Akutní poškození ledvin je obecně definováno jako náhlé zhoršení funkce ledvin podmíněné poklesem glomerulární filtrace a je diagnostikováno hromaděním konečných produktů metabolizmu dusíku, sníženým výdejem moči nebo obojím. Akutní poškození ledvin je důsledkem řady onemocnění, které mohou akutně poškodit funkce ledvin. Jde o častou komplikaci hospitalizace a je velmi časté i u kriticky nemocných pacientů jako důsledek jiných závažných onemocnění. S včasnou diagnózou mohou pomoci nové diagnostické možnosti včetně renálních biomarkerů. Žádná specifická terapie akutního poškození, která by zmírnila tíži poškození anebo urychlila reparaci funkce ledvin, dosud neexistuje. Léčba nadále zůstává symptomatická. K náhradě funkce ledvin pomocí dialýzy přistupujeme v případě těžkého renálního poškození spojeného s nárůstem cirkulujícího volumu, biochemickými změnami a uremickými komplikacemi. Obvykle dojde k reparaci renálních funkcí a pacient není závislý na dialyzační léčbě. Pacienti po prodělaném akutním poškození ledvin mají zvýšené riziko chronického onemocnění ledvin. Obecně je akutní poškození ledvin spojeno se závažnými a dalekosáhlými důsledky nejen pro nemocného (nutnost dialýzy, zvýšení morbidity a mortality), ale i s významnými ekonomickými dopady pro zdravotní systém., Acute kidney injury is generally defined as a condition characterised by the rapid loss of the kidney's excretory function with a decline of the glomerular filtration rate and is typically diagnosed by the accumulation of the end products of nitrogen metabolism or decreased urine output, or both. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. Chronické onemocnění ledvin patří k nejběžnějším klinickým problémům, často však poddiagnostikovaným. Snížení renální funkce vede k ovlivnění všech farmakokinetických vlastností léčiv, nemocní s chronickým onemocněním ledvin jsou proto ve zvýšené míře ohroženi nežádoucími účinky farmakoterapie – především předávkováním léky, které jsou primárně vylučovány ledvinami. Dalšími zásadními problémy u pacientů s onemocněním ledvin jsou polymorbidita a léková polypragmazie. Před nasazením každého nového léčiva je nutné především určit glomerulární filtraci, přičemž samotné stanovení hladiny sérového kreatininu není spolehlivým indikátorem očišťovací funkce ledvin. U léků eliminovaných převážně nebo výhradně ledvinami se dávkovací schéma upravuje snížením dávky léku, prodloužením intervalu mezi podáním jednotlivých dávek nebo kombinací obou metod., Chronic kidney disease belongs among the most frequent clinical problems, but often remains underdiagnosed. In hemodialyzed patients hormonal disturbances are known to occur. However, melatonin levels have not been completely studied. The aim of the study was to find whether changes in calcaemia affect melatonin secretion. For this reason we followed the nocturnal serum concentrations of melatonin and parathyroid hormone (PTH) in 9 hemodialyzed patients (6 women and 3 men, aged 37-65 years) both before and 1-3 months after parathyroidectomy at 6 p.m., 9 p.m., 11 p.m., 2 a.m., 5 a.m. and 7 a.m. At 6 p.m. blood samples to evaluate the levels of calcium and phosphate were also collected. Parathyroidectomy resulted in an increase in nocturnal melatonin levels. As expected, the parathyroidectomy was followed by considerable PTH decrease. PTH showed no nocturnal variation before or after parathyroidectomy. Calcium levels significantly decreased after the operation while phosphate levels increased. In summary, in hemodialyzed patients with hyperparathyroidism, parathyroidectomy significantly increases the nocturnal secretion of melatonin. Diabetická nefropatie postihuje 20–40 % pacientů s diabetem a je hlavní příčinou selhání ledvin. Diabetici s alterovanou funkcí ledvin mají omezené terapeutické možnosti vzhledem k riziku akumulace perorálních antidiabetik a jejich metabolitů při redukované glomerulární filtraci. Dobrá metabolická kontrola je důležitá v časných fázích nefropatie ke snížení rizika progrese a ve stadiu selhání ledvin redukuje riziko progrese aterosklerózy a zlepšuje životní prognózu. Metabolizmus inzulinu je při renálním selhání změněn, clearance inzulinu je prodloužena a zvyšuje se riziko hypoglykemií. Inzulinová analoga krátkodobě působící mají rychlejší absorpci a dlouhodobě působící nižší riziko hypoglykemií. Mohou tak pozitivně ovlivnit kompenzaci diabetu u pacientů s diabetem a renální dysfunkcí., The main cause of renal failure is diabetic nephropathy which affects 20–40 % of diabetic patients. Kalcifikující uremická arteriolopatie, neboli