V průběhu posledních let dochází k nárůstu počtu imunosuprimovaných nemocných a tím i k vzestupu invazivních mykotických onemocnění. Terapie hlubokých mykóz je mnohdy obtížná, mortalita a morbidita těchto infekčních komplikací jsou vysoké. Stále lepší dostupnost nových antimykotik rozšiřuje naše léčebné možnosti a zlepšuje výsledky terapie těchto invazivních mykotických infekcí. Předkládaná práce podává informace o využití jednotlivých antimykotik v léčbě invazivních mykóz, shrnuje léčbu invazivní aspergilózy (IA), invazivní kandidózy (IC) a empirickou antimykotickou terapii. Doporučení vychází ze závěrů prezentovaných Europen Conference on Infection in Leukemia (ECIL)., The frequency of invasive fungal infections has significantly increased with the rise in at-risk populations of patients in the last years. Management of deep fungal infection is difficult and the morbidity and mortality of these infections are very high. New antifungal agents provide the managment options and improve therapeutic outcomes of these infections. This article informs about the role of available antifungal agents in the management of invasive fungal infections and reviews empirical antifungal treatmnet and the treatment of invasive aspergillus and candida infections. Literature data support that green tea and its major component epigallocatechin gallate (EGCG) have powerful antioxidant effects. Contrary, hepatotoxicity can be induced by high-dose EGCG. The timing of exposure to green tea in relation to administration of hepatotoxic agent plays an import role too. The aim of our work was a verification of antioxidative effect of EGCG on D-galactosamine-induced injury in primary culture of rat hepatocytes. Hepatocytes were incubated with EGCG at concentrations of 1.25-10 μM and toxic D-galactosamine (GalN) for 24 hrs. Alternatively, hepatocytes were pretreated with EGCG for 24 hrs, and then incubated with EGCG and GalN for further 24 hrs. Cytotoxicity was analysed by lactate dehydrogenase activity, functional capacity by albumin production. Oxidative stress was evaluated from a production of malondialdehyde and glutathione content in the cells. EGCG protected hepatocytes against GalN-induced cytotoxicity but preventive treatment of intact hepatocytes with EGCG was required to diminish the development of hepatocyte injury. Oxidative stress induced in our study seems to overcome the ability of hepatocytes to improve GSH depletion and albumin production. The aim of our study was to determine the effect of selected cytostatics on a human ovarian cancer cell line A2780 as a model system for ovarian cancer treatment. This cell line is considered cisplatin-sensitive. Panel of tested cytostatics included cisplatin, paclitaxel, carboplatin, gemcitabine, topotecan and etoposide. These cytostatics have a different mechanism of action. To evaluate cytotoxic potential of the tested compounds, the methods measuring various toxicological endpoints were employed including morphological studies, MTT assay, dynamic monitoring of cell proliferation with xCELLigence, cell cycle analysis, caspase 3 activity and expression of proteins involved in cell cycle regulation and cell death. Invazivní metody léčby chronické formy ischemické choroby srdeční byly po dlouhou dobu považovány za kauzální terapii, která bezpochyby snižuje morbiditu a mortalitu nemocných. Na druhé straně životní styl a farmakologická terapie byla nahlížena jako spíše doplňující, ale nikoliv rozhodující léčba. V posledních letech je možno vidět změnu v pohledu na ischemickou chorobu srdeční. Je to mnohem difuznější proces v koronráním řečišti a není to tedy jen otázka kritických stenóz, ale stejně nemocného ohrožují i další úseky věnčitých tepen, kde se hovoří o vulnerabilních placích. Překvapením pro experty byly závěry řady studií potvrzujících neschopnost invazivch metod prodloužit život nemocným s chronickou formou ICHS. Na druhé straně přibývají důkazy o významném vlivu úpravy životního stylu a rozrůstajících se možnostech farmakolgoické terapie s pozitivním vlivem na morbiditu a mortalitu. Článek prezentuje současné názory na tyto otázky, které jsou nyní živě diskutovány., Invasive methods for treatment of coronary artery disease in chronic form were, for a long time, believed to be causal treatment that definitely diminish patient morbidity and mortality. By contrast, lifestyle and pharmacological therapy were seen as valuable but not lifesaving. In the last few years a change can be seen in the view on coronary artery disease. It is a much more diffusive and complicated process in the coronary tree. It is not merely a question of the critical stenotic areas; the new idea of vulnerable plaque, which presents the same danger of complication as critically stenotic areas, is also important. A great surprise for experts was the result of various studies confirming the inability of invasive procedures to prolong life in patients with chronic coronary disease. On the other hand, there has been more evidence that lifestyle changes and expanding pharmacological possibilities can have a significant positive effect on morbidity and mortality. Vzniku trombózy v arteriálním nebo venózním cévním systému předchází složitá souhra faktorů vnějšího prostředí a faktorů genetických. Rozsáhlé genomové asociační studie se ukazují jako velice přínosné v identifikaci lokusů spojených s kardiovaskulárními onemocněními a souvisejícími rizikovými faktory. Ještě zbývá mnoho práce v identifikaci určujících genů a jejich kauzálních variant, stejně jako v definici mechanizmů, které ovlivňují vývoj a progresi onemocnění. Z výsledků studií lokusů spojených s dříve identifikovanými onemocněními se očekává zlepšení našich znalostí o patofyziologii kardiovaskulárních onemocnění a možnost identifikovat nové cíle pro léčbu. V oblasti aterotrombózy a trombofilie jsou významné výsledky z asociačních studií zaměřených na oblast ischemické choroby srdeční, ischemické cévní mozkové příhody a žilního tromboembolizmu. The genomewide association approach has proved successful in identifying loci associated with cardiovascular disease and related risk factors. However, much work remains to identifyning the culprit genes and causal variants as well as the mechanisms whereby they influence disease development and progression. In-depth studies of previously identified disease-associated loci are expected to improve our understanding of the pathophysiology of cardiovascular disease and identify novel targets for treatment. In the field of atherothrombosis and thrombophilia are significant results from association studies focused on the area of coronary artery disease, ischemic stroke, venous thromboembolism.
