, ktorá označuje primárnu poruchu myelínu a buniek tvoriacich myelín. Klasicky sa do tejto skupiny radí šesť klinických jednotiek: X‑adrenoleukodystrofia, metachromatická leukodystrofia, Krabbeho globoidná leukodystrofia, Canavanovej choroba, Pelizaeus‑Merzbacherova choroba a Alexandrova choroba. Pribúdajúcimi poznatkami sa skupina rozširuje o stále nové primárne poruchy. Pre presnú diagnostiku je dôležitý klinický nález, významné sú zmeny na MR mozgu a laboratórne vyšetrenia. Precíznou analýzou MR obrazu je možné diagnostický proces urýchliť. Jedným zo spoločných znakov leukodystrofií je progresívny priebeh. U detí nastáva zväčša rýchla strata motorických a kognitívnych funkcií. V prípade adolescentov a adultných pacientov je ťažisko zmien v psychickej a mentálnej oblasti, pričom motorický deficit nastupuje neskôr. Priebeh X‑adrenoleukodystrofie, metachromatickej leukodystrofie, Krabbeho globoidnej leukodystrofie sa dá vo včasnom štádiu alebo predklinickom období ovplyvniť transplantáciou kostnej drene alebo kmeňových buniek. V prípade metachromatickej leukodystrofie je v skorých štádiách indikovaná génová a enzýmová terapia. Pre ostatné leukodystrofie je dostupná len symptomatická liečba. Cieľom práce je zhrnutie najdôležitejších poznatkov a liečebných možností., Myelin disorders of the central nervous system are also known as leukoencephalopathies. This term includes diseases, in which changes of the white matter are dominant, or even exclusive, while neither the pathophysiological mechanism nor the histopathological basis are decisive. This broad selection of inborn and acquired disorders includes a group of leukodystrophies characterized by primary dysfunction of myelin and myelin-producing cells. Typically, six separate clinical entities are included in this group: X-linked adrenoleukodystrophy, metachromatic leukodystrophy, Krabbe’s globoid cell leukodystrophy, Canavan disease, Pelizaeus-Merzbacher disease and Alexander disease. The group is constantly expanding as our knowledge develops. Clinical and laboratory findings, as well as MRI scans, are crucial for exact diagnosis. Accurate analysis of MRI images accelerates the diagnostic process considerably. A common feature of leukodystrophies is their progressive nature. Children exhibit rapid loss of motor and cognitive functions. In adolescent and adult patients, psychological and mental changes are the most prominent, while motor deficit has a later onset. The course of X-linked adrenoleukodystrophy, metachromatic leukodystrophy, and Krabbe’s leukodystrophy can be managed at the early or pre-clinical stages with bone marrow or stem cell transplantation. Gene and enzyme therapy are indicated at the earliest stages of metachromatic leukodystrophy. For the remaining leukodystrophies, only symptomatic therapy is available. Neuromyelitis optica (NMO), také známá jako Devicova choroba, je devastující autoimunitní onemocnění centrálního nervového systému klinicky charakterizované záněty zrakového nervu, míchy a méně často mozkového kmene. Objev protilátek proti akvaporinu‑4 (NMO‑IgG/AQP4-IgG) definitivně odlišil tuto chorobu od roztroušené sklerózy. Testování AQP4-IgG je klíčové v diferenciálně diagnostickém procesu, ve kterém se dále neobejdeme bez magnetické rezonance mozku a míchy. Typickým nálezem jsou míšní ložiska přesahující délku tří míšních segmentů. Změny na magnetické rezonanci mozku diagnózu NMO nevylučují, jsou naopak relativně časté a mají poměrně typický charakter. Důležité informace přináší i optická koherentní tomografie, která kvantifikuje postižení sítnice a zrakového nervu. Neurologický deficit u této choroby je následkem relapsů onemocnění, a proto je zcela zásadní časné stanovení diagnózy a nasazení adekvátní terapie, která může změnit nepříznivou prognózu pacienta. Mezi hlavní léčebné strategie patří agresivní léčba relapsů (vysokodávkované steroidy či plazmaferéza) následované zahájením chronické terapie (mezi léky první volby dnes řadíme azathioprin a rituximab)., Neuromyelitis optica (NMO), also known as Devic‘s disease, is a devastating autoimmune disorder of the central nervous system and is clinically characterized by inflammation of the optic nerve, spinal cord, and, less frequently, brainstem. The discovery of antibodies against aquaporin‑4 (NMO‑IgG/AQP4-IgG) has definitively distinguished this disease from multiple sclerosis. AQP4-IgG testing is crucial in differential diagnosis that also includes magnetic resonance imaging of the brain and spinal cord. Typically, spinal cord lesions exceeding the length of three vertebral segments are found. Changes in magnetic resonance imaging of the brain do not exclude the diagnosis of NMO; by contrast, they are relatively common and are fairly typical. Optical coherence tomography provides important information, and quantifies the involvement of the retina and optic nerve. Neurologic deficit in this disease is a result of disease relapse, and it is therefore crucial to establish the diagnosis early and initiate an appropriate treatment that can change the unfavorable prognosis. The main therapeutic strategies include aggressive treatment of relapses (with high‑dose corticosteroids or plasmapheresis), followed by chronic therapy (first choice drugs include azathioprine and rituximab). Apendicitida je nejčastější břišní náhlá příhoda. Diagnóza může být ihned jasná u pacientů s klasickými příznaky a znaky. Atypické příznaky mohou výrazně ztížit stanovení diagnózy a zpozdit léčbu. Laboratory data usually reveal an elevated leukocytosis with a left shift and elevated C-reactive protein. To establish the diagnosis greatly help native abdominal X-ray, ultrasound or CT. Pánevní kongesce je zvláštní forma chronického žilního onemocnění postihující ženy. Definovaná je jako klinicko‑anatomický syndrom charakterizovaný chronickou pánevní bolestí, perineálním dyskomfortem, mikčními obtížemi a postkoitální bolestí, které jsou způsobeny ovariálním a/nebo pánevním žilním refluxem. Bývá příčinou varixů vulvy, perinea, hýždí a dolních končetin. Těsný vztah je mezi symptomatickými pánevními varixy a renálním syndromem louskáčku. Duplexní ultrasonografie žil břicha, pánve, dolních končetin a vulvoperineální oblasti je hlavní neinvazivní diagnostickou metodou. Magnetická rezonanční venografie nebo jen MR venografie má vysokou senzitivitu v hodnocení pánevní kongesce a může být užita jako screeningové vyšetření nebo v případě nejasného nálezu na ultrazvuku a klinických pochybností. Možnosti medikamentózní terapie jsou omezené, základními léky jsou venofarmaka. Endovaskulární intervence – terapeutická embolizace ovariálních/pánevních žil a implantace ilikokaválního a renálního endovenózního stentu – se stává léčbou první volby u žen s těžkými příznaky, které nereagují na konzervativní terapii., Pelvic congestion is a special form of chronic venous disease in women. This is defined as s syndrome which includes chronic pelvic pain, perineal discomfort, difficulties of micturition, and post‑coital pain in association with ovarian and/or pelvic vein reflux. It may cause vulvar, perineal, gluteal and lower extremity varices. There is a close relation between symptomatic pelvic varices and renal nutcracker syndrome. Duplex ultrasound of abdominal, pelvic and lower extremity veins and vulvoprineal region is the cardinal method of non‑invasive examination. MRI Venography is a highly sensitive technique for evaluating pelvic congestion. It may be used as a screening method or in cases of ambiguous sonographic findings and doubts. Pharmaceutical treatment options are limited, and therapy is based on venoactive drugs.
