Ortotopická transplantace srdce je zavedenou metodou léčby pacientů v terminálním stadiu srdečního selhání, která zlepšuje přežívání těchto pacientů. Cenou za podávání imunosupresivní léčby je řada potenciálních rizik, jedněmi z nejzávažnějších jsou infekční komplikace. Kazuistika popisuje případ pacientky se srdečním selháním na podkladě primární amyloidózy, která podstoupila srdeční transplantaci. Po transplantaci byla v plánu kurativní hematoonkologická léčba, která však vzhledem ke kompletní remisi základního onemocnění nebyla uskutečněna. V časném potransplantačním období pacientka prodělala řadu komplikací, z nichž nejzávažnější byla invazivní plicní aspergilóza. Tato infekce byla úspěšně zaléčena vorikonazolem a pacientka je nyní 28 měsíců po srdeční transplantaci ve velmi dobrém klinickém stavu bez známek aktivní infekce., Orthotopic heart transplantation is an established method of treatment for patients with end‑stage heart failure, which improves the survival of these patients. The use of immunosuppressive therapy comes at the price of a number of potential risks, the most serious being infectious complications. Our case report describes a patient with heart failure on the basis of primary amyloidosis who underwent cardiac transplantation. Hemato‑oncological curative treatment was planned after transplantation, but has not been applied due to the complete remission of the primary disease. In the early post‑transplant period, the patient experienced a number of complications, the most serious being invasive pulmonary aspergillosis. Spinální arteriovenózní malformace jsou skupina vzácných onemocnění, jejichž klinické projevy mohou být velmi rozmanité. Optimální léčebný postup je nutno vždy volit individuálně a pacienty směřovat do specializovaných center. Cílem sdělení je uvést přehlednou klasifikaci SAVM, jejich diferenciální diagnostiku a terapii a dále prezentovat dvě kazuistiky. První kazuistika se zmiňuje o případu 27letého muže, u kterého se rozvinuly silné bolesti bederní páteře s dominující meningeální symptomatikou v klinickém obraze. Na MR hrudního úseku páteře (Th11/12) byla nalezena AVM zásobovaná hypertrofickou radikulopiální tepnou. Na specializovaném pracovišti byla provedena úspěšná embolizace AV zkratu. Druhá kazuistika uvádí případ 41leté ženy, které vznikla při lyžování náhlá prudká bolest za krkem. V neurologickém obraze došlo k rozvoji inkompletní léze míšní s kvadruparézou s levostrannou akcentací do těžkého stupně s hranicí čití ve výši C1. Na provedeném MR a MRAG krční míchy a následně na DSA vertebrálních tepen byla detekována AVM zásobovaná levou arteria spinalis posterior. Terapeuticky byla indikována elektivní endovaskulární embolizace a pacientka byla ošetřena na specializovaném pracovišti ve Francii., Spinal arteriovenous malformations are a group of rare diseases with highly diverse clinical manifestations. The optimal therapeutic practice should always be selected individually and patients should be sent to specialized centres. The objective of this paper is to provide a structured classification of spinal AVMs, their differential diagnosis and therapy, and to present two case reports. The first report is concerned with a case of a 27-year-old man who developed severe pain in the lumbar spine with predominantly meningeal symptomatology. An AVM fed by a hypertrophic radiculopial artery was found on the thoracic spine (Th11/12) MR. A specialized centre performed successful embolization of an arteriovenous shunt. The second report presents a case of a 41-year-old woman, who felt sudden sharp pain in the neck while skiing. Neurologically, she developed an incomplete spinal cord lesion with quadriparesis with left-sided accentuation of severe intensity and sensation threshold at C1. The MR and MRAG of the cervical spinal cord and the following DSA of vertebral artery detected AVM fed by the left posterior spinal artery. Střelná poranění mozku jsou velmi závažná poranění a přežití závisí na včasné diagnostice a intervenci neurochirurga. Důležitým aspektem je i volba zbraně. V této kazuistice je uváděn případ pacienta hospitalizovaného na Neurochirurgické klinice ve FN Brno. Použitá zbraň je mezi replikami historických zbraní velmi raritní a statistiky poranění dlouhou předovkou neuvádějí., Missile brain injuries are very serious injuries, survival of which depends on early diagnosis and neurosurgical intervention. The choice of a