kalcifylaxe, je raritní onemocnění vyznačující se systémovou kalcifikací médie arteriol vedoucí k ischemii a podkožním nekrózám. Nejčastěji se vyskytuje u pacientů s konečným selháním ledvin v chronickém hemodialyzačním programu (incidence se udává 1–4 %) a po transplantaci ledviny. Kalcifylaxe je zrádná svou rychlou progresí v nekrózu tkáně, obtížným hojením a vysokým rizikem sekundární infekce, která je nejčastější příčinou úmrtí při tomto onemocnění. Úmrtnost se udává v rozmezí 60–80 % v průběhu několika měsíců od stanovení diagnózy. Základem léčebného úspěchu je včasné stanovení diagnózy a správně a trpělivě vedená léčba. Na příkladu 3 pacientů zařazených v našem chronickém hemodialyzačním programu demonstrujeme typické klinické projevy a léčbu kožních defektů při kalcifylaxi, jež vyžaduje těsnou spolupráci pacienta a ošetřujícího personálu. Základem léčby u všech našich pacientů byla korekce kalciofosfátového metabolizmu a sekundární hyperparatyreózy. Na konci dialýz byl všem aplikován tiosulfát sodný. Další důležitou součástí byla péče o ránu, citlivý debridement a intenzivní lokální péče. Po 5–6 měsících došlo k úplnému zhojení kožních defektů u první pacientky, výraznému zlepšení v případě druhého pacienta a progresi u třetí pacientky. Nebyly pozorovány žádné nežádoucí účinky aplikace tiosulfátu sodného., Calcific uremic arteriolopathy or calciphylaxis is a rare disorder characterized by systemic medial calcification of arterioles that leads to ischemia and subcutaneous necrosis. It most commonly occurs in patients with end-stage renal disease who are on haemodialysis or who have received a renal transplant. Calciphylaxis is dangerous by its fast progression into tissue necrosis, difficult healing process and a great risk of secondary infection which is the most common cause of death in this condition. The reported mortality rates are as high as 60–80 % in a couple of months once it is diagnosed. The key to successful treatment of calciphylaxis is fast diagnosing of the disease and appropriate treatment management. On the examples of three patients from our haemodialysis centre we demonstrate typical clinical manifestation of calciphylaxis and its treatment, which requires close patient-medical staff cooperation. The basic principle of treatment of all our patients was normalization of calcium-phosphate metabolism and secondary hyperparathyroidism. Sodium thiosulfate had been administered to all patients at the end of haemodialysis session. The wound care played another major role with gentle debridement and intensive local care. After five to six months the skin defects resolved in the first patient, partially resolved in the second patient and deteriorated in the third patient. This review article summarizes the problems of metabolic disorders and nutrition imbalances that often occur in chronic kidney failure (CKF) or following regular dialysis treatment. In this survey, we cover the pathogenesis of these disorders, their clinical consequences, and their association with the most severe complications of chronic kidney failure and dialysis treatment. These complications are primarily at herosclerosis, malnutrition, anemia, hyperparathyroidism, and other serious problems that markedly and negatively affect prognosis and the quality of life of uremic patients. Risk factors for cardiovascular disease are discussed in-depth because cardiovascular disease is the leading cause of death in patients with chronic kidney failure. Pohybová aktivita je v léčbě diabetu neprávem často podceňována a málo využívána. Jedním z důvodů, proč není v praxi často využívána, je obava z poškození, zvláště u pacientů s komplikacemi diabetu. Pohybová aktivita hraje důležitou roli v prevenci progrese periferní neuropatie a vliv má především na rozvoj svalové síly a schopností nahradit funkci poškozenou postižením nervových vláken. Prokazatelný efekt na neuropatii je zaznamenáván již po několika týdnech pravidelného cvičení, dlouhodobé programy pak prokazují bezpečnost z hlediska výskytů ulcerací při dodržování základní péči o dolní končetiny. Při přítomné autonomní neuropatii nelze předvídat reakci srdeční resp. tepové frekvence ani krevního tlaku. Vzhledem k dalším rizikům (němé ischemie apod) je vhodné provést před plánovanou preskripcí pohybové aktivity zátěžový test. Monitorování krevního tlaku, srdeční frekvence a glykemie v průběhu prvních hodin pohybové aktivity je samozřejmostí. U pacientů s autonomní neuropatií gastrointestinálního traktu může významně ovlivnit složení stravy nejen schopnost pohybové aktivity, ale může ovlivnit i vznik hypoglykemií vzhledem ke zpomalenému vstřebávání sacharidů u těchto pacientů. Dalším rizikem u pacientů s autonomní neuropatií je ortostatická hypotenze, která může být potencována antihypertenzivy při hypertenzi bílého pláště. Preskripce u pacientů s retinopatií záleží na formě a stupni retinopatie. Pouze proliferativní retinopatie může významně omezit preskripci pohybové aktivity a vždy je třeba konzultovat s očním lékařem. U pacientů s nefropatií je pro preskripci rozhodující stadium renální insuficience. Preskripce je pak ovlivněna kromě stupně renální insuficience přítomností dalších komorbidit. Pohybová aktivita je zásadní i u pacientů v dialyzačním programu resp. po transplantaci ledvin, protože zachování periferních svalů a jejích funkcí nejen, že zlepšuje prognózu, ale především zlepšuje kvalitu života a udržuje soběstačnost., Physical activity is often underestimated and little used in the treatment of diabetes. Demonstrable effect on neuropathy is already recorded a few weeks of regular exercise, long-term programs then demonstrate the safety of occurrence of ulcers in compliance with basic foot care. Present autonomic neuropathy cannot predict response to cardiac respectively, heart rate and blood pressure. Due to other risks (silent ischemia, arrhythmia etc.), it is appropriate to stress test before a prescription of exercise programs. Monitoring of blood pressure, heart rate and blood glucose during the first hours of physical activity is necessary. In patients with autonomic neuropathy of the gastrointestinal tract may significantly affect the composition of the diet not only the ability of physical activity, but can also affect hypoglycaemia due to a slow carbohydrate absorption in these patients. Another risk in patients with autonomic neuropathy is orthostatic hypotension, which may potentiate antihypertensive drugs in „white coat“ hypertension. Prescription of patients with retinopathy depends on the form and degree of retinopathy. Only proliferative retinopathy can significantly reduce exercise prescription, and it is always necessary to consult with ophthalmologist. In patients with nephropathy is an important stage of renal insufficiency for prescription of physical activity. Prescription is then influenced by the degree of renal insufficiency in addition to the presence of other associated diseases (anemia, hypertension, osteopathy etc.). Cíl: Stručný přehled analytických a klinických aspektů měření parathyroidního hormonu v séru s důrazem na posouzení srovnatelnosti současně používaných metod v klinických laboratořích. Metoda: Prostudování a vyhodnocení recentní odborné literatury z let 2009-2014, doplněné experimentem u souboru pacientů léčených na hemodialýze. V něm bylo sledováno 99 pacientů třemi metodami druhé generace (Roche, DiaSorin, Siemens DPC) a dvěma metodami 3. generace (DiaSorin, Roche). Výsledky a diskuse: Výsledky získané různými metodami jsou velmi odlišné. Hodnoty mediánů jsou až dvojnásobně odlišné a u zvýšených hodnot jsou rozdíly ještě vyšší. Systematické diference jsou významné nejen mezi metodami různých generací, ale (méně) i mezi metodami generací stejných. Četnost výsledků uvnitř rozhodovacího limitu KDIGO 2009 se v závislosti na použitém kitu lišila u pacientů na hemodialýze až 2,5 násobně. Prvním krokem k postupné harmonizaci měření PTH u renálně nemocných pacientů by mělo být použití 3. generace imunochemických metod. Principiálním řešením bude standardizace na bázi separačních metod LC-MS/MS., Method: Overview of recent literature on the PTH comparability from 2009-2014. Comparison of PTH results obtained from 99 patients on the hemodialysis during their regular controls. We used for this purpose three methods of 2nd generation (DiaSorin, Roche, Siemens DPC) and two methods of 3th generation (DiaSorin, Roche). Results and discussion: Results obtained by different methods are not comparable. Median values are two times or more (in higher concentrations) different between results obtained by 2nd or 3th generation of methods. Differences between methods of the same generation are also significant. Number of patients on the hemodialysis with values in required interval of decision limits according to KDIGO 2009 was strongly dependent on the used method and maximum of these differences was approximately 250%. In our opinion is introducing the 3th generation methods of measurement PTH first step to improving the use of PTH in chronic renal diseased patients. Definitive step to rational use of PTH measurement is standardization.