weapon is an important aspect. In this case report, we describe a case treated at the Department of Neurosurgery, University Hospital Brno. The weapon causing this injury was a quite rare replica of a historical weapon and injury statistics do not report injuries caused by a longrifle. Our case-based review focuses on limb salvage through operative management of Charcot neuroarthropathy of the diabetic foot. We describe a case, when a below-knee amputation was considered in a patient with chronic Charcot foot with a rocker-bottom deformity and chronic plantar ulceration. Conservative treatment failed. Targeted antibiotic therapy and operative management (Tendo-Achilles lengthening, resectional arthrodesis of Lisfranc and midtarsal joints, fixation with large-diameter axial screws, and plaster cast) were performed. On the basis of this case, we discuss options and drawbacks of surgical management. Our approach led to healing of the ulcer and correction of the deformity. Two years after surgery, we observed a significant improvement in patient's quality of life. Advanced diagnostic and imaging techniques, a better understanding of the biomechanics and biology of Charcot neuroarthropathy, and suitable osteosynthetic material enables diabetic limb salvage. and T. Autoři referují o vzácném případu traumatické totální avulze očního bulbu s optickým nervem v úrovni chiazmatu. Kazuistické sdělení retrospektivně popisuje průběh ročního sledování mladé ženy, která byla zraněna při hasičských závodech. Avulze bulbu a optiku, kombinovaná s poraněním chiazmatu, rezultovala v hemianopický defekt druhostranného zorného pole. Při traumatu vzniklo subarachnoidální krvácení do oblasti cisteren chiazmatu, jež spontánně ustoupilo. Nedošlo k rozvoji vaskulárních spazmů mozku. Celková kortikoterapie a neuroprotekce neměla vliv na průběh onemocnění. Během sledovací doby objektivně došlo k descendentní atrofii korespondujících axonů a retinálních gangliových buněk. V současné době neexistuje efektivní terapie poranění v oblasti chiazmatu., The authors report a rare case of complete traumatic avulsion of the eyeball with the optic nerve at the level of chiasm. The case report retrospectively describes a year-long monitoring of a young woman who was injured in fire-fighter races. Avulsion of the globe and optic nerve combined with injured chiasm resulted in contralateral hemianopic visual field defect. The trauma led to subarachnoid hemorrhage in cisterns of chiasm, which spontaneously resolved. There was no development of vascular spasms in the brain. Systemic corticosteroid therapy and neuroprotection did not affect the course of the disease. During the observation period, descending atrophy of the corresponding axons and retinal ganglion cells developed. There is currently no effective therapy of chiasm injuries., and L. Hejsek, V. Vazospazmy jsou považovány za hlavní příčinu vysoké mortality a špatné prognózy pacientů se subarachnoidálním krvácením způsobeným rupturou aneuryzmatu. Námi popisovaná kazuistika se týká 30leté pacientky, která byla přijata s Glasgow Coma Scale 14 a těžkou levostrannou hemiparézou. Počítačová tomografie odhalila subarachnoidální krvácení. Jeho příčinou byla ruptura aneuryzmatu na arteria cerebri media vpravo. Aneuryzma bylo zaklipováno. Pro symptomatické vazospazmy jsme opakovaně provedli mechanickou i farmakologickou angioplastiku. Přesto došlo k rozvoji ischemie v pravé mozkové hemisféře, což nás vedlo k dekompresivní kraniektomii. Finálně vazospazmy regredovaly a nastala úplná úprava neurologického deficitu. Čtyři roky po operaci je pacientka plně soběstačná, bez poruchy hybnosti končetin, s nedotčenými kognitivními funkcemi., Vasospasms are considered to be the major cause of high mortality and bad prognosis in patients with subarachnoid hemorrhage caused by a rupture of an aneurysm. Our case report describes a 30-year-old patient admitted with Glasgow Coma Scale 14 and severe left-sided hemiparesis. Computed tomography revealed subarachnoid hemorrhage. Its etiology was ruptured aneurysm of the right middle cerebral artery. The aneurysm was clipped. Mechanical and pharmacological angioplasty was repeatedly performed to manage symptomatic vasospasms. Despite these interventions, an ischemic lesion developed in the right brain hemisphere that prompted us to perform decompressive craniectomy. Finally, vasospasms, as well as the neurological deficit, disappeared.